1.Hepatitis C virus infection:surveillance report from China Healthcare-as-sociated Infection Surveillance System in 2020

Objective To investigate the infection status and changing trend of hepatitis C virus(HCV)infection in hospitalized patients in medical institutions,and provide reference for formulating HCV infection prevention and control strategies.Methods HCV infection surveillance results from cross-sectional survey data reported to China Healthcare-associated Infection(HAI)Surveillance System in 2020 were summarized and analyzed,HCV positive was serum anti-HCV positive or HCV RNA positive,survey result was compared with the survey results from 2003.Results In 2020,1 071 368 inpatients in 1 573 hospitals were surveyed,738 535 of whom underwent HCV test,4 014 patients were infected with HCV,with a detection rate of 68.93%and a HCV positive rate of 0.54%.The positive rate of HCV in male and female patients were 0.60%and 0.48%,respectively,with a statistically sig-nificant difference(x2=47.18,P<0.001).The HCV positive rate in the 50-<60 age group was the highest(0.76%),followed by the 40-<50 age group(0.71%).Difference among all age groups was statistically signifi-cant(x2=696.74,P<0.001).In 2003,91 113 inpatients were surveyed.35 145 of whom underwent HCV test,resulting in a detection rate of 38.57%;775 patients were infected with HCV,with a positive rate of 2.21%.In 2020,HCV positive rates in hospitals of different scales were 0.46%-0.63%,with the highest in hospital with bed numbers ranging 600-899.Patients'HCV positive rates in hospitals of different scales was statistically signifi-cant(X2=35.34,P<0.001).In 2020,12 provinces/municipalities had over 10 000 patients underwent HCV-rela-ted test,and HCV positive rates ranged 0.19%-0.81%,with the highest rate from Hainan Province.HCV posi-tive rates in different departments were 0.06%-0.82%,with the lowest positive rate in the department of pedia-trics and the highest in the department of internal medicine.In 2003 and 2020,HCV positive rates in the depart-ment of infectious diseases were the highest,being 7.95%and 3.48%,respectively.Followed by departments of orthopedics(7.72%),gastroenterology(3.77%),nephrology(3.57%)and general intensive care unit(ICU,3.10%)in 2003,as well as departments of gastroenterology(1.35%),nephrology(1.18%),endocrinology(0.91%),and general intensive care unit(ICU,0.79%)in 2020.Conclusion Compared with 2003,HCV positive rate decreased significantly in 2020.HCV infected patients were mainly from the department of infectious diseases,followed by departments of gastroenterology,nephrology and general ICU.HCV infection positive rate varies with gender,age,and region.
2.Expression of miR-205 and miR-367 in cutaneous malignant melanoma tissue and their clinical significances

Objective To study the expression of miR-205 and miR-367 in cutaneous malignant melanoma(CMM)tissue and their relationships with clinicopathological characteristics and prognosis.Methods The tumor tissues from 85 patients with CMM who were admitted to our hospital from May 2013 to December 2019 were selected as the CMM group,and the nevus tissue samples from 80 patients with benign pigmented nevi who were treated in our hospital during the same period were selected as the control group.The expression level of miR-205 and miR-367 of two groups were detected by qRT-PCR.The clinical and pathological data of patients with CMM were collected,and the relationships between the expression of miR-205 and miR-367 and clinicopathological characteristics were analyzed.Kaplan-Meier was used to draw the survival curves of patients 3 years after surgery,and the survival rate of patients was analyzed by Log-rank test.The influencing factors of prognosis was analyzed by COX proportional risk regression model.Results The expression level of miR-205 of patients in the CMM group was lower than that in the control group,and the expression level of miR-367 was higher than that in the control group(P<0.05).The proportions of patients with ulceration,high clinical stage,tumor invasion,low KPS score,thick primary lesion,high Clark grade,lymph node metastasis in the miR-205 low-expression group were higher than those in the miR-205 high-expression group(P<0.05),the proportions of patients with the above indexes in the miR-367 high-expression group were higher than those in the miR-367 low-expression group(P<0.05).Patietns were followed up for 3 years,with a median time of 28.3 months,and 1 case was lost to followed-up.The Kaplan-Meier survival curve showed that the 3-year survival rate of patients in the miR-205 high-expression group was higher than that in the miR-205 low-expression group(P<0.05),and the 3-year survival rate of patients in the miR-367 low-expression group was higher than that in the miR-367 high-expression group(P<0.05).COX multivariate regression analysis showed that the high clinical stage,low KPS score,high Clark grade,lymph node metastasis,and high expression of miR-367 were the risk factors for prognosis of patients,while high expression of miR-205 was a protective factor(P<0.05).Conclusion Low expression of miR-205 and high expression of miR-367 in CMM tissue are associated with ulceration,high clinical stage,tumor invasion,low KPS score,thick primary lesion,high Clark grade,lymph node metastasis,and poor prognosis.These two indicators may serve as potential biomarkers for prognostic evaluation in CMM patients.
3.Lifestyle improvement and the reduced risk of cardiovascular disease: the China-PAR project.

