1.Observation of curative effect of Qiju Dihuang pill combined with fluorometholone eye drops on dry eye disease due to Yin deficiency of liver and kidney

Objective To investigate the efficacy of Qiju Dihuang pill combined with fluorometholone eye drops on dry eye disease(DED)due to Yin deficiency of liver and kidney and its impact on the ocular surface function of patients.Methods Ninety-two patients with DED admitted to our hospital from April 2021 to May 2024 were gathered and assigned into two groups using a random number table method,with 46 patients in each group.The conventional group was treated with fluorometholone eye drops,while the combination group was treated with Qiju Dihuang pills on the basis of the conventional group.The efficacy,tear film break up time(TBUT),length of tear infiltration,corneal fluorescein staining(FLS)and ocular surface disease index(OSDI)of DED patients were compared between the two groups,and adverse reactions were evaluated.Results After treatment,the length of TBUT and tear infiltration were significantly increased in the two groups(P<0.05),OSDI and FLS scores were significantly decreased(P<0.05),and changes were more significant in the combined group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).The efficacy of the combination group was 91.30%(42/46),which was higher than that of the conventional group,76.09%(35/46)(P<0.05).Conclusion The combination of Qiju Dihuang pill and flumilone eye drops has good therapeutic effect for patients with DED of Yin deficiency of liver and kidney.

3.Clinical, endoscopic, and pathological analyses of 14 cases of gastric adenocarcinoma of the fundic gland type

Objective:Gastric adenocarcinoma of the fundic gland type (GA-FG) is rare and often occurs in patients who are not infected with Helicobacter pylori. The current study analyzed and summarized the clinical, endoscopic, and pathological features of GA-FG, in an effort to improve its diagnosis. Methods:Patients who were diagnosed with GA-FG and treated with endoscopic submucosal dissection (ESD) resection at the Department of Gastroenterology, First Affiliated Hospital of Zhejiang Chinese Medical University from January 1st 2020 to October 1st 2022 were included in the study. Their clinical manifestations, endoscopic features, pathological immunohistochemistry, and other characteristics were analyzed.Results:A total of 14 patients with GA-FG were included in the study, 5 males and 9 females, with a mean age of 59 years. Most had no substantial clinical manifestations. Twelve patients were H. pylori-negative, all patients underwent ESD resection, and all patients survived during the follow-up period of 13±9 months. Eleven patients had postoperative endoscopic follow-up records, and no recurrence was detected. Fifteen lesions were detected (2 were present in 1 patient). Twelve were located in the upper 1/3 of the stomach, 10 were ≤ 1 cm in diameter, 12 had a morphology of type 0-Ⅱa, 8 had visible discoloration changes, and 12 had visible vasodilation on the surface. Magnified endoscopy and narrow-band imaging indicated that 12 of the lesions had enlarged marginal crypt epithelium, without any obvious microvascular pattern abnormalities and no obvious borderline. After resection the pathological specimens were all without vascular infiltration, and there was no atrophy of the mucosa at the edge of the lesion. In immunohistochemistry analyses MUC-2 was negative in all cases. MUC5AC was negative in 11 cases, MUC-6 was positive in all cases, and Ki-67 was ≤ 5% in 12 cases. Conclusions:GA-FG is a newly identified type of gastric cancer with low malignancy and a good prognosis. Characteristic discoloration and surface dilated vessels are often evident endoscopically. Enlarged marginal crypt epithelium and no visible boundary lines are often apparent in magnification endoscopy and narrow band imaging.
4.Effect of Using Hydrogen Peroxide for Periodic Disinfection Combined With Continuous Disinfection to Control Contamination in Dental Unit Waterline

