1.Correlation between the family cohesion and adaptability of naval officers and sailors and their negative emotions

Objective:To explore the correlation between the family cohesion and adaptability of naval officers and sailors and their negative emotions.Methods:A total of 610 naval officers and sailors from four units of a naval troop were selected by cluster and convenience samplings. The Chinese version of Family Adaptability and Cohesion Evaluation Scale(FACES-CV),the Patient Health Questionnaire 9-Item(PHQ-9),and the Generalized Anxiety Disorder 7-Item(GAD-7)Scale were used for measurement. Variance tests and correlation analyses were conducted.Results:The anxiety of naval officers and sailors was negatively correlated with their family cohesion and adaptability( r=-0.235, r=-0.200,both P<0.01);and their depression was negatively correlated with their family cohesion and adaptability( r=-0.294, r=-0.257,both P<0.01). Conclusions:The negative emotions of naval officers and sailors are negatively correlated with their family cohesion and adaptability. Stronger family cohesion and adaptability are conducive to reducing negative emotions.
2.Investigation of the anxiety and depression of officers and soldiers stationed on islands and reefs and the related influencing factors

Objective:To investigate the severity of anxiety and depression of officers and soldiers stationed on islands and reefs and the related influencing factors,so as to provide a theoretical basis for the subsequent psychological screening and mental health maintenance of these officers and soldiers.Methods:Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),Neuroticism-Extraversion-Openness Five-Factor Inventory(NEO-FFI),Interpersonal Relationship Comprehensive Diagnostic Scale(IRCDS),Connor-Davidson Resilience Scale(CD-RISC)were used to evaluate the emotional status and its related factors of the officers and soldiers stationed on islands and reefs.Results:Among all the research objects,34.14% of them showed anxiety,and 65.51% showed depression. The scale ratings of the officers and soldiers stationed on islands and reefs were significantly different from Chinese norm( P<0.01). According to the ratings of IRCDS,16.11% of the officers and soldiers had serious problems in interpersonal relationship,and they also had varying degrees of distress in conversation,making friends,manners,and heterosexual interactions. Anxiety and depression were significantly and positively correlated with neuroticism in personality traits( P<0. 01),while they were significantly and negatively correlated with other personality traits( P<0.01);they were significantly and positively correlated with all factors of IRCDS( P<0.01),while they were significantly and negatively correlated with all factors of CD-RISC( P<0.01). Through multiple linear regression analysis,the neuroticism of NEO-FFI,the conversation and manners of IRCDS,and the strength of CD-RISC have impacts on anxiety;the strength and optimism of CD-RISC,the neuroticism of NEO-FFI,and the conversation of IRCDS had impacts on depression. Conclusion:Officers and soldiers stationed on islands and reefs have obvious anxiety and depression,and the factors of personality traits,interpersonal relationship,and resilience can affect the emotions of anxiety and depression to varying degrees.
3.A cross-sectional study on sleep status and mental health of officers and soldiers stationed islands and reefs in a certain sea area of China

Objective:To provide a reference and basis for developing more targeted and effective intervention measures for the mental health of officers and soldiers stationed on islands and reefs in a certain sea area of China.Methods:Based on the previous studies,the research group conducted a cross-sectional survey on the mental health and sleep status of the officers and soldiers of different specialties stationed on islands and reefs in a certain sea area,and evaluated the mental health and sleep status of officers and soldiers of different specialties stationed on islands and reefs for the first time.Results:Among the officers and soldiers in the survey,106 were with suboptimal mental health status,accounting for 23.77% of all objects of study. Among them,there were 42(29.37%)in the ground unit group,41(20.92%)in the aviation unit group,18(30.51%)in the observation unit group,and 5(10.42%)in the electronics unit group. There were statistically significant differences among the 4 groups( P=0.023). The total average SCL-90 scores of the ground unit group,the aviation unit group,the observation unit group,and the electronics unit group were 1.35,1.25,1.36,and 1.20,respectively. The total scores of SCL-90 were 121.10,112.73,122.02,and 108.17,respectively. Among the officers and soldiers in the 4 groups,there were statistically significant differences in the factors and in total scores of SCL-90,i.e.,somatization,obsessive-compulsive symptoms,interpersonal sensitivity,depression,anxiety,paranoia,sleep,and diet( P < 0.05). After adjusting for other factors,the results of pairwise comparison showed the aviation unit group was superior to the ground unit group. The average PSQI scores of the ground unit group,the aviation unit group,the observation unit group,and the electronics unit group were 9.1,7.5,8.5,and 5.5,respectively. There were statistically significant differences in sleep quality score,sleep time score,sleep efficiency score,hypnotic drugs score,daytime function score,and in the PSQI total scores among the 4 groups( P<0.05). Conclusion:The officers and soldiers in the ground unit and observation unit stationed on islands and reefs in a certain sea area of China have a relatively poor mental health and sleep status. The length of time on the islands and reefs affects mental health and sleep. A more scientific regular rotation scheme should be developed.
4.Clinical efficacy of hyperbaric oxygen comprehensive treatment of sudden sensorineural hearing loss and the analysis of the influencing factors

