1.Early effectiveness of navigation-free robot-assisted total knee arthroplasty in treating knee osteoarthritis with extra-articular deformities.

OBJECTIVE:
To evaluate the early effectiveness of navigation-free robot-assisted total knee arthroplasty (TKA) compared to traditional TKA in the treatment of knee osteoarthritis combined with extra-articular deformities.
METHODS:
The clinical data of 30 patients with knee osteoarthritis combined with extra-articular deformities who met the selection criteria between June 2019 and January 2024 were retrospectively analyzed. Fifteen patients underwent CORI navigation-free robot-assisted TKA and intra-articular osteotomy (robot group) and 15 patients underwent traditional TKA and intra-articular osteotomy (traditional group). There was no significant difference in age, gender, body mass index, affected knee side, extra-articular deformity angle, deformity position, deformity type, and preoperative knee range of motion, American Knee Society (KSS) knee score and KSS function score, and lower limb alignment deviation between the two groups ( P>0.05). The operation time, intraoperative blood loss, and complications of the two groups were recorded and compared. The knee range of motion and lower limb alignment deviation were recorded before operation and at 6 months after operation, and the knee joint function was evaluated by KSS knee score and function score.
RESULTS:
There was no significant difference in operation time between the two groups ( P>0.05); the intraoperative blood loss in the robot group was significantly less than that in the traditional group ( P<0.05). Patients in both groups were followed up 6-12 months, with an average of 8.7 months. The incisions of all patients healed well, and there was no postoperative complication such as thrombosis or infection. At 6 months after operation, X-ray examination showed that the position of the prosthesis was good in both groups, and there was no loosening or dislocation of the prosthesis. The knee joint range of motion, the lower limb alignment deviation, and the KSS knee score and KSS function score significantly improved in both groups ( P<0.05) compared to preoperative ones. The changes of lower limb alignment deviation and KSS function score between pre- and post-operation in the robot group were significantly better than those in the traditional group ( P<0.05), while the changes of other indicators between pre- and post-operation in the two groups were not significant ( P>0.05).
CONCLUSION
Compared to traditional TKA, navigation-free robot-assisted TKA for knee osteoarthritis with extra-articular deformities results in less intraoperative blood loss, more precise reconstruction of lower limb alignment, and better early effectiveness. However, long-term effectiveness require further investigation.

2.Efficacy of cannulated screw internal fixation combined with quadratus femoris bone flap with preservation of the posterior superior retinaculum for femoral neck fracture in young and middle-aged patients

