1.Regional distribution of depressive symptoms in middle-aged and portioulate matter elderly Chinese population and its ecological association with particulate matter 25

Objective:Against the backdrop of population aging and air pollution,this study examined the re-gional distribution of depressive symptoms among middle-aged and elderly people in China and explores the influ-ence of patrticulate matter 2.5(PM2.5)concentration on these symptoms.This study examined the regional distri-bution of depressive symptoms in middle-aged and elderly people in China and explores the influence of PM2.5 concentration and related factors on these symptoms.Methods:Middle-aged and elderly people aged 45 years and above in CHARLS database in 2013,2015 and 2018 were selected as research objects,and the Center for Epidemio-logic Studies Depression Scale(CESD-10)to obtain the detection rate of depressive symptoms.The PM2.5 concen-tration data were derived from the annual data of the China Environmental Monitoring Station.t-test,x2 test and DID model were used to analyze the related factors of depressive symptoms.Stata17.0 and Arc GIS10.8 were used for spatial autocorrelation analysis to explore the regional distribution and relationship of depressive symptoms in mid-dle-aged and elderly people in China.Results:From east to west,the detection rate of depressive symptoms in mid-dle-aged and elderly people in China showed an increasing trend.The regional distribution of PM2.5 mainly showed that the central region was higher than the northern and southern regions and the western region(except Xinjiang)was higher than the eastern region.Conclusion:Both the PM2.5 concentration and depression symptoms exhibit spatial clustering across China.PM2.5 has a negative impact on depression symptoms in middle-aged and elderly people,with other related factors including gender,age,education level,marital status,living area,smoking status.
2.Clinical observation on the treatment of chemotherapy-induced peripheral neuropathic pain with Tianchan capsule

Objective:To observe the efficacy and safety of Tianchan capsule on chemotherapy-induced peripheral neuropathic pain (CIPNP) .Methods:A total of 120 CIPNP patients with Karnofsky performance status (KPS) score higher than 60 admitted to the Nantong Tumor Hospital, Jiangsu Province and Suzhou Hospital of Traditional Chinese Medicine from April 2023 to April 2024 were selected as study objects. Patients were divided into control group (cisplatin/paclitaxel) and observation group (cisplatin/paclitaxel+Tianchan capsule) according to random number table method, with 60 cases in each group. After 28 days of treatment, the clinical efficacy, adverse reactions, numberical rating scale (NRS) score, quality of life score, Traditional Chinese Medicine (TCM) syndrome score and neurotoxicity grading score before and after treatment were observed and compared between the two groups.Results:Twenty-eight days after treatment, the total effective rate of the control group was 0 (0/60) , while the total effective rate of the observation group was 83.33% (50/60) , with a statistically significant difference ( χ2=85.71, P<0.001) . The NRS scores of the control group before treatment and at 7, 14, and 28 days after treatment were 6.18±1.71, 6.17±1.72, 6.20±1.70 and 6.22±1.70, respectively, with no statistically significant difference ( F=-1.43, P=0.160) . The NRS scores of the observation group before treatment and at 7, 14, and 28 days after treatment were 6.05±1.76, 5.17±1.42, 3.76±1.16, and 2.00±0.80, respectively, with a statistically significant difference ( F=22.89, P<0.001) . The NRS scores of the observation group at 7, 14, and 28 days after treatment were all lower than those of the control group, with statistically significant differences (all P<0.001) . Further pairwise comparison showed that, NRS scores in the observation group gradually decreased from before treatment to after 7, 14 and 28 days of treatment (all P<0.05) . Before treatment and 7, 14, and 28 days after treatment in the control group, the KPS scores were 67.33±6.43, 67.25±6.29, 67.08±6.14 and 