The C1–2 level is a relatively common location for extradural neurogenic tumors. However, surgical access to this region is anatomically demanding because of its close relationship to the vertebral artery and the spinal cord. To our knowledge, there have been no previous reports of using the unilateral biportal endoscopic (UBE) technique for such cases. We present the first successful removal of a C1–2 extradural tumor using UBE, demonstrating the feasibility, safety, and effectiveness of this approach.

Purpose: Non-operative management (NOM) has emerged as a promising organ-preserving strategy for patients with rectal cancer who achieve a clinical complete response (cCR) after neoadjuvant chemoradiotherapy (CRT). However, no standardized treatment protocol has been established for watch-and-wait strategies. Materials and Methods: This prospective study evaluated oncological outcomes of NOM combined with 4 months of adjuvant capecitabine. Patients with resectable rectal cancer (≤ 8 cm from the anal verge, cT2-4 or N+) underwent CRT (50-54 Gy in 25-27 fractions with capecitabine). Eight weeks post-CRT, a multidisciplinary team assessed cCR. Patients achieving cCR received six cycles of capecitabine (2 weeks on/1 week off) and were actively monitored. Results: Among 89 patients receiving CRT (2018-2023), 17 (19.1%) achieved cCR and were included. The median age was 65 years, and 64.7% were male. Eleven (64.7%) completed all six cycles of adjuvant therapy. After a median follow-up of 31.4 months, 11 patients (64.7%) remained disease-free. Local regrowth occurred in six patients (35.3%) with 2- and 4-year rates of 34.5% and 47.6%, respectively. Five underwent radical surgery, and one received transanal excision with systemic chemotherapy. At the time of assessment, 15 patients (88.2%) showed no evidence of disease, while two (11.8%) received palliative chemotherapy. All patients were alive. Conclusion NOM with adjuvant capecitabine showed promising oncological outcomes, offering an alternative to passive watch-and-wait approaches. Further refinement through multidisciplinary strategies is warranted.

Background: and Purpose This study evaluated the diagnostic utility of an anti-signal-recognition particle 54 (anti-SRP54) antibody-based enzyme-linked immunosorbent assay (ELISA) as well as the clinical, serological, and pathological characteristics of patients with SRP immune-mediated necrotizing myopathy (IMNM). Methods: We evaluated 87 patients with idiopathic inflammatory myopathy and 107 healthy participants between January 2002 and December 2023. The sensitivity and specificity of the ELISA for anti-SRP54 antibodies were assessed, and the clinical profiles of patients with antiSRP54 antibodies were determined. Results: The ELISA for anti-SRP54 antibodies had a sensitivity and specificity of 88% and 99%, respectively, along with a test–retest reliability of 0.92 (p<0.001). The 32 patients diagnosed with anti-SRP IMNM using a line-blot immunoassay included 28 (88%) who tested positive for anti-SRP54 antibodies using the ELISA, comprising 12 (43%) males and 16 (57%) females whose median ages at symptom onset and diagnosis were 43.0 years and 43.5 years, respectively. Symptoms included proximal muscle weakness in all 28 (100%) patients, neck weakness in 9 (32%), myalgia in 15 (54%), dysphagia in 5 (18%), dyspnea in 4 (14%), dysarthria in 2 (7%), interstitial lung disease in 2 (7%), and myocarditis in 2 (7%). The median serum creatine kinase (CK) level was 7,261 U/L (interquartile range: 5,086–10,007 U/L), and the median anti-SRP54 antibody level was 2.0 U/mL (interquartile range: 1.0–5.6 U/mL). The serum CK level was significantly higher in patients with coexisting anti-Ro-52 antibodies. Conclusions This study has confirmed the reliability of the ELISA for anti-SRP54 antibodies and provided insights into the clinical, serological, and pathological characteristics of South Korean patients with anti-SRP IMNM.

Background: Monopolar radiofrequency devices for facial skin tightening offer a promising non-invasive approach that targets the deep dermis to prompt immediate collagen contraction and subsequent remodeling.However, non-clinical evidence supporting the efficacy and mechanisms of action of these treatments remains limited, despite their increasing use. This study aimed to evaluate the effectiveness and safety of a dualfrequency non-invasive monopolar radiofrequency (NMRF) device using a porcine model, directly comparing a conventional 6.78 MHz setting with a dual-frequency (6.78 + 2 MHz) mode. Methods: Female porcine subjects were treated using an innovative NMRF device (XERF; Cynosure Lutronic Inc.). Efficacy was assessed histologically for up to 1 month. Safety was evaluated through thermal skinsurface monitoring and standard assessments. Results: When compared with 6.78 MHz alone, the dual-frequency (6.78 + 2 MHz) mode produced greater immediate thickening and shortening of deep dermal collagen bundles and more prominent remodeling of subcutaneous fibrous septa. At 1 month, the dual-frequency treatment yielded larger increases in dermal collagen density, more regular bundle alignment, and higher elastin signals. Surface temperature remained below 43ºC, and no tissue damage was observed following NMRF treatment. Conclusion Our research strongly indicates that immediate tissue contraction occurs following treatment and that dual-frequency application results in more substantial collagen reconstruction. In the long term, NMRF stimulates the production of new collagen and elastin. The dual-frequency mode enhances these effects, supporting its use in effective and safe skin tightening and rejuvenation.

Background and Objectives: Hypertrophic cardiomyopathy (HCM) increases the risk of sudden cardiac death. Despite international guidelines for sudden death prevention and implantable cardiac defibrillator (ICD) use, clinical practices vary. This study assessed the practices and opinions of Korean cardiologists on HCM management, focusing on ICD use and reimbursement policies. Methods: A structured electronic survey was distributed to members of the Korean Society of Cardiology, covering HCM management, ICD preferences, and reimbursement policies. Results: A total of 119 cardiologists participated, with most being arrhythmia specialists (56.3%) and university professors (81.5%). Cardiac magnetic resonance imaging and genetic testing were underutilized (30.3% and 11.8%, respectively). Beta-blocker-refractory premature ventricular complexes were primarily managed with continued medical therapy (77.3%). ICD preferences varied by physician specialty and patient age, with subcutaneous ICDs preferred in younger patients and dual-chamber ICDs in older patients. Nearly half of respondents believed ICD reimbursement criteria required revision. Conclusions Significant variability in HCM management exists among Korean cardiologists, influenced by restrictive reimbursement criteria rather than clinical guidelines. Updated reimbursement policies and improved access to advanced diagnostics are needed to align with international standards and optimize patient care.
