1.In silico evaluation of human hookworm antigen, Glutathione-S-Transferase (GST-1) protein interaction with human dendritic cell responses towards eliciting immune response

Hookworm disease is one of the tropical neglected diseases that significantly impacts human health
to varying degrees. Hookworms produce various proteins to facilitate host invasion and immune
evasion. Despite available treatments, reinfection is common, underscoring the need for effective
vaccines. However, the complexity of the hookworm’s life cycle poses a challenge in understanding the
immune response in the vaccine candidates. Reverse vaccinology (RV) offers a powerful approach to
understand the immune response by using various bioinformatics tools. This study begins by identifying
hookworm antigens capable of inducing host immune responses, followed by docking analysis with
different dendritic cell (DC) receptors to investigate the immunological response of antigenic peptides
and further correlated to the immunogenicity findings in clinical trial. Necator americanus GlutathioneS-Transferase-1 (Na-GST-1), a known immunogenic protein from Necator americanus, was selected for
docking due to its strong antigenic properties. Fifteen DC receptors were evaluated against Na-GST-1, of
which seven receptors (TLR2, TLR3, TLR4, TLR7, DEC-205, CD206, and CD36) exhibited stronger predicted
interactions, as indicated by stronger binding affinities with Na-GST-1 utilizing various immunoinformatic
tools. These receptors are associated with the mediation of Th1/Th2 immune responses, suggesting
a potential correlation between docking affinity and the predicted immunogenicity of Na-GST-1.
Overall, this study provides valuable insights into DC receptor–antigen interactions and demonstrate
a computational approach for assessing the potential of hookworm antigens to engage DC receptors,
thereby supporting rational hookworm vaccine design. These findings support the application of early
in silico strategies for advancing vaccine candidates against hookworm infection and strengthening
control efforts for neglected tropical diseases.

Sarcopenia, which is characterized by progressive and generalized loss of skeletal muscle mass and strength, has been well described to be associated with numerous poor postoperative outcomes, such as increased perioperative mortality, postoperative sepsis, prolonged length of stay, increased cost of care, decreased functional outcome, and poorer oncological outcomes in cancer surgery. Multimodal prehabilitation, as a concept that involves boosting and optimizing the preoperative condition of a patient prior to the upcoming stressors of a surgical procedure, has the purported benefits of reversing the effects of sarcopenia, shortening hospitalization, improving the rate of return to bowel activity, reducing the costs of hospitalization, and improving quality of life. This review aims to present the current literature surrounding the concept of sarcopenia, its implications pertaining to colorectal cancer and surgery, a summary of studied multimodal prehabilitation interventions, and potential future advances in the management of sarcopenia.
4.Budget Impact Analysis of Ferric Derisomaltose for the Treatment of Iron-Deficiency in Malaysia

Introduction: Untreated iron deficiency (ID) can lead to severe anaemia, requiring blood transfusion, or increased
mortality risk. Globally intravenous (IV) iron is increasingly recognised as a recommended option for patients. This
study aims to evaluate the budget impact associated with introducing a new intravenous (IV) iron, ferric derisomaltose (Monofer® [IIM]) as one of the treatment options for the management of ID in the Ministry of Health Malaysia
(MOHM) setting. Methods: A 5-year budget impact model was developed from 2020 to 2024 for patients with ID
that require a high iron dose (≥500 mg), using the perspective of MOHM. The model was built with four external
medical specialists, each with experience and deep knowledge of ID management, to support estimations on the
future development of iron use in Malaysia. Results: Compared to the current market mix with the existing IV iron
products (i.e., iron sucrose and iron dextran), a cost-saving of MYR 53,910 could be achieved with the introduction
of IIM in 2020. The uptake of IIM into MOHM over five years is estimated to lead to an overall budget saving of MYR
11,837,524 over a 5-year time horizon. Conclusion: The use of IIM in place of the current IV iron products in MOHM
resulted in a significant cost saving by reducing the number of visits required to achieve the targeted iron dose and
the shorter IV infusion time with IIM.

6.Paediatric emergency department attendances during COVID-19 and SARS in Singapore.