66.75±5.80, respectively, with no statistically significant difference ( F=2.18, P=0.340) . Before treatment and 7, 14, and 28 days after treatment in the observation group, the KPS scores were 67.17±6.49, 68.33±6.32, 71.25±7.68 and 79.42±5.30, respectively, with a statistically significant difference ( F=-19.33, P<0.001) . The KPS scores of the observation group at 14 and 28 days after treatment were both higher than those of the control group, with statistically significant differences ( t=-3.33, P<0.001; t=-12.66, P<0.001) . Further pairwise comparison showed that, the KPS scores of the observation group were higher at 28 days after treatment compared to pre-treatment levels, as well as to those at 7 and 14 days after treatment (all P<0.05) . The scores of TCM syndrome showed that the scores of burnout and fatigue were 3.03±0.66 before treatment and 3.03±0.66 after 28 days of treatment in the control group, respectively, with no statistically significant difference ( t<0.01, P>0.999) . In the observation group, the scores were 3.02±0.60 and 1.97±0.52, respectively, with a statistically significant difference ( t=21.00, P<0.001) . The scores of hypochondriac pain were 3.05±0.68 before treatment and 3.07±0.63 after 28 days of treatment in the control group, respectively, with no statistically significant difference ( t=-0.57, P=0.568) . In the observation group, the scores were 3.03±0.66 and 2.02±0.57, respectively, with a statistically significant difference ( t=18.25, P<0.001) . The scores of pale complexion were 2.87±0.50 and 2.85±0.48 in the control group before treatment and 28 days after treatment, respectively, with no statistically significant difference ( t=-0.57, P=0.568) . In the observation group, the scores were 2.93±0.55 and 1.93±0.55, respectively, with a statistically significant difference ( t=24.29, P<0.001) . The scores of loss of appetite were 2.90±0.60 and 2.90±0.63 in the control group before treatment and after 28 days of treatment, respectively, with no statistically significant difference ( t<0.01, P>0.999) . In the observation group, the scores were 2.95±0.57 and 1.98±0.60, respectively, with a statistically significant difference ( t=20.42, P<0.001) . After 28 days of treatment, the scores of burnout and fatigue, hypochondriac pain, pale complexion and loss of appetite in the observation group were lower than those in the control group (all P<0.001) . Comparison of neurotoxicity grading showed that the scores of peripheral sensory nerve disorder were 2.54±0.50 before treatment and 2.58±0.49 after 28 days of treatment in the control group, respectively, with no statistically significant difference ( t=-0.40, P=0.690) . The scores in the observation group were 2.52±0.50 and 2.00±0.00, respectively, with a statistically significant difference ( t=7.29, P<0.001) . The scores of peripheral motor nerve disorder were 2.44±0.51 before treatment and 2.36±0.48 after 28 days of treatment, respectively, with no statistically significant difference ( t=0.81, P=0.419) . In the observation group, the scores were 2.46±0.50 and 2.00±0.10, respectively, with a statistically significant difference ( t=6.49, P<0.001) . Neuralgia score: Before treatment and after 28 days of treatment, the scores in the control group were 2.14±0.49 and 2.18±0.48, respectively, with no statistically significant difference ( t=-0.41, P=0.683) . The scores in the observation group were 2.16±0.51 and 1.72±0.46, respectively, with a statistically significant difference ( t=4.56, P<0.001) . After 28 days of treatment, the scores of peripheral sensory nerve disorder, peripheral motor nerve disorder and neuralgia in the observation group were lower than those in the control group (all P<0.001) . Patients in both control group and observation group had different degrees of diarrhea, nausea and vomiting, but there was no statistically significant difference in total incidence of adverse reactions between the two groups ( χ2=0.22, P=0.637) . Conclusions:Tianchan capsule can effectively reduce the pain degree of patients with CIPNP, improve the quality of life of patients and has good safety.
