Objective To investigate the impact of nutritional risk-based intervention on the quality of life and survival prognosis of patients with the locally advanced pancreatic cancer and undergoing the chemotherapy.Methods A total of 118 patients with the pancreatic cancer and admitted to the Department of Hepatobiliary and Minimally Invasive Surgery,Lincang People's Hospital from April 2021 to April 2024 were selected and randomly divided into the nutritional intervention group and the control group,with 59 patients in each group.All patients received the palliative chemotherapy,and the control group received the routine dietary guidance,while the intervention group received the nutritional intervention based on the nutritional risk score.The nutritional status,quality of life score,and survival prognosis of the two groups were compared.Results The NRS2002 score of the intervention group and the control group gradually increased from the first to the sixth chemotherapy,and the NRS2002 score of the intervention group was lower than that of the control group at the 4th,5th,and 6th chemotherapy(P<0.05).The incidence of malnutrition in the intervention group was lower than that of the control group at the 1st,2nd,and 3rd chemotherapy(11.9%,30.9%,and 32.2%,respectively),but there was no significant difference(P>0.05);The incidence of malnutrition in the intervention group was significantly lower than that of the control group at the 4th,5th,and 6th chemotherapy(32.2%,45.8%,and 52.5%,respectively),while the incidence of malnutrition in the control group was 59.3%,69.5%,and 88.5%,respectively(P<0.05);The quality of life score of the intervention group in all dimensions was significantly higher than that of the control group after the treatment(P<0.05).After the nutritional intervention,the median OS time and median PFS time of the patients were significantly improved(P<0.05).Conclusion Nutritional intervention can reduce the nutritional risk during the chemotherapy for locally advanced pancreatic cancer patients and improve their quality of life.

Objective To explore the distribution of healthcare-associated infection(HAI)and direct economic burden in neurosurgical patients based on disease diagnosis-related grouping(DRG),providing data support for in-fection prevention and control.Methods Clinical data of neurosurgical patients in a hospital from January to Decem-ber 2023 were retrospectively investigated,the average length of hospital stay and average hospitalization expense of HAI and non-HAI groups of the subgroups of DRG were analyzed.Results A total of 102 cases of HAI occurred among 2 180 neurosurgical patients,with HAI incidence being 4.68％.The main infection sites were lower respira-tory tract and organ space,accounting for 53.92％and 25.49％respectively.HAI patients distributed in 16 DRG subgroups,out of which AH 19 subgroup(invasive ventilator support≥96 hours or extracorporeal membrane oxy-genation[ECMO]or total artificial heart transplantation)had the highest incidence(58.82％),followed by BC19 subgroup(intracranial vascular surgery accompanied with hemorrhage diagnosis)(17.65％)and BB2A subgroup(craniotomy other than trauma,with severe or general complications and comorbidities)(12.81％).There was no statistically significant difference in resource consumption between HAI group and control group of AH19 group(all P＞0.05).HAI in BB2A group increased the average length of hospital stay and average hospitalization expense by 5.00 days and 34 600 Yuan,respectively.HAI in BC19 group increased the average length of hospital stay and ave-rage hospitalization expense by 8.50 days and 42 800 Yuan,respectively.Lower respiratory tract infection had a significant impact on resource consumption,while organ space infection only increased length of hospital stay of pa-tients.Conclusion Analysis of incidence of HAI and resource consumption of major infection sites based on DRG can clarify the focus of infection prevention and control,formulate targeted intervention measures,control medical expense and improve the quality of medical services.

Objective To observe the value of apparent diffusion coefficient(ADC)for evaluating short-term efficacy of TACE for treating colorectal cancer liver metastases(CRLM).Methods Data of 60 liver metastases in 28 CRLM patients who underwent TACE were retrospectively analyzed.Based on MRI after the first TACE,according to the response evaluation criteria of solid tumors,the liver metastases were divided into response group(n=38)and non-response group(n=22).ADC parameters obtained with diffusion weighted imaging(DWI)before and after TACE,including ADC before TACE(ADCpre),after the first TACE(ADCpost1)and after the second TACE(ADCpost2)were compared between groups,while ADC change value(ΔADC)and the percentage of ΔADC were calculated.The maximum diameter of the target foci were measured,and the correlation between ΔADCpost1 and the change of the maximum diameter of target foci were analyzed.Receiver operating characteristic curve was drawn,the area under the curve(AUC)was calculated to observe the efficacy of ΔADCpost1 for evaluating short-term efficacy of TACE for CRLM.Results No significant difference of ADCpre was found between groups(P=0.484).After the first TACE,ADCpost1,ΔADCpost1 and percentage of ΔADCpost1 in response group were all higher than those in non-response group(all P＜0.05).After the second TACE,no significant difference of ADCpost2,ΔADCpost2 nor percentage of ΔADCpost2 was found between groups(all P＞0.05).The maximum diameter change of the target foci after the first TACE was(-0.48±0.93)cm,which was negatively correlated with ΔADCpost1(rs=-0.347,P=0.007).AUC of ΔADCpost1 for evaluating short-term efficacy of TACE for CRLM was 0.717.Conclusion ADC had good efficacy for evaluating short-term efficacy of TACE for treating CRLM.

