1.Research on the application of data platform in refined management for medical consumables under DRG mode

Objective:To build a data platform for medical consumables,so as to improve the management efficiency for medical consumables under the mode of diagnosis related groups(DRG)for disease.Methods:Based on the framework of data platform,the data extraction and transformation,data warehousing,big data analysis,and data mining were adopted to establish a data platform that can integrate rapid data integration,retrieval and statistics,and mining applications and other functions in one for medical consumables.A total of 122409 procurement records,2301337 records of incoming and outgoing inventory,and 740833 usage records of medical consumables at Shanghai General Hospital during March 2020 and July 2024 were selected.The data platform system and conventionally manual methods were respectively used to predict the total consumption amount of medical consumables during August to October 2024.The accuracy and duration of the predictive results between the two prediction modes were compared.In the usage records of medical consumables during August and October 2024,the usage records of 80 consumables with high price in the departments of orthopedics and neurosurgery were selected,and they were set as the tested objects after they were mixed with the un-rational usage records of 20 medical consumables that were manually set.Then,the detection functions of data platform system for rational usage of medical consumables were assessed.Results:The accuracy of the procurement plan of medical consumables that was assisted by the data platform of medical consumables closed to that of the conventional manual method,with an accuracy rate of 100%.The efficiency of generating plan was significantly better than that of conventional manual method,and the difference was statistically significant(t=1.987,P<0.05).In the application of rational analysis of the usage of medical consumables,both the accuracy rate and overall rate of the check of the data platform of medical consumables for the events of un-rational using medical consumables were equal to those of conventional method.However,the detection efficiency of the data platform of medical consumables was significantly better than that of conventional manual method,and the difference was statistically significant(t=1.972,P<0.05).Conclusion:The data platform of medical consumables connects various business links in management for medical consumables,which can reduce data barriers,and accelerate the conversion efficiency of data value by establishing data update,data governance,services of data application and other methods.It promotes the transformation of medical behavior from experience-driven to data-driven,and improves the refined level of management for medical consumables.

3.A study on the effect of autologous bone marrow nucleated cell transplantation in the treatment of ovarian dysfunction

Objective:To evaluate the clinical efficacy of autologous bone marrow nucleated cell transplantation on ovarian dysfunction-related diseases.Methods:From September 2020 to August 2021, we conducted a before-after study in Shanghai Institute of Planned Parenthood Research Hospital and Shanghai New Hongqiao International Medical Center. A total of 28 patients with premature ovarian insufficiency, perimenopausal syndrome or repeated assisted reproduction failure were treated with ultrasound-guided autologous bone marrow nucleated cell transplantation into the ovaries. Clinical efficacy was evaluated by comparing the patients' self-reported symptoms, endometrial thickness, bilateral ovarian volume, the number of bilateral ovarian follicles, the maximum diameter of bilateral ovarian follicles, and the level of plasma estradiol, follicle-stimulating hormone (FSH), luteinizing hormone (LH), progesterone, prolactin, and testosterone.Results:After autologous bone marrow nucleated cell transplantation, the number of bilateral ovarian follicles and the maximum diameter of bilateral ovarian follicles statistically significantly increased by an average of 1.26±2.12 ( P=0.005) and (5.40±8.92) mm ( P=0.006), respectively. Endometrial thickness and bilateral ovarian volume increased after treatment although without statistically significance (all P>0.05). However, the ovarian volume, the number of follicles, and the maximum diameter of follicles with greater changes between the left and right sides of the treatment, were found to statistically significantly increase after treatment ( P=0.007, P<0.001, P=0.002). Besides, the levels of FSH, LH, progesterone, and prolactin decreased with no statistical significance (all P>0.05). The stratified analysis of disease types found that endometrial thickness, the number of bilateral ovarian follicles, and the maximum diameter of bilateral ovarian follicles after treatment tended to increase, but only the number of bilateral follicles and the maximum diameter of bilateral ovarian follicles in patients with perimenopausal syndrome showed statistical significance ( P=0.008, P=0.047). Conclusion:The present study suggested that autologous bone marrow nuclear cell therapy could improve ovarian function to some extent, and is expected to be a new treatment method for patients with premature ovarian insufficiency, perimenopausal syndrome, and repeated assisted reproduction failure. However, further studies with larger sample sizes are needed to corroborate the findings.

5.Performance verification of 3 kinds of dry chemical analyzers used in ALT screening before blood donation

Objective To perform the comparative analysis on the use situation of 3 kinds of dry chemical analyzers according to the industrial standards YY/T0655-2008.Methods With the Roche Cobas P800 automatic biochemical analyzers as the reference instrument,the accuracy,intra-assay precision,linearity,etc.in 3 kinds of dry chemical analyzers were evaluated.Results 3 kinds of dry chemical analyzers met the requirements of the industrial standards YY/T0655-2008,but the precision of imported instrument was significantly better than that of the domestic instrument.Conclusion In order to ensure the comparability and consistency of the detection results among instruments,the instruments should be regularly calibrated and performed the comparative experi-ments,the premise of calibration and comparison should be using 1/4 of the precision level in the CLIA′88 indicators as the indica-tor,not satisfying with the precision requirements in YY/T0655-2008,only in this way can the waste of blood and donors going a-way be avoided.
6.Effect of Blood-activating and Stasis-removing Method for Acute Tubular Necrosis After Kidney Transplantation

[Objective] To observe the effect of Xiangdan injection with the actions of activating blood and removing stasis for the treatment of acute tubular necrosis (ATN) after kidney transplantation. [Methods] Twenty-six patients with ATN after kidney transplantation were randomized into groups A and B. The two groups were treated with triple immuno-suppressive agents of cyclosporin A, prednisone and mycophenolic acid, with medicines for protecting liver function and gastric mucosa, and with preventive measures from infection. Additionally, group A (n = 16) was treated with Xiangdan injection, while group B ( n = 10) was not. After treatment, time for hemodialysis, renal function and renal flow resistance index of the transplanted kidney were compared between the two groups after the occurrence of ATN. [Results] Compared with group B, time for hemodialysis was shortened, the decrease of serum creatine (SCr) in the transplanted kidney quickened, and renal flow resistance index (RI) of the transplanted kidney decreased (P 0.05). [Conclusion] Xiangdan injection with the actions of activating blood and removing stasis can promote the early recovery of ATN after kidney transplantation.
