1.Efficacy and Safety of Blinatumomab in Adult Patients with B-Cell Acute Lymphoblastic Leukemia

Objective:To evaluate the efficacy and safety of blinatumomab in adult patients with relapsed/refractory(R/R)or measurable residual disease(MRD)positive B-cell acute lymphoblastic leukemia(B-ALL)in the real world.Methods:The clinical data of 30 B-ALL patients received at least 1 course of blinatumomab therapy in the Chinese PLA General Hospital from January 1st,2021 to December 31st,2023 were retrospectively analyzed,including pre-treatment baseline clinical feature,post-treatment complete response(CR),CR with partial hematologic recovery(CRh),CR with incomplete hematologic recovery(CRi),complete MRD response rate,MRD response rate(MRD<10-4),overall survival(OS),and disease-free survival(DFS),as well as drug-related adverse reactions.Results:Among 5 patients who were not assessed 4 were MRD negative and 1 did not receive bone marrow biopsy.In the R/R B-ALL group(13 cases),11 patients achieved CR/CRh/CRi and 10 patients achieved complete MRD response.In MRD+group(12 cases),9 patients achieved overall MRD response and 7 patients achieved complete MRD response.The median follow-up time was 8.4(95%CI:6.3-10.4)months.The median OS was 15.5(95%CI:0.7-30.3)months in the R/R group,while not reached in the MRD+group.The median DFS of the two groups were not reached.Drug-related adverse reactions occurred in 22 patients,and pyrexia was the most common(13 cases).Grade ≥3 adverse reactions occurred in 15 patients,and neutropenia was the most common(9 cases).Cytokine release syndrome occurred in 6 patients,including 5 cases with grade 1 and 1 case with grade 3.No patients interrupted therapy or died due to drug-related adverse reactions.Conclusion:Blinatumomab is effective in the treatment of R/R or continuous MRD+B-ALL with acceptable adverse reactions.
2.Epidemiological analysis of imported malaria in Yunnan Province,2020-2023

This study analyzed the epidemiological characteristics of imported malaria in Yunnan Province from 2020 to 2023,to provide scientific evidence for formulating measures to decrease imported malaria and prevent re-establishment of malaria transmission.Malaria data reported by the China Disease Prevention and Control Information System were analyzed to determine parasite species;sources of infection;temporal,spatial,and population distributions;and importation routes.A total of 828 malaria cases were reported in the province.Plasmodium vivax and Plasmodium falciparum accounted for 89.98%and 8.33%of cases,respectively.A total of 47.58%of cases were imported from Myanmar,and all P.falciparum malaria ca-ses were from Africa.Thirteen(81.25%)prefectures or municipalities reported malaria,among which Dehong,Baoshan,Kunming,and Lincang reported 94.32%of cases.A total of 52.54%of cases were in young men.The proportion of cross-bor-der personnel flow,land input,and aircraft input were 88.89%and 11.11%respectively.A total of 98.19%of patients sought medical care within 7 days after fever onset,and 82.85%initiated diagnosis for malaria,and 84.90%of diagnoses were con-firmed by health facilities at or below the county level.Imported malaria is a major challenge in preventing re-establishment of transmission in Yunnan.Most imported cases involved cross-border malaria transmission of mainly Plasmodium vivax between China and Myanmar.To achieve malaria elimination,vigilance of health staff in malaria diagnosis and treatment should be pro-moted,and intensive malaria health education should be provided to people traveling to malaria endemic territories,to enable individual protection,and timely diagnosis and treatment after return from endemic countries.

5.Effects of Xuefu Zhuyu Decoction Combined with Cyproheptanidine Hydrochloride Tablets on Intracranial Arterial Hemodynamics,Oxygen Free Radicals and Vasoactive Factors in Patients with Migraine Caused by Blood Stasis Blocking Meridians

Objective:To explore the effects of Xuefu Zhuyu Decoction combine with Cyproheptanidine Hydrochloride Tablets on intracranial arterial hemodynamics,oxygen free radicals and vasoactive factors in patients with migraine caused by blood stasis blocking meridians.Methods:This study was a single-center prospective randomized controlled study,90 patients with migraine caused by blood stasis blocking meridians who were admitted to Zaozhuang Municipal Hospital Medical Group Canal Hospital from October 2022 to July 2024 were divided into control group(treated with Cyproheptanidine Hydrochloride Tablets,n=45)and study group(treated with Xuefu Zhuyu Decoction combined with Cyproheptanidine Hydrochloride Tablets,n=45)according to the random number table method.The TCM syndrome scores,visual analog scale(VAS),intracranial arterial hemodynamics[anterior cerebral artery(ACA),vertebral artery(VA),middle cerebral artery(MCA),basilar artery(BA)],serum oxygen free radicals[superoxide dismutase(SOD),malondialdehyde(MDA)],serum vasoactive factors[endothelin-1(ET-1),nitric oxide(NO)]between the two groups were compared.Results:The TCM syndrome scores,VAS score,MDA and ET-1 levels in the study group after treatment were lower than those in the control group,while ACA,MCA,VA and BA hemodynamics,and SOD and NO levels were higher than those in the control group(P<0.05).Conclusion:Xuefu Zhuyu Decoction combined with Cyproheptanidine Hydrochloride Tablets in the treatment of patients with migraine caused by blood stasis blocking meridians,can effectively improve intracranial arterial hemodynamics,control serum oxygen free radicals and vasoactive factors,and prevent the progression of the disease.

