1.Construction and application of a refined intelligent management platform for the supply chain of hospital preparations

OBJECTIVE To construct a refined intelligent management platform for the supply chain of hospital preparations and to evaluate its implementation effectiveness.METHODS Machine learning and classical time series model were used to predict the demand for hospital preparations. Based on the prediction results, an intelligent management platform was constructed around each link of the supply chain of hospital preparations, which included three functional modules: intelligent auxiliary decision-making, daily expiry date monitoring, and intelligent push and continuous improvement. By comparing indicators such as the mean of average inventory turnover days of each preparation, average monthly expired and scrapped amount, and average clinical satisfaction before and after the application of the platform in the First Affiliated Hospital of Soochow University, the application effectiveness was evaluated from three dimensions: cost control, operational efficiency, and pharmaceutical care service quality.RESULTS The constructed intelligent management platform could automatically generate dynamic inventory grading alerts, production plan suggestions, material procurement suggestions, and expiry date alerts, and intelligently push them to relevant responsible persons. Compared with before the application, in terms of cost control, the mean of average inventory turnover days of each preparation was shortened from 64.1 days to 48.4 days, and the average monthly expired and scrapped amount was reduced from 1 973 yuan to 1 369 yuan; in terms of operational efficiency, the average time for production plan formulation was shortened from 120 min to 60 min, and the average lag days of expiry date alert information was shortened from 15 days to 0.5 days; in terms of pharmaceutical care service quality, the average clinical satisfaction increased from 85.4% to 94.6%.CONCLUSIONS The refined intelligent management platform for the supply chain of hospital preparations has been successfully constructed, which can assist managers in decision-making regarding production plan formulation, procurement, and disposal of near-expiry preparations and materials, thereby achieving the purpose of cost reduction and efficiency improvement.
2.A case of febrile infection-related epilepsy syndrome in children successfully treated with tocilizumab and literature review

Objective:To report a case of tocilizumab successfully used in a child with febrile infection-associated epilepsy syndrome (FIRES), and to provide a new idea for the treatment of FIRES in children.Methods:The diagnosis and treatment of 1 case of FIRES admitted in Children′s Hospital Affiliated to Zhengzhou University on February 15, 2021 were described, and the prognosis and follow-up of the child were evaluated. At the same time, the literatures on tocilizumab in the treatment of children′s FIRES were reviewed.Results:A 5-year-old case of FIRES was reported. The child was extremely refractory to immunotherapy and anti-seizure medicines, anesthetics and ketogenic diet. So he was treated with tocilizumab (each time 4 mg/kg) at the 36th day and 43rd day, and epileptic seizures were controlled 10 days after the 2nd doses of tocilizumab. During a follow-up of 10 months, his epileptic seizures were controlled and the cognitive behavior and speech function were well recovered. At present, only 3 cases of FIRES in children have been reported all over the world. All the seizures were well controlled and no obvious adverse reactions were observed.Conclusions:FIRES is a rare refractory epilepsy syndrome, resistant to many kinds of anti-seizure medicines or even anesthetic agents, which is difficult to treat and has poor prognosis. Preliminary trials have shown that tocilizumab is effective and well tolerated in children with FIRES. It may be a potential therapeutic modality for children with FIRES.
3.Impact of WeChat-assisted follow-up on the self-administer medication capability of stroke survivors

Objective To explore the impact of WeChat-assisted follow-up on self-administer medication capability of stroke survivors.Methods Seventy patients were recruited from a tertiary hospital and randomly divided into intervention group (n=35) and control group (n=35).Patients in intervention group were given WeChat-assisted follow-up including health education program and behavior guidance,and patients in control group received regular discharge care.The effects of WeChat-assisted follow-up and regular care on self-administer medication ability,cognitive and behavior level were assessed.Results After three months' follow-up,the scores of self-administer medication capability (38 (32,42)),experience (25 (20,28) and the integral level (87 (71,92)) in intervention group were significantly higher than those in control group (capability 25(16,38),experience 21 (8.75,27),total score 68 (38,87)) (Z=-2.511,-2.033,-2.209,P<0.05).The self-administer medication experience,cognitive,capability and total scores increased significantly in intervention group after intervention(Z=-4.525,-4.610,-3.806,-4.718,all P< 0.01),while only the cognitive level increased slightly in control group (18 (10,24) vs 13 (11,18),Z =-1.794,P=0.073).Conclusions Follow-up intervention based on WeChat platform can improve the stroke patients' self-administration medication capability,however,the long-term effects on cognitive level and further reform need to be strengthened.
4.Study on Chemical Constituents from Chemical Split Fractions ofMori Cortex

This article was aimed to study the chemical constituents from the chemical split fractions of Mori Cortex. The compounds were isolated with Diaion HP-20, Toyopearl HW-40, Sephadex LH-20, MCI Gel CHP-20, Silica gel column chromatography and preparative HPLC. Structures of compounds were identified by physicochemical properties and spectral analysis. The results showed that 23 compounds were obtained. And their structures were identified. The 16 compounds were obtained from the 30% ethanol fraction as vanillic acid (1), 3,4-dimethoxyphenol (2), benzoic acid (3), syringic acid (4), kelampayoside A (5), p-hydroxyphenylpropionic acid (6), caffeic acid (7), hydroferulic acid (8), 6,7-dihydroxycoumarin (9), 5,7-dihydroxycoumarin (10), morin-7-O-β-D-glucopyranoside (11), liriodendrin (12), 2,3-trans-dihydromorin (13), 2,3-cis-dihydromorin (14), 2,3-trans-dihydroquercetin (15), 2,3-cis-dihydroquercetin (16). The 4 compounds were obtained from the 50% ethanol fraction as scopoletin (17), morin (18), kaempferol-7-O-β-D-glucopyranoside (19), umbelliferone (20). The 3 compounds were obtained from the 80% ethanol fraction as sanggenon R (21), cis-mulberroside A (22), resveratrol (23). It was concluded that compounds 2, 4-6, 11, 16, 19 were isolated from this plant for the first time.