Objective To observe the effect of using hydrogen peroxide in periodic disinfection combining with continuous disinfection of dental unit waterlines and to provide references for the selection of waterway disinfection measures.Methods A total of 4 dental units in a hospital of stomatology were selected through convenience sampling.The dental unit waterlines received periodic disinfection once every 4 weeks in addition to continuous disinfection(When the dental units were not used for more than 3 days,an additional periodic disinfection would be performed.).Periodic disinfection referred to filling up the waterlines with a disinfectant solution(1.4%hydrogen peroxide)by using the waterline disinfection device that came with the dental unit,immersing for 24 hours,and then emptying out the disinfectant solution.Continuous disinfection referred to using hydrogen peroxide at a concentration of 0.014%as dental treatment water and using it to flush the waterlines for 2 minutes before any dental treatment in the morning and to flush the waterlines for 30 seconds after each dental treatment.The study lasted for 25 weeks,with periodic disinfection being performed for 7 times and continuous disinfection carried out for the rest of the dental treatment time.During the 25 weeks,water samples were collected from air/water syringes and high-speed handpieces.Then,the water samples were incubated and the bacterial concentration and the qualification rates were calculated accordingly.When the bacterial concentration≤100 CFU/mL,the water samples were considered to be qualified.Waterline tubes of 1 cm were collected before and after the 25 weeks of disinfection with hydrogen peroxide.Biofilms in the waterline tube were observed under scanning electron microscope.Results A total of 352 water samples were collected.Eight water samples were collected before disinfection with hydrogen peroxide,with the median of bacterial concentration being 3140 CFU/mL.On the first day of disinfection with hydrogen peroxide,the median bacterial concentration in dental treatment water was 7.5 CFU/mL.There was a significant difference between the bacterial concentration of the water samples before the disinfection and that after the disinfection(P=0.012).A total of 344 water samples were collected after the disinfection,with the median bacterial concentrations for air/water syringes and high-speed handpieces being 11 CFU/mL and 11CFU/mL and the qualified rates being 83.7%and 82.0%,respectively.There was no significant difference in bacterial concentration or the qualification rates.During week 1 through week 9 of the disinfection,the qualification rates of the dental treatment water always exceeded 80%in 8 weeks,with week 3 being the exception.In the two four-week disinfection periods of week 14 through week 17 and week 18 through week 21,the qualification rate was maintained at above 80%for only the first two weeks and started to decrease from the third week.Biofilm morphology was observed under scanning electron microscope.Before the disinfection,the biofilm was found to be a dense structure and the mixture of a large number of bacteria.After 25 weeks of the disinfection,the biofilm structure appeared to be loose and did not show consistent characteristics of a large number of bacteria retained.Conclusion Periodic disinfection combined with continuous disinfection using hydrogen peroxide can effectively control contamination in dental unit waterlines.But the cycles of periodic disinfection and the concentration of hydrogen peroxide for continuous disinfection should be further discussed according to the actual clinical situation.
5.The construction of clinical skills training and assessment system based on whole process and staged type

To meet the requirements of training objectives for medical students , The national Experimental Teaching Demonstration Center for Clinical Skill (Xi'an Jiaotong University) has established three training platforms for "basic clinical skills", "clinical specialist skills" and "clinical comprehensive skills", and constructed a "whole-process and staged" clinical skills training and assessment system lasting from entering clinical study to graduation, in which clinical courses as the carrier, the clinical skill center as the main training base and containing clinical novitiate, internships and pre-graduation stage. This system integrates clinical skills training and evaluation into the whole process of clinical medicine teaching. More than two years of practice shows that the system has effectively improved the clinical practice ability of medical students, and the quality of medical education, and helped the school achieve the expected teaching objectives.

7.Expression and intratumoral heterogeneity of LN-5γ2 in esophageal squamous cell carcinoma

Objective The present study aimed to evaluate the expression and intratumoral heterogeneity of LN?5γ2 in esophageal squamous cell carcinoma ( ESCC) . Methods The expression of LN?5γ2 protein was examined in 135 ESCC cases by immunohistochemistry, and to analyze its relationship with the clinical relevance of patients. The protein expressions in different regions in the same tumor as well as different nests in the same region were compared. Results Moderate and high expression of LN?5γ2 protein was detected in 40.0% (54/135) of tumor tissues. Positive immunohistochemical staining was observed in 31.1% (23/74) of early stage (stages Ⅰ/Ⅱ) cases and 50.8% (31/61) of late stage (stage Ⅲ) cases, with a significant difference between these two groups (P=0.023). There was no statistical association of LN?5γ2 expression with age, sex of patients, PT sage, lymph node metastasis and degree of tumor differentiation ( P>0.05) . However, differential expression of LN?5γ2 protein was found at different sampling sites in the same tumor and the same sampling site in different carcinomas. Conclusion High expression of LN?5γ2 is positively correlated with tumor clinical stages and there existed intratumoral heterogeneity of LN?5γ2 expression in ESCC tissues.

10.Protoplast mutagenesis for improving beta-glucosidase production of Aspergillus niger.

The aims of this research were to isolate a Aspergillus niger strain with higher beta-glucosidase activity. We utilized the beta-glucosidase producing strain Aspergillus niger CGMCC 3.316 as the original strain to first obtain a mutant 3-3M through ultraviolet irradiation. Then we studied the conditions of protoplast release and regeneration for strain 3-3M. We treated the protoplasts of strain 3-3M via ultraviolet irradiation and obtained another isolated mutant 60B-3D. The strain 60B-3D showed much higher beta-glucosidase production than the original strain and 3-3M strain. The beta-glucosidase activity of strain 60B-3D was 23.4 IU/mL, with an improvement of 39% compared with the original strain, and 23% compared with strain 3-3M. We also studied the fermentation process of strain 60B-3D, and compared it with the original strain and strain 3-3M. We found the strain 60B-3D exhibited an improvement in xylanase production. The comparison results also showed that the strain 60B-3D secreted more protein. These results were beneficial for producing beta-glucosidase through this productive mutant.
