1.Brucea javanica Seed Oil Emulsion and Shengmai Injections Improve Peripheral Microcirculation in Treatment of Gastric Cancer.

OBJECTIVE:
To explore and verify the effect and potential mechanism of Brucea javanica Seed Oil Emulsion Injection (YDZI) and Shengmai Injection (SMI) on peripheral microcirculation dysfunction in treatment of gastric cancer (GC).
METHODS:
The potential mechanisms of YDZI and SMI were explored through network pharmacology and verified by cellular and clinical experiments. Human microvascular endothelial cells (HMECs) were cultured for quantitative real-time polymerase chain reaction, Western blot analysis, and human umbilical vein endothelial cells (HUVECs) were cultured for tube formation assay. Twenty healthy volunteers and 97 patients with GC were enrolled. Patients were divided into surgical resection, surgical resection with chemotherapy, and surgical resection with chemotherapy combining YDZI and SMI groups. Forearm skin blood perfusion was measured and recorded by laser speckle contrast imaging coupled with post-occlusive reactive hyperemia. Cutaneous vascular conductance and microvascular reactivity parameters were calculated and compared across the groups.
RESULTS:
After network pharmacology analysis, 4 ingredients, 82 active compounds, and 92 related genes in YDZI and SMI were screened out. β-Sitosterol, an active ingredient and intersection compound of YDZI and SMI, upregulated the expression of vascular endothelial growth factor A (VEGFA) and prostaglandin-endoperoxide synthase 2 (PTGS2, P<0.01), downregulated the expression of caspase 9 (CASP9) and estrogen receptor 1 (ESR1, P<0.01) in HMECs under oxaliplatin stimulation, and promoted tube formation through VEGFA. Chemotherapy significantly impaired the microvascular reactivity in GC patients, whereas YDZI and SMI ameliorated this injury (P<0.05 or P<0.01).
CONCLUSIONS
YDZI and SMI ameliorated peripheral microvascular reactivity in GC patients. β-Sitosterol may improve peripheral microcirculation by regulating VEGFA, PTGS2, ESR1, and CASP9.

2.Thoughts and strategies on the diagnosis and treatment of metabolic diseases in offspring after organ transplantation

The incidence of metabolic disorders is relatively high among transplant recipients and is closely related to the use of immunosuppressive agents. Metabolic diseases can trigger or accelerate the progression of cardiovascular conditions, becoming an important factor affecting long-term survival after transplantation. This article discusses the reasons for the high incidence of post-transplant metabolic disorders, and introduces the management strategies for post-transplant diabetes, dyslipidemia, and hyperuricemia, including early screening, timely diagnosis, and continuous management. It also highlights current challenges in the management of metabolic disorders after organ transplantation in China, such as insufficient interdisciplinary collaboration, lack of research data, and low awareness among recipients. It is recommended to establish a multidisciplinary follow-up system, improve the general medical capabilities of follow-up teams, and achieve precise management of transplant recipients.
3.Mechanism of Lizhong decoction in treating cold-damp diarrhea through network pharmacology,molecular docking and animal experiments

Aim To explore the key components and mechanisms of Lizhong decoction in treating rats with cold-damp diarrhea based on network pharmacology,molecular docking technology and animal experiments.Methods By literature review and database collec-tion,the components of Lizhong decoction,therapeutic targets,and the mapping with diarrhea disease targets were conducted to construct an intersection target pro-tein-protein interaction network for screening core tar-gets,and GO and KEGG pathway enrichment analysis was performed to build an"active component-target-pathway"network,followed by molecular docking vali-dation.Forty-eight rats were randomly divided into the normal control group(K),model group(DG),Lizhong decoction group(LZDG),and Pulsatilla decoction group(BTDG).Subsequently,a rat cold-damp diar-rhea model was established using Senna combined with low-temperature high-humidity environment,and the rats were intervened with Lizhong decoction and Pul-satilla decoction.HE staining was used to detect path-ological changes in intestinal tissue,ELISA was em-ployed to measure the levels of peripheral blood IL-6,IL-10,IL-1 β,and TNF-α,and western blot was used to determine the expression of colon tight junction pro-teins.Results Network pharmacology initially identi-fied 125 compounds in Lizhong decoction,5 186 drug target components,438 disease targets,and 60"drug-disease"shared targets.GO and KEGG enrichment a-nalysis showed that signaling pathways such as IL-17 and TNF were highly enriched.Molecular docking in-dicated that the core components of the drug had good binding activity with corresponding key targets.Liz-hong decoction could effectively improve the clinical symptoms of rats with cold-damp diarrhea,and com-pared with the DG group,the diarrhea rate,diarrhea in-dex,and other related indicators also gradually de-creased to normal levels.Compared with the DG group,the LZDG group showed reduced inflammation levels and a recovery in energy metabolism levels.Conclusion It can regulate targets such as MMP9 and IL-17 signaling pathways through multi-components like Calycosin and formononetin to exert its therapeutic effect on cold-damp diarrhea.
4.Comparison of clinical manifestations,laboratory characteristics,and treatment outcomes of 258 patients with acute and chronic brucellosis