This article was aimed to study the chemical constituents of seeds of Descurainia Sophia (L.) Webb. ex Prantl., in order to lay the material foundation for further interpretation of seeds of D. Sophia and provide pharmacodynamic basis as well as the basis for attributing its nature and taste. The compounds were isolated and purified by Diaion HP-20, Toyopearl HW-40, MCI Gel CHP-20, ODS, Silica gel chromatography combining with Pre-HPLC. The structures were identified on the basis of spectral data and physicochemical properties. Twenty eight compounds were isolated and identified from 20% and 80% ethanol fraction. Thirteen compounds were identified from 20% ethanol fraction: kaempferol-3-O-β-D-glucopyranosyl-7-O-β-D-gentiobioside(1), quercetin-3-O-β-D-glucopyranosyl-7-O-β-D-gentiobioside (2), isorhamnetin-3-O-β-D-glucopyranosyl-7-O-β-D-gentiobioside (3), isorhamnetin-3,7-di-O-β-D-glucopyranoside (4), quercetin-3,7-di-O-β-D-glucopyranoside (5), kaempferol-3, 7-di-O-β-D-glucopyranoside (6), kaempferol-3-O-β-D-xylopyranosyl (1 → 2)-β-D-glucopyranoside (7), methyl sinapate (8), syringaldehyde (9), (S)-p- hydroxyphenyl lactate acid (10), (S)-2-hydroxy-phenylpropionic acid (11), scopoletin (12), sinapic acid (13). Fifteen compounds were identified from 80% ethanol fraction: isorhamnetin-3-O-β-D-glucopyranoside (14), quercetin-3-O-β-D-glucopyranoside (15), kaempferol-3-O-β-D-glucopyranoside (16), quercetin (17), kaempferol (18), isorhamnetin (19), syringic acids (20), quercetin-3-O-β-D-arabinopyranoside (21), quercetin-3-O-β-D-xylopyranoside (22), 6-O-[E]-Sinapoyl-(α- and β)-D-glucopyranoside (23), dimethyl (E, E)-4,4'-dihydroxy-3,3',5,5'-tetramethoxylign-7,7'-dien-9,9'-dioate (24), dimethylthomasidioate (25), 2-hydroxy-3-(1H-indol-3-yl) propanoic acid (26), 2-hydroxyl-3-(1H-indol-3-yl) propanoic acid methyl ester (27), 4'-O-methyl-dihydroquercetin (28). It was concluded that compounds 7-11 and 21-28 were isolated from seeds of D. sophia (L.) Webb. ex Prantl. for the first time.
6.Effect of motivational interviewing health education on the self-efficacy of diabetes patients

Objective To evaluate the effect of health education through motivational interviewing on the self-efficacy of diabetes patients. Methods From January to June in 2013, 120 patients with type 2 diabetes who were extracted conveniently from our hospital were randomly divided into the experimental group ( n=60 ) and control group (n=60). Diabetes specialist nurses who had been trained in motivational interview were the interviewers. After the baseline evaluations for patients, motivational interview, the traditional health education and the telephone follow-up were used for the patients in the experimental group, but only traditional education and telephone follow-up for those in the control group. The self-efficacy and the Diabetes Management Self-Efficacy Scale was evaluated before intervention, after 3 months and 6 months intervention respectively. Results Before the intervention, the score of self-efficacy was not significantly different between the two groups (P>0. 05). After 3 and 6 months intervention, the total scores of self-efficacy in the experimental group were (20.13 ±3.78) and (24.93 ±4.38), which were significantly higher than those before the intervention (t=4. 397, 11. 716, respectively;P <0. 05). Compared with the scores before the intervention, the scores of the self-efficacy were significantly different after 3 and 6 months intervention (F = 81. 850,P < 0. 05). Conclusions Health education through motivational interviewing can improve the self-efficacy of patients with type 2 diabetes.
7.Therapeutic progress on the treatment of hepatitis B recurrence after liver transplantation

Hepatitis B recurrence is an important factor to show whether liver transplantation is successful or not. Recrrrence rate is nearly 80% in the patients liver transplantation, and HBV reinfection is the main cause of death. After the operation, HBV may copy very quickly, because of taking in immunosuppressant therapy and graft diseases develop rapidly. Recently, the exploitation and application of therapy has become a focus. Now the drugs of preventing and treating HBV reinfection are mainly sorted into passive immunity agents(Hepatitis B immunoglobulin) and antivirus therapeutic agents(interferon and nucleotide analog antivirus drugs, etc.). Lamivudine is considered the most promising new drug of treating HBV infection. This article introduced the therapeutic characteristic of these drugs.
