1.Diagnostic value of pelvic floor ultrasound measurement of musculi levator ani strain rate in assessing postpartum pelvic floor muscles contractile force

Objective To explore the application value of pelvic floor ultrasound measurement of musculi levator ani strain rate in evaluating postpartum pelvic floor muscles contractile force,and to analyze the correlation between the musculi levator ani strain rate and the evaluation results of surface electromyography of the pelvic floor muscles.Methods A total of 153 postpartum women who admitted to Wenzhou People's Hospital from January to October 2024 were selected as the subjects.All of them underwent pelvic floor ultrasound examinations 42 days after childbirth to obtain the area and anteroposterior diameter of levator ani hiatus in the resting and squeezing states,and calculate the contraction musculi levator ani strain rate.Meanwhile,surface electromyography assessment of the pelvic floor muscles was carried out.The correlation between the contraction musculi levator ani strain rate and the assessment results of surface electromyography of the pelvic floor muscles were compared,and the value of the contraction levator ani hiatus strain rate predicting the strength of the pelvic floor muscles was further evaluated.Results Among the 153 postpartum women,78 cases had stress urinary incontinence,and 75 cases had no obvious clinical symptoms.The potential of pelvic floor muscle fibers in those with urinary incontinence was significantly lower than that in those without clinical symptoms.In the state of anal contraction,the anteroposterior diameter and area of the levator ani hiatus of all research subjects were smaller than those in the resting state.Correlation between the contraction strain rate and the potential during the contraction phase of the pelvic floor muscles was significant positive(r>0,P<0.05).The areas under the curves of the contraction strain rates of the anteroposterior diameter and area of the levator ani hiatus in predicting the abnormal potentials during the contraction phase of type Ⅰ and type Ⅱ muscle fibers were 0.820,0.875 and 0.943,0.971,respectively,which showed high sensitivity and specificity.Conclusion By observing the size of musculi levator ani hiatus under different conditions,pelvic floor ultrasound can reflect the contractility of musculi levator ani,and they have significant correlation with strength of the pelvic floor muscles.It has certain value for the diagnosis of female pelvic floor dysfunction.
2.Analysis of risk factors of bronchopneumonia and lobar pneumonia caused by Mycoplasma pneumoniae

Objective To compare the laboratory indicators of bronchopneumoniaand lobar pneumonia caused by Mycoplasma pneumoniae(MP)in children,and to explore the risk factors of lobar pneumonia in children.Methods A total of 424 children diagnosed with pneumonia caused by MP infection,admitted to the Second Hospital of Weihai,Qingdao University(Weihai Maternal and Child Health Hospital)from July to October 2023 were selected as subjects.Based on chest X-ray findings at admission,the childron were divided into bronchopneumonia group(n=178)and lobar pneumonia group(n=246).Age,hospitalization duration,laboratory parameters,and clinical characteristics were compared between two groups.Results The age and hospitalization duration of children with lobar pneumonia group were significantly higher than those in bronchopneumonia group(P<0.05).The lactic acid dehydrogenase(LDH),adenosine deaminase(ADA),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),and D-dimer levels in lobar pneumonia group were markedly elevated compared to bronchopneumonia group(P<0.05).Rates of mixed infections,pleural effusion,concurrent organ damage,infection with multidrug-resistant MP,and bronchoscopy procedures were higher in lobar pneumonia group than those in bronchopneumonia group(P<0.05).Elevated LDH,ADA,CRP,ESR,D-dimer levels and older children were identified as risk factors for developing lobar pneumonia following MP infection.LDH,ADA,CRP,ESR,D-dimer demonstrated predictive value for lobar pneumonia development,with their area under the curve(AUC)values were 0.612,0.704,0.659,0.645,and 0.679 respectively.When combined for prediction,the AUC was 0.793.Conclusion There are significant differences in various laboratory indicators between bronchopneumonia and lobar pneumonia caused by MP,and comprehensive analysis of multiple indicators is helpful for differential diagnosis of two types of pneumonia.
3.Analysis of virtual labor medication rules and clinical experience based on data mining

Objective To analyze the medication rules and clinical experience of Qin Zujie in treating virtual labor based on data mining.Methods Outpatient medical records of Qin Zujie,diagnosed with deficiency syndrome at Ethnic Medicine Characteristic Diagnosis and Treatment Center,Guangxi International Zhuang Medical Hospital from January to December 2024 were selected.A standardized database was established and processed,yielding 148 clinical prescriptions containing 171 herbal ingredients.Frequency analysis,association rule analysis,and cluster analysis were employed to examine medication rules and clinical experience.Results The 20 Chinese herbal ingredients used more than 30 times each,with the first three being Zhigancao,Baizhu,Danggui.These herbs are primarily warm in nature,neutral in temperature,and slightly cold in effect,characterized by sweet,pungent,and bitter flavors.They mainly affect the spleen meridian and function to tonify deficiency,with Qi-tonifying herbs being the most common.Through association rule analysis,the most frequently combined pairs were Baizhu-Dangshen.Cluster analysis identified three core prescription clusters.Conclusion Qin Zujie's academic thought of treating virtual labor is to supplement deficiency as the main,detoxification as the auxiliary,and treat both the symptoms and the causes.
4.Efficacy analysis of Degludec insulin Liraglutide on obese T2DM patients with poor treatment effect of Metformin