OBJECTIVE:
To assess the safety and topical efficacy of prim-O-glucosylcimifugin (POG) and investigate the molecular mechanisms of its therapeutic effects in atopic dermatitis (AD).
METHODS:
The effects of POG on human keratinocyte cell viability and its anti-inflammatory properties were evaluated using cell counting kit-8 assay and reverse transcription-quantitative polymerase chain reaction (RT-qPCR). Subsequently, the impact of POG on the differentiation of cluster of differentiation (CD) 4+ T cell subsets, including T-helper type (Th) 1, Th2, Th17, and regulatory T (Treg), was examined through in vitro experiments. Network pharmacology analysis was used to elucidate POG's therapeutic mechanisms. Furthermore, the therapeutic potential of topically applied POG was further evaluated in a calcipotriol-induced mouse model of AD. The protein and transcript levels of inflammatory markers, including cytokines, lymphocyte-specific protein tyrosine kinase (Lck) mRNA, and LCK phosphorylation (p-LCK), were quantified using immunohistochemistry, RT-qPCR, and Western blot analysis.
RESULTS:
POG was able to suppress cell proliferation and downregulate the transcription of interleukin 4 (Il4) and Il13 mRNA. In vitro experiments indicated that POG significantly inhibited the differentiation of Th2 cells, whereas it exerted negligible influence on the differentiation of Th1, Th17 and Treg cells. Network pharmacology identified LCK as a key therapeutic target of POG. Moreover, the topical application of POG effectively alleviated skin lesions in the calcipotriol-induced AD mouse models without causing pathological changes in the liver, kidney or spleen tissues. POG significantly reduced the levels of Il4, Il5, Il13, and thymic stromal lymphopoietin (Tslp) mRNA in the AD mice. Concurrently, POG enhanced the expression of p-LCK protein and Lck mRNA.
CONCLUSION
Our research revealed that POG inhibits Th2 cell differentiation by promoting p-LCK protein expression and hence effectively alleviates AD-related skin inflammation. 10.Analysis of the whole genome characteristics of influenza A(H1N1)pdm09 subtype in Wuxi city, 2018-2023

Objective:To understand the evolution and variation characteristics of the H1N1 influenza virus in Wuxi City from 2018 to 2023.Methods:Real time PCR was used to perform nucleic acid testing on throat swab samples of influenza like cases sent to sentinel hospitals for testing. The influenza A (H1N1) pdm09 positive samples were subjected to cell culture, and nucleic acid was extracted from strains with a red blood cell agglutination test (HA) ≥1∶8. The whole genome sequence was amplified, and a library was constructed. The MiSeq sequencer was used for sequencing on the machine. Using NC_026431.1 as a reference strain, we analyzed the offline data using CLC Genomics Workbench (Version 23) software. MEGA 7.0 software was used to construct a phylogenetic tree, and NetNGlyc 1.0 Server software was used to predict N-glycosylation sites.Results:The nucleotide and amino acid homology between 38 strains of A (H1N1) pdm09 influenza virus from 2018 to 2023 were 96.06%-100% and 96.12%-100%, respectively. From February to May 2023, all 12 strains of A (H1N1) pdm09 had two amino acid mutation sites occurring in the HA antigenic determinant cluster, namely the Ca region (A203T) and the Cb region (K71Q). No mutations were found in the HA receptor binding site and NA resistance site. The strains from January to June 2018 belong to the 6B. 1A evolutionary branch, the strains from December 2018 to January 2020 belong to three evolutionary branches: 6B. 1A. 1, 6B. 1A. 5a, and 6B. 1A. 7, and the strains from February to May 2023 belong to the 6B. 1A. 5a. 2a evolutionary branch. 38 strains of A (H1N1) pdm09 HA gene all have 7 potential N-glycosylation sites, while NA gene has 7-8 potential N-glycosylation sites.Conclusions:There are characteristic amino acid mutation sites of H1N1 influenza A in Wuxi City from 2018 to 2023. The emergence of these mutation sites may affect the virus′s transmission and antigenic changes.
