BACKGROUND:Asperosaponin Ⅵ has good osteogenic effects,but its ability to promote cellular osteogenesis under hypoxia environment is not yet clear.OBJECTIVE:To investigate the effect and potential mechanism of Asperosaponin Ⅵ on osteogenic differentiation of MC3T3-E1 cells under hypoxia environment.METHODS:MC3T3-E1 cells were divided into three groups.Cells in the control group were cultured in a complete medium under normoxic conditions(volume fraction of 21％O2);cells in the hypoxia group were cultured in the complete medium under hypoxia conditions(volume fraction of 0.5％O2);and cells in the Asperosaponin Ⅵ group were cultured in the complete medium containing Asperosaponin Ⅵ under hypoxia conditions(volume fraction of 0.5％oxygen).After 24 hours of culture,cell counting kit-8 method and EdU staining were used to detect cell proliferation activity,TUNEL staining and western blot assay were performed to detect cell apoptosis,and flow cytometry was used to detect intracellular reactive oxygen species levels and cell cycle distribution.Each group of cells was cultured in an osteogenic induction medium containing 1×10-5,1×10-6,and 1×10-7 mol/L Asperosaponin Ⅵ under hypoxia conditions for 7 days.The optimal concentration of Asperosaponin Ⅵ for intervention was identified using alkaline phosphatase staining under optical microscopy.Western blot was used to detect the expression of bone morphogenetic protein 2,osteopontin,and PI3K/AKT signaling axis-related proteins.RESULTS AND CONCLUSION:(1)Compared with the control group,the proliferation ability of MC3T3-E1 cells decreased under hypoxia conditions.1×10-6 mol/L Asperosaponin Ⅵ could significantly improve the cell proliferation ability under hypoxia conditions and reduce cell apoptosis.(2)Compared with the hypoxia group,the Asperosaponin Ⅵ group showed a decrease in intracellular reactive oxygen species and a significant increase in the proportion of cells in the S phase.(3)Compared with the hypoxia group,the cell morphology in the 1×10-6 mol/L Asperosaponin Ⅵ group was elongated,with more protrusions and darker alkaline phosphatase staining.(4)Compared with the hypoxia group,the expression of bone morphogenetic protein 2 and osteopontin increased in the Asperosaponin Ⅵ group,while the expression of p-PI3K and p-AKT proteins decreased.These findings indicate that under hypoxia conditions,Asperosaponin Ⅵcan promote osteogenic differentiation of MC3T3-E1 cells,possibly by regulating the PI3K/AKT pathway.

Objective:To explore the efficacy and safety of one-hole split endoscope (OSE) minimally invasive surgery for the treatment of single-segment thoracic ossification of the ligamentum flavum (TOLF).Methods:This retrospective non-randomized controlled study included 41 patients with single-segment TOLF who underwent surgery at Qilu Hospital of Shandong University between July 2019 and July 2023. Patients were divided into two groups: the OSE group (19 cases) treated with one-hole split endoscope minimally invasive surgery and the open group (22 cases) treated with traditional laminectomy and pedicle screw fixation. There were no significant differences between the two groups on gender, age, disease duration, affected segment, presence or absence of dural ossification, and residual cross-sectional vertebral canal area on CT ( P>0.05). Additionally, perioperative surgical time, estimated blood loss (EBL), incision length, hospital stay duration, hospitalization costs and follow-up duration were compared. The Japanese Orthopaedic Association (JOA) score and Oswestry Disability Index (ODI) were compared preoperatively and at the last follow-up. Complications were also recorded. Results:All patients successfully completed the surgery with no significant differences at the last follow-up ( P>0.05). Compared with the open group, the OSE group had a significantly shorter operative time (133.1±16.8 vs. 160.5±22.6 min), lower EBL (91.2±15.0 vs. 192.5±43.8 ml), shorter incision length (2.6±0.5 vs. 7.9±1.9 cm), reduced hospital stay (3.9±0.8 vs. 5.6±0.8 days), and lower hospitalization costs (34,874.9±4,568.6 vs. 53,162.3±9,815.6 yuan) (all P<0.05). AAt the final follow-up, JOA scores (8.5±0.8 vs. 8.6±1.2) and ODI values (16.7%±2.1% vs. 17.7%±4.4%) showed no significant differences between the OSE and open groups ( P>0.05). During the perioperative period and follow-up, complications occurred in 2 patients in the OSE group (1 cerebrospinal fluid leak, 1 poor wound healing) and in 8 patients in the open group (5 cerebrospinal fluid leaks, 1 neurological deterioration, 2 poor wound healing). Conclusion:OSE minimally invasive surgery is an effective treatment for single-segment thoracic ossification of the ligamentum flavum. Compared with open surgery, it provides advantages such as minimal invasiveness and fewer complications.

