1.Risk Factor Evaluation for Venous Thromboembolism during the Perioperative Period in Gynecology and Rationality Analysis of Anticoagulant Drug Use

Objective To evaluate the high-risk factors for venous thromboembolism(venous throm-boembolism,VTE)during the perioperative period in gynecology and analyze the rationality of anticoagulant drug use.Methods Clinical data of 183 gynecological surgical patients treated at Kunming Maternal and Child Health Hospital from January 2023 to December 2024 were collected.Among them,29 diagnosed with postoperative VTE were classified as the VTE group and 154 diagnosed with no postoperative VTE during the same period were classified as the non-thrombosis group.Independent sample t-test,Mann-Whitney U test,or χ2 test were used to analyze high-risk factors for VTE during the perioperative period.Logistic regression analysis was employed to identify risk factors for VTE after the gynecological surgery and a predictive regression model was established.The ROC curve was plotted to evaluate the model's validity.Results Elevated D-dimer levels,open abdominal surgery,and history of thrombosis were independent risk factors for VTE in postoperative gynecological patients(OR>1,P<0.05).The AUC of the regression-based predictive model was higher than that of the Caprini and G-Caprini scores,indicating that this model had the good discriminatory ability its resolution was better than G-Caprini and Caprini risk assessment tools.The identified issues in VTE prophylaxis at the hospital included the insufficient mechanical prevention time limit and delayed initiation of anticoagulant therapy in patients at moderate risk of bleeding.Conclusion This predictive model has certain clinical value for broader application.For issues related to drug therapy such as delayed use of anticoagulant drugs,clinical pharmacists should collaborate with physicians to assess patients'bleeding and thrombosis risks,jointly develop precise anticoagulation regimens.
2.Immunomodulatory effect of umbilical cord mesenchymal stem cells on type 2 diabetes mellitus

BACKGROUND:Umbilical cord mesenchymal stem cells present immunomodulatory function and multidirectional differentiation potential.Umbilical cord mesenchymal stem cells can actively improve the chronic low-grade inflammation state in type 2 diabetes mellitus and which is important for preventing the development of type 2 diabetes mellitus progression.OBJECTIVE:To review the research progress of umbilical cord mesenchymal stem cells in the treatment of type 2 diabetes mellitus through immunomodulatory effect.METHODS:Using"umbilical cord mesenchymal stem cells,UC-MSCs,mesenchymal stem cells,MSCs,type 2 diabetes mellitus,T2DM,immunity,insulin resistance,inflammation"as Chinese and English search terms,articles were searched on CNKI,Web of Science and PubMed databases.Relevant reviews,research papers and theses were screened and 88 papers were collected for summarization based on the inclusion and exclusion criteria.RESULTS AND CONCLUSION:Umbilical cord mesenchymal stem cells can effectively delay the occurrence and development of type 2 diabetes mellitus by reducing inflammatory response,inducing macrophages to M2 polarization,increasing the sensitivity of target organs to insulin,improving insulin resistance,and alleviating the dysfunction of islet beta cells through immune regulation.However,before umbilical cord mesenchymal stem cell transplantation can become a routine treatment,large-scale basic and applied basic research is needed to determine the optimal treatment regimen and efficacy.

5.Determination of 9 components Simultaneously in Swertia chirayita by HPLC method

Objective:To establish a HPLC method for determinating 9 components simultaneously in Swertia chirayita. Methods:By useing water Sunfire C18 column (4.6 mm× 250 mm,5 μm); Gradient elution was carried out with methanol-0.05% phosphoric acid solution as mobile phase. Setting the column temperature at 30 ℃, the flow rate at 1.0 ml/min, and the detection wavelength at 254 nm.Results:9 components showed good linear relationship within the injection quality range. The recovery rates of wertiamarin, Gentiopicroside, Angelica glycosides,Mangiferin, Isolysine, Gentianoside, Diol glycoside, 8-hydroxy-1,3,5 trimethoxyketone, and Daisy leaf gentinone were 95.38%, 92.41%, 95.14%, 91.87%, 92.24%, 92.51%, 95.08%, 91.72%, 95.74% ( n=6). Conclusion:The method is simple, efficient, sensitive, accurate, economical and practical, with repeatability and stability. It could provide reference for the quality control and comprehensive utilization of Swertia chirayita.
6.Constructing an evaluation system for hierarchical management of clinical nurse in top three hospitals based on three-dimensional structure theory of medical quality

Objective:To provide a reference for the hierarchical management of clinical nurses in top three hospitals, the hierarchical management evaluation system was conducted and evaluated in the present study.Methods:On the context of three-dimensional structure theory of medical quality, the Delphi method was applied to conduct two rounds of correspondence consultation among 23 nursing experts, and then related indexes were established based on the evaluation results and recommendations.Results:An evaluation system for hierarchical management of clinical nurse in top three hospitals was finally constructed, which included 3 first-grade and 19 second-grade indexes. The effective recovery rate of the two rounds of expert questionnaires and authority Cr of two rounds of expert consultation were 100% and 0.915, respectively. The Kendall coefficients of concordance for significance and operability of the first-grade and second-grade indexes were 0.353/0.515 and 0.344/0.469, respectively ( P<0.01). Conclusions:The hierarchical management of clinical nurses in top three hospitals constructed based on three-dimensional structure theory has reliability and feasibility.
7.A grounded theory study of primary caregiver coping trajectories in patients with permanent enterostomy

Objective:To explore the coping trajectory of primary caregivers in patients with permanent enterostomy, and to provide theoretical basis for clinical intervention.Methods:A semi-structured interview was conducted among 20 primary caregivers of patients with permanent enterostomy hospitalized in Qilu Hospital of Shandong University from May to July 2021. The data were analyzed by three-level coding and continuous comparison.Results:The coping trajectory of primary caregivers of permanent enterostomy patients experienced confusion and fatigue period, coping exploration period, acceptance and coexistence period, transformation and transcendence period.Conclusions:Medical staff should give corresponding health education and timely formulate effective intervention measures according to different stages. At the same time, we should effectively integrate external resources to promote the construction of "Hospital-Family-Community" tripartite care model to relieve the care pressure of caregivers.

