1.Molecular characterization analyses of a human metapneumovirus outbreak in Gongshu District of Hangzhou City

ObjectiveTo analyze the epidemiological and etiological characteristics of a cluster of human metapneumovirus (HMPV) infection in a kindergarten in Gongshu District of Hangzhou City in May 2024, and to provide reference for the prevention and control of similar outbreaks. MethodsAn on-site investigation was conducted using an epidemiological case investigation form. Throat swab specimens collected from cases were screened for 13 respiratory pathogens using real-time fluorescent polymerase chain reaction (PCR). For HMPV nucleic acid positive specimens, the F gene of HMPV was used as the target gene for amplification and sequencing. The sequencing results were then compared with sequences in GenBank database to determine the virus subtypes and perform phylogenetic analyses. ResultsThe outbreak occurred in a kindergarter junior class with a total of 28 preschoolers and 3 teachers and childcare workers. A total of 11 cases (10 preschoolers and 1 teacher) were identified, including 8 male cases and 3 female cases. Clinical manifestations included fever in all 11 cases (100.00%), cough in 8 cases (72.72%), catarrhal symptoms in 4 cases (36.36%), and headache in 3 cases (27.27%). All symptoms were mild, and no severe cases were observed. A total of 11 throat swab samples were collected. Real-time fluorescent PCR test results showed that 3 samples were positive for HMPV nucleic acid, 2 samples were positive for both HMPV and Streptococcus pneumoniae, and 1 sample was positive for both HMPV and rhinovirus. The sequences of the 6 HMPV nucleic acid positive specimens were amplified and analyzed using specific primers, and all were determined to be HMPV subtype A2b. The F gene fragment sequence showed the highest similarity to PV081665.1/Brazil/2024 (99.65%), and also exhibited high similarity to PP683455.1/Indonesia/2021 (99.48%), PV016275.1/Beijing/2024 (99.31%), and PV052230.1/USA/2024 (99.13%). ConclusionThis cluster of acute respiratory tract infection was caused by HMPV subtype A2b, with co-infection of rhinovirus and Streptococcus pneumoniae. The F gene fragment sequences of the HMPV in this outbreak were highly homologous to those of the A2b strains isolated from Brazil, Beijing, Indonesia, and the the United States.
2.Knowledge, attitude and practice of infectious disease prevention and control among primary and middle school students in Gongshu District

Objective:
To investigate the knowledge, attitude and practice of infectious disease prevention and control among primary and middle school students in Gongshu District, Hangzhou City, so as to provide the evidence for implementing health education on infectious disease prevention and control in schools.
Methods:
Students from 12 primary and middle schools in Gongshu District were selected using a random cluster sampling method. Basic information, knowledge, attitude and practice of infectious disease middle and control were collected through questionnaire surveys. Influencing factors of practice of primary and middle school students' infectious disease prevention and control were analyzed using multivariable logistic regression model.
Results:
A total of 3 439 primary and middle school students were surveyed, including 1 717 boys (49.93%) and 1 722 girls (50.07%). There were 1 237 primary school students (35.97%), 1 092 junior high school students (31.75%), and 1 110 senior high school students (32.28%). The awareness of infectious disease prevention and control knowledge, the percentage of attitude towards infectious disease prevention and control, and the percentage of practice towards infectious disease prevention and control were 73.22%, 96.83%, and 60.19%, respectively. Multivariable logistic regression analysis revealed that girls (OR=1.460, 95%CI: 1.265-1.685), those who had received health education on infectious disease prevention (OR=2.088, 95%CI: 1.656-2.632), those with adequate knowledge (OR=2.129, 95%CI: 1.816-2.496), and those with positive attitude (OR=4.320, 95%CI: 2.683-6.957) were more likely to form practice for the prevention and control of infectious disease. Conversely, senior high school students (OR=0.667, 95%CI: 0.561-0.794) and those with poor self-rated health status (OR=0.678, 95%CI: 0.495-0.930) were less likely to form practice for the prevention and control of infectious disease.
Conclusions
The attitude towards infectious disease prevention and control among primary and middle school students in Gongshu District is relatively high, but their awareness and practice are low. It is recommended to strengthen health education on infectious disease prevention and control knowledge in a targeted manner to promote the formation of prevention and control practice among senior high school students and primary and middle school students who self-assess their health status as poor.
3.Feature of Cardiovascular-kidney-metabolic Syndrome Among Ethnic Minorities in Yunnan,China