Purpose/Significance Based on the concepts of healthy cities and smart cities,the paper introduces a new concept-smart healthy cities.It explores the definition,scope,function,challenges,and strategic responses associated with the concept.Meth-od/Process Through literature review and case study analysis,the theoretical foundations,characteristics,objectives,and implementa-tion strategies of healthy cities and smart cities are compared,revealing differences and points of convergence,proposing the origins,def-initions,and delineations of smart healthy cities,and exploring the relationships among healthy cities,smart cities,and smart healthy cit-ies.Result/Conclusion Smart healthy cities enhance urban health governance and the well-being of residents through technological in-novation.Effective integration of advanced technologies with urban governance policies is essential,alongside the implementation of di-verse strategies to drive progress.Future urban development should focus more on the theoretical and practical collaborative development within the smart healthy cities framework.

Objective:To investigate the prevalence and distribution of childhood maltreatment, internet addiction, and emotional and behavioral problems among middle school students in Guangzhou, and to explore the relationship between childhood maltreatment and internet addiction and the mediating role of emotional and behavioral problems.Methods:A total of 1 957 students from 10 middle schools in Guangzhou were recruited using multistage cluster random sampling and were investigated with anonymous questionnaires including the general demographic data, the Childhood Trauma Questionnaire-Short form (CTQ-SF), the Internet Addiction Test (IAT) and the Strengths and Difficulties Questionnaire (SDQ). Linear regression analysis was conducted to explore the relationship between childhood maltreatment and internet addiction, and the Bootstrap approach was used to reveal the potential mediating effect of emotional and behavioral problems on the relationship between childhood maltreatment and internet addiction.Results:The prevalence of internet addiction among middle school students in Guangzhou was 14.4%(270/1 873), and the median ( IQR) score for IAT was 33 (16). The linear regression analysis revealed that after adjusting for potential confounders, trauma exposure (β=2.217, P<0.001) and abuse exposure (β=4.689, P<0.001) were positively associated with internet addiction. Bootstrap approach and path analysis showed that there was a partial mediating effect of internal problems (β=0.305, 95% CI=0.154-0.480) and external problems (β=1.306, 95% CI=0.992-1.667) on the relationship between trauma exposure and internet addiction. Meanwhile, internal problems (β=0.413, 95% CI=0.185-0.678) and external problems (β=1.835, 95% CI=1.338-2.379) also partially mediate the relationship between abuse exposure and internet addiction. Conclusion:There is a relatively high prevalence of internet addiction among middle school students in Guangzhou. Childhood maltreatment is a risk factor for internet addiction among middle school students, and there is a partial mediating effect of emotional and behavioral problems.

Jinlingzi San is a formula frequently used in treating pain syndrome， first recorded in the Collection of Writings on the Mechanism of Disease， Suitability of Qi， and the Safeguarding of Life as Discussed in the 'Basic Questions' written by LIU Wansu in the Jin Dynasty. Jinlingzi San is composed of 2 Chinese medicinals Toosendan Fructus and Corydalis Rhizoma with a concise composition and exact clinical efficacy， having been included in the Catalogue of Ancient Classical Formulas （Second Batch： Han Chinese Medicine）. The formula name， historic evolution， medicine origins， composition， dosage， decocting methods， and ancient and modern clinical application were sorted out and analyzed with the bibliometric method. A total of 209 pieces of information were collected from ancient books and literature. After screening， 49 pieces of effective data involving 45 ancient books were included. Results showed that the name of Jinlingzi San was first recorded in Secret Formulas of the Yang Family written by Yang Tan in the Southern Song Dynasty and developed into 3 other versions of the decoction. The Jinlingzi San included in the Collection of Writings on the Mechanism of Disease， Suitability of Qi， and the Safeguarding of Life as Discussed in the 'Basic Questions' written by LIU Wansu invariably plays a dominant role. As for the 3 other versions， although they have the same name of Jinlingzi San， their composition and indications are different from those of the original formula， which were therefore viewed as prescriptions based on Jinlingzi San and also included in the research. The medicine origins and processing of Jinlingzi San are suggested： Toosendan Fructus is the dry mature fruit of Melia toosendan of Meliaceae， and the crude is used after cleansing without putamen. Corydalis Rhizoma is the dry tuber of Corydalis yanhusuo of Papaveraceae， which is used after impurity removal， cleaning， and drying. Depending on the conversion from the measurement system in the Jin Dynasty to modern measurement， it is suggested that Toosendan Fructus and Corydalis Rhizoma （41.3 g each） are ground into fine powder， and one dose includes 12.39 g of the powder， which should be taken with an appropriate amount of wine. If wine is not suitable for the patient， the decoction can also be taken with warm water. Jinlingzi San has the effects of soothing the liver， discharging heat， and activating blood to stop pain. As recorded in ancient books， Jinlingzi San is specialized in treating heart pain caused by reversal heat， chest and abdominal pain， hypochondriac pain， jaundice， hernia， and other diseases. Modern studies have shown that modified Jinlingzi San can be used in treating diseases involving the digestive system， the integumentary system， the gynecological system， the reproductive system， and other systems and has wide clinical application in treating epigastric pain， herpes zoster， dysmenorrhea， and other diseases. This study has made clear the key information of Jinlingzi San by textual research of ancient books and literature in the hope of providing a theoretical reference for the clinical application， set prescriptions， and new drug development.
