1.Etiological analysis of 864 children with pneumonia

Objective To analyze the etiological characteristics of children with pneumonia and provide a reference for regional pneumonia prevention and control.Methods A retrospective analysis was conducted on the clinical data of 864 children with pneumonia admitted to this hospital from January to December 2023.The disease types,common pathogen infections,and drug-resistant Mycoplasma pneumoniae infections in dif-ferent age groups were analyzed.Results Among the 864 children with pneumonia,there were 466 cases(53.94%)of bronchopneumonia,290 cases(33.56%)of asthmatic bronchopneumonia,and 108 cases(12.50%)of lobar pneumonia.The infections were caused by Mycoplasma pneumoniae in 470 cases(54.40%),Haemophilus influenzae in 196 cases(22.69%),Streptococcus pneumoniae in 172 cases(19.91%),Moraxella catarrhalis in 143 cases(16.55%),Staphylococcus aureus in 142 cases(16.43%),Re-spiratory syncytial virus in 54 cases(6.25%).The drug resistance rate of Mycoplasma pneumoniae was 20.00%(94/470),and that of Staphylococcus aureus was 7.75%(11/142).Among 864 children with pneumo-nia,there was 1 case(0.12%)of drug-resistant of Mycoplasma pneumoniae infection in the 0-<1 year age group,29 cases(3.36%)in the 1-<4 years age group,39 cases(4.51%)in the 4-<7 years age group,21 cases(2.43%)in the 7-<10 years age group,and 4 cases(0.46%)in the 10-<15 years age group.Conclu-sion Mycoplasma pneumoniae is the main pathogen in children with pneumonia,and its drug resistance rate is relatively high.
2.Application of automatic laser navigation in percutaneous bone marrow puncture biopsy

Objective To investigate the advantages and disadvantages of percutaneous bone marrow puncture biopsy guided by automatic laser navigation and free-hand percutaneous bone marrow biopsy guided by CT.Methods Seventy patients with bone marrow biopsy at the department of nuclear medicine in this hos-pital from January to June 2022 were selected as the study subjects and divided into the laser navigation group (bone marrow biopsy guided by automatic laser navigation,n=30) and free-hand puncture group (percutane-ous bone marrow biopsy guided by CT,n=40) based on different guidance methods.The duration of the pro-cedure,total radiation dose,total number of scans,positive rate of bone lesion biopsy and puncture complica-tions occurrence were compared between the two groups.Results Compared to the free-hand puncture group,the operation duration in the laser navigation group was shorter[11.00(7.00,13.00)min vs. 13.00(11.00,17.75)min],the total radiation dose was lower[439.00(291.00,568.75)mGy vs. 510.00(431.25,708.50) mGy],total number of scans was less[4.50(3.00,6.00)times vs. 6.00(5.00,7.00)times],and the differ-ences were statistically significant (P<0.05).There was no statistically significant difference in the positive rate of bone lesion biopsy between the laser navigation group and free-hand puncture group[96.67%(29/30) vs. 85.00%(34/40),P>0.05].No puncture complications occurred in both groups.Conclusion Automatic laser navigation could assist the operators to better complete percutaneous bone marrow biopsy procedures.
3.PGRMC1-mediated autophagy decreases the sensitivity of hepatocellular carcinoma cells to 125I particle irradiation

Objective To investigate the effect of progesterone receptor membrane component 1(PGRMC1)mediated autophagy on the sensitivity of liver cancer cells to 125I particles irradiation.Methods Hepatoma cell lines Huh7 and LM3 were exposed to different doses(0,2,4,6 and 8 Gy)of 125I particles,and cell autophagy was observed by transmission electron microscopy(TEM).Then,autophagy inhibitor chloroquine(CQ),agonist rapamycin(Rapa),and PGRMC1 inhibitor AG-205 were used respectively to verify that PGRMC1-mediated autophagy plays a key role in the sensitivity of hepatocellular carcinoma cells to 125I particle irradiation.Cell proliferation,colony formation and apoptosis were detected by CCK-8 assay,clonal formation test and flow cytometry,respectively.The expression levels of PGRMC1,microtubule-associated protein light chain 3-Ⅰ(LC3-Ⅰ),LC3-Ⅱ and p62 were detected by Western blotting.Results Different doses of 125I particles irradiation significantly decreased the proliferation and clonogenesis of Huh7 and LM3 cells(P<0.05),and increased the apoptotic cells(P<0.01),in a dose-dependent manner.Compared with the 0 Gy group,the ratio of LC3-Ⅱ/LC3-Ⅰ in Huh7 and LM3 cells was obviously increased,and the expression of p62 was significantly down-regulated in the 6 Gy group.The proliferation capacity and clonal formation ability of Huh7 and LM3 cells were decreased significantly,and their apoptotic cells were increased notably in the 6 Gy+CQ group than the 6 Gy group,while the above results were on the contrary in the 6 Gy+Rapa group.The 6 Gy+AG205 group had notably decreased LC3-Ⅱ/LC3-Ⅰ ratio in the Huh7 and LM3 cells,up-regulated p62 expression,reduced cell proliferation capacity and clone formation ability,and enhanced cell apoptosis when compared with the 6 Gy group,and the above results of the 6 Gy+PGRMC1 group were opposite.Conclusion Increment of PGRMC1 induced by 125I irradiation can promote autophagy,increase the proliferation and clonogenesis,and reduce the apoptosis in hepatocellular carcinoma cells.