Objective:To investigate the clinical efficacy of hyperbaric oxygen(HBO)comprehensive treatment of sudden sensorineural hearing loss(SSHL)and the related influencing factors.Methods:A total of 310 SSHL patients admitted to PLA No.971 Hospital from January 2017 to June 2019 were selected and divided into control group( n=154)and observation group( n=156). The control group was on medication only,while the observation group was treated with medication combined with HBO. The therapeutic effects were compared between the two groups. The rank sum test was adopted to test the significance of gender,age,comorbid diabetes mellitus(DM),hypertension,onset to treatment time(OTT),type of hearing loss curve,and degree of hearing impairment between the patients whose treatment was effective and those whose treatment was ineffective in the observation group. Logistic regression was used to analyze the independent factors affecting the prognosis. Results:After treatment,the total effective rate was 80.33% in the observation group and 66.90% in the control group,and there were statistically significant differences between the two groups( P<0.05). Among the patients in the observation group,there was no statistically significant difference between those whose treatment was effective and those whose treatment was ineffective in terms of gender,DM,and hypertension( P>0.05),while there were statistically significant differences in terms of age,OTT,type of hearing loss curve,and degree of hearing loss( P<0.05). The results of logistic regression analysis showed that the OTT and the type of hearing loss curve were the independent influencing factors for the HBO comprehensive treatment of SSHL. Conclusion:HBO comprehensive treatment has obvious curative effects in treating SSHL. The OTT and the type of hearing loss curve are the influencing factors of the clinical efficacy of HBO combined with medication in treating SSHL.
5.Aeromedical evaluation of cerebrovascular malformations in military aircrew:a clinical analysis of 20 cases

Objective:To explore the methods and standards of aeromedical evaluation for military aircrew with cerebrovascular malformations(CVMs).Methods:A retrospective analysis was conducted on the medical records of 20 aircrew with CVMs who were admitted to the PLA Air Force Medical Center from January 2000 to January 2020.Results:There were 11 cases with cerebral cavernous angioma(CCA)[CCA locations:occipital lobe(three cases),frontal lobe(two cases),temporal lobe(two cases),cerebellum(one case),posterior horn of left ventricle(one case),right basal ganglia region(one case),brainstem(one case)]. There were four cases with cerebral aneurysm(CA),three cases with cerebral venous aneurysms(VAs),and two cases with brain arteriovenous malformations(bAVMs). Among them,11 aviators were qualified for flight,three were temporarily disqualified from flight,and six were disqualified from flight.Conclusion:The aeromedical evaluation of CVMs in military aircrew should consider the type,size,location of the lesions,symptoms,signs,and complications of the malformations,as well as the types of aircraft,duties,flight experience,personal plan,and the opinions of working unit. It should be carried out in accordance with relevant standards and learn from the standard practices in developed countries and the 1% rule of civil aviation.
6.Design of the personnel decontamination system in a certain type of integrated rescue ship