Objective To explore the therapeutic effect of Degludec insulin Liraglutide on obese type 2 diabetes mellitus(T2DM)with poor treatment effect of Metformin.Methods A total of 95 obese T2DM patients with inadequate glycemic control after Metformin therapy admitted to the Second Affiliated Hospital of Guilin Medical University from July 2022 to February 2024 were enrolled.Using a randomized digital table method,patients were divided into observation group(n=47)and control group(n=48).The patients of observation group received Degludec insulin Liraglutide,while the patients of control group was treated with Glargine insulin.Body mass index(BMI),serum C-peptide levels,blood glucose parameters,pancreatic function,lipid profiles,and adverse reaction incidence were compared between two groups.Results After treatment,BMI,postprandial 2-hour blood glucose,fasting blood glucose,glycated hemoglobin,total cholesterol,and low-density lipoprotein cholesterol were significantly lower in observation group than those in control group(P<0.05).Pancreatic β-cell function index,insulin resistance index,postprandial 2-hour C-peptide,fasting C-peptide,and high-density lipoprotein cholesterol were significantly higher in observation group than those in control group(P<0.05).There were fewer adverse reactions in observation group than those in control group(P<0.05).Conclusion The use of Degludec insulin Liraglutide in the treatment of obese T2DM patients with poor effect of Metformin can achieve satisfactory results during the treatment,and pancreatic function,blood glucose,blood lipid and BMI have been significantly improved.The clinical application has a high safety.
5.Research on medication rules for treating diseases related to spleen and stomach in LAN Shi Mi Cang

Objective To analyze the medication patterns of LAN Shi Mi Cang in treating diseases related to the spleen and stomach.Methods Search for prescriptions for treating spleen and stomach-related diseases in LAN Shi Mi Cang,a total of 95 prescriptions and 150 traditional Chinese medicines were included.The usage frequency,four Qi,five Wei and channel tropism of traditional Chinese medicine were analyzed,and the correlation degree,core drugs and new formula combinations were analyzed by using correlation analysis,cluster analysis and complex network analysis.Medication rules of Li Dongyuan in treating diseases related to spleen and stomach were summarized.Results The top three most frequently used herbs are Danggui,Shengma,and Gancao.These herbs generally exhibit warm,neutral,or cold properties,with flavors predominantly pungent,sweet,or bitter,primarily affecting the spleen and stomach meridians.The most closely associated herb pair is Danggui-Shengma.Core herbs include Huangqi,Shengma,and Chaihu,et al.Cluster analysis identified four distinct herbal combinations.Conclusion Li Dongyuan's treatment of diseases related to the spleen and stomach mainly focuses on tonifying the spleen and stomach,harmonizing Qi and blood,sweet and warm to remove heat,and raising Yang to disperse fire.It also treats external infections and internal injuries,as well as symptoms of excess and root deficiency.
6.Exploring the characteristics and medication patterns for sweating syndrome in Chinese Medical Canon based on data mining

Objective To analyze the formulas and drugs related to sweating syndrome in Chinese Medical Canon through data mining,and explore the medication patterns of sweating syndrome treatment.Methods Literature related to sweating syndrome were searched the Chinese Medical Canon electronic database.A total of 2392 prescriptions were collected,including 1974 for spontaneous sweating syndrome,368 for night sweating syndrome,and 50 for yellow sweating syndrome.Association rule analysis and cluster analysis were performed on the included drugs.Results For spontaneous sweating syndrome,the key herbs were Gancao and Renshen,mainly pungent in flavor,primarily affecting the spleen meridian.Association rule analysis revealed 22 core herb pairs,clustering into 5 patterns.For night sweating syndrome,the key herbs were Danggui and Renshen,mainly sweet in flavor,primarily affecting the lung meridian.Association rule analysis revealed 12 core herb pairs,clustering into 5 patterns.For yellow sweating syndrome,the key herbs were Guizhi and Huangqi,mainly pungent in flavor,primarily affecting the lung meridian.Association rule analysis revealed 8 core herb pairs,clustering into 4 patterns.Conclusion This study systematically reveal the complex syndrome structures within three types of sweating syndromes and identify representative herbal combinations and their pathogenesis by cluster and association rule analysis.
7.Visual analysis of the application of wearable devices in nursing care both domestically and internationally