Background/Aims:
Despite the high efficacy of direct-acting antivirals (DAAs), approximately 1–3% of hepatitis C virus (HCV) patients fail to achieve a sustained virological response. We conducted a nationwide study to investigate risk factors associated with DAA treatment failure. Machine-learning algorithms have been applied to discriminate subjects who may fail to respond to DAA therapy.
Methods:
We analyzed the Taiwan HCV Registry Program database to explore predictors of DAA failure in HCV patients. Fifty-five host and virological features were assessed using multivariate logistic regression, decision tree, random forest, eXtreme Gradient Boosting (XGBoost), and artificial neural network. The primary outcome was undetectable HCV RNA at 12 weeks after the end of treatment.
Results:
The training (n=23,955) and validation (n=10,346) datasets had similar baseline demographics, with an overall DAA failure rate of 1.6% (n=538). Multivariate logistic regression analysis revealed that liver cirrhosis, hepatocellular carcinoma, poor DAA adherence, and higher hemoglobin A1c were significantly associated with virological failure. XGBoost outperformed the other algorithms and logistic regression models, with an area under the receiver operating characteristic curve of 1.000 in the training dataset and 0.803 in the validation dataset. The top five predictors of treatment failure were HCV RNA, body mass index, α-fetoprotein, platelets, and FIB-4 index. The accuracy, sensitivity, specificity, positive predictive value, and negative predictive value of the XGBoost model (cutoff value=0.5) were 99.5%, 69.7%, 99.9%, 97.4%, and 99.5%, respectively, for the entire dataset.
Conclusions
Machine learning algorithms effectively provide risk stratification for DAA failure and additional information on the factors associated with DAA failure.
5. PPARfJ involved in endothelial cell injury induced by high glucose via nitrative stress

Aim To investigate the role of PPARβ and nitrative stress in human umbilical vein endothelial cells(HUVECs)injury induced by high glucose.Methods The cell viability was detected by CCK-8.The cell proliferation was detected by EdU proliferation detection kit.The protein expression level of PPARβ,eNOS,iNOS,and 3-nitrotyrosine was detected by Western blot.The content of peroxynitrite and nitric oxide(NO)was determined by peroxynitrite kit and Griess Reagent,respectively.Results Glucose(30,40,50 mmol·L-1)significantly reduced the cell viability of HUVECs in a dose-dependent manner.Glucose at 30 mmol·L-1(high glucose,HG)significantly reduced the proliferation of HUVECs,down-regulated the expression of PPARβ,eNOS at protein level and NO content,and increased iNOS,3-nitrotyrosine protein expression and peroxynitrite level.The above effects of HG were reversed by PPARβ agonist GW0742(1 μmol·L-1).Both PPARβ antagonist GSK0660(1 μmol·L-1)and NOS inhibitor L-NAME(10 μmol·L-1)blocked the protective effects of GW0742.Conclusion The down-regulation of PPARβ is involved in the injury of HUVECs induced by high glucose,which may be mediated,at least partly,by the stimulation of nitrative stress.

