1.Comparison of optical coherence tomography image features between traumatic macular hole and idiopathic macular hole

Objective:To comparatively observe optical coherence tomography (OCT) image features between traumatic macular hole (TMH) and idiopathic macular hole (IMH).Methods:A retrospective clinical study. A total of 174 patients (174 eyes) with macular hole (MH) diagnosed at Shantou International Eye Center from December 2008 to May 2024 were included in the study. Among them, there were 75 patients (75 eyes) with TMH and 99 patients (99 eyes) with IMH, and they were divided into the TMH group and the IMH group accordingly. All the affected eyes underwent best corrected visual acuity (BCVA) and OCT examinations. The BCVA was examined using a standard logarithmic visual acuity chart, and was converted to the logarithm of the minimum angle of resolution (logMAR) visual acuity for statistical analysis. The minimum diameter and basal diameter of the MH, as well as the average, nasal, superior, inferior, and temporal center retinal thickness (CRT) around the MH were measured by OCT. The independent-sample t test was used to compare the logMAR BCVA, hole diameter, and CRT at the hole margin between the groups. Results:There were significant differences in age ( t=-15.857) and gender ratio ( χ2=28.154) between the TMH group and the IMH group ( P<0.05), while there was no significant difference in logMAR BCVA ( t=1.962, P>0.05). The minimum diameter of the hole in the TMH group was smaller than that in the IMH group, but the basal diameter was larger, with significant differences ( t=-3.322, 2.570; P<0.05). The thickness of the neuroepithelial layer at the hole margin in the TMH group was thinner than that in the IMH group, with significant differences in the superior ( t=-2.747), inferior ( t=-2.316), and nasal ( t=-2.851) regions ( P<0.05), and no significant difference in the temporal region ( t=-1.586, P>0.05). In the TMH group, the number of eyes with macular cystoid edema (CME), posterior vitreous detachment (PVD), retinal atrophy, subretinal hemorrhage, choroidal laceration, and focal neuroepithelial detachment was 36 (48.00%, 36/75), 4 (5.33%, 4/75), 4 (5.33%, 4/75), 15 (20.00%, 15/75), 8 (10.67%, 8/75), and 19 (25.33%, 19/75) eyes, respectively. In the IMH group, the number of eyes with CME and PVD was 95 (95.96%, 95/99) and 94 (94.95%, 94/99) eyes, respectively. Conclusion:Compared with IMH, TMH has a larger basal diameter, a thinner CRT at the hole margin, a lower incidence of CME and PVD, and a higher incidence of subretinal hemorrhage, focal neuroepithelial detachment, choroidal laceration, and retinal atrophy.

Objective To establish a stable,reliable,and clinically relevant porcine model of endotoxin-induced acute respiratory distress syndrome(ARDS).Methods Ten 8-month-old male Bama minipigs were deeply sedated,followed by invasive mechanical ventilation and electrocardiographic monitoring.Lipopolysaccharide(LPS)was intravenously pumped at 600 μg/(kg·h)for 3 hours,then maintained at 15 μg/(kg·h)thereafter.Dynamic monitoring was performed at five time points after LPS injection(LPS 0,1,3,5,and 8 h),including arterial blood gas analysis and chest computed tomography(CT)scans.Pathological examination of lung tissues obtained via bronchoscopic biopsy(HE staining and transmission electron microscopy)was conducted.These indicators were comprehensively used to evaluate the success of the animal model.Results At 5 hours after LPS administration,8 minipigs developed symptoms such as skin cyanosis,elevated body temperature,and respiratory distress.The oxygenation index decreased to<300 mmHg.Chest CT scans showed diffuse pulmonary infiltrates.Histopathology revealed alveolar edema and hyaline membrane formation.Transmission electron microscopy demonstrated disruption of pulmonary blood-air barrier,depletion of lamellar bodies in type Ⅱ pneumocytes,inflammatory cell infiltration,and exudation of plasma proteins and fibrin.Compared with LPS 0 h,at LPS 8 h,the oxygenation index and arterial blood pH were significantly decreased(P<0.001),while blood lactic acid and serum potassium were significantly increased(P<0.05);serum calcium and base excess were significantly decreased(P<0.05),and the lung injury score based on HE-stained lung sections was significantly increased(P<0.01).Conclusion The porcine ARDS model established by continuous LPS injection can dynamically simulate the pathophysiological characteristics and typical pathological manifestations of clinical septic ARDS,making it an effective tool to study the pathogenesis,prevention,and treatment strategies of septic ARDS.

Executive function(EF) is an advanced cognitive function of the central nervous system, and is closely related to an individual's capacity for daily living and adaptation. Patients with attention deficit hyperactivity disorder (ADHD) typically exhibit significant executive dysfunction. While most existing studies on the executive function of individuals with ADHD are cross-sectional, and little is known about the longitudinal maturation process of related brain structures and functional connectivity patterns. The findings indicate that ADHD patients exhibit differential developmental trajectories in brain structural and functional connectivity compared with typically developing group.Furthermore, there is a lifespan association between abnormal brain network development and ADHD symptoms. This article aims to elucidate the characteristics of executive function deficits in ADHD patients across different developmental stages, examining their relationship with the nervous system’s development from a development perspective.