3.Effects of myocardial extracellular matrix remodeling on connexin 43 and its Ser368 phosphorylation and electrical conduction

BACKGROUND:Our previous studies found that decreased expression of connexin 43 and its Ser368 phosphorylation after myocardial hypothermic ischemia-reperfusion was closely associated with decreased cardiac conduction velocity and reperfusion arrhythmia.OBJECTIVE:To observe the effect of changes in membrane-type matrix metalloproteinase 2,matrix metalloproteinase 2 and collagen type Ⅳ on the expression of connexin 43 and its Ser368 phosphorylation and electrical conduction in the myocardial extracellular matrix after hypothermic ischemia-reperfusion.METHODS:Sixteen Langendorff extracorporeal cardiac perfusion models were successfully established from SD rats and randomly divided into a control group(n=8)and a hypothermic ischemia-reperfusion group(n=8).The control group was balanced perfused with 37 ℃ Krebs-Henseleit solution for 15 minutes and then continued to be perfused with 37 ℃ Krebs-Henseleit solution for 90 minutes.The hypothermic ischemia-reperfusion group was balanced perfused with 37 ℃ Krebs-Henseleit solution for 15 minutes,and then the heart was arrested for 60 minutes by injection of 4 ℃ Thomas solution.During the cardiac arrest,the periphery was protected by 4 ℃ Krebs-Henseleit solution.Half-volume 4 ℃ Thomas solution was reperfused 30 minutes after the arrest.After stopping the arrest,the heart was reperfused with 37 ℃ Krebs-Henseleit solution for 30 minutes.The occurrence of arrhythmias,rebeating time,and the duration of arrhythmias were recorded from the immediate time point to the end of the reperfusion period.Conduction velocity,absolute inhomogeneity,and inhomogeneity index were measured using the Mapping Lab multi-channel electrophysiological mapping system at the time of balanced perfusion for 15 minutes(T1),reperfusion for 15 minutes/continuous perfusion for 90 minutes(T2),and reperfusion for 30 minutes/continuous perfusion for 105 minutes(T3).The relative expression levels of membrane-type matrix metalloproteinase 2,matrix metalloproteinase 2,collagen type Ⅳ,connexin 43,and its Ser368 phosphorylation in ventricular tissue were detected by western blot assay.RESULTS AND CONCLUSION:(1)No arrhythmia occurred in the control group.There were six cases of arrhythmia in the hypothermic ischemia-reperfusion group during reperfusion.Rebeating time and duration of arrhythmias were(25.38+12.02)and(158.67±67.68)seconds,respectively.(2)The conduction sochronal diagrams at T1,T2,and T3 in the control group were uniform and regular in direction,and the conduction velocity at T2 and T3 was not different from that at T1(P>0.05).The conduction isochronal diagrams at T2 and T3 in the hypothermic ischemia-reperfusion group were uneven and irregular in direction,and the conduction velocity was slower than that at T1(P<0.01).The conduction velocity at T2 and T3 in the hypothermic ischemia-reperfusion group was slower than that in the control group(P<0.01).Conduction dispersion was greater in the hypothermic ischemia-reperfusion group than that in the control group at T2 and T3(P<0.05).(3)Compared with the control group,the protein expressions of membrane-type matrix metalloproteinase 2 and matrix metalloproteinase 2 in the hypothermic ischemia-reperfusion group were increased(P<0.05 or P<0.01),and the protein expression levels of type Ⅳ collagen,connexin 43 and its Ser368 phosphorylation were decreased(P<0.05 or P<0.01).(4)The results indicate that after hypothermic ischemia-reperfusion,myocardial extracellular matrix remodeling may mediate the downregulation of myocardial connexin 43 and its Ser368 phosphorylation,slowed conduction velocity and increased conduction dispersion,thereby increasing the risk of arrhythmia.