Objective To analyze the clinical characteristics of pulmonary tuberculosis patients compli-cated with infection of Nocardia farcinica aiming to improve the diagnosis and treatment the disease.Methods The clinical data of 22 cases of pulmonary tuberculosis complicated with Nocardia farcinica infection admitted to Guang-zhou Chest Hospital from June 2020 to December 2023 were collected and the clinical manifestations,imaging,laboratory tests,treatment process,and disease outcomes were retrospectively analyzed.Results Among the 22 patients,there were 13 males and 9 females,aged 20～86 years,with a median age of 52 years.Common clinical manifestations included cough(22/22),sputum(21/22),and underlying diseases(13/22).One case was positive for cerebrospinal fluid culture,and 21 cases were positive for sputum culture.The culture period was 5～26 days,with a median culture period of 18 days.The imaging manifestations were mainly plaques,plaques and cavities,and the lesions were spread in both lungs(17/22)and cavities(11/22).After anti-tuberculosis treatment,the absorption of lung lesions in some patients was poor,and the absorption of the lesions was improved after anti-nocardia treat-ment.8 cases were cured,13 cases were improved and 1 case died.Conclusion The clinical symptoms of pa-tients with pulmonary tuberculosis complicated with Nocardia farcinica were atypical,the culture period of Nocardia was long,and the imaging manifestations were similar to pulmonary tuberculosis,which is prone to misdiagnosis and missed diagnosis.For patients with poor response to anti-tuberculosis treatment and slow lesion absorption,the possi-bility of a concurrent Nocardia infection should be considered.

OBJECTIVE To establish the correlation evaluation and quality evaluation method of HPLC fingerprint grade of color sorting Lonicerae Japonicae Flos, and provide technical basis for the grade standard of color sorting Lonicerae Japonicae Flos. METHODS The chromatographic column was SVEA C18(250 mm×4.6 mm, 5 μm); mobile phase was acetonitrile(A)- 0.2% formic acid aqueous solution(B); gradient elution; injection volume was 10 μL; detection wave length was 245 nm; volume flow rate was 0.5 mL·min-1; column temperature was 38 ℃. The common peak determination and similarity evaluation of HPLC chromatogram data were carried out by using the Similarity Evaluation System of Traditional Chinese Medicine Chromatographic Fingerprints(Version 2012); the color sorting grade evaluation was carried out by CA, PCA and PLS-DA. The first part of Chinese Pharmacopoeia 2020 Edition was used to measure the quality control indicators, and the data were analyzed comprehensively. RESULTS A total of 28 common peaks were identified in the fingerprints, and 7 components were identified. The similarity of 24 batches of color sorting grade samples was 0.936-0.968. CA and PCA divided 28 batches of Lonicerae Japonicae Flos samples into 4 categories, which were basically consistent with the classification of color sorting, and PLS-DA achieved a discrimination result that was very consistent with the classification of color sorting. The color sorting grade was negatively correlated with the diameter, flowering rate, damage rate, and luteolin content of Lonicerae Japonicae Flos. The color sorting grade was positively correlated with chlorogenic acid, 3,5-di-O-caffeoyl quinic acid and 4,5-di-O-caffeoyl quinic acid. There was a clear correlation between the color sorting of Lonicerae Japonicae Flos and established fingerprint overall. There were differences in the quality of Lonicerae Japonicae Flos in the color sorting grade. Based on the sensory indicators of diameter, flowering rate, and damage rate, the content, diameter, flowering rate, and damage rate of luteolin showed a trend from high to low, ranging from third grade>second grade>first grade>special grade. The content of three phenolic acids showed a trend from high to low, ranging from special grade>first grade>second grade>third grade. Among the special grade, the content of three phenolic acids was the highest. CONCLUSION Combining the content of luteolin and phenolic acids as evaluation and control indicators for color selection grade is feasible and scientific, which can achieve intelligent color sorting grade production of Lonicerae Japonicae Flos grade.

