1.Correlation Analysis between Disease Duration and Anxiety,Depression,Sleep Disorders and Quality of Life in Patients with Inflammatory Bowel Disease

Objective To explore the correlation between disease duration and anxiety,depression,sleep quality and quality of life in patients with inflammatory bowel disease(IBD).Methods A cross-sectional survey study was conducted from September 2021 to May 2022 in 42hospitals in 22 provinces(autonomous regions and municipalities directly under the central government).General clinical information and disease duration of adult patients with IBD who voluntarily participated in the study were collected,and the anxiety,de-pression,sleep quality,and quality of life of IBD patients were assessed by using the Generalized Anxiety Disorder(GAD-7),the Pa-tient Health Questionnaire-9(PHQ-9),the Pittsburgh Sleep Quality Index(PSQI),and the Inflammatory Bowel Disease Questionnaire(IBDQ).Results A total of 2476 valid questionnaires were collected.Among them,458(18.50%)patients had a disease duration within 1 year,481(19.43%)patients had a disease duration of 1-2 years,764(30.86%)had a disease duration of 2-5 years,529(21.37%)had a disease duration of 5-10 years,and 244(9.85%)had a disease duration of more than 10 years.The GAD-7score(P=0.005),PHQ-9score(P=0.002),and PSQI score(P=0.005)were statistically significant in different disease duration groups.In pairwise comparisons,IBD patients with a disease duration of 5-10 years were more likely to have depression,anxiety,and sleep disorder.IBDQ scores were statistically significant in different disease duration groups(P=0.005).Among them,bowel symptom score(P=0.001),emotional competence score(P=0.013),and social competence score(P<0.001)were statistically significant.Patients with a disease duration of 2-5 years seemed to have a better quality of life,including milder bowel symptoms,better emotional,and social competence.Patients with a disease duration of 10 years or more had better social competence.Conclusion Anxiety,depres-sion,sleep disorders,and quality of life are associated with disease duration in patients with IBD,and patients with IBD with a disease duration of 5-10 years are more likely to have depressive states,anxiety and sleep states.
2.Influence of platelet-albumin-bilirubin score on textbook outcome of patients with hepatocellular carcinoma after hepatectomy

Objective To investigate the influence of platelet-albumin-bilirubin(PALBI)score on the textbook outcome(TO)of patients with hepatocellular carcinoma(HCC)after hepatectomy,as well as the association of different PALBI scores before surgery with the achievement of TO after hepatectomy in HCC patients.Methods A retrospective analysis was performed for the data of HCC patients who underwent hepatectomy in West China Hospital of Sichuan University and Ziyang Central Hospital from January 2013 to January 2022.TO was defined as no serious complication within 30 days after surgery,no death within 90 days,no rehospitalization within 30 days after discharge,no blood transfusion in the perioperative period,RO resection,and no prolongation of hospital stay.The chi-square test was used for comparison of categorical data between two groups.The univariate and multivariate Logistic regression analyses were used to investigate the influencing factors for the achievement of TO after hepatectomy in HCC patients.The Kaplan-Meier method was used to plot the survival curves of HCC patients,and the Log-rank test was used for comparison.Results A total of 3 599 patients were included in this study,among whom 2 369(65.8%)achieved TO.The multivariate Logistic regression analysis showed that PALBI grade(PALBI grade 2:odds ratio[OR]=1.562,95%confidence interval[CI]:1.308-1.864,P<0.001;PALBI grade 3:OR=2.216,95%CI:1.463-3.359,P<0.001)was an independent risk factor for achievement of TO after surgery in HCC patients.The proportion of patients achieving TO decreased with the increase in PALBI grade.Among the patients with PALBI grade 1,2 or 3,the patients achieving TO accounted for 70.2%,54.2%,and 38.4%,respectively(χ2=106.295,P<0.001).The incidence rate of serious complications within 30 days,the mortality rate of patients within 90 days after hepatectomy,readmission rate within 30 days after discharge,perioperative blood transfusion rate,and the rate of prolonged hospital stay all increased with the increase in PALBI grade(all P<0.05).For the patients achieving TO,the 1-,3-,and 5-year relapse-free survival rates were 79.5%,60.6%,and 51.5%,respectively,and the overall survival rates were 92.1%,80.0%,and 71.1%,respectively;for those who did not achieve TO,the 1-,3-,and 5-year relapse-free survival rates were 68.5%,52.7%,and 46.2%,respectively,and the overall survival rates were 83.3%,66.0%,and 57.1%,respectively.The patients who achieved TO had significantly better relapse-free survival rate and overall survival rate than those who did not achieve TO(χ2=18.936 and 79.371,both P<0.001).Conclusion Preoperative PALBI grade can affect the achievement of TO after hepatectomy in HCC patients,and it is more difficult for patients with a higher PALBI grade to achieve TO.Preoperative PALBI score can be used to early identify the patients with a high risk of postoperative complications,provide early intervention,and enhance perioperative management,thereby improving the perioperative safety and long-term prognosis of HCC patients after hepatectomy.
3.One-pot Synthesis of Sulfhydryl-protected Fluorescent Silver Nanoclusters and Its Application in Detection of Tetracycline

