Objective To propose a lead attention network-based ST-T change recognition algorithm to detect ECG ST-T changes accurately. Methods Firstly, heartbeat signals were extracted through R-wave localization, and a 12-lead heartbeat matrix was generated by correlation-based screening and merging to realize data augmentation.Secondly,a lead attention module was constructed by combining depthwise convolution(DWConv)with the channel attention squeeze-and-excitation block(SE-block)structure to perceive the differences in ST-T status among electrocardiogram leads.Thirdly, the mapping output by two independent attention modules was fused and splicing with the original signal residual was carried out,so that attention information extraction and original information transfer were enhanced effectively.Finally,SE-ResNet was used as the backbone network to extract signal features to complete the classification and identification of ST-T changes. To validate the recognition performance of the proposed algorithm for ST-T changes in ECG,the 12-lead ECG data of 97 472 patients containing different ECG rhythms were collected for ablation and comparison experiments at the First Affiliated Hospital of Army Medical University. Results The proposed algorithm achieved an AUC of 0.965 with a sensitivity of 90.51%,specificity of 90.23%,positive predictive value of 89.24%and overall accuracy of 90.36%on an independent test set.Comparative analysis demonstrated superior performance to four benchmark architectures, including VGG16,ResNet18,MobileNetV3-Small and ShuffleNet,in terms of both classification accuracy and computational efficiency. Conclusion The algorithm designed can accurately detect ST-T changes and can be used for wearable ECG automatic analysis to assist in the early warning of cardiovascular diseases in both acute and chronic patients and highland residents.[Chinese Medical Equipment Journal,2025,46(7):1-11]

Objective:To construct a full-length infectious clone of enterovirus D68(EV-D68)expressing enhanced green fluorescent protein(EGFP)by reverse genetics in order to provide an efficient tool for studying the biological characteristics and screening antiviral drugs for EV-D68.Methods:Gene synthesis and overlap PCR techniques were used to construct the full-length clone plasmid pUC57-EV-D68 of EV-D68. The full-length viral sequence was then transferred into the pCAGGS plasmid to obtain the pCAGGS-EV-D68 plasmid. The EGFP gene was amplified and inserted into the pCAGGS-EV-D68 plasmid to construct the pCAGGS-EGFP-EV-D68 plasmid. Then, the two constructed plasmids were transfected into human rhabdomyosarcoma(RD)cells to rescue recombinant viruses RV-EV-D68 and RV-EGFP-EV-D68. The rescued viruses were identified using PCR,Western blot, and immunofluorescence techniques. The antiviral effect of doxycycline was evaluated using the rescued RV-EGFP-EV-D68. Statistical analysis was performed using the two independent samples t-test. Results:The recombinant virus RV-EGFP-EV-D68 capable of expressing EGFP was successfully rescued. Even after 15 serial passages, the virus retained EGFP expression with no significant difference in viral titers compared to the parental virus, indicating its stable passage in RD cells. Besides, the rescued strains exhibited similar replication characteristics to the parental virus. While at 24 and 36 h after infection, the titers of the rescued strains were significantly lower than that of the parental strain(both P<0.05). This study demonstrated that doxycycline significantly reduced the fluorescence intensity of RV-EGFP-EV-D68-infected RD cells( P<0.01). Meanwhile,a negative correlation was observed between the doxycycline concentration and the fluorescence intensity, indicating that the rescued virus could be used for antiviral drug evaluation. Conclusions:This study successfully constructs an infectious clone of EV-D68 expressing EGFP. The rescued recombinant virus RV-EGFP-EV-D68 has been verified to be applicable for the evaluation of antiviral drugs.

Nonobstructive azoospermia (NOA), one of the most severe types of male infertility, etiology often remains unclear in most cases. Therefore, this study aimed to detect four biallelic detrimental variants (0.5%) in the minichromosome maintenance domain containing 2 ( MCMDC2 ) genes in 768 NOA patients by whole-exome sequencing (WES). Hematoxylin and eosin (H&E) demonstrated that MCMDC2 deleterious variants caused meiotic arrest in three patients (c.1360G>T, c.1956G>T, and c.685C>T) and hypospermatogenesis in one patient (c.94G>T), as further confirmed through immunofluorescence (IF) staining. The single-cell RNA sequencing data indicated that MCMDC2 was substantially expressed during spermatogenesis. The variants were confirmed as deleterious and responsible for patient infertility through bioinformatics and in vitro experimental analyses. The results revealed four MCMDC2 variants related to NOA, which contributes to the current perception of the function of MCMDC2 in male fertility and presents new perspectives on the genetic etiology of NOA.