Objective: To provide basis for early selection of drug intervention or surgical treatment for premature patent ductus arteriosus(PDA) by a dynamic monitoring of serum N-terminal pro-brain natriuretic peptide(NT-proBNP)levels with ultrasonic examination, so as to improve the prognosis of premature infants. Methods: A total of 108 premature infants with gestational age less than 32 weeks and body weight less than 1.5 kg, within 24 h of birth were admitted to the Department of NICU, Harbin Children′s Hospital from June 2016 to December 2018.The serum NT-proBNP levels were measured at 3 d, 6 d, 9 d after birth, and echocardiography was performed at the same time.According to the results of echocardiography and clinical symptoms, infants were divided into haemodynamically significant PDA(hsPDA)(n=29), asymptomatic PDA(asPDA)(n=24)and non-PDA as control group(n=55). Among them, the hsPDA group was further divided into drug treatment group(n=21) and surgical treatment group(n=8). The surgical treatment group was those who failed 2 courses of oral ibuprofen treatment or had contraindications to drug treatment.Arterial catheter ligation was used for surgical treatment group on 9 to 21 days after birth.NT-proBNP levels were detected at 24 hours and 3 days after surgery, and echocardiography was performed at the same time.The levels of serum NT-proBNP were compared between the three groups and before and after the treatment, and the ROC curve of NT-proBNP was drawn to analyze its diagnostic value. Results: (1) At 3 and 6 days after birth, serum NT-proBNP levels were 8 346 pg/ml and 3 340 pg/ml in the hsPDA group, and 2 536 pg/ml and 1 079 pg/ml in the asPDA group, 1 132 pg/ml and 879 pg/ml in the control group, respectively.The levels of NT-proBNP in the hsPDA group were significantly higher than those in the asPDA group(P<0.05), and the levels of NT-proBNP in the asPDA group were higher than those in the control group(P<0.05). At 9 days after birth, serum NT-proBNP levels were 2 231 pg/ml in the hsPDA group, 834 pg/ml in the asPDA group, and 808 pg/ml in the control group.The levels of serum NT-proBNP in the hsPDA group were significantly higher than those in the asPDA group and the control group (P<0.05). There was no significant difference between the asPDA group and the control group (P>0.05). (2)At 3, 6, and 9 d after birth, serum NT-proBNP levels were 9 000 pg/ml, 8 989 pg/ml, 9 000 pg/ml in the surgical treatment group, and 3 741 pg/ml, 2 544 pg/ml and 1 032 pg/ml in the drug treatment group, respectively.The levels of serum NT-proBNP in the surgical treatment group were significantly higher than those in the drug treatment group (P<0.05). The serum NT-proBNP levels in both drug treatment group and surgical treatment group were significantly lower than those before treatment (P<0.05). At 3 d after the operation, the serum NT-proBNP level was 941 pg/ml in the surgical treatment group and 736 pg/ml in the drug treatment group.There was no significant difference between the two groups (P>0.05). (3)At 3 d after birth, the area under the ROC curve of serum NT-proBNP was 0.91 (95%CI 0.865-0.964), and the sensitivity and specificity of NT-proBNP to diagnose hsPDA at 2 343.5 pg/ml were 93.1% and 73.41.% respectively. Conclusion NT-proBNP level monitoring can be used as a sensitive indicator for early identification of hsPDA, and it has a clinical significance for intervention strategy and intervention time selection.

Objective To comprehensively grasp the grassroots medical institutions blood transfusion(blood) management present situation,put forward improvement strategy for the universality of existence weak link,the stand-ardized management of clinical blood transfusion to strengthen grass -roots medical institutions to provide necessary theoretical support.Methods 62 grassroots medical institutions blood transfusion(blood)in Yantai district were selected as the research subjects,to set the planning,layout,basic construction standards,related technology and equipment application aspects of quantitative evaluation,preliminary conclusions.Results By examination of 62 grassroots medical institutions transfusion quality management level both in terms of hardware and software facilities, emergency plan formulation,system file support informatization construction,staff education training,etc,found many problems,there was a certain gap between the existing department of blood transfusion standards.Conclusion The current grassroots medical institutions transfusion management is conditioned by the objective conditions,there are many problems,such as not improved,will cause a certain hidden trouble on the safety of clinical blood transfusion. Therefore,should combine their own reality,strengthen the quality of blood transfusion management system construc-tion,and give full play to the supervision and management of the hospital transfusion quality management committee. At the same time,the local administrative department of health also should formulate corresponding policies,ensure transfusion management level significantly increased.

Objective To detect the gene mutation of thyroid hormone receptor β ( TRβ ) in a family with thyroid hormone resistance syndrome.Methods The genomic DNA was extracted from peripheral blood leukocytes of the patient and his 5 family members.The exons 1-10 ofTRβ gene were amplified by PCR.The products of PCR were sequenced directly to detect the gene mutation.Results Two members of this family were confirmed to have the C y A transition mutation at nucleotide 1642 site within exon 10 of TRβ gene,which was a missense mutation causing the substitution of Proline to Threonine (P453T).The mutation was Heterozygous.Conclusions It was confirmed that the patient has TRβ gene mutation P453T in exon 10.The mutation may lead to the occurrence of thyroid hormone resistance syndrome.

Objective To investigate the association of single nucleotide polymorphisms (SNPs) in the SCGB3A2(secretoglobin family 3A member 2) gene promoter with susceptibility of Graves' disease.Methods One-hundred and seventy-nine SNPs within a 3.0 Mb region surrounding marker D5s2090 were scanned in a case-control study.The size of the region(s) associated with GD was then narrowed.Results Total 179 SNPs within a 3.0 Mb region surrounding marker D5s2090 were analyzed.The most significant association signal was found at SNP rs1368408 (P =3.69 × 10-5).Subsequent association analysis was then performed and the results suggested that the SNP76 (P =4.11 × 10-8) and SNP75 (P =1.37 × 10-8) in the promoter of SCGB3A2 gene may be the causal variants of GD.Logistic regression analysis suggested these 2 SNPs in this region may contribute to GD susceptibility.Conclusion A significant association seems to exist between GD with the SCGB3A2 gene.