Objective To analyze the research status,hotspots,and trends of wearable devices in nursing applications both domestically and internationally.Methods Using China National Knowledge Infrastructure,Wanfang Data Knowledge Service Platform,and Embase core databases as data sources,VOSviewer was used to visualize and analyze the publication time,keywords,and other relevant literature.Results Total of 428 articles were included,including 196 Chinese articles and 232 English articles.The overall publication volume showed an upward trend.Domestic research focuses on chronic diseases,artificial intelligence,and nursing,with the main research subjects being the elderly;Foreign research focuses on smart devices,self-monitoring,and quality of life,with the main research subjects being adults.Conclusion Currently,the number of publications on the application of wearable devices in nursing is relatively small,but the overall research heat is on the rise,mainly used for chronic diseases and self-monitoring.In the future,the application scope of wearable devices should be expanded and their potential value should be explored to promote the innovation and progress of nursing models.
8.Explore the causal relationship between the gut microbiota and inflammatory bowel disease based on two-sample Mendelian randomization

Objective Two-sample Mendelian randomization(TSMR)was used to explore the causal relationship between inflammatory bowel disease(IBD)and gut microbiota(GM).Methods GM was taken as the exposure factor,and two subtypes of IBD,Crohn's disease(CD)and ulcerative colitis(UC),were selected as the outcome factors.The potential causal relationship between IBD and gut microbiota was screened by TSMR statistical method.Results The results of comprehensive multiple analysis showed that 15 kinds of intestinal microbiota might be related to IBD.Among them,the Gastroenterococcaceae,Eulogicum,Clostridium 1,Agnobacter,Ruminococcus 2 and Ruminococcaceae UCG009 may be potential protective factors for IBD(OR<1,P<0.05);The UCG-011 of the Alfalfa family,the UCG001 of the Clostridium genus,the Actinobacillus genus,the Trichoderma genus,the Rumen genus UCG014,the NB1n order,the Eubacterium genus,the UCG008 of the Trichoderma family and the Clostridium order may be potential risk factors for IBD(OR>1,P<0.05);No pleiotropy or significant heterogeneity of instrumental variables genes was found.Conclusion The intestinal flora may have a dual effect on IBD.It may be a protective factor or increase the risk of IBD.The specific mechanism of action still needs to be further explored.
9.Based on data mining to explore the characteristics of acupoint selection and the law of acupoint compatibility in the treatment of Guillain-Barré syndrome

Objective To explore the characteristics of acupoint selection and the law of acupoint compatibility in the treatment of Guillain-Barré syndrome(GBS)based on data mining.Methods Literature on acupuncture treatment of GBS was searched and screened from China National Knowledge Infrastructure,Wanfang Data Knowledge Service Platform,VIP,SioMed,PubMed,Embase,Web of Science,and Cochrane Library from the establishment of the database to December 1,2024 and to build database.The author information and acupuncture prescriptions were modified and imported according to the data entry requirements of the Traditional Chinese medicine inheritance computing platform V3.5,the relevant data were analyzed and integrated through the"Acupoint Analysis"module of the platform.Results A total of 140 related papers,145 valid prescriptions,involving 237 acupoints.The acupoints with higher frequency were Zusanli,Hegu,Quchi,Yanglingquan and etc.The analysis yielded 42 core acupoints,22 sets of strong association rules for core acupoints,and 5 new core acupuncture prescriptions.Conclusion Acupuncture treatment for GBS follows the principle of"treating impotence by taking Yangming alone",takes"benefiting Qi and draining heat,supporting the correctness and opening up the collaterals"as the main method,and attaches importance to"regulating the spirit and connecting the internal organs".
10.Clinical value of high-frequency ultrasound combined with MIPO technique in preserving the supraclavicular nerve in clavicle fractures

Objective To explore the clinical value of high-frequency ultrasound combined with minimally invasive plate osteosynthesis(MIPO)in preserving the supraclavicular nerve in clavicle fractures.Methods Sixty patients with mid-clavicle fractures who were treated at the First Affiliated Hospital of Ningbo University from September 2022 to May 2024 were selected.Patients were divided into minimally invasive incision group(n=30)treated with high-frequency ultrasound combined with MIPO and traditional incision group(n=30)treated with open reduction and locked plate fixation.The perioperative indicators,visual analogue scale scores for pain,Constant-Murley shoulder scoring scale(CMS),and incidence of complications were compared between two groups.Results All patients completed the surgery and were followed up for 6 months.The bone call formation time and incision length in minimally invasive incision group were lower than those in traditional incision group(P<0.05);The CMS at 3 months and 6 months after surgery in minimally invasive incision group were higher than those in traditional incision group(P<0.05).The incidence of skin sensory abnormalities in minimally invasive incision group at 2 weeks,1 month,and 3 months after surgery was lower than that in traditional incision group(P<0.05).Conclusion Compared with traditional open reduction and plate internal fixation,the high-frequency ultrasound combined with MIPO technique can effectively reduce the iatrogenic injury of the supraclavicular nerve during surgery,reduce the incidence of postoperative skin sensory abnormalities,and also have significant advantages in terms of shoulder aesthetics,shoulder joint function,and fracture healing.