BACKGROUND:
The benefits of healthy lifestyles are well recognized. However, the extent to which improving unhealthy lifestyles reduces cardiovascular disease (CVD) risk needs to be discussed. We evaluated the impact of lifestyle improvement on CVD incidence using data from the China-PAR project (Prediction for Atherosclerotic Cardiovascular Disease Risk in China).
METHODS:
A total of 12,588 participants free of CVD were followed up for three visits after the baseline examination. Changes in four lifestyle factors (LFs) (smoking, diet, physical activity, and alcohol consumption) were assessed through questionnaires from the baseline to the first follow-up visit. Cox proportional hazard models were used to estimate hazard ratios (HRs) and corresponding 95% confidence intervals (CIs). The risk advancement periods (RAPs: the age difference between exposed and unexposed participants reaching the same incident CVD risk) and population-attributable risk percentage (PAR%) were also calculated.
RESULTS:
A total of 909 incident CVD cases occurred over a median follow-up of 11.14 years. Compared with maintaining 0-1 healthy LFs, maintaining 3-4 healthy LFs was associated with a 40% risk reduction of incident CVD (HR = 0.60, 95% CI: 0.45-0.79) and delayed CVD risk by 6.31 years (RAP: -6.31 [-9.92, -2.70] years). The PAR% of maintaining 3-4 unhealthy LFs was 22.0% compared to maintaining 0-1 unhealthy LFs. Besides, compared with maintaining two healthy LFs, improving healthy LFs from 2 to 3-4 was associated with a 23% lower risk of CVD (HR = 0.77, 95% CI: 0.60-0.98).
CONCLUSIONS
Long-term sustenance of healthy lifestyles or improving unhealthy lifestyles can reduce and delay CVD risk.
4.Association of Late-life Marital Status with Mild Cognitive Impairment and Dementia in Old Age

ObjectiveTo explore the relationship between marital status and the prevalence of mild cognitive impairment (MCI) and dementia in the community dwelling old adults, so as to provide sociological data basis for the prevention and control of cognitive impairment. MethodsPeople aged 65 and above in urban and rural areas of Hubei Province were selected by cluster random sampling, and face-to-face interviews were conducted in community health service centers to complete general investigation and neuropsychological evaluation, as well as clinical physical examination and laboratory examination. The diagnostic expert panels, including two neuroscientists and two neuropsychologists with specialized knowledge of dementia, diagnosed MCI and dementia according to Petersen's MCI standard and Chinese Diagnostic guidelines for dementia and Cognitive impairment. Multivariate Logistic regression was used to analyze the correlation between marital status and MCI and dementia in different age groups. ResultsDuring 2018~2020, 8 221 people completed the full neurocognitive assessment and clinical physical examination, with an average age of (71.96±5.895) years, and 1 953 people, accounting for 23.75% of the total number, were without spouses. The crude prevalence of MCI and dementia in HMACS were 25.98%, 95%CI (24.99%, 26.98%) and 7.24%, 95%CI (6.68%,7.80%), respectively in total participants. After standardization for age and sex using data from the 2010 population census in Hubei Province, the prevalence of MCI and dementia were 26.28% and 6.98%.The prevalence of MCI and dementia in non-spouse [28.07%, 95%CI (26.08%, 30.06%);12.45%, 95%CI (10.99%, 13.91%)] was significantly higher than that in spouse [22.85%, 95%CI(21.81%, 23.89%; 5.62%, 95%CI (5.05%, 6.19%)]. Multivariable logistic regression model showed that the rates of MCI and dementia for married men aged 65~69 [P=0.028, OR=0.639,95%CI(0.429, 0.952)] and above 80 years [P=0.004,OR=0.460, 95%CI (0.269, 0.785)] were significantly lower than those of widowed men. However, there was no statistical difference between the presence or absence of spouses and the prevalence of MCI and dementia in older women. ConclusionsThe prevalence of MCI and dementia is higher in older adults without spouses. Especially for men aged 65~69 and 80 and above, having spouses is important to protect cognitive function.
5.Mechanisms and Therapeutic Effects of Human Olfactory Mucosa-Derived Mesenchymal Stem Cells on Experimental Autoimmune Encephalomyelitis in Mice