6.Effect of Temperature Cycle Preservation on Platelet Aggregation Rate and Routine Parameters

Objective:To compare and analyze the changes of aggregation rate and routine parameters of platelets stored in temperature cycle,cold storage at 4 ℃ and oscillating storage at 22 ℃,so as to provide more experimental data for platelet preservation methods.Methods:Blood samples were collected at 5 time points on the 1st,2nd,3rd,4th and 6th day after platelet cycling preservation at temperature,cold storage at 4 ℃,and oscillating storage at 22 ℃.Platelet maximum aggregation rate(MAR)and routine parameters including platelet count(PLT),mean platelet volume(MPV),platelet distribution width(PDW)and platelet-larger cell ratio(P-LCR)were detected.Results:The platelet MAR of three groups showed a significant decrease trend with the preservation time,the fastest decrease was in the 22 ℃ group,the slowest was in the 4 ℃ group,and the temperature cycle group was between the two groups.On the 3rd day of preservation,the platelet MAR in 4 ℃ group was still in the normal range(MAR>60%),while in temperature cycle group was about 50%,and in 22 ℃ group was the lowest.On the 4th day of preservation,platelet MAR in all the three groups was lower than 50%,and that in temperature cycle group was significantly lower than in 4 ℃ group but higher than in 22 ℃ group(both P<0.05).On the 6th day of preservation,platelet MAR in the temperature cycle group was significantly lower than that in the 4 ℃ group(P<0.05),but there was no statistically significant difference compared to 22 ℃ group(P>0.05).PLT values in the three groups were all significantly decreased with the preservation time extension,and were significantly lower than those in the early stage of preservation within 6 days(all P<0.05).PDW in temperature cycle group had no significant change within 6 days of preservation,but MPV and P-LCR were significantly increased.MPV,PDW and P-LCR all decreased significantly in 4 ℃ group within 6 days of preservation but increased in 22 ℃ group.Under the same storage days,PLT value of temperature cycle group had no significant difference with that of 4 ℃ group and 22 ℃ group,while MPV,PDW and P-LCR values were significantly higher than 4 ℃ group but lower than 22 ℃ group(all P<0.05).Conclusion:The aggregation function and routine parameters changes of temperature circulating preserved platelets are between 4 and 22 ℃.

Objective To assess the biomechanical differences between the artificial ankle joints INBONE Ⅱ and INFINITY after total ankle arthroplasty(TAA)to provide more scientific and individualized treatment options for patients.Methods A patient-individualized TAA lower extremity MSK MBD model was established using the musculoskeletal(MSK)multibody dynamics(MBD)software AnyBody.The ankle joint contact force,motion,and contact characteristics of artificial ankle joint surfaces were predicted.Results The geometric shape of the articular surface of the artificial ankle had no significant effect on the ankle contact force,but it affected the range of motion and contact characteristics of the articular surface.Compared with INBONE Ⅱ,the coronal plane articular surface arc height of INFINITY was lower,and increased the inversion-eversion and external rotation-internal rotation movements by 7.91%and 2.61%,respectively.Sagittal plane matching was lower,and reduced posterior-anterior and inferior-superior movements by 21.75%and 21.23%,respectively,and medial-lateral movement increased by 49.26%.INFINITY exhibited lower matching with decreases of 18.48%,30.42%,and 26.36%in the medial,lateral,and total joint surface contact areas,respectively.However,the center of the pressure motion trajectory was concentrated on the medial side,while avoiding edge contact stress concentration and reducing the risk of joint dislocation and premature wear of the tibial component.Conclusions The smaller geometric constraints of the INFINITY artificial ankle joint demonstrated better biomechanical performance,thereby promoting improved postoperative ankle joint functional recovery.

There are huge differences between tumor cells and normal cells in material metabolism, and tumor cells mainly show increased anabolism, decreased catabolism, and imbalance in substance metabolism. These differences provide the necessary material basis for the growth and reproduction of tumor cells, and also provide important targets for the treatment of tumors. Ferroptosis is an iron-dependent form of cell death characterized by an imbalance of iron-dependent lipid peroxidation and lipid membrane antioxidant systems in cells, resulting in excessive accumulation of lipid peroxide, causing damage to lipid membrane structure and loss of function, and ultimately cell death. The regulation of ferroptosis involves a variety of metabolic pathways, including glucose metabolism, lipid metabolism, amino acid metabolism, nucleotide metabolism and iron metabolism. In order for tumor cells to grow rapidly, their metabolic needs are more vigorous than those of normal cells. Tumor cells are metabolically reprogrammed to meet their rapidly proliferating material and energy needs. Metabolic reprogramming is mainly manifested in glycolysis and enhancement of pentose phosphate pathway, enhanced glutamine metabolism, increased nucleic acid synthesis, and iron metabolism tends to retain more intracellular iron. Metabolic reprogramming is accompanied by the production of reactive oxygen species and the activation of the antioxidant system. The state of high oxidative stress makes tumor cells more susceptible to redox imbalances, causing intracellular lipid peroxidation, which ultimately leads to ferroptosis. Therefore, in-depth study of the molecular mechanism and metabolic basis of ferroptosis is conducive to the development of new therapies to induce ferroptosis in cancer treatment. Ferroptosis, as a regulated form of cell death, can induce ferroptosis in tumor cells by pharmacologically or genetically targeting the metabolism of substances in tumor cells, which has great potential value in tumor treatment. This article summarizes the effects of cellular metabolism on ferroptosis in order to find new targets for tumor treatment and provide new ideas for clinical treatment.