9.Application of enhanced recovery after surgery in perioperative care for intracerebral aneurysm

Objective? To explore the effects of enhanced recovery after surgery (ERAS) in perioperative care for intracerebral aneurysm. Methods? Totally 353 patients with intracerebral anterior circulation aneurysm who were treated with the craniotany clipping technique in a ClassⅢ Grade A hospital from November 2016 to November 2018 were selected. A total of 291 patients admitted from November 2016 to December 2017 were included in the control group which received conventional perioperative nursing care, while 62 patients admitted between January and November 2018 were included into the ERAS group which received ERAS nursing care. The length of hospital stay, Glasgow Outcome Scale (GOS) and incidence of complications were compared between the two groups. Results? The length of hospital stay and the GOS score of the ERAS group was (14.426±4.264) d and (4.019±0.533), respectively, while those of the control group were (15.931±6.814) d and (4.676±0.705), respectively (t'=2.117, 7.937; P<0.05). There was no statistical difference in the incidence rate of central nervous system infection (CNSI) and pulmonary infection between the two groups (P>0.05). However, the incidence rate of lower limb vein thrombosis of the ERAS group was 4.61% (1/62), while that of the control group was 9.32% (27/219) (χ2=6.185,P< 0.05). Conclusions? The ERAS perioperative care for intracerebral aneurysm helps to reduce the length of hospital stay, improve GOS scores and curtail the incidence rate of lower limb vein thrombosis in these patients.

Objective To study the clinical characteristics of outpatients with hand,foot and mouth disease (HFMD) caused by different serotypes of enteroviruses.Methods This was a prospective study.From February 2017 to March 2018,563 outpatients with HFMD were enrolled by systematic sampling in the Department of Infectious Diseases,Henan Children's Hospital.Throat swabs were collected to determine the serotypes via PCR.Demographic,clinical,and laboratory data were collected by standard questionnaire.All cases were followed up twice at 2 and 9 weeks after the initial outpatient visit through telephone interview.A total of 563 cases were enrolled and 555 (98.6%) cases were positive for human enteroviruses,including 338 (60.9%) males.Analyses were stratified by enterovirus serotypes,Chi square test or Fisher's exact test,Rank sum test was used for comparison among different groups.Results The age of 555 cases was 24.2 (16.4,41.3) months.Among them 44.0% (224 cases) were identified as coxsackievirus (CV)-A6,while 189 cases,35 cases,14 cases and 73 cases were identified as CV-A16,enterovirus (EV)-A71,CV-A10 and other serotypes,respectively.Fever (≥37.5 ℃C) was present in 51.4%(285/555) of laboratory confirmed cases.The proportions of fever in cases of CV-A6 (68.9%(168/244)) and CV-A10 (12/14) were significantly higher than those in cases of CV-A16 (31.7%(60/189),x2=57.344,14.313,both P=0.000),other serotypes (43.8%(32/73),x2=15.101 and 8.242,P=0.000 and 0.004) and EV-A71 (37.1%(13/35),x2=13.506 and 9.441,P=0.000 and 0.002) respectively.There was no significant difference between CV-A6 and CV-A10 in presentation of fever (x2=1.785,P=0.182).There were 359 cases (64.7%) with eruptions in mouth,hands,feet and buttocks.Cases infected with EV-A71 had the highest proportions (74.3%(26/35)) of rash emerging simultaneously in mouth,hands,feet,and buttocks.The proportion in cases of CV-A 16,CV-A6,CVA 10 and other serotype were 73.5% (139/189),61.9% (151/244),7/14 and 49.3% (36/73),respectively.The proportion of rash on other parts of body,such as face,limbs or torso in cases infected with CV-A6 (16.8% (41/244)) was the higherest and the proportion in cases of CV-A16,EV-A71,CV-A10 or other serotypes were 8.5%(16/189),5.7%(2/35),1/14,6.8%(5/73),respectively.None of these cases developed serious complications.Desquamation occurred in 45.5% (179/393) cases 7.5 (5.0,9.0) days after disease onset and 13.5% (53/393) cases showed onychomadesis 31.0 (18.0,33.5) days after disease onset.The proportion of desquamation and onychomadesis associated with CV-A6 (64.2% (95/148) and 31.8% (47/148)) was significantly higher than CV-A16 (31.8% (49/154) and 1.3% (2/154),x2=33.601 and 52.482,both P=0.000) and other serotypes (38.0%(19/50) and 6.0%(3/50),x2=10.236 and 12.988,P=0.001 and 0.000).Desquamation appeared more in cases of CV-A6 than in cases of CV-A10 (2/11,x2=9.386,P=0.002),with the proportion of onychomadesis higher in CV-A6 than in EV-A71 (3.3% (1/30),x2=11.088,P=0.001).Conclusion Clinical manifestation such as fever,rash emerging parts,desquamation and onychomadesis are different among outpatient HFMD cases infected with CV-A16,CV-A6,EV-A71,CV-A10 and other enteroviruses.