【Objective】 To study the mechanisms and therapeutic effects of human olfactory mucosa-derived mesenchymal stem cells(OMSC)on experimental autoimmune encephalomyelitis(EAE)in mice.【Methods】Under local anesthesia by using nasal endoscopy,olfactory epithelia of healthy donors were obtained,digested and cultured up to the 5th passage. OMSC were identified,differentiated and stained. EAE models were induced in C57 female mice by myelin oligodendrocyte glycoprotein(MOG35- 55)and pertussis toxin(PT). Neurological function was documented daily. On day 16 after immunization(peak of incidence),the mice were divided randomly into two groups and treated with OMSC and
PBS via tail vein injection respectively. On day 24 after immunization ,blood was collected from angular vein and levels of IL-10,IL-17,IFN-γ and IL-6 were determined by cytometric beads array(CBA). The size of the spinal cord lesion in mice was observed and measured by using HE and LFB staining. Peripheral blood lymphocytes(PBL)of healthy donors were obtained and then co-cultured with OMSC. The proportions of CD4+ T cells secreting IFN- γ(Th1 cells)in lymphocyte group and co-culture group were compared after 2 days of cultivation. Adding IDO or COX pathway inhibitor to co- culture group and cultivating for 2 days,we observed and compared the proportions of Th1 cells in lymphocyte group,co-culture group and inhibitor treatment group respectively.【Results】OMSC exhibited certain mesenchymal stem cell-like characteristics with respect to expression of stem cell surface markers and multilineage differentiation potentials. After induced by MOG35- 55 and PT,EAE models showed different levels of neurological damage. Compared with those in PBS treatment group,in OMSC treatment group,the severity of neural dysfunction in mice was significantly reduced(P =0.002),the level of IFN-γ in serum was lower(P = 0.032),but no significant differences in the levels of IL-10,IL-17 and IL-6 were found between two groups. HE and LFB staining revealed that the inflammatory infiltration and demyelinating areas in OMSC treatment group were less than those in PBS treatment group. The proportion of Th1 cells was lower in co-culture group than that in lymphocyte group(P = 0.001),higher in IDO inhibitor group than that in co-culture group(P = 0.01),but no significant difference was found between IDO inhibitor group and lymphocyte group or between COX inhibitor group and co-culture group.【Conclusions】OMSC may regulate the proportion of Th1 lymphocytes through IDO pathway so as to inhibit the demyelinating injuries of EAE in mice. This study provides a new idea for the clinical treatment of multiple sclerosis.
6.Association of cardiovascular diseases with milk intake among general Chinese adults.

BACKGROUND:
The association of milk intake with cardiovascular disease (CVD) and cause-specific mortality remained controversial and evidence among the Chinese population was limited. We aimed to study the relationship between milk intake and CVDs among general Chinese adults.
METHODS:
A total of 104,957 participants received questionnaire survey. Results of physical examination such as anthropometric measurements and biochemical tests during 2007 to 2008, demographic data and their information on milk intake were collected through standardized questionnaires. Cox proportional hazard regression models were used to calculate hazard ratios (HRs) and their corresponding 95% confidence intervals (CIs) of CVD incidence, cause-specific mortality and all-cause mortality related to milk intake. Restricted cubic splines (RCSs) were applied to examine dose-response associations.
RESULTS:
Among the 91,757 participants with a median follow-up period of 5.8 years, we documented 3877 CVD cases and 4091 all-cause deaths. Compared with participants who never consumed milk, the multivariate-adjusted HRs (95% CIs) of CVD incidence for 1 to 150 g/day, 151 to 299 g/day, and ≥300 g/day were 0.94 (0.86-1.03) (P > 0.05), 0.77 (0.66-0.89) (P < 0.05), and 0.59 (0.40-0.89) (P < 0.05), respectively; each 100 g increase of daily milk intake was associated with 11% lower risk of CVD incidence (HR, 0.89; 95% CI: 0.85-0.94; P < 0.001), and 11% lower risk of CVD mortality (HR, 0.89; 95% CI: 0.82-0.97; P = 0.008) after adjustment for age, sex, residential area, geographic region, education level, family history of CVD, smoking, alcohol drinking, physical activity level, body mass index, and healthy diet status (ideal or not). RCS analyses also showed a linear dose-response relationship with CVD (P for overall significance of the curve <0.001; P for non-linearity = 0.979; P for linearity <0.001) and stroke (P for overall significance of the curve = 0.010; P for non-linearity = 0.998; P for linearity = 0.002) incidence, and CVD mortality (P for overall significance of the curve = 0.045; P for non-linearity = 0.768; P for linearity = 0.014) within the current range of daily milk intake.
CONCLUSIONS
Daily milk intake was associated with lower risk of CVD incidence and mortality in a linear inverse relationship. The findings provide new evidence for dietary recommendations in CVD prevention among Chinese adults and people with similar dietary pattern in other countries.
7.Effects of Mesenchymal Stem Cells on Proliferation and Immunomodulation of B Cells