Objective:To design a shipboard personnel decontamination system for a certain type of integrated rescue ship, which can be used to decontaminate the wounded in nuclear emergency rescue at sea.Methods:To decontaminate as many people as a shipboard decontamination system could in a limited space, we redesigned the layout of cabins and the decontamination process of the system. We broke down the system into different functions, estimated the time consumed by each function, and used the FlexSim to model and simulate the system.Results:The simulation system was set to run as fast as a common deep submersible lifeboat did in the case of a rescue. The utilization rate of most functioning objects remained between 48.6% and 82.9%. The average utilization rate of the skin drying positions and the dressing positions was less than 40%. However, it was acceptable considering multiplicity was required to keep at least one object operational.Conclusions:The decontamination system presented in this study has a scientific layout and a clear decontamination process. It can meet most of the needs of decontamination in nuclear emergency rescue at sea.
7.The mental health status and risk factors of the troops stationed and training in remote islands and reefs during COVID-19 pandemic

Objective:To investigate and analyze the mental health status and influencing factors of the troops stationed and training in remote islands and reefs during COVID-19 pandemic.Methods:A total of 577 officers and sailors were selected as objects of study from the troops stationed and training in remote islands and reefs in 2020 by convenient sampling method.The investigation was carried out by handing out and returning a questionnaire on-the-spot.The questionnaire was developed from the 90-item symptom list(SCL-90),and all the questions and choices in the questionnaire were uniformed according to the same instruction.A total of 579 questionnaires were handed out,and 577 valid questionnaires were collected;the effective rate was 99.65%.The univariate and multivariate analyses were used to screen the influencing factors related to the results of the above-mentioned investigation.Results:The average score of SCL-90 was 122.7±49.76,which was significantly lower than the military norm( P<0.01).The multivariate analysis showed that the total score was negatively correlated with the relationship with other colleagues,the relationship with parents,and the relationship with spouse( P<0.01);while it was positively correlated with the military situation in the South China Sea and the family economic situation( P<0.05). Conclusion:The mental health status of the troops stationed and training in remote islands and reefs were generally good.The mental status of the troops were closely related to intimate relationship,interpersonal relationship,and military environment.
8.Observation on the clinical efficacy of 3D printed individual model combined with hyperbaric oxygen therapy in 24 cases of complex ankle fracture

Objective:To investigate the clinical efficacy of 3D printed individual model combined with hyperbaric oxygen(HBO)therapy in treating complex ankle fracture,and to evaluate the therapy’s effect on prognosis.Methods:The clinical data of 45 patients with complex ankle fractures treated in the Department of Orthopedics of Penglai People’s Hospital from January 2017 to January 2021 were analyzed retrospectively. According to different treatment methods,they were divided into 3D model group( n=21)and combined group( n=24). In addition,other 37 complex ankle fracture patients,who were treated in the Department of Orthopedics of Penglai People’s Hospital from January 2014 to December 2016 and had complete clinical data and detailed follow-up records,were selected as control group(conventional group). The conventional group was given traditional surgery for complex ankle fractures,i.e.,after fracture reduction,the medial malleolus was fixed with steel wire tension band or screws,and the lateral malleolus was fixed with screws and steel plates. In the 3D model group,the strategy and approach of surgical operation were simulated and designed based on 3D printed individual models before operation. The combined group was additionally treated with HBO on the basis of the treatment of the 3D model group. The operation time,hospital length-of-stay,time to first ambulation,clinical fracture healing time,and the intraoperative bleeding loss of the three groups were observed and recorded,so as to compare the therapeutic effects of the three groups. All patients were followed up for one year. The functional recovery of postoperative ankle fracture was evaluated by Olerud-Molander Ankle Score(OMAS). The incidences of postoperative complications were recorded. Results:The total effective rate of the 3D model group(95.24%)and the combined group(100.00%)were significantly higher than that of the conventional group(72.97%)( P<0.05). Regarding the operation time,hospital length-of-stay,time to first ambulation,clinical fracture healing time,and intraoperative bleeding loss,the results of the 3D model group and the combined group were significantly shorter or lower than those of the conventional group( P<0.05);the time to first ambulation and clinical fracture healing time of the combined group were significantly shorter than those of the control group( P<0.05);there was no statistically significant difference in the operation time,hospital length-of-stay,and intraoperative bleeding loss between the 3D model group and the combined group( P>0.05). All patients were followed up with a follow-up rate of 100.0%. The OMAS outcomes of the 3D model group and the combined group at the 1st,3rd,6th,and 12th month after operation were significantly higher than those of the conventional group at the same time points( P<0.05),and the OMAS outcomes of the combined group at the 1st,3rd,6th,and 12th month after operation were significantly higher than those of 3D model group at the same time points( P<0.05). The incidence of postoperative complications was 16.22% in the conventional group,the incidence of deep venous thrombosis(DVT)was 4.8% in the 3D model group,and no complication was found in the combined group. Conclusion:The 3D printed individual model combined with HBO therapy has sound clinical efficacy and prognosis in treating complex ankle fracture. It can shorten the clinical fracture healing time and facilitate the rapid recovery of ankle function,therefore,it should be widely promoted in clinical practices.
9.Investigation of oral health status for crew of a ship