INTRODUCTION:
We evaluated the impact of public health measures on paediatric emergency department attendances during the COVID-19 and severe acute respiratory syndrome (SARS) outbreaks in Singapore.
METHODS:
Between 1 January 2020 and 31 July 2020, we retrospectively reviewed paediatric emergency department attendances and admissions in a tertiary paediatric hospital in Singapore before and after a national lockdown to combat the spread of COVID-19 in Singapore. Hospital attendances and admissions were compared with data from a corresponding period in 2019 (1 January 2019 to 31 July 2019), as well as during and after the SARS outbreak (1 January 2003 to 31 December 2004).
RESULTS:
Compared with a corresponding non-outbreak period, emergency department attendances decreased in line with nationwide public health measures during the COVID-19 and SARS outbreaks (2020 and 2003 respectively), before increasing gradually following lifting of restrictions, albeit not to recorded levels before these outbreaks. During the COVID-19 outbreak, mean daily attendances decreased by 40%, from 458 per day in January-July 2019, to 274 per day in January-July 2020. The absolute number of hospital inpatient admissions decreased by 37% from January-July 2019 (19,629) to January-July 2020 (12,304). The proportion of emergency department attendances requiring admission remained similar: 20% in January-July 2019 and 21% in January-July 2020.
CONCLUSION
Nationwide public health measures in Singapore have had an impact on paediatric emergency department attendances and hospital inpatient admissions. Data from this study could inform planning and resource allocation for emergency departments in Singapore and internationally.

7.Psychosocial impact of the COVID-19 pandemic on paediatric healthcare workers.

INTRODUCTION:
Frontline healthcare workers (HCWs) exposed to coronavirus disease 2019 (COVID-19) are at risk of psychological distress. This study evaluates the psychological impact of COVID-19 pandemic on HCWs in a national paediatric referral centre.
METHODS:
This was a survey-based study that collected demographic, work environment and mental health data from paediatric HCWs in the emergency, intensive care and infectious disease units. Psychological impact was measured using the Depression, Anxiety, Stress Scale-21. Multivariate regression analysis was performed to identify risk factors associated with psychological distress.
RESULTS:
The survey achieved a response rate of 93.9% (430 of 458). Of the 430 respondents, symptoms of depression, anxiety and stress were reported in 168 (39.1%), 205 (47.7%) and 106 (24.7%), respectively. Depression was reported in the mild (47, 10.9%), moderate (76, 17.7%), severe (23, 5.3%) and extremely severe (22, 5.1%) categories. Anxiety (205, 47.7%) and stress (106, 24.7%) were reported in the mild category only. Collectively, regression analysis identified female sex, a perceived lack of choice in work scope/environment, lack of protection from COVID-19, lack of access to physical activities and rest, the need to perform additional tasks, and the experience of stigma from the community as risk factors for poor psychological outcome.
CONCLUSION
A high prevalence of depression, anxiety and stress was reported among frontline paediatric HCWs during the COVID-19 pandemic. Personal psychoneuroimmunity and organisational prevention measures can be implemented to lessen psychiatric symptoms. At the national level, involving mental health professionals to plan and coordinate psychological intervention for the country should be considered.

8.The Current and Future Challenges of Hip Fracture Management in Malaysia

By 2050, it is predicted that six million hip fractures will
occur each year of which the majority will happen in Asia.
Malaysia is not spared from this predicted rise and its rate of
increase will be one of the highest in this region. Much of
this is driven by our unprecedented growth in the number of
older people. Characteristics of individuals with hip fractures
in Malaysia mirror what has been reported in other countries.
They will be older multimorbid people who were already at
risk of falls and fractures. Outcomes were poor with at least
a quarter do not survive beyond 12 months and in those that
do survive have limitation in their mobility and activities of
daily living. Reviewing how these fractures are managed and
incorporating new models of care, such as orthogeriatric
care, could address these poor outcomes. Experts have
warned of the devastating impact of hip fracture in Malaysia
and that prompt action is urgently required. Despite that,
there remains no national agenda to highlight the need to
improve musculoskeletal health in the country
