OBJECTIVE:
To investigate the current status of clinical genetics specialization development and the diagnostic and therapeutic capabilities for hereditary diseases across medical institutions in Shanghai, and to assess the necessity and feasibility of establishing training bases for clinical genetics specialists.
METHODS:
By employing a cross-sectional survey design, the Clinical Genetics Committee of Shanghai Medical Association has conducted questionnaire surveys from March to April 2025 across 54 healthcare institutions in Shanghai (including 33 tertiary hospitals and 21 secondary hospitals). The survey involved administrative departments and medical personnel from 15 clinical specialties. The survey has covered current genetic disease diagnosis and treatment practices, relevant and specialised disease types, genetic department establishment, testing capabilities, personnel teams, and training requirements.
RESULTS:
The results revealed that 78.0% of clinical departments surveyed had treated patients with hereditary disorders. Shanghai possesses diagnostic and therapeutic expertise for over 95% of hereditary diseases listed in its rare disease catalogue, reflecting both the practical clinical demand for such conditions and the city's overall diagnostic and therapeutic strengths in this field. Nevertheless, significant disparities exist in the development of genetics departments across different tiers of healthcare institutions. Resources for genetic testing capabilities (including molecular, cellular, and biochemical testing) are also unevenly distributed across different tiers of hospitals. The survey further revealed that only 26.0% of departments believe that their current physician structure fully meets the diagnostic and treatment demands. Over 90% of departments consider standard training for clinical genetic specialists necessary, with 74.0% expressing willingness to participate in establishing training bases. Based on above findings and thorough deliberation, the Clinical Genetics Committee of the Shanghai Medical Association proposes advancing specialist training and discipline development through establishing a standard training system. The committee has drafted a three-year training protocol featuring a "joint training"-centered model, recommending a pilot-first, dynamically optimized strategy for steadily advancing training base development.
CONCLUSION
Shanghai faces substantial demand for genetic disease diagnosis and treatment, yet exhibits shortcomings in clinical genetics specialization development, resource allocation, and talent pipeline cultivation. To establish a standard training system holds significant practical importance and is underpinned by a broad demand.

Acta Universitatis Medicinalis Anhui 2025;60(7):1258-1265
Objective :
To explore the effect and mechanism of total saponins of astragalus membranaceus combined with bone marrow mesenchymal stem cells (BMSCs) regulating the stromal cell-derived factor-1 (SDF-1) / chemo- kine receptor 4 ( CXCR4) pathway in the treatment of autoimmune premature ovarian failure ( POF) model rats.
Methods :
Rats were randomly divided into POF group, total saponins of astragalus membranaceus group,BMSCs group ,combination group ,combination + SDF-1 / CXCR4 pathway inhibitor ( AMD3100 ) group ,and control group, with 12 rats in each group.Except for the control group, all other groups of rats needed to construct a POF model. After successful modeling, treatment would begin. The drug injected into the tail vein once a week, and the drug injected into the abdominal cavity once a day for 4 weeks.ELISA was applied to detect the levels of anti ovari- an antibodies (AOAB) ,β-endorphin ( β-EP) ,vascular endothelial growth factor (VEGF) ,tumor necrosis factor- α (TNF-α) ,and interferon-γ ( INF-γ) in serum.Changes in ovarian index were detected.HE staining was ap- plied to detect the number of mature follicles, developing follicles, and blocked follicles in the ovary.Flow cytome- try was applied to detect the proportion of BMSCs in the ovaries.QRT-PCR was applied to detect the mRNA ex- pression of mesenchymal transition factor (cMET) ,hepatocyte growth factor (HGF) ,SDF-1,and CXCR4 in ovar- ian tissue.Western blot was applied to detect SDF-1 and CXCR4 proteins in ovarian tissue.
Results :
Compared with the control group, the serum levels of AOAB,TNF-α , INF-γ , and the number of blocked follicles in the ova- ries were higher, while the serum levels of β-EP,VEGF,ovarian index, the numbers of mature follicles and devel- oping follicles in the ovary, the expression of SDF-1,CXCR4 mRNA and protein were lower in the POF group (P < 0. 05) .Compared with the POF group, the serum levels of AOAB,TNF-α , INF-γ , and the number of blocked follicles in the ovaries were lower, while the serum levels of β-EP,VEGF,ovarian index, the numbers of mature follicles and developing follicles in the ovary, the proportion of BMSCs,the expression of cMET ,HGF,SDF-1, CXCR4 mRNA and SDF-1,CXCR4 protein were higher in the total saponins of astragalus membranaceus group, BMSCs group, and combination group, and the corresponding indicators in the combination group showed the most obvious change trend (P<0. 05) .AMD3100 reversed the effect of total saponins of astragalus membranaceus com- bined with BMSCs on POF rats.
Conclusion
The combination of total saponins of astragalus membranaceus and BMSCs may inhibit inflammation in POF rats, promote follicular development, increase the proportion of BMSCs in the ovaries, and promote their homing by activating the SDF-1 / CXCR4 pathway, thereby improving ovarian func- tion.