Objective:To analyze the oral health condition of the crew of a ship by carrying out oral examination,so as to provide reference for offering better medical services.Methods:A total of 816 crew members of a ship were divided into two groups:new crew group with 186 members(onboard time ≤ 1 year)and old crew group with 630 members(onboard time > 1 year). Indicators related to dental caries,periodontal disease,and wisdom teeth were recorded and analyzed statistically.Results:Among the 816 crew members,the caries prevalence rate was 42.7%,the per capita caries was 0.98,the periodontal health rate was 22.3%,the calculus detection rate was 75.9%,the materia alba detection rate was 9.27%,and the wisdom teeth detection rate was 49.6%. The new crew had worse rank sum test results than the old crew regarding the number of dental caries,decayed-missing-filled teeth,and the calculus index(CI)( P<0.05). While the new crew had better results than the old crew regarding the number of teeth loss due to caries( P<0.05). In all,799 teeth needed dental treatment;71 teeth were missing and needed to be repaired;619 patients needed supragingival scaling;and 1,005 impacted wisdom teeth needed to be removed. Conclusion:The oral health of the crew is not optimistic,therefore,more oral medical services are needed. Early prevention of oral diseases and oral health education should be strengthened,early treatment of oral diseases should be carried out routinely,and oral examination and intensive treatment should be conducted regularly.
10.Epidemiological investigation on the prevalence of hyperuricemia and urinary calculi in tropical island population

Objective:To investigate the prevalence of hyperuricemia in tropical island population,and to analyze the risk factors of urinary calculi in patients with hyperuricemia or gout.Methods:A total of 638 samples of tropical island people who received annual physical examinations were the subjects of our study. Their general information,renal function indicators,and urological ultrasound data were collected. The intergroup comparison and binary Logistic regression were applied for statistical analysis.Results:The incidences of hyperuricemia and gout were 36.4% and 1.3%,respectively,in tropical island population. The incidences of urinary calculi in cases with normal uric acid,asymptomatic hyperuricemia,and gout were 14.3%,23.9%,and 72.7%,respectively. A positive correlation was found between serum creatinine and uric acid levels among the 638 cases( r = 0.25, P < 0.001),and the level of urea nitrogen increased slightly with the increase of uric acid( r = 0.07, P = 0.051). The elevated serum uric acid( OR = 1.0, P = 0.013)and history of gout( OR = 7.9, P = 0.004)were risk factors for the occurrence of urinary calculi in tropical island population. The combination of these two parameters could predict the occurrence of urinary calculi with AUC = 0.601,which was slightly better than using serum uric acid level alone with AUC = 0.598( P = 0.158). Conclusion:The proportion of patients with hyperuricemia in tropical islands is significantly higher than that in non-tropical regions. The serum creatinine and urea nitrogen levels are positively correlated with uric acid levels among the cases. The elevated serum uric acid and history of gout are risk factors for the occurrence of urinary calculi in tropical island population. The combination of these two parameters has a certain role in predicting the occurrence of urinary calculi.