Objectives:To investigate the epidemiological characteristics and ethnic differences of cardiovascular-kidney-metabolic syndrome(CKM)among the Hani,Dai,Bai,and Lisu populations in Yunnan Province,and to provide evidence for developing effective prevention and control strategies for CKM.Methods:A cross-sectional survey was conducted among four ethnic minority groups.A total of 3 906 permanent residents aged 18 years and older were enrolled using a multistage cluster random sampling method.CKM stages(0-4)were defined based on the 2023 American Heart Association criteria,stages 3-4 were classified as advanced CKM.Descriptive statistics and chi-square tests were used to compare the prevalence of CKM stages across ethnic groups.Modified Poisson regression was applied to estimate relative risk(RR)and 95%confidence intervals(CI)for factors associated with advanced CKM.Results:The prevalence rates of CKM stage 1 and above among the Hani,Dai,Bai and Lisu ethnic groups were 80.1%,87.3%,84.8%and 67.8%,respectively.The prevalence of CKM was generally higher in males than in females,and the prevalence of CKM increased significantly with age.The Dai ethnic group had the highest prevalence of advanced CKM(24.7%,95%CI:22.1%-27.4%),while the Lisu ethnic group had the lowest prevalence of advanced CKM(13.7%,95%CI:11.5%-15.9%).Modified Poisson regression analysis showed that older age and higher body mass index were common risk factors for advanced CKM across all four ethnic groups.Additionally,except for the Lisu ethnic group,the other three ethnic groups had specific individual risk factors:among the Hani ethnic group,low educational attainment(RR=2.18,95%CI:1.12-4.25)and low income(RR=1.47,95%CI:1.00-2.18)were the primary risk factors of CKM.Among the Dai ethnic group,smoking(RR=1.60,95%CI:1.07-2.37)and a family history of cardiovascular disease(RR=1.61,95%CI:1.14-2.27)are the primary risk factors of CKM.Among the Bai ethnic group,male gender(RR=0.48,95%CI:0.29-0.79)was the primary risk factor of CKM.Conclusions:The prevalence of CKM stage 1 or higher is relatively high among the four minority ethnic groups in Yunnan province.There are significant differences in staging characteristics and primary risk factors across ethnic groups,necessitating the development of stratified,differentiated intervention strategies to achieve precise prevention and control and ethnic health equity in terms of CKM.
4.Literature study on traditional Chinese medicine syndrome type and syndrome element distribution of in vitro fertilization and embryo transfer failure

Objective To analyze the clinical research literature of in vitro fertilization and embryo transfer(IVF-ET)failure,and summarize the traditional Chinese medicine(TCM)syndromes and syndrome elements,so as to provide theoretical basis for TCM syndrome differentiation and treatment of IVF-ET failure.Methods Using"in vitro fertilization and embryo transfer""IVF-ET"and"embryo transfer failure"as search terms,the literature related to TCM that had appeared in CNKI and Wanfang Data Knowledge Service Platform were searched since these database was constructed.After excluding irrelevant literature,statistical analysis was performed on the frequency and frequency of symptoms and signs.Results Kidney Yang deficiency,kidney deficiency,blood stasis,kidney Yin deficiency and liver qi stagnation were the common syndroms of failure.The disease location with high frequency of syndrome elements was kidney,and the disease was blood stasis.Conclusion Kidney-Yang deficiency syndrome is the most common type of failure,and TCM disease location syndrome is the most common in kidney.The disease deficiency is mostly Yang deficiency,and the excess is mostly blood stasis.