4.Establishment of animal models of rat abdominal wall with implantation of polypropylene mesh and mersi-lene tape and comparative study on their biomechanical properties and histocompatibility

Objective To establish animal models by implanting Gynemesh polypropylene mesh and Mersilene tape into the abdominal walls of rats,followed by conducting mechanical experiments and performing HE staining on abdominal wall tissues at 30 and 90 days post-implantation,respectively,in order to evaluate the biomechanical properties and histocompatibility of the two types of meshes.Methods The Gynemesh mesh and Mersilene tape were implanted into the abdominal wall of adult female rats(n=10)using W6977M polyester non-absorbable sutures and V-Loc absorbable sutures.The rats were randomly assigned to either a 30-day group or a 90-day group(n=5 per group)based on different experimental time points.Mechanical tests were conducted at these time points to evaluate the ultimate load required for avulsion of the meshes from the abdominal wall.Following the mechanical experiments,the tissues surrounding the meshes were harvested for hematoxylin and eosin(HE)staining.The inflammatory response,neovascularization,and fibroblast proliferation in the tissues were scored to compare the histocompatibility of the two types of meshes.Results(1)In the 30-day group,the ultimate load values were as follows:Gynemesh+6977(14.96±2.22)N,Gynemesh+V-Loc(12.73±1.11)N,Mersilene+6977(10.65±0.91)N,and Mersilene+V-Loc(8.70±1.18)N.No statistically significant difference was observed in the ultimate load between the Gynemesh+6977 and Gynemesh+V-Loc groups(P=0.12),whereas statistically significant differences were noted among the other groups(P<0.05).(2)In the 90-day group,the ultimate load values were as follows:Gynemesh+6977(18.97±0.59)N,Gynemesh+V-Loc(18.18±0.54)N,Mersilene+6977(13.87±0.67)N,and Mersilene+V-Loc(10.41±0.73)N.No statistically significant difference was observed in the ultimate load between the Gynemesh+6977 and Gynemesh+V-Loc groups(P=0.06),while statistically significant differences were noted among the other groups(P<0.05).(3)The ultimate load at 90 days for each group was significantly greater than that at 30 days,with statistically significant differences observed across all groups(P<0.05).(4)In the 30-day group,Gynemesh exhibited a lower inflammatory response compared to Mersilene tape(2.0±0.69 vs.3.10±0.71,P<0.05),with no statistically significant differences in neovascular-ization or fibroblast proliferation(2.37±0.61 vs.2.40±0.62,P=0.84;2.43±0.73 vs.2.63±0.67,P=0.27).In the 90-day group,Gynemesh demonstrated a lower inflammatory response score(1.10±0.66 vs.2.00±0.74,P<0.05),reduced fibroblast proliferation(2.87±0.68 vs.3.27±0.67,P<0.05),and no significant difference in neovascular proliferation(2.20±0.55 vs.2.13±0.68,P=0.68)compared to Mersilene tape.(5)The inflam-matory response for both mesh types was higher in the 30-day group compared to the 90-day group(Gynemesh group:2.0±0.69 vs.1.10±0.66,P<0.05;Mersilene group:3.13±0.73 vs.2.0±0.74,P<0.05).Additionally,the degree of fibroblast proliferation was lower in the 30-day group than in the 90-day group(Gynemesh group:2.43±0.73 vs.2.87±0.68,P<0.05;Mersilene group:2.63±0.67 vs.3.27±0.69,P<0.05).However,there was no statistically significant difference in neovascularization proliferation between the two groups(Gynemesh group:2.53±0.74 vs.2.47±0.74,P=0.81;Mersilene group:2.40±0.62 vs.2.13±0.68,P=0.12).Conclusion Compared with Mersilene tape,Gynemesh polypropylene mesh exhibits superior tensile strength and enhanced biocompatibility.

