1.Exosome derived from human adipose-derived mesenchymal stem cells prevented bone loss induced by estrogen deficiency.

OBJECTIVE:
To investigate the effect of human adipose-derived mesenchymal stem cells (hASCs) exosomes on osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs) extracted from osteoporotic mice, and to evaluate the effect of hASCs exosomes on preventing bone loss induced by estrogen deficiency.
METHODS:
hASCs exosomes were extracted by ultracentrifugation. The osteoporotic mice were established by bilateral ovariectomy (OVX). BMSCs were isolated from osteo-porotic mice and cultured for further analysis. In the experimental group, these BMSCs were exposed to an osteogenic induction medium supplemented with hASCs exosomes to evaluate their potential effects on osteogenesis. In contrast, the control group was treated with the same osteogenic induction medium, but without the addition of hASCs exosomes, to serve as a baseline comparison for the study. To comprehensively assess the osteogenic differentiation of BMSCs influenced by hASCs exosomes, alkaline phosphatase (ALP) staining, ALP activity quantitative analysis and quantitative reverse transcription polymerase chain reaction (qPCR) were performed. These evaluations provided critical insights into the role of hASCs exosomes in promoting osteoblast differentiation and bone formation in osteoporotic conditions. The fluorescence labeled hASCs exosomes were injected via the tail vein to observe the biodistribution of exosomes. Two weeks after OVX, the mice were divided into three groups: The experimental group consisted of estrogen-deficient mice receiving hASCs exosome injections; the negative control group consisted of estrogen-deficient mice receiving phosphate-buffered saline (PBS) injections; and the positive control group consisted of mice that underwent Sham surgery and received PBS injections.The injections were administered once every 3 days, for a total of 8 injections. Afterward, the femurs were collected from the mice, and micro-computed tomography (micro-CT) was performed to measure bone mineral density and conduct bone morphometric analysis.
RESULTS:
hASCs exosomes were successfully extracted using ultracentrifugation. After the induction by hASCs exosomes, ALP staining and ALP activity in the BMSCs extracted from osteoporotic mice were significantly enhanced, the expression of osteogenesis related genes in BMSCs were significantly up-regulated. More trabecular bone and higher bone mineral density were observed in estrogen-deficient mice injected with hASCs exosomes compared with estrogen-deficient mice injected with PBS, and there was no significant decrease in bone mineral density compared with the Sham operation group.
CONCLUSION
hASCs exosomes promoted the osteogenic differentiation of BMSCs extracted from osteoporotic mice. hASCs exosomes prevented bone loss induced by estrogen deficiency.

2.Role of different dose nicorandil in prevention of contrast-induced nephropathy in elderly CHD patients

Objective To study the role of different dose nicorandil in prevention of CIN in elderly CHD patients.Methods One hundred and twenty-one elderly CHD patients were divided into standard dose nicorandil treatment group (n=42),5 mg nicorandil treatment group (n=39) and 10 mg nicorandil treatment group (n=40).Their Scr levels,and eGFR were measured for 3 days before and after PCI.The urine KIM-1 and NGAL levels were measured before and 24 h after PCI.The incidence of CIN and adverse reactions were compared in 3 groups.Results The incidence of (IN and the eGFR ≥25% were significantly lower,the serum Scr level was significantly lower while the eGFR was significantly higher in 5 mg and 10 mg nicorandil treatment groups than in standard dose nicorandil treatment group on days 1 and 2 after PCI (P<0.05,P<0.01).The serum Scr level was significantly lower while the eGFR was significantly higher in 10 mg nicorandil treatment group than in 5 mg nicorandil treatment group on day 2 after PCI (P<0.05).The urine KIM-1 and NGAL level were significantly lower in 10 mg nicorandil treatment group than in standard dose nicorandil treatment group and 5 mg nicorandil treatment group at 24 h after PCI (2.77±0.33 μg/L vs 5.63±0.27 μg/L,4.82±0.32 μg/L,P<0.05;41.64±8.42 μg/L vs 66.51±10.72 μg/L,57.11±9.67 μg/L,P<0.05).Conclusion Different oral doses of nicorandil play a role in prevention of CIN and 10 mg nicorandil plays a greater role than standard dose nicorandil and 5 mg nicorandil in prevention of CIN in elderly CHD patients undergoing PCI.
3.Analysis of Antimicrobial Therapy Participated by Clinical Pharmacists for a Patient with MRSA Infection