Objective:To compare the efficacy of cannulated screw internal fixation combined with quadratus femoris bone flap with preservation of the posterior superior retinaculum and cannulated screw internal fixation alone in the treatment of femoral neck fracture in young and middle-aged patients.Methods:A retrospective cohort study was conducted to analyze the clinical data of 83 young and middle-aged patients with femoral neck fracture admitted to the 920th Hospital of Joint Logistic Support Force of PLA from January 2018 to January 2023, including 56 males and 27 females, aged 28-55 years [(42.7±3.2)years]. According to Garden classification, the fractures were classified as type III in 22 patients and type IV in 61. Based on Pauwels classification, the fractures were classified as type I in 15 patients, type II in 38 and type III in 30. Forty patients were treated with cannulated screw internal fixation combined with modified quadratus femoris bone flap (cannulated screw combined with bone flap group) and 43 with cannulated screw internal fixation alone (cannulated screw group). The two groups were compared in terms of the operation time, intraoperative blood loss, time to weight-bearing, length of hospital stay, and wound healing. The visual analogue scale (VAS) scores and Harris hip function scores at 1, 3, 6, 12 months after surgery and at the last follow-up. The postoperative complication rate was detected.Results:All the patients were followed up for 20-70 months [(40.0±1.2)months]. The operation time and intraoperative blood loss were (105.2±2.7)minutes and (100.6±16.3)ml in the cannulated screw combined with bone flap group, which were longer or more than (92.4±4.7)minutes and (92.5±14.6)ml in the cannulated screw group ( P<0.01). The time to weight-bearing was (12.1±1.4)weeks in the cannulated screw combined with bone flap group, shorter than (23.6±1.2)weeks in the cannulated screw group ( P<0.01). There was no statistically significant difference in the length of hospital stay between the two groups (P>0.05). The incisions in both groups were healed by first intention. At 1 month after surgery, no statistically significant difference was observed in VAS scores between the two groups ( P>0.05); at 3, 6, 12 months after surgery and at the last follow-up, the VAS scores were (6.6±0.2)points, (4.5±0.3)points, (3.2±0.5)points, and (2.6±0.4)points in the cannulated screw combined with bone flap group, lower than (7.0±0.1)points, (5.2±0.2)points, (3.9±0.4)points, and (3.3±0.1)points in the cannulated screw group ( P<0.05 or 0.01). At 1 and 3 months after surgery, no statistically significant difference was observed in the Harris hip function scores between the two groups ( P>0.05); at 6, 12 months after surgery and at the last follow-up, the Harris hip function scores were (82.2±1.7)points, (90.0±1.4)points, and (91.6±1.0)points in the cannulated screw combined with bone flap group, higher than (75.2±1.7)points, (83.4±1.9)points, and (85.2±0.7)points in the cannulated screw group ( P<0.01). At the last follow-up, in the cannulated screw combined with bone flap group, the Harris hip function was rated excellent in 32 patients, good in 5, and fair in 3, with an excellent and good rate of 92.5%, while in the cannulated screw group, the Harris hip function was rated excellent in 20 patients, good in 13, and fair in 10, with an excellent and good rate of 76.7% ( P<0.05). The postoperative complication rate was 5.0% (2/40) in the cannulated screw combined with bone flap group, significantly lower than 23.2% (10/43) in the cannulated screw group ( P<0.05). Conclusion:Compared with cannulated screw internal fixation alone, cannulated screw internal fixation combined with quadratus femoris bone flap with preservation of the posterior superior retinaculum has the advantages of earlier weight-bearing, less pain, better recovery of hip joint function, and lower incidence of postoperative complications in the treatment of femoral neck fracture in young and middle-aged patients, despite longer operation time and more intraoperative blood loss.
3.Dosimetry effect of fluence smoothing in Monaco Treatment Planning System for short-course volumetric modulated arc therapy of preoperative rectal cancer

Objective To investigate the dosimetric differences in preoperative short-course volumetric modulated arc therapy(VMAT)for rectal cancer using different fluence smoothing(FS)levels in the Monaco Treatment Planning System(Monaco TPS).Methods Twenty rectal cancer patients who received preoperative neoadjuvant short-course VMAT at some hospital from September 2021 to December 2022 were retrospectively selected.Four groups of radiotherapy plans were formulated using the Monaco TPS for each case,which were classified into an off group,a low group,a medium group and a high group based on the FS levels.Then the four groups were compared in terms of the dosimetric parameters,monitor unit and number of the segments in the planning target volume(PTV)and organ at risk(OAR).Statistical analysis was performed using SPSS 27.0 software.Results All the four groups had the doses to the target volume meeting clinical requirements,which had no significant differences in the doses to 5%(D5%)and 95%(D95%)to the target volume and the maximum dose(Dmax),minimum dose(Dmin),mean dose(Dmean)and conformity index(all P>0.05).Statistical differences were found between the homogeneity indexes of the four groups(P<0.05),with the medium group behaving the best.The number of the segments rose while the mornitor units decreased siginificantly with the increase of FS levels,with the differences being statistically significant(P<0.05).There were no significant differences between the V25,V20,V15 and V10 of the small intestine,the V25 and V20 of the bladder and the V15 and V10 of the left and right femur(all P>0.05).Conclusion In preoperative short-course VMAT for rectal cancer,clinical requirements can be met with different FS levels in the Monaco TPS,and medium-level FS results in optimal overall dose distribution in terms of treatment planning.[Chinese Medical Equipment Journal,2025,46(5):48-53]
4.Epidemiological characteristics of Mycoplasma pneumoniae infection among hospitalized children with respiratory tract infections at Children's Hospital of Hebei Province,2016-2024