BACKGROUND:Our previous studies found that decreased expression of connexin 43 and its Ser368 phosphorylation after myocardial hypothermic ischemia-reperfusion was closely associated with decreased cardiac conduction velocity and reperfusion arrhythmia.OBJECTIVE:To observe the effect of changes in membrane-type matrix metalloproteinase 2,matrix metalloproteinase 2 and collagen type Ⅳ on the expression of connexin 43 and its Ser368 phosphorylation and electrical conduction in the myocardial extracellular matrix after hypothermic ischemia-reperfusion.METHODS:Sixteen Langendorff extracorporeal cardiac perfusion models were successfully established from SD rats and randomly divided into a control group(n=8)and a hypothermic ischemia-reperfusion group(n=8).The control group was balanced perfused with 37 ℃ Krebs-Henseleit solution for 15 minutes and then continued to be perfused with 37 ℃ Krebs-Henseleit solution for 90 minutes.The hypothermic ischemia-reperfusion group was balanced perfused with 37 ℃ Krebs-Henseleit solution for 15 minutes,and then the heart was arrested for 60 minutes by injection of 4 ℃ Thomas solution.During the cardiac arrest,the periphery was protected by 4 ℃ Krebs-Henseleit solution.Half-volume 4 ℃ Thomas solution was reperfused 30 minutes after the arrest.After stopping the arrest,the heart was reperfused with 37 ℃ Krebs-Henseleit solution for 30 minutes.The occurrence of arrhythmias,rebeating time,and the duration of arrhythmias were recorded from the immediate time point to the end of the reperfusion period.Conduction velocity,absolute inhomogeneity,and inhomogeneity index were measured using the Mapping Lab multi-channel electrophysiological mapping system at the time of balanced perfusion for 15 minutes(T1),reperfusion for 15 minutes/continuous perfusion for 90 minutes(T2),and reperfusion for 30 minutes/continuous perfusion for 105 minutes(T3).The relative expression levels of membrane-type matrix metalloproteinase 2,matrix metalloproteinase 2,collagen type Ⅳ,connexin 43,and its Ser368 phosphorylation in ventricular tissue were detected by western blot assay.RESULTS AND CONCLUSION:(1)No arrhythmia occurred in the control group.There were six cases of arrhythmia in the hypothermic ischemia-reperfusion group during reperfusion.Rebeating time and duration of arrhythmias were(25.38+12.02)and(158.67±67.68)seconds,respectively.(2)The conduction sochronal diagrams at T1,T2,and T3 in the control group were uniform and regular in direction,and the conduction velocity at T2 and T3 was not different from that at T1(P>0.05).The conduction isochronal diagrams at T2 and T3 in the hypothermic ischemia-reperfusion group were uneven and irregular in direction,and the conduction velocity was slower than that at T1(P<0.01).The conduction velocity at T2 and T3 in the hypothermic ischemia-reperfusion group was slower than that in the control group(P<0.01).Conduction dispersion was greater in the hypothermic ischemia-reperfusion group than that in the control group at T2 and T3(P<0.05).(3)Compared with the control group,the protein expressions of membrane-type matrix metalloproteinase 2 and matrix metalloproteinase 2 in the hypothermic ischemia-reperfusion group were increased(P<0.05 or P<0.01),and the protein expression levels of type Ⅳ collagen,connexin 43 and its Ser368 phosphorylation were decreased(P<0.05 or P<0.01).(4)The results indicate that after hypothermic ischemia-reperfusion,myocardial extracellular matrix remodeling may mediate the downregulation of myocardial connexin 43 and its Ser368 phosphorylation,slowed conduction velocity and increased conduction dispersion,thereby increasing the risk of arrhythmia.
8.Relationship between parental mediation and adolescents'tendency of online game addiction

Objective:To explore the relationship between parental mediation and adolescents'tendency of online game addiction,and the effect of self-control in it.Methods:Totally 553 senior high school students were tested with the Chinese version of Parental Mediation of Online Activities Questionnaire,Internet Gaming Disorder Scale-Short-form(IGDS9-SF)and Dual-mode of Self-control Scale for Adolescents(DMSC-S).Parental active me-diation,restrictive mediation,Internet game addictive tendency,adolescent self-control impulsive system,control system were evaluated respectively.SPSS macro program PROCESS was used to test the mediating role.Results:The scores of restrictive mediation of parents were negatively associated with the scores of adolescents'tendency of online game addiction(β=-0.14,P<0.001).Self-control scores played an independent mediating role between parental active mediation scores and online game addiction tendency scores(indirect effect=-0.09,95%CI=-0.09--0.01)and a partial mediating role between parental restrictive mediation scores and online game addiction tendency scores(indirect effect=-0.04,95%CI=0.00-0.07).Conclusion:Parental active and restrictive medi-ation are related to adolescents'tendency of online game addiction.Self-control plays a partial mediating role.