7.Overexpressed SIRT6 ameliorates doxorubicin-induced cardiotoxicity and potentiates the therapeutic efficacy through metabolic remodeling.

Since the utilization of anthracyclines in cancer therapy, severe cardiotoxicity has become a major obstacle. The major challenge in treating cancer patients with anthracyclines is minimizing cardiotoxicity without compromising antitumor efficacy. Herein, histone deacetylase SIRT6 expression was reduced in plasma of patients treated with anthracyclines-based chemotherapy regimens. Furthermore, overexpression of SIRT6 alleviated doxorubicin-induced cytotoxicity in cardiomyocytes, and potentiated cytotoxicity of doxorubicin in multiple cancer cell lines. Moreover, SIRT6 overexpression ameliorated doxorubicin-induced cardiotoxicity and potentiated antitumor efficacy of doxorubicin in mice, suggesting that SIRT6 overexpression could be an adjunctive therapeutic strategy during doxorubicin treatment. Mechanistically, doxorubicin-impaired mitochondria led to decreased mitochondrial respiration and ATP production. And SIRT6 enhanced mitochondrial biogenesis and mitophagy by deacetylating and inhibiting Sgk1. Thus, SIRT6 overexpression coordinated metabolic remodeling from glycolysis to mitochondrial respiration during doxorubicin treatment, which was more conducive to cardiomyocyte metabolism, thus protecting cardiomyocytes but not cancer cells against doxorubicin-induced energy deficiency. In addition, ellagic acid, a natural compound that activates SIRT6, alleviated doxorubicin-induced cardiotoxicity and enhanced doxorubicin-mediated tumor regression in tumor-bearing mice. These findings provide a preclinical rationale for preventing cardiotoxicity by activating SIRT6 in cancer patients undergoing chemotherapy, but also advancing the understanding of the crucial role of SIRT6 in mitochondrial homeostasis.
8.Investigation on outbreaks of acute respiratory tract infection caused by respiratory syncytial virus in kindergartens in Hangzhou

Objective:
In order to analyze the characteristics of the outbreak of acute respiratory tract infection in children caused by respiratory syncytial virus(RSV).
Methods:
The field epidemiological investigations were conducted for the two outbreaks in kindergartens in Hangzhou. Data were analyzed by descriptive method. Samples with positive respiratory syncytial virus nucleic acid were sequenced using PCR.
Results:
The two outbreaks occurred in kindergartens. There were 21 cases in kindergarten A, lasting 11 days, and 43 cases in kindergarten B, lasting 33 days. The epidemic curve showed a proliferation pattern. The cases were concentrated in nurseries and K1 classes, primarily among children aged 2-4 years. The most common symptoms were fever and cough, mainly upper respiratory tract infection, and no severe cases were found. Upper respiratory tract samples were collected and detected as positive for RSV. Four samples were sequenced and identified as subgroup B.
Conclusion
During the outbreak of acute respiratory infection in kindergartens, respiratory syncytial virus should be given primary consideration in the process of identification of the outbreak caused by other respiratory infections, and strictly control measures should be taken to reduce the long term impact of the epidemic.
9.Advances on research of human exposure to triclosan.

Triclosan, a broad-spectrum antimicrobial agent, was reported to have been widely detected in various human biological samples such as urine, blood and human milk among foreign populations. In China, limited reports have been found on human exposure to triclosan, and the reported urinary triclosan concentrations were significantly lower than that of American populations. Besides, the potential influencing factors still remain unclear regarding human exposure to triclosan, but evidences suggest that those in middle age and with higher household income and higher social class tend to have higher urinary triclosan concentrations. Furthermore, triclosan exposure tend to differ by sex, geography, heredity, metabolism and life style.