Objective To investigate the epidemiological characteristics of Mycoplasma pneumoniae(MP)infection among pediatric inpatients with respiratory tract infections(RTIs)at Children's Hospital of Hebei Province,providing evidence for clinical diagnosis and treatment.Methods A retrospective analysis was conducted on 79 546 children hospitalized for RTIs between January 2016 and February 2024.Nasopharyngeal aspirates or deep sputum samples were collected,and polymerase chain reaction(PCR)was used to detect nucleic acids of 13 respiratory pathogens,including MP and adenovirus.The epidemiological trends across different years,seasons,genders,and age groups were analyzed.Results Among the 79 546 enrolled cases(male:47 437,59.6%;female:32 109,40.4%),the MP-positive rate was 17.7%(14 106/79 546),peaking in 2023(28.8%)and reaching the lowest in 2021(7.0%).Except for the period from July 2020 to March 2021 with exceptionally low MP positivity,epidemic peaks consistently occurred between August and February or March of the following year.Seasonal analysis revealed significantly higher MP positivity in autumn and winter compared to spring(P<0.001).Female children exhibited a higher MP-positive rate(20.1%,6444/32 109)than males(16.2%,7662/47 437)(P<0.001).The MP-positive rate increased with age:infancy(3.2%,849/26 741),toddlerhood(9.3%,1935/20 763),preschool(21.2%,3918/18 448),and school-age(54.5%,7404/13 594)(P<0.001).Co-infections with other respiratory pathogens were observed in 37.3%(5264/14 106)of MP-positive cases,with human rhinovirus(HRV)being the most frequent co-pathogen(43.3%,2279/5264 of mixed infections).Conclusion MP is a major pathogen of RTIs in hospitalized children at Children's Hospital of Hebei Province.Infections occur year-round but predominantly in autumn,with higher susceptibility in females and school-age children.Targeted preventive measures should be implemented during peak seasons to mitigate transmission risks.

6.circHERC4_041 Inhibits the Fibrotic Phenotype of Cardiac Fibroblasts by Encoding Protein

A mounting body of research suggests that circRNAs significantly contribute to the develop-ment of myocardial fibrosis.The microarray results of human circular RNA expression profile indicated that circHERC4_041 expression increased in the myocardium of patients with heart failure,RT-qPCR a-nalysis confirmed that the myocardial expression level of circHERC4_041 in individuals with heart failure were considerably elevated compared to that in healthy organ donors.Fluorescence in situ hybridization(FISH)confirmed that circHERC4_041 was abundant in the cytoplasm of human cardiomyocyte AC16.Overexpression of circHERC4_041 in mouse myocardial fibroblasts(mCFs)mediated by adenovirus in-hibited the expression of fibrosis-related proteins in mCFs.Experiments involving cell proliferation,wound healing,and Transwell assays demonstrated that overexpression of circHERC4_041 suppressed the growth and mobility of mCFs(P<0.001).Sequence analysis results suggested that circHERC4_041 con-tains potential ribosome entry sequence(IRES)and open reading frame(ORF).Western blot confirmed that circHERC4_041 could translate the 516 amino acid HERC4-516aa protein,which was mainly located in the cytoplasm of the cell.Cell functional experiments confirmed that circHERC4_041 inhibited the fi-brotic phenotype of mCFs by specifically translating HERC4-516aa(P<0.05).The specific interaction between HERC4-516aa and transglutaminase 2(TGM2)was confirmed by IP-MS screening and Co-IP i-dentification.Further results found that the degradation of TGM2 was promoted through proteasome path-way.The overexpression of TGM2 in mCFs facilitated by adenoviral vectors could counteract the suppres-sive effects of HERC4-516aa on the fibrotic phenotype of mCFs.Therefore,this study confirmed that the HERC4-516aa protein translated by circHERC4_041 can specifically bind to TGM2 to inhibit the fibrotic phenotype of myocardial fibroblasts.