【Objective】To study the effect of mesenchymal stem cells(MSC)on proliferation and immunomodulation of B cells in vitro.【Methods】Bone marrow 30 mL from healthy donors was isolated by density gradient centrifugation. Isolated MSC were cultivated adherently with L- DMEM medium containing 10% fetal bovine serum(FBS). Phenotype and differentiation capacity of MSC was detected after the sixth passage. Peripheral blood mononuclear cells(PBMC)of healthy donors were also obtained by density gradient centrifugation. CD19+ B cells were sorted by fluorescence activated and co-cultured with MSC(CD19+ B∶MSC = 5∶1)group. Stimulated by CpG+ CD40L,the proliferation of B cells of two groups were checked 96 hours after co-culture respectively. The CD19+CD5+ B cells percentage and its secretion of IL-10 were detected by FACS. The effects of CD19+ B cells on the proliferation of CD4+ T cells and the secretion of IFN-γ were continually observed. Anti-IL-10 was used to confirm the effect of B cells on proliferation and IFN-γ secretion of CD4+ T cells.【Results】MSC collected from healthy donors remained differentiation capacity. MSC derived from bone marrow expressed CD29,CD44,CD73,CD90,CD105,and CD166,but did not express CD45 and CD34. Compared with control group,the proportion of CD19+ B cells proliferation in co-cultured group was significantly increased after 3 days,meanwhile,the level of IFN-γ,which secreted by CD4+ T cells was significantly restrained. To make a further analysis of the subsets of CD19+ B cells,the percentage of CD5+ B cells in co-cultured group increased from(21.31±1.22)% to(31.27±0.92)%(P=0.014),and its IL-10 secretion increased from(1.09±0.08)% to(2.44±0.06)%(P<0.001)compared with control group. After anti-IL-10 was used,the B cells co-cultured with MSC showed a decreased capacity of inhibiting the proliferation and IFN-γ secretion of CD4+ T cells.【Conclusions】MSC could promote the proliferation of CD19+ B cells to inhibit the secretion of IFN-γ by CD4+ T cells,which may be related to the increasing expression of IL-10 by CD19+CD5+B cells.
9.Risk factors related to HIV new infections among men who have sex with men in a cohort study.

Objectives: To analyze and understand the risk factors related to HIV new infections among men who have sex with men (MSM). Methods: A longitudinal observational study among MSM was conducted to collect information on HIV related behaviors and sero-conversion. Univariate and multivariate generalized estimating equations (GEE) were used to discuss the risk factors for HIV new infection. Results: A total number of 4 305 MSM were followed during 2013-2015. Among those self-reported MSM who are seeking partners on the Interner tended to have higher proportion on receptive anal intercourse and consistent condom use during anal intercourse than the subgroups seeking their partners in gay bars or bathrooms. HIV incidence among followed MSM during the study period appeared as 4.3/100 person years, with adjusted RR (aRR) of HIV infection for receptive anal intercourse as group 2.20 (95% CI: 1.49-3.24) times than that of insertion anal intercourse group. Those who used rush-poppers (aRR=1.55, 95% CI: 1.10-2.17), unprotected anal intercourse (aRR=2.24, 95%CI: 1.62-3.08), and those with syphilis infection (aRR=2.95, 95%CI: 2.00-4.35) were also risk factors for HIV new infections. After controlling other factors, the relationship between the ways of seeking partners and HIV new infection was not statistical significant. Conclusion: Risk factors for HIV new infection among MSM appeared complex and interactive, suggesting that further studies are needed to generate tailored strategies for the prevention of HIV epidemic among MSM population.

10.A simple rat model of in situ reversible obstructive jaundice in situ reversible obstructive jaundice model.

PURPOSE: To develop a simple and reliable rat model of in situ reversible obstructive jaundice with low morbidity and mortality rates. METHODS: Rats were divided into 4 groups with 8 rats each: the sham-operated (SH) group only underwent laparotomy, the control internal drainage (ID-C) group underwent choledochoduodenostomy, the new internal drainage (ID-N) group and the long-term internal drainage (ID-L) group underwent choledochocholedochostomy. Common bile duct ligation was performed in all the drainage groups 7 days before reversal procedures. All rats were sacrificed for samples 7 days after the last operation except rats of the ID-L group that survived 28 days before sacrifice. Body weight, liver function, histopathological changes, morbidity and mortality were assessed. RESULTS: One rat died and 2 rats had complications with tube blockage in the ID-C group. No death or complications occurred in the ID-N and ID-L groups. The drainage tube remained patent in the long-term observation ID-L group. Body weight showed no significant difference between the ID-C and ID-N groups after 7 days drainage. Liver function was not fully recovered in the ID-C and ID-N groups after 7 days drainage, but statistical differences were only observed in the ID-C group compared with the SH and ID-L groups. Periportal inflammation and bile duct proliferation showed severer in the ID-C group than in the ID-N group. CONCLUSION: The present study provided an efficient, simple, and reliable rat model that is especially suitable for long-term or consecutive studies of reversible obstructive jaundice.
