This study analyzed the epidemiological characteristics of imported malaria in Yunnan Province from 2020 to 2023,to provide scientific evidence for formulating measures to decrease imported malaria and prevent re-establishment of malaria transmission.Malaria data reported by the China Disease Prevention and Control Information System were analyzed to determine parasite species;sources of infection;temporal, spatial, and population distributions;and importation routes.A total of 828 malaria cases were reported in the province.Plasmodium vivax and Plasmodium falciparum accounted for 89.98%and 8.33%of cases, respectively.A total of 47.58%of cases were imported from Myanmar, and all P.falciparum malaria ca-ses were from Africa.Thirteen(81.25%)prefectures or municipalities reported malaria, among which Dehong,Baoshan,Kunming, and Lincang reported 94.32%of cases.A total of 52.54%of cases were in young men.The proportion of cross-bor-der personnel flow, land input, and aircraft input were 88.89%and 11.11%respectively.A total of 98.19%of patients sought medical care within 7 days after fever onset, and 82.85%initiated diagnosis for malaria, and 84.90%of diagnoses were con-firmed by health facilities at or below the county level.Imported malaria is a major challenge in preventing re-establishment of transmission in Yunnan. Most imported cases involved cross-border malaria transmission of mainly Plasmodium vivax between China and Myanmar. To achieve malaria elimination, vigilance of health staff in malaria diagnosis and treatment should be pro-moted,and intensive malaria health education should be provided to people traveling to malaria endemic territories,to enable individual protection, and timely diagnosis and treatment after return from endemic countries.

Journal of Regional Anatomy and Operative Surgery 2025;34(11):1008-1011
With the development of echocardiographic technology, the evaluation of left atrial function has become increasingly crucial in predicting cardiovascular risks in minors. The conventional two-dimensional and Doppler ultrasound can assess the left atrial volume and diastolic function, while have limited accuracies. In recent years, the emerging ultrasound two-dimensional speckle tracking echocardiogra-phy(2D-STE)has made it possible to evaluate the three phases of left atrial function(reservoir, conduit, booster pump),which is more sensitive and suitable for high-risk minors with cardiomyopathy, obesity,or undergoing chemotherapy. This article reviews the progress of 2D-STE in evaluating left atrial function in minors, and discusses its potential and challenges in early clinical diagnosis and prognosis assessment, with the aim of providing a reference for clinical practice.

Objective:To investigate the early diagnostic value of contrast-enhanced ultrasound(CEUS)combined with serum creatinine(Scr)and urea nitrogen(BUN)in children with renal parenchymal injury caused by urinary tract infections(UTIs).Methods:A total of 120 children with UTIs admitted from January 2020 to January 2025 were selected and divided into a renal parenchymal injury group(n=65)and a non-injury group(n=55)based on the results of 99mTc-DMSA renal static imaging. All children underwent CEUS examination within 24 hours of admission to obtain hemodynamic parameters of the renal cortex and medulla(time-to-peak[TTP],peak intensity[PI],and area under the curve[AUC]),and Scr and BUN levels were measured.Multivariate logistic regression analysis was used to identify independent risk factors, and the diagnostic efficacy was evaluated using receiver operating characteristic(ROC)curves. Results:In the renal parenchymal injury group,Scr(0.82±0.25 vs 0.53±0.15 mg/dL),BUN(16.23±4.82 vs 12.42±3.27 mg/dL),and cortical TTP(51.47±12.38 vs 43.45±12.55 s)were significantly elevated, while cortical PI(32.63±6.37 vs 39.23±7.31 dB)and AUC(2915.36±536.37 vs 3415.35±681.5 dB·s)were significantly decreased(all P<0.05).Multivariate analysis showed that Scr(OR=6.17),BUN(OR=2.58),cortical TTP(OR=1.98),and PI(OR=0.60)were independent predictors. In the ROC curve analysis, the combination of CEUS parameters(cortical TTP,PI,AUC)with Scr and BUN demonstrated the best diagnostic efficacy(AUC=0.953,sensitivity 87.3%,specificity 90.3%).Conclusion:CEUS can sensitively identify renal parenchymal injury associated with UTIs by dynamically assessing abnormal renal cortical blood perfusion. When combined with Scr and BUN,it significantly improves diagnostic accuracy, providing an important basis for early intervention and renal function protection in children with UTIs.

Objective:To evaluate the efficacy and long-term outcomes of fludarabine, cyclophosphamide, and rituximab (FCR) in treatment-na?ve patients with chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) .Methods:Clinical data from 68 CLL/SLL patients treated with FCR at Jiangsu Province Hospital (August 2008–May 2021) were retrospectively analyzed to assess efficacy, safety, and survival outcomes.Results:Among 68 patients [46 males, 22 females; median age 55 (47, 60) years], 13.1% (8/61) had a complex karyotype, 32.3% (20/62) had immunoglobulin heavy variable region mutated (IGHV-M) type, 6.6% (4/61) had del (17p), and 14.8% (8/54) had del (11q). Patients received a median of 6 (4, 6) FCR cycles. The overall response rate was 88.2% (60/68), including 47.0% (32/68) complete remissions. Over a median follow-up of 82 (59, 98) months, 66.2% (45/68) experienced disease progression. Median progression-free survival was 56 (21, 123) months, while median overall survival was not reached. The 5- and 10-year PFS rates were 42.6% (95% CI: 31.9–56.8% ) and 28.7% (95% CI: 19.0–43.4% ), respectively. Poor PFS was associated with del (17p) ( HR=5.04, 95% CI: 1.72–14.74, P=0.003), del (11q) ( HR=5.27, 95% CI: 2.11–13.15, P<0.001), IGHV unmutated (IGHV-UM) ( HR=4.11, 95% CI: 1.72–9.79, P=0.001), complex karyotype (CK) ( HR=3.53, 95% CI: 1.58–7.85, P=0.002), β 2-microglobulin >3.5 mg/L ( HR=2.87, 95% CI: 1.37–6.01, P=0.005). In multivariate analysis, IGHV-UM remained an independent predictor of PFS ( HR=8.63, 95% CI: 1.09–68.40, P=0.042). Sixteen patients with IGHV-M and lacking del (17p) or CK had a median PFS of 123 (58,123) months and a 5-year PFS rate of 70.7% (95% CI: 49.7–99.1% ), reaching a plateau after 5 years with no recurrences by 10 years. Common grade 3–4 adverse events included hematologic toxicity (44.1%, 30/68), infection (36.7%, 25/68), and liver dysfunction (4.4%, 3/68). Among 25 patients receiving single-agent BTK inhibitors after FCR progression, median follow-up was 45 (26, 64) months; 36% (9/25) experienced disease progression, with a median PFS time of 55 (27, 55) months. Conclusion:First-line FCR provides durable long-term benefits for patients with IGHV-M CLL without del (17p) or CK.