9.Clinical and imaging analyses of primary mediastinal yolk sac tumor

Objective·To summarize the clinical features,imaging features,and diagnosis and treatment experience of primary mediastinal yolk sac tumor(YST).Methods·Data of 29 patients with primary mediastinal YST,who attended Shanghai Chest Hospital,Shanghai Jiao Tong University School of Medicine from September 2016 to May 2023,were collected and comprehensively analyzed,including imaging examination results,serum indicators,pathology reports and treatment methods.Results·There were 22 cases of pure YST and 7 cases of mixed YST comprising 28 males and 1 female.The mean age of onset was(24.5±5.9)years.The initial symptoms were chest tightness(34.5%),chest pain(27.8%),cough(34.5%),expectoration(34.5%)and no specific symptoms(24.1%).Chest computerized tomography(CT)enhancement showed that all the 29 lesions were located in the anterior mediastinum.The maximum diameter of the lesions ranged from 5.6 cm to 18.2 cm.The lesions were irregular in shape,uneven in density,partially cystic and solid in density.The enhancement scan showed the solid part was slightly and moderately enhanced,and the low-density area was not enhanced.Tumor boundary was not clear because tumors often compressed and invaded surrounding tissues.Among the 29 newly diagnosed patients,serum alpha-fetoprotein(AFP)was significantly increased in 28 cases(1 case was not tested).Patients received multidisciplinary comprehensive treatment,including chemotherapy(25/29),surgery(26/29),and radiotherapy(8/29).Seven patients directly received surgery after diagnosis.Nineteen patients received chemotherapy first and then surgery;16(84.2%)cases were evaluated as lesion shrinkage after chemotherapy.After surgery,73.1%(19/26)patients had a significant decrease in serum AFP.After chemotherapy,56.0%(14/25)patients had decreased serum AFP.Conclusion·Primary mediastinal YST usually occurs in middle-aged and young men,with certain clinical and radiographic features and elevated serum AFP,which requires multidisciplinary comprehensive treatment.
10.Ultrasound-guided argon-helium knife cryoablation for the treatment of advanced non-small cell lung cancer with supraclavicular lymph node metastases:a clinical observation

Objective To investigate the short-term efficacy of ultrasound-guided argon-helium knife cryoablation for the treatment of advanced non-small cell lung cancer with supraclavicular lymph node metastases.Methods A total of 14 patients with advanced non-small cell lung cancer complicated by supraclavicular lymph node metastases were treated with ultrasound-guided argon-helium knife cryoablation on the basis of intravenous chemotherapy or targeted therapy.The short-term therapeutic effect and adverse reactions were analyzed.Results All the 14 patients were successfully treated with argon-helium knife cryoablation under ultrasound guidance.The overall effective rate was 78.6%,and the local compression symptoms of the patients were improved.The scores evaluated by the QLQ-C30 scale were improved when compared with preoperative values.The postoperative fever and local pain of puncture site,which were the common accompanying symptoms of argon-helium knife cryoablation,were relieved or disappeared within a few days after symptomatic treatment.Conclusion Ultrasound-guided argon-helium knife cryoablation is an effective method for local palliative treatment of advanced non-small cell lung cancer with supraclavicular lymph node metastases,which can monitor the real-time changes of lesion and evaluate the therapeutic effect in the treatment course.The clinical symptoms can be relieved within a short time after treatment,and the quality of life of patients will also be improved.