To compare and analyze the clinical manifestations,laboratory characteristics,imaging findings,and treatment outcomes of patients with acute and chronic brucellosis,a retrospective analysis was conducted on 258 patients with brucellosis(202 in the acute group and 56 in the chronic group)hospitalized in Xinkang Hospital in Dalad Banner,Ordos City,Inner Mongolia Autonomous Region,from November 2023 to November 2024.General data,epidemiological characteristics,clinical presentations,laboratory test results,imaging findings,treatment outcomes,and prognosis were collected.The incidences of fever(51.5%vs 7.1%),fatigue(30.2%vs 12.5%),joint pain(42.9%vs 16.1%),and muscle pain(9.9%vs.1.8%)were significantly higher in the acute phase group(all P<0.05).The incidence of osteoarthritis complications was higher in the chronic brucellosis group(51.8%vs 8.9%,χ2=75.697,P<0.01).Univariate ANOVA analysisshowed that the Serum Agglutination Tests(SAT),alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL),creatinine(CRE),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),and bone destructionexhibited statistically significant differences between the acute and chronic phases of brucellosis(all P<0.05).Multivariate logistic regression analysis indicated that abnormal ALT(OR=14.18,95%CI:1.11-181.72;P=0.041)and bone destruction(OR=0.16,95%CI:0.04-0.63;P=0.009)were associated with chronic brucellosis.After treatment,all patients experienced have symptom relief in varying degrees,with 157 patients(60.9%)cured and 101 patients(39.1%)symptomatic improved(P<0.01).In conclusion,the incidences of fever,fatigue,and joint pain in patients during the acute phase is significantly higher than that those in patients during the chronic phase,while the incidence of osteoarthritis complications is higher in chronic phase patients.The incidences of abnormal SAT,ALT,AST,TBIL,CRE,CRP,and ESR,and bone destruction varies at different stages of brucellosis.Of those,abnormal ALT and bone destruction show a stronger association with,which can assist the clinical staging of brucellosis.

7.Effect of high ligation of spermatic vein on leukocytes in expressed prostate secretion of patients with chronic prostatitis and varicocele

Objective:To investigate the effect of laparoscopic high ligation of spermatic cord vein in patients with chronic prostatitis and varicocele prostatitis.Methods:A total of 90 varicocele patients were selected from January 2016 to December 2020,including 33 patients with chronic prostatitis.Changes of white blood cell count,National Institutes of Health chronic prostatitis symp-tom index(NIH-CPSI)score and serum testosterone level in the expressed prostate secretion(EPS)were observed before and after the operation of laparoscopic high ligation of spermatic vein.Results:All patients were followed up three months after the surgery.There was no significant difference in the white blood cell counts in EPS,NIH-CPSI score,and serum testosterone level in patients with vari-cocele-only who underwent high ligation surgery after the operation.However,the white blood cell count in the EPS of patients with chronic prostatitis was lower than that before 3 months of operation([12.39±4.23]× 10 9/L vs[21.36±5.05]× 10 9/L).The NIH-CPSI score was significantly lower than that before operation([12.71±6.21]vs[26.76±8.43]).And the serum testosterone level was higher than that before operation([4.34±1.77]ng/ml vs[2.36±1.05]ng/ml).Conclusion:Laparoscopic high liga-tion of the spermatic vein in patients with chronic prostatitis and varicocele could effectively reduce the number of white blood cells in the EPS,boost the level of serum testosterone and improves symptoms of chronic prostatitis.

