Objectives: Pediatric and adolescent oral and maxillofacial trauma and infections differ significantly from adults in anatomy, development, and immunity, making diagnosis and treatment challenging. This study retrospectively evaluated the etiology, clinical features, and management of oral and maxillofacial trauma and infections among patients aged 3 to 15 years. Materials and Methods: A retrospective review was conducted of 824 pediatric and adolescent patients who presented to the emergency department of Pusan National University Yangsan Hospital from January 2022 to December 2024. Patients were categorized based on chief complaints into trauma, infection, or other conditions. Primary analyses focused on trauma and infections, and secondary analyses included etiology, diagnosis, treatment, and outcomes. Results: Trauma accounted for 742 cases (90.0%) and infections for 44 cases (5.3%). Boys (65.7%) outnumbered girls (34.3%), and school-aged children (6-11 years) were most frequently affected (56.5%). Lacerations (49.2%) and abrasions (34.1%) were the most common traumas, followed by dental trauma (31.0%) and facial fractures (5.0%). Among fractures, the mandibular condyle (35.1%) was most commonly involved. Most fractures were managed non-surgically (54.0%), while 30.0% required surgery. Infections were mainly odontogenic abscesses (44.7%), cellulitis (36.4%), and sialadenitis (15.9%). Abscesses were treated with incision and drainage (57.1%) or antibiotics (19.1%), with 23.8% requiring hospitalization. Conclusion Trauma was the most frequent emergency cause in pediatric and adolescent patients, particularly among school-aged boys. Most cases were successfully managed with appropriate treatment. Early diagnosis and timely intervention are essential for favorable outcomes in pediatric maxillofacial emergencies.

Patients and Methods: A total of 796 emergency patients whose visits were attributed to dental procedures, of 4,241 patients who visited the ER at Pusan National University Yangsan Hospital from January 2021 to January 2024, was included in the study. Patients were categorized based on the reason for visit into bleeding, inflammation, and other categories. Analysis was conducted on variables such as types of dental treatment, underlying conditions, and emergency treatment methods using ER records. Results: Among the 796 patients, 68.4% (539 patients) were in the bleeding group, 27.7% (219 patients) in the inflammation group, and 4.8% (38 patients) in the other complications group. Among the bleeding group, there were no associations between postoperative bleeding and systemic diseases, antithrombotic medications, or dental treatments. In 36.2% of cases, compression hemostasis alone was sufficient to resolve the bleeding. In the inflammation group, 29% of the cases required extraoral incision and drainage as emergency treatment, while 53% of the cases required subsequent hospitalization. Factors associated with hospitalization included underlying diseases (especially diabetes mellitus), procedures on mandibular teeth (especially third molars), and age older than 30 years. Conclusion Ensuring adequate hemostasis after dental procedures is essential regardless of the patient’s underlying medical conditions. Dentists must also educate patients on pressure hemostasis techniques. Patients with underlying medical conditions, such as diabetes, have a higher possibility of requiring hospitalization if inflammation occurs. Therefore, preventive measures against inflammation should be implemented in these patients.

Background: The Laboratory Medicine Foundation (LMF) checklists explain the accreditation requirements of the program and reflect quality standards like those of the Clinical Laboratory Standards Institute (CLSI) or quality system essentials (QSE) of the World Health Organization (WHO). In this study, we have analyzed how the LMF checklists correlate with the 12 QSE elements of the WHO. Methods: The LMF checklists for laboratory management (LM) (version 2019) were classified into the 12 specific QSE elements by five laboratory physicians. Each checklist item was classified into specific element if four or more participants agreed, and into overlapping elements when two or more of them agreed for two different items. Any changes in checklist items and chapter structuring of the checklist since 2009 were investigated. Results: The LM checklists consisted of 183 checklist items, including 20 (10.9%) classified into overlapping QSE elements. The QSE element with the highest number of items was the facilities and safety (62, 33.9%), followed by the personnel (22, 12.0%), and the process control (17, 9.3%). In contrast, QSE elements with the lowest number of items were the customer service (4, 2.2%) and process improvement (4, 2.2%). Items belonging to the customer service and the organization elements have increased since 2017. Conclusions The LMF checklists reflect current quality goals for clinical laboratories and play a leading role in the laboratory's quality improvement. The results of our study will be of help in potentiating the quality leader role of LMF checklists, and international harmonization of our laboratory accreditation program.

BACKGROUND: Medical device adverse event reporting is an essential activity for mitigating device-related risks. Reporting of adverse events can be done by anyone like healthcare workers, patients, and others. However, for an individual to determine the reporting, he or she should recognize the current situation as an adverse event. The objective of this report is to share observed individual differences in the perception of a medical device adverse event, which may affect the judgment and the reporting of adverse events. METHODS: We trained twenty-three participants from twelve Asia-Pacific Economic Cooperation (APEC) member economies about international guidelines for medical device vigilance. We developed and used six virtual cases and six questions. We divided participants into six groups and compared their opinions. We also surveyed the country's opinion to investigate the beginning point of ‘patient use’. The phases of ‘patient use’ are divided into: 1) inspecting, 2) preparing, and 3) applying medical device. RESULTS: As for the question on the beginning point of ‘patient use,’ 28.6%, 35.7%, and 35.7% of participants provided answers regarding the first, second, and third phases, respectively. In training for applying international guidelines to virtual cases, only one of the six questions reached a consensus between the two groups in all six virtual cases. For the other five questions, different judgments were given in at least two groups. CONCLUSION: From training courses using virtual cases, we found that there was no consensus on ‘patient use’ point of view of medical devices. There was a significant difference in applying definitions of adverse events written in guidelines regarding the medical device associated incidents. Our results point out that international harmonization effort is needed not only to harmonize differences in regulations between countries but also to overcome diversity in perspectives existing at the site of medical device use.