Objective: To evaluate the clinical value of clinical pharmacists in clinical treatment team through participating in the treatment of one case of MRSA infection.Methods: According to the infection site, MRSA infection treatment principle and the characteristics of drug treatment, clinical pharmacists assisted physicians in optimizing the therapy plan and provided the pharmaceutical care.Results: Physicians adopted the clinical pharmacist's suggestions, and the symptoms of patient were improved with effectively reduced ADR.Conclusion: The participation of clinical pharmacists in the optimization of anti-infective therapy plan can improve efficacy and security.
4.Application of Deluxe-ps knee prostheses in simultaneous bilateral total knee arthroplasty

BACKGROUND:Deluxe-PS knee prosthesis is designed based on the anatomical characteristics of the Chinese peoples’ knee joint, especial y the morphological features of femoral condyle. The distance between median and lateral knee prosthesis femoral condyle is less than that of the imported prosthesis for 3.5 mm.
OBJECTIVE:To explore the short-term efficacy of Deluxe-ps knee prosthesis in simultaneous bilateral total knee arthroplasty.
METHODS:Fifteen knee osteoarthritis patients (30 knees) were included as the experimental group, and treated with simultaneous bilateral knee arthroplasty by Deluxe-ps knee prosthesis. Twenty patients (40 knees) in the control group were treated with simultaneous bilateral knee arthroplasty by PFCSigma knee prosthesis. The knee society score, hospital for special surgery knee score and knee joint range of motion were compared between two groups;the operation time and the intraoperative blood loss were compared between two groups.
RESULTS AND CONCLUSION:The patients in both groups were fol owed-up for 12 to 24 months, with an average of 16 months. Postoperative knee pain was relieved, and the joint function was recovered satisfactorily. The knee society score, hospital for special surgery knee score and knee joint range of motion in two groups were improved significantly after treatment (P<0.05). There were no significant differences in postoperative knee society score, hospital for special surgery knee score and knee joint range of motion between two groups (P>0.05), and there was no significant difference in intraoperative blood loss between two groups (P>0.05), while there was significant difference in operation time between two groups (P<0.05). The results showed that the short-term effect of Deluxe-ps knee prosthesis in simultaneous bilateral total knee arthroplasty was satisfied.
5.An analysis of relevant factors for depressive symptoms in residents aged 60 and over in Shimen Erlu Community of Shanghai

Objective It aims to investigate prevalence of depressive symptoms among the elderly in community and to understand their related social factors. Methods Four neighborhood committees were randomly selected with cluster sampling as survey sites. Enrolled residents at age of 60 to 89 years were evaluated with a general background inventory and a geriatric depression scale(GDS). All the data were input to a microcomputer and were analyzed for its relevant factors with SPSS13. 0 software by multivariate logistic regression model. Results Totally, 1731 elderly people in the community were interviewed and rated with the inventory and scale mentioned above, and depressive symptoms were found in 297 of them, with prevalence of 17. 2 percent. Multivariate logistic regression analysis showed that there were seven factors associated to their depressive symptoms in the elderly, including family expenditure, housing condition, medical condition, familial support, hobbies in spare-time, ability to take care of oneself and participation to social activities. Conclusions A few social factors, such as living alone, insufficient income for family expenditure, disability of self care-taking, lack of hobby in spare-time, less participation to social activities, lack of familial support and severe chronic diseases by serf-evaluation, are all associated with depressive symptoms in the elderly.