9.Alvianolic acid B exerts a protective effect on random skin flaps through autophagy mediated by activation of TFE3

Aim To observe the role of salvianolic acid B(Sal B)in enhancing the survival of random skin flaps and to preliminarily explore its potential mecha-nisms.Methods The appearance,degree of edema,color and hair condition of the skin flap were evaluated seven days after operation.The vascular network and blood flow of random flaps were measured by laser Doppler flow measurement.HE staining was used to detect the growth of microvessels in random flaps.The expressions of VEGF and CD34 were detected by im-munohistochemistry,the expressions of RIPK1,2 and LC3 Ⅱ were detected by immunofluorescence,and the effects of autophagy related proteins and signaling path-ways were detected by Western blot.Results The ex-perimental results showed that Sal B induced autophagy in the random skin flaps,promoted angiogenesis,and reduced oxidative stress and necrotic apoptosis,signifi-cantly increasing the survival rate of the flaps.Immu-nohistochemistry,immunofluorescence staining,and Western blot confirmed that Sal B induced autophagy in the random skin flaps by activating TFE3 protein.Conclusion Sal B can promote autophagy in cells of random skin flaps and reduce their necrotic apoptosis by activating TFE3 protein.
10.Effect and mechanism of astaxanthin on improving intestinal injury of sep-tic mice

Objective To understand the effect of astaxanthin on intestinal injury of septic mice,and explore the mechanism.Methods Septic mice model was constructed by cecum ligation and puncture(CLP).Sixty-two male Balb/c mice were randomly divided into 4 groups by random number method:Sham surgery+solvent control group(Sham+Vehi group,n=11),Sham surgery+astaxanthin group(Sham+Asta group,n=11),sepsis model+sol-vent control group(CLP+Vehi group,n=20),and sepsis model+astaxanthin group(CLP+Asta group,n=20).In astaxanthin-containing groups,astaxanthin was dissolved in edible olive oil(40 mg/mL),and 100 mg/(kg·d)was gavaged for 7 days before surgery.In solvent-containing groups,the solvent was treated with an equal amount of olive oil by gavage(2.5 mL/kg).Five mice from the Sham groups and 12 mice from the CLP groups were ran-domly selected to observe their 7-day survival after surgery.The remaining mice were given fluorescent isothiocya-nate dextran(FD-40)gavage at 18 hours after surgery.Changes in mice intestinal tissue morphology,intestinal functional injury indicators,intestinal tissue oxidative stress indicators,inflammatory factors expression,and ex-pression of key protein of peroxisome proliferator-activated receptor γ(PPARγ)/nuclear factor kappa B(NF-κB)were detected 24 hours after surgery.Results There were no statistical differences in mice survival rate,intestinal injury indicators,intestinal inflammatory factor levels,oxidative stress indicators,and intestinal tissue injury scores between Sham+Vehi and Sham+Asta groups(all P>0.05).Compared with the Sham+Vehi group,the survival rate of mice in the CLP+Vehi group decreased significantly;serum diamine oxidase(DAO)activities,levels of in-testinal fatty acid binding protein(I-FABP),D-lactate,and FD-40 increased significantly;levels of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6)and malondialdehyde(MDA)in intestinal tissue in-creased significantly;superoxide dismutase(SOD)activity decreased;intestinal morphological injury score was higher;the expression of PPARγ in intestinal tissue increased,and the ratios of both p-IκBα/IκBα and p-p65/p65 in-creased(all P<0.05).Compared with the CLP+Vehi group,the survival rate of mice in the CLP+Asta group im-proved;serum DAO activities,levels of I-FABP,D-lactate and FD-40 all decreased significantly;levels of TNF-α,IL-1β,IL-6 and MDA in intestinal tissue decreased significantly;SOD activity increased;intestinal morphological injury score decreased;PPARγ expression in intestinal tissue increased,and the ratios of both p-IκBα/IκBα and p-p65/p65 decreased(all P<0.05).Conclusion Astaxanthin decreases intestinal injury in CLP-induced septic mice,and its mechanism may be related to the regulation of PPARγ/NF-κB signaling pathway,as well as the inhibi-tion of inflammatory response and oxidative stress.