Shengmai formula, composed of Ginseng Radix et Rhizoma,Ophiopogon Radix and Schisandrae Chinensis Fructus,is a classic and famous formula. It is a representative formula for"supplementing qi,nourishing yin,and generating fluid"in Traditional Chinese Medicine theory. To date,a wide range of Shengmai formulae have been developed according to different medical applications, but the quality evaluation standards are at a relatively low level, and most of them only specify the individual components of a single herb, making it difficult to ensure clinical efficacy and safety. At the same time, the physical and chemical identification methods of Shengmai formula have been constantly updated, allowing for greater progress in research on its main chemical components such as saponins, lignans and flavonoids. However, there is little systematic summarization of the chemical components and analytical methods.Based on the existing references,we systematically summarized ginsenosides, ophiopogonins, schisandra lignans, homoisoflavonoids and some other compounds in this paper,as well as the quality standards of Shengmai formulae and their analytical methods in order to aid clinical research and formulation manufacture.

Objective:To explore the relationship of telomere length, mitochondrial DNA copy number of peripheral blood with hypertension and the interaction between telomere length and mtDNA-CN on hypertension in coal miners. Methods:A case control study was conducted in a coal mine of Shanxi province from July to December of 2013, in which 325 healthy workers were selected as the control group and 378 workers with hypertension as the case group. The information about general demographic characteristics and life behavior habits of the subjects were collected through questionnaire. Levels of telomere length and mtDNA-CN in peripheral blood were detected by real-time PCR. Unconditional logistic regression was used to examine the association between hypertension and telomere length, mtDNA-CN. The interaction test between telomere length and mtDNA-CN on hypertension was performed by adding the interaction term in the corresponding model. Results:The mean telomere length of the workers in the case group was (1.50±0.55) kb, and that of the control group was (2.01±0.62) kb, the difference between two groups was significant ( t=11.68, P<0.001). The correlation analysis showed that telomere length was positively correlated with mtDNA-CN ( r=0.157, P=0.002) in the case group. Multivariate analysis showed that telomere length ( OR=4.408, 95% CI: 3.012-6.452), age ( OR=0.417, 95% CI: 0.284-0.613), BMI ( OR=1.357, 95% CI: 1.162-1.584), monthly household income level ( OR=0.656, 95% CI: 0.553-0.778) and work duration ( OR=1.249, 95% CI: 1.100-1.417) were influencing factors of hypertension. The multiply interaction between telomere length and mtDNA-CN was significant on hypertension ( OR=1.267, 95% CI: 1.094-1.468). Conclusions:The results suggest shorter telomere length is a risk factor of hypertension. There is a multiply interaction between telomere length and mtDNA-CN on hypertension. However, the association between mtDNA-CN and hypertension was not found.