OBJECTIVES:
To study the effect of glucose metabolism disorders on the short-term prognosis in neonates with asphyxia.
METHODS:
A retrospective analysis was performed on the medical data of the neonates with asphyxia who were admitted to 52 hospitals in Hubei Province of China from January to December, 2018 and had blood glucose data within 12 hours after birth. Their blood glucose data at 1, 2, 6, and 12 hours after birth (with an allowable time error of 0.5 hour) were recorded. According to the presence or absence of brain injury and/or death during hospitalization, the neonates were divided into a poor prognosis group with 693 neonates and a good prognosis group with 779 neonates. The two groups were compared in the incidence of glucose metabolism disorders within 12 hours after birth and short-term prognosis.
RESULTS:
Compared with the good prognosis group, the poor prognosis group had a significantly higher proportion of neonates from secondary hospitals (48.5% vs 42.6%,
CONCLUSIONS
Recurrent hyperglycemia in neonates with asphyxia may suggest poor short-term prognosis, and it is necessary to strengthen the early monitoring and management of the nervous system in such neonates.

7.Mechanism of Modified Erzhiwan Combined with Epimedium for Atherosclerosis Based on Network Pharmacology

Objective::In this study, a network pharmacology-based method was applied to analyze the mechanism of modified Erzhiwan combined with epimedium in treatment of atherosclerosis. Method::The compounds and targets of modified Erzhiwan combined with epimedium were screened in traditional Chinese medicine(TCM) systems pharmacology database and analysis platform (TCMSP) and bioinformatics analysis tool for molecular mechanism of TCM (BATMAN-TCM). Mang related databases were applied to find the target-related to atherosclerosis.The common targets of modified Erzhiwan combined with epimedium and atherosclerosis were got by venn diagrams.Cytoscape 3.6.1 was used to construct ingredients-disease-targets networks.Then protein-protein interaction (PPI) analysis of ingredients-disease-targets was builed in STRING database, and was visualized by Cytoscape 3.6.1, then important modules were analyzed with Moleculaar complex detection(MCODE). Biological information annotation databases (DAVID) was used to carry on gene ontology (GO) analysis and enrichment analysis of gene encyclopedia kyoto encyclopedia of genes and genomes (KEGG) pathway. 8.Progress in Molecular Chaperone Regulation of Heat Shock Protein 90 and Cancer

Heat shock protein 90 (HSP90) is a member of genetically conserved heat shock protein family. As an important molecular chaperone in eukaryotic cells, HSP90 plays a key regulatory role in maintaining cellular protein homeostasis. HSP90 clients encompass a wide range of proteins, thus HSP90 is involved in diverse biological process. With the deeper study, it is found that HSP90 takes an important part in the development and metastasis of cancer,and has become a promising target for the study of anticancer biology. We review the progress of HSP90 as molecular chaperone and its relationship with cancer.

Differences of protonated and lithiated leucine-enkephalin(LE) were investigated by hydrogen deuterium exchange-mass spectrometry(HDX-MS) combined with quantum chemistry calculation. The results revealed that the protonated ions possessed very high product yield with all hydrogen atoms being exchanged, while the reaction of lithiated LE stopped after exchanging five hydrogen atoms in the same experimental conditions. The different HDX behaviours probably indicated their conformational differences. To further clarify the experimental results, the most stable conformations of protonated and lithiated leucine-enkephalin were calculated by density functional theory. It was found that terminal amino group was the most thermodynamically stable protonation site,while Li+in coordination of four carbonyl oxygen atoms formed the most favourable lithiated LE. The reaction field reduction of lithium LE was probably due to the less acidity of hydrogen atoms and the increasing rigid conformation change induced by lithium ion.
10.Diagnostic Power of Diffusion-weighted Magnetic Resonance Imaging for the Presence of Lymph Node Metastasis: A Meta-analysis

Present work was designed to quantitatively evaluate the performance of diffusion-weighted magnetic resonance imaging (DWI) in the diagnosis of the presence of metastasis in lymph nodes (LNs).Eligible studies were identified from systematical PubMed and EMBASE searches.Data were extracted.Meta-analyses were performed to generate pooled sensitivity and specificity on the basis of per-node,per-lesion and per-patient,respectively.Fourteen publications (2458 LNs,404 lesions and 334 patients) were eligible.Per-node basis demonstrated the pooled sensitivity and specificity was 0.82 (P<0.0001) and 0.90 (P<0.0001),respectively.Per-lesion basis illustrated the pooled sensitivity and specificity was 0.73 (P=-0.0036) and 0.85 (P<0.0001),respectively.Per-patient basis indicated the pooled sensitivity and specificity was 0.67 (P=0.0909) and 0.86 (P<0.0001),respectively.In conclusion,DWI has rather a negative predictive value for the diagnosis of LN metastasis presence.The difference of the mean apparent diffusion coefficients between benign and malignant LNs is not yet stable.Therefore,the DWI technique has to be further improved.