Water-soluble silver nanoclusters(DHLA-AgNCs)with red fluorescence emission were synthesized using silver nitrate(AgNO3)as silver source,dihydrolipoic acid(DHLA)as ligand and sodium borohydride(NaBH4)as reducing agent by one-pot method.Based on the selective quenching of DHLA-AgNCs by tetracycline(TC),a rapid and selective fluorescence analysis method for detection of TC was constructed by monitoring the fluorescence intensity change at 650 nm.Under the optimal detection conditions,the fluorescence quenching efficiency of DHLA-AgNCs showed good linear relationship with concentration of TC within the range of 10.0-120.0 μmol/L,and the limit of detection(LOD)was 0.39 μmol/L.This method was successfully applied to detection of TC in milk samples,with spiked recoveries ranging from 99.5%to 102.5%,and relative standard deviations(RSDs)less than 5%.This method had the advantages of simplicity,rapidity,strong specificity and low cost,and thus provided a simple and feasible strategy for selective detection of TC.

6.Transient Expression of Monkeypox Virus Recombinant Protein B6R-Fer in Nicotiana benthamiana

Monkeypox is a viral zoonotic disease,and there is currently a lack of safe and effective vac-cines against the monkeypox virus.Therefore,screening and developing vaccine candidates is of signifi-cant practical importance.With the rapid advancement of molecular biology and plant genetic engineer-ing,plant bioreactors offer promising potential for producing vaccine proteins due to their advantages,in-cluding safety,cost-effectiveness,and scalability.In this study,we focused on the monkeypox protein B6R.The recombinant expression plasmid pFolia40108-B6R-Fer was successfully constructed using am-plification,enzyme digestion,and flexible linker tandem ferritin technology.A complete transient expres-sion system in Nicotiana benthamiana and a purification system for the recombinant monkeypox protein were established.The optimal expression time was determined to be 12-14 days,with a final purified pro-tein concentration of approximately 1 mg/mL and a yield of 0.85 mg/kg fresh weight.The purified B6R-Fer recombinant protein self-assembled into spherical virus-like particles(VLPs)with an average particle size of 24 nm.The B6R-Fer recombinant protein from this study shows promising potential for use in the development and screening of plant-derived monkeypox vaccine candidates.

8.Application of 3D-T1WI surface-based morphometry in assessment of brain structure in preschool children with non-lesional epilepsy

Objective To evaluate the changes of brain structure in preschool children with non-lesional epilepsy(NLE)by analyzing their 3D-T1WI brain structural images with surface-based morphometry(SBM).Methods Thirty-five preschool children with diagnosed NLE in the neurology department of some hospital from January 2021 to September 2022 were enrolled into a NLE group,and 35 healthy preschool children from the same period were included in a control group.The two groups were compared in terms of cortical thickness,sulcus depth,gyrus index,cerebrospinal fluid volume,gray matter volume and white matter volume.SPSS 25.0 software was used for statistical analysis.Results When compared with the control group,the NLE group had the thicknesses of temporal lobe cortex decreased at both left and right sides while the thickness of cingulate cortex increased on the right,and the differences were statistically significant(P<0.05);the NLE group had the depths of sulcus in lingual gyrus and middle frontal gyrus of right cerebral hemisphere reduced,and the differences were statistically significant(P<0.05);the NLE group had the gyrus index of left cerebral hemisphere talus cortex highered,and the differences were statistically significant(P<0.05).There were no significant differences between the two groups in cerebrospinal fluid,gray matter and white matter(P>0.05).Conclusion Cortical structural abnormalities in children with NLE may be a major factor in its pathogenesis.SBM can be used to evaluate localized brain structural abnormalities in NLE children by analyzing their 3D-T1WI brain structural images,and thus can be an effective tool for determining the potential pathogenesis of NLE in children.[Chinese Medical Equipment Journal,2024,45(2):62-66]
9.Clinical efficacy analysis of the SCCLG-ALL-2016 protocol for the treatment of pediatric B-ALL with CD20+

