1.Construction and Policy Recommendations for An Integrated Pharmaceutical Care Model for Chronic Respiratory Diseases Based on the Rainbow Model

Objective To construct an integrated pharmaceutical care model for chronic respiratory system disease patients,and to provide policy recommendations.Methods A field survey,a semi-structured interview,and a questionnaire survey were comprehensively used to investigate the effect of the reform and the status quo of pharmaceutical care for chronic respiratory diseases after the medicine consistency reform in the medical alliance of Chongqing.Subsequently,the rainbow model was used to assess the challenges in the macro,meso,micro,and supporting element's dimensions,and the integrated pharmaceutical care model was established as a consequence.Results The primary issues involved in the medical alliance were as followed:insufficient joint reformation for public health services,medical insurance,and medical production and circulation(JRPMM);deficient supporting policies;a dearth of information support;an inadequately list of medications;insufficient collaboration among chronic disease specialties;and unstandardized pharmaceutical care.Consequently,the following recommendations were:in the macro dimension,to strengthen the JRPMM to enhance the service capacity of primary hospitals and to develop suitable human resource coordination mechanisms.In the meso dimension,to intensify the reform of pharmaceutical care in the medical alliance and enhance specialty collaboration horizontally.In the micro dimension,to establish a pharmaceutical management committee in the medical alliance,to formulate pharmaceutical technical specifications for the hierarchical medical system,and to combine"talent and technology"training innovatively.In the supporting elements dimension,to bolster information support in the area and to develop suitable incentives and assessment institutions.Conclusion This paper reported an integrated pharmaceutical care model for chronic respiratory diseases,which could serve as a reference for standardizing the hierarchical medical system for chronic diseases.
2.Clinical analysis of pulmonary tuberculosis complicated with Nocardia farcinica

Objective To analyze the clinical characteristics of pulmonary tuberculosis patients compli-cated with infection of Nocardia farcinica aiming to improve the diagnosis and treatment the disease.Methods The clinical data of 22 cases of pulmonary tuberculosis complicated with Nocardia farcinica infection admitted to Guang-zhou Chest Hospital from June 2020 to December 2023 were collected and the clinical manifestations,imaging,laboratory tests,treatment process,and disease outcomes were retrospectively analyzed.Results Among the 22 patients,there were 13 males and 9 females,aged 20~86 years,with a median age of 52 years.Common clinical manifestations included cough(22/22),sputum(21/22),and underlying diseases(13/22).One case was positive for cerebrospinal fluid culture,and 21 cases were positive for sputum culture.The culture period was 5~26 days,with a median culture period of 18 days.The imaging manifestations were mainly plaques,plaques and cavities,and the lesions were spread in both lungs(17/22)and cavities(11/22).After anti-tuberculosis treatment,the absorption of lung lesions in some patients was poor,and the absorption of the lesions was improved after anti-nocardia treat-ment.8 cases were cured,13 cases were improved and 1 case died.Conclusion The clinical symptoms of pa-tients with pulmonary tuberculosis complicated with Nocardia farcinica were atypical,the culture period of Nocardia was long,and the imaging manifestations were similar to pulmonary tuberculosis,which is prone to misdiagnosis and missed diagnosis.For patients with poor response to anti-tuberculosis treatment and slow lesion absorption,the possi-bility of a concurrent Nocardia infection should be considered.
3.Combined determination of 210Pb and 210Po in soil samples around a decommissioned uranium mine

Objective To evaluate the current situation of radioactive contamination in the soil around a decommissioned uranium mine by combined determination of 210Pb and 210Po activity concentrations. Methods Soil samples were collected around a decommissioned uranium mine. 210Po and the 210Pb decay product 210Bi were plated on a copper sheet by self-deposition at a constant temperature. Gross α and gross β counts were measured using a low-background α/β counter. The activity concentrations of 210Pb and 210Po in soil were calculated. The radioactive contamination was evaluated using geological accumulation index. Results The activity concentrations of 210Pb and 210Po in the soil around the decommissioned uranium mine were 873.7-2315.5 and 956.0-2315 Bq·kg−1, respectively. The activity concentrations of 210Pb and 210Po in nearby farmland soil were 54.2-257.8 and 59.1-234.5 Bq·kg−1, respectively. Evaluation with the geological accumulation index showed that the level of nearby farmland contamination was 0-2, indicating zero to moderate radioactive contamination of the farmland. Conclusion The relatively high radioactivity around the decommissioned uranium mine requires attention to the transfer, diffusion, and ingestion of the uranium decay products 210Pb and 210Po in the surrounding soil to avoid radioactive contamination of the environment.
4.Efficacy and safety of laparoscopic versus open radical resection for hilar cholangiocarcinoma: a meta-analysis

Objective:This article aimed to systematically evaluate the safety, feasibility and short-term outcomes of laparoscopic versus open radical resection for hilar cholangiocarcinoma.Methods:Relevant medical literatures published before November 30, 2021 were searched using PubMed, EMbase, Cochrane Library, Web of Science, CNKI, Wanfang, VIP, CBM databases. Based on predetermined inclusion and exclusion criteria, articles were selected, and RevMan5.4 software was used to conduct the meta-analysis of this study.Results:Twelve comparative cohort articles were enrolled, which included 634 patients. There were 275 patients who underwent laparoscopic radical resection for hilar cholangiocarcinoma in the laparoscopic group, and 359 patients who underwent open radical resection for hilar cholangiocarcinoma in the open group. Meta-analysis showed that when compared with the open group, operation time of the laparoscopic group was significantly longer ( MD=58.36, 95% CI:13.54-103.18). However, intraoperative blood loss ( MD=-79.70, 95% CI:-90.01--69.39), hospital stay ( MD=-3.37, 95% CI:-4.29--2.45) and incision-related complications ( OR=0.36, 95% CI:0.16-0.80) were all significantly less (all P<0.05). The number of lymph node harvested, R 0 resection rates, postoperative bile leakage, intraperitoneal infection and effusion, total complication rates, and perioperative mortality and 1-year survival rates after operation between the two groups were not significantly different (all P>0.05). Conclusion:Laparoscopic radical resection for hilar cholangiocarcinoma was feasible and safe in selected patients carried out in an experienced hepatobiliary surgery center.