Processing for deodorization is widely used in the production of animal-derived Chinese medicinal materials. In this study, Heracles Neo ultra-fast gas-phase electronic nose combined with chemometrics was employed to analyze the overall odor difference of Cervi Cornu Pantotrichum(focusing on that derived from Cervus nippon Temminck in this study) before and after processing. The results showed that the electronic nose effectively distinguished between the medicinal materials and decoction pieces of Cervi Cornu Pantotrichum. HS-GC-MS was used to identify and quantify the volatile components in the medicinal materials and decoction pieces of Cervi Cornu Pantotrichum, and 35 and 37 volatile components were detected in the medicinal materials and decoction pieces, respectively. The medicinal materials and decoction pieces contained 28 common volatile components contributing to the odor of Cervi Cornu Pantotrichum. The odor activity value(OAV) of each volatile component was calculated based on the olfactory threshold and relative content. The results showed that there were 17 key odor substances such as isovaleraldehyde, 2-methylbutanal, isobutyraldehyde, hexanal, and methanethiol in the medicinal materials and decoction pieces of Cervi Cornu Pantotrichum. All of them had bad odor and were the main source of the odor of Cervi Cornu Pantotrichum. The results of principal component analysis(PCA) and orthogonal partial least squares-discriminant analysis(OPLS-DA) showed that there were significant differences in volatile components between the medicinal materials and decoction pieces of Cervi Cornu Pantotrichum. Based on the thresholds of P<0.05 and Variable Importance in Projection(VIP)>1, 21 differential volatile odor components were screened out. Among them, isopentanol, isovaleraldehyde, 2-methylbutanal, n-nonanal, and dimethylamine were the key differential odor compounds between the medicinal materials and decoction pieces of Cervi Cornu Pantotrichum. The odor compounds and their relative content reduced, and some flavor substances such as esters were produced after processing with wine, which was the main reason for the reduction of the odor after processing of Cervi Cornu Pantotrichum.

Aromatic traditional Chinese medicine(TCM) has played an important role against epidemics and viruses, and volatile components are the main components that exert the pharmacological effects of aromatic TCM. By screening the related monomer components in aromatic TCM against epidemic and viruses and analyzing and endowing TCM with medicinal properties based on its clinical application and pharmacological research according to the theoretical thinking of TCM, the key technical issues of compatibility of TCM monomer components were solved from a theoretical perspective, providing new ideas and methods for screening raw materials and formulas for the development of new TCM drugs. Based on the conditions of antiviral activity, clinical application foundation, definite therapeutic effect, and high safety, a gradient screening of aromatic TCM was carried out. Firstly, 30 aromatic TCM were screened from anti-epidemic literature and clinical trial formulas, and seven volatile monomers were further screened from them. Then, four monomer components with significant effects, namely patchouli alcohol, carvacrol, p-cymene, and eucalyptol were screened. By adopting the "four-step method for a systematic study of TCM properties", the four monomer components were endowed with medicinal properties, and compatibility and combination studies were conducted to explore the theoretical basis of monomer formulas and form monomer formulas guided by TCM theory. The screening results of volatile monomers in aromatic TCM against viral pneumonia included patchouli alcohol, carvacrol, p-cymene, and eucalyptol. The medicinal properties and compatibility theory of volatile monomer components in TCM were explored. Patchouli alcohol was the main herb, with a cool and pungent nature. It entered the lung meridian to dispel evil Qi and has the effects of aromatization, detoxification, and epidemic prevention. Carvacrol was a minister drug with a cool and pungent taste. It had the effects of aromatizing, moistening, and dissolving the exterior, as well as strengthening the spleen and stomach. p-Cymene was an adjunctive medicine with a mild and pungent nature. It entered the lungs and kidneys and had the effects of aromatic purification, cough relief, and asthma relief. Eucalyptol was also an adjunctive medicine with a pungent and warm taste. It had the functions of aromatic purification, cough relief, phlegm reduction, and pain relief. The combination of the four medicines had the effects of aromatizing, moistening, detoxifying, and epidemic prevention, as well as relieving cough and asthma and strengthening the spleen and stomach. They were used to treat viral pneumonia caused by upper respiratory tract viral infections, with symptoms such as chest tightness, cough, wheezing, fatigue, nasal congestion, runny nose, nausea, and vomiting. This study has laid a literature and theoretical foundation for further drug efficacy verification experiments, compatibility efficacy experiments, and subsequent product development and clinical applications, and it serves as an innovative practice that combines literature research, theoretical research, experimental research, and clinical practice to develop new products.