Abstract: Objective To analyze the clinical data of children with B-cell acute lymphoblastic leukemia (B-ALL) treated under the SCCLG-ALL-2016 protocol, to explore the impact of CD20 expression on the prognosis of children with B-ALL, and to provide a scientific basis for future clinical personalized and precision clinical treatment. Methods A retrospective analysis of the clinical data of 273 newly diagnosed B-ALL children treated by the South China Children's Acute Lymphoblastic Leukemia Treatment Cooperation Group from six centers between October 2016 and June 2023 was conducted. Clinical features, chromosomal karyotypes, fusion genes, clinical risk stratification, early and long-term outcomes were analyzed, and survival analysis was performed by plotting survival curve methods. Results Among the 273 newly diagnosed B-ALL pediatric patients, the CD20-positive expression rate was 26.9%. When compared to CD20-negative individuals, there were statistically significant differences in terms of leukemia gene fusion and mutations (P<0.05). However, no statistically significant differences were observed in initial age, gender, initial white blood cell count, liver-spleen condition, chromosomal karyotype, induction bone marrow leukemia residual on days 15 and 33, and relapse (P>0.05). The CD20-positive group had a total survival rate (OS) of (71.9±10.1)%, lower than the CD20-negative group which was (86.8±4.0)%. The 3-year event-free survival rate (EFS) was (81.2±5.6)%, lower than the CD20-negative group which was (89.5±2.2)%, but the difference between the two groups was not statistically significant (P>0.05). Moreover, there were no statistically significant differences in the total survival rate and 3-year EFS between CD20-positive patients with or without IKZF1 segment deletion (P>0.05). Univariate and multivariate Cox regression analyses related to 3-year EFS and OS indicated that spleen condition and risk stratification were related to 3-year EFS in the univariate analysis; multivariate analysis showed that spleen condition and risk stratification were independent prognostic factors affecting 3-year EFS. Factors related to OS in the univariate analysis were spleen condition, bone marrow on day 15, risk stratification, and relapse, with multivariate analysis indicating that relapse was an independent prognostic factor affecting OS. Conclusions In pediatric B-ALL, CD20 expression is related to the expression of fusion genes, yet it is not a prognostic factor.

Pharmacokinetics of traditional Chinese medicine(TCM)is a discipline that adopts pharmacokinetic research methods and techniques under the guidance of TCM theories to elucidate the dynamic changes in the absorption,distribution,metabolism and excretion of active ingredients,active sites,single-flavour Chinese medicinal and compounded formulas of TCM in vivo.However,the sources and components of TCM are complex,and the pharmacodynamic substances and mechanisms of action of the majority of TCM are not yet clear,so the pharmacokinetic study of TCM is later than that of chemical medicines,and is far more complex than that of chemical medicines,and its development also confronts with challenges.The pharmacokinetic study of TCM originated in the 1950s and has experienced more than 70 years of development from the initial in vivo study of a single active ingredient,to the pharmacokinetic and pharmacodynamic study of active ingredients,to the pharmacokinetic study of compound and multi-component of Chinese medicine.In recent years,with the help of advanced extraction,separation and analysis technologies,gene-editing animals and cell models,multi-omics technologies,protein purification and structure analysis technologies,and artificial intelligence,etc.,the pharmacokinetics of TCM has been substantially applied in revealing and elucidating the pharmacodynamic substances and mechanisms of action of Chinese medicines,research and development of new drugs of TCM,scientific and technological upgrading of large varieties of Chinese patent medicines,as well as guiding the rational use of medicines in clinics.Pharmacokinetic studies of TCM have made remarkable breakthroughs and significant development in theory,methodology,technology and application.In this paper,the history of the development of pharmacokinetics of TCM and the progress of cutting-edge research was reviewed,with the aim of providing ideas and references for the pharmacokinetics of TCM and related research.