Trauma is the leading cause of death for people under 40 years old in the world. At present, the rescue and treatment system of trauma patients in China is not yet well established, and the mortality of trauma patients is higher than those in the developed countries. Improving the treatment system is the key to reducing the trauma mortality. In order to innovate the service mode of trauma first aid, further promote the establishment of regional trauma first aid system, improve the ability of trauma treatment, reduce the mortality and disability rate of trauma patients in Jiangxi Province, recently Health Commission of Jiangxi Province and the First Affiliated Hospital of Nanchang University have reached a consensus on the establishment of Jiangxi trauma first aid center. In order to provide reference for the construction of trauma treatment system, the author analyzes the following aspects including functional positioning, basic requirements, organization management, and evaluation of core indicators.

Objective:
To provide basis for early selection of drug intervention or surgical treatment for premature patent ductus arteriosus(PDA) by a dynamic monitoring of serum N-terminal pro-brain natriuretic peptide(NT-proBNP)levels with ultrasonic examination, so as to improve the prognosis of premature infants.
Methods:
A total of 108 premature infants with gestational age less than 32 weeks and body weight less than 1.5 kg, within 24 h of birth were admitted to the Department of NICU, Harbin Children′s Hospital from June 2016 to December 2018.The serum NT-proBNP levels were measured at 3 d, 6 d, 9 d after birth, and echocardiography was performed at the same time.According to the results of echocardiography and clinical symptoms, infants were divided into haemodynamically significant PDA(hsPDA)(
7.Association study between G-protein β3 subunit gene polymorphism and antipsychotic agent-induced obesity

Objective To explore the relationship between G-protein β3 subunit (GNB3) gene C825T polymorphism and antipsychotic agent-induced obesity.Methods 126 schizophrenic inpatients with long-term antipsychotics treatment were collected.According to body mass index ( BMI),patients were divided into obesity group ( n =62) and non-obesity group ( n =64).The GNB3 gene C825T polymorphism was detected by polymerase chain reaction and DNA sequencing technique.Levels of fasting blood glucose,2-hour postprandial blood glucose,blood lipids and blood uric acid of all patients were routinely measured.Results (1)The GNB3 gene C825T polymorphism were found in obesity group and non-obesity group respectively,and the distribution of genotypes in two groups were both consistent with Hardy-Weinberg equilibrium.(2)There was no significant difference in genotype frequencies between obesity group ( CC 17.75%,CT 58.06%,TT 24.19% ) and non-obesity group( CC 18.75%,CT 62.50%,T T 18.75% )( x2 =0.59,P > 0.05 ).There was also no significant difference in allele frequencies between obesity group ( C 46.77%,T 53.23 % ) and non-obesity group ( C 50%,T 50% ) ( x2 =0.26,P > 0.05 ).(3)No significant differences were observed in BMI,fasting blood glucose,2-hour postprandial blood glucose,blood lipids and blood uric acid among different genotype groups (all P > 0.05 ).Also no significant differences were observed in BMI,fasting blood glucose,2-hour postprandial blood glucose,blood lipids and blood uric acid between Tallele carrier (TT and CT genotypes) and T-allele non-carrier( CC genotype) ( all P > 0.05 ).Conclusion The GNB3 gene C825T polymorphism may not be a genetic risk factor for antipsychotic agent-induced obesity.
8.Revision after total hip arthroplasty in 41 patients

Forty-four patients suffering from hip pain after the hip and femoral head replacement were treated by revision for total hip arthroplasty in Spinal and Joint Surgery of Gaozhou People’s Hospital between August 2000 and August 2006. Forty-one patients were followed up. The surgeon, an associate chief physician, had been engaging in artificial joint field for 9 years, and performed artificial joint replacement for 620 patients. ①There were 26 patients with pain after artificial femoral head replacement, including 17 males and 9 females with an average age of 72.3 years (range, 67-79 years). Of the 26 patients, 12 had loosening of prosthesis stem, 8 acetabular wear, 3 dislocation of artificial femoral head, 2 ossification around the joints, and 1 periprosthetic fracture. ②Fifteen patients suffered with pain after total hip replacement, including 9 males and 6 females with an average age of 73.4 years (range, 66-80). Of them, there were 6 with loosening of prosthesis, 2 with loosening of acetabular prosthesis, 2 with wear of acetabular prosthesis, 2 with dislocation of artificial femoral head, 1 with infection of incision, 1 with delayed infection, and 1 with periprosthetic fracture. ③During follow up, 4 cases developed bone cement response symptoms such as acute hypotension, hypoxemia, and arrhythmia, but no rejection induced by artificial joint was found. X-ray films showed that no metal corrosion or acetabular polyethylene prosthesis degradation occurred. ④White blood count was generally elevated the day after revision of total hip arthroplasty, and mostly recovered 7 days after revision. Three patients had venous thrombosis of lower extremities. All 41 patients were followed up for 1-7 years, and odynolysis rate was 90.2%. The results indicate that the complications such as loosening of prosthesis, acetabular wear, dislocation of artificial femoral head, and ossification around the joints are main causes of hip pain after hip prosthesis replacement. Revision of total hip arthroplasty is an efficient method to relieve the pain.