9.Effects of Tripterygium wilfordii polyglycosides on a rat model of pulmonary fibrosis via miR-146/NF-κB signaling pathway

AIM To investigate the role of Tripterygium wilfordii polyglycosides as an inhibitor of pulmonary fibrosis progression in rats.METHODS In contrast to the six rats randomly selected into the blank group,another 34 rats were induced into a rat model of pulmonary fibrosis by bleomycin sulfate ( 5 mg/mL) injection using tracheal puncture followed by 7 days later random assignment of the survival thirty rats into the model group,the prednisone acetate group (1.17 mg/kg) and the low,medium and high dose tripterygium wilfordii polyglycosides groups (1,3,6 mg/kg) for 21 days corresponding dosing of the drugs.The general characteristics of rats were observed during administration.After the administration,the rats had their pulmonary morphology observed;their lung coefficients of each group compared;their structural changes of lung tissue observed using HE staining;their collagen deposition observed by Masson staining;and their hyperplastic conditions assessed by the criteria for judging pulmonary fibrous hyperplasia;their serum HYP,TNF-α,IL-1β and IL-6 levels detected by ELISA;their pulmonary expressions of miR-146a,NF-κB and Col-I mRNA detected by RT-qPCR method;and their pulmonary protein expressions of p-P65,IRAK1,TRAF6 and α-SMA detected by Western blot.RESULTS Compared with the blank group,the model group displayed increased levels of serum TNF-α,IL-6,IL-1β and HYP ( P<0.01);decreased pulmonary miR-146a mRNA expression ( P<0.01);and increased expressions of NF-κB,Col-I mRNA and p-P65,IRAK1,TRAF6 and α-SMA protein ( P<0.01 ).Compared with the model group,the groups intervened with either prednisone acetate or Tripterygium wilfordii polyglycoside shared differently improved alveolar structure and inflammatory cell infiltration;reduced fibrosis;increased pulmonary miR-146a mRNA expression ( P<0.01);increased levels of TRAF6-α,IL-6,IL-1β and HYP in serum,and expressions of NF-κB and Col-I mRNA and expression of p-P65,IRAK1,TRAF6 and α-SMA protein of the lung tissue ( P<0.05,P<0.01) as well.CONCLUSION Tripterygium wilfordii polyglycosides can delay the progression of pulmonary fibrosis in rats,and its mechanism may be related to the regulation via miR-146/NF-κB signaling pathway.

AIM To investigate the role of Tripterygium wilfordii polyglycosides as an inhibitor of pulmonary fibrosis progression in rats.METHODS In contrast to the six rats randomly selected into the blank group,another 34 rats were induced into a rat model of pulmonary fibrosis by bleomycin sulfate ( 5 mg/mL) injection using tracheal puncture followed by 7 days later random assignment of the survival thirty rats into the model group,the prednisone acetate group (1.17 mg/kg) and the low,medium and high dose tripterygium wilfordii polyglycosides groups (1,3,6 mg/kg) for 21 days corresponding dosing of the drugs.The general characteristics of rats were observed during administration.After the administration,the rats had their pulmonary morphology observed;their lung coefficients of each group compared;their structural changes of lung tissue observed using HE staining;their collagen deposition observed by Masson staining;and their hyperplastic conditions assessed by the criteria for judging pulmonary fibrous hyperplasia;their serum HYP,TNF-α,IL-1β and IL-6 levels detected by ELISA;their pulmonary expressions of miR-146a,NF-κB and Col-I mRNA detected by RT-qPCR method;and their pulmonary protein expressions of p-P65,IRAK1,TRAF6 and α-SMA detected by Western blot.RESULTS Compared with the blank group,the model group displayed increased levels of serum TNF-α,IL-6,IL-1β and HYP ( P<0.01);decreased pulmonary miR-146a mRNA expression ( P<0.01);and increased expressions of NF-κB,Col-I mRNA and p-P65,IRAK1,TRAF6 and α-SMA protein ( P<0.01 ).Compared with the model group,the groups intervened with either prednisone acetate or Tripterygium wilfordii polyglycoside shared differently improved alveolar structure and inflammatory cell infiltration;reduced fibrosis;increased pulmonary miR-146a mRNA expression ( P<0.01);increased levels of TRAF6-α,IL-6,IL-1β and HYP in serum,and expressions of NF-κB and Col-I mRNA and expression of p-P65,IRAK1,TRAF6 and α-SMA protein of the lung tissue ( P<0.05,P<0.01) as well.CONCLUSION Tripterygium wilfordii polyglycosides can delay the progression of pulmonary fibrosis in rats,and its mechanism may be related to the regulation via miR-146/NF-κB signaling pathway.