Objective To explore and analyze the epidemiological characteristics of scarlet fever caused by group A Streptococcus (GAS) in Minhang District, Shanghai from 2023 to 2024, providing relevant information for clinical treatment and infection control. Methods Throat swab specimens were collected from children clinically diagnosed with scarlet fever at a sentinel hospital in Shanghai from May 2023 to June 2024. GAS strains were isolated and cultured using blood agar plates, and mass spectrometry performed species identification. The polymerase chain reaction (PCR) method was used to assess emm gene typing and 11 superantigens, while the broth microdilution method was used to determine the resistance of the strains to 14 antibiotics. Whole genome sequencing was performed on 68 GAS strains. The chi-square test and Fisher's exact probability method were conducted for intergroup comparisons. Results A total of 178 GAS strains were isolated from 305 throat swab specimens of scarlet fever cases in children. All GAS strains were sensitive to penicillin, cefotaxime, linezolid, vancomycin, meropenem, chloramphenicol, levofloxacin, teicoplanin, and cefepime, while the resistance rates to erythromycin, clarithromycin, and tetracycline were 99.44%, 99.44%, and 94.94%, respectively. There are two types of emm genes, with emm12 being the main type (89.33%, 159/178), followed by emm1 10.7% (19/178). There were 26 types of superantigen gene spectrums. The speA gene carrying rate of emm1 type GAS (94.74%, 18/19) was significantly higher than emm12(12.00%,32/159) (χ2=46.77, P<0.001). The simultaneous carrying rate of speH and speI genes in emm12 type GAS (59.75%, 95/159) was higher than in emm1 type (0) (χ2=24.35, P<0.001). Whole-genome sequencing revealed that all 68 GAS strains carried the drug resistance gene ermB, with 98.52% (67/68) carrying tetM. Conclusions In Minhang District of Shanghai, non-invasive scarlet fever in children is predominantly caused by emm12 GAS, which shows high resistance to erythromycin, clindamycin, and tetracycline, with higher carriage rates of speH and speI in emm12 types compared to other strains.

This paper discusses the mechanism of acupuncture for diabetes mellitus from the perspective of molecular biology, aiming to reveal the potential rules of restoring the balance in the body and fighting against diabetes mellitus (DM). By searching the basic research literature of acupuncture treatment for DM, it is found that the molecular biological mechanism of acupuncture treatment for DM is closely related to the regulation of insulin signaling pathway, the modulation of inflammatory response, the protection and regeneration of islet β-cells, fat metabolism and energy balance. It points out that there are few studies of acupuncture on the type 1 diabetes mellitus and the large-scale randomized controlled trials, as well as the studies on the upstream regulation mechanism, the specific cellular molecules and the interaction mechanism of various molecules. It needs to deepen the multi-level, multi-target and multi-dimensional exploration on the molecular biological mechanism of acupuncture for diabetes mellitus.

Chinese Journal of Child Health Care 2024;32(5):507-510
【Objective】 To generate reference values for inspiratory muscle of preschool children in Nanjing, so as to provide a reference index for evaluating children′s lung function and exercise performance. 【Methods】 A total of 236 preschool children were selected from the main urban area of Nanjing by stratified cluster sampling. The inspiratory muscles were evaluated by breath link respiratory function evaluation system to obtain the maximum inspiratory pressure (MIP) and inspiratory peak flow rate.Pearson analysis was used to determine the correlation of MIP and inspiratory peak flow rate with gender, age, height and weight.Multiple linear stepwise regression analysis was used to obtain the formula of MIP and inspiratory peak flow rate. 【Results】 Differences in MIP and peak inspiratory flow rate were not significant between boys and girls (P>0.05), but were statistically significant among different age groups and showed an increasing trend with age (F=13.660, 33.581, P<0.001).MIP and peak inspiratory flow rate were positively correlated with children′s age, height and weight (P<0.001).The regression model, proved to be statistically significant(F=12.913、22.398, P<0.08), indicated that height was the best predictor of MIP and age was the best predictor of inspiratory peak flow rate. 【Conclusions】 This study is the first study on the predicted value of inspiratory muscle in preschool children in China. The predicted value formula can provide a reference for clinical inspiratory muscle evaluation.
