1.Effect of Angong Niuhuang pill combined with conventional treat-ment on neurological function recovery in patients with phlegm-heat internal closed cerebral hemorrhage:A randomized controlled trial based on the AMPK-PGC1α-NRF2 pathway

AIM:To examine the impact of An-gong Niuhuang pill in conjunction with convention-al therapy on the recovery of neurological function in patients with cerebral hemorrhage(ICH)charac-terized by phlegm-heat internal obstruction,and to evaluate its efficacy in enhancing neurological func-tion,promoting brain tissue repair,and mitigating oxidative stress.Additionally,this study aims to elu-cidate the potential mechanisms underlying these effects through the AMPK-PGC1α-Nrf2 pathway.METHODS:A total of 130 patients with ICH charac-terized by phlegm-heat internal closure were re-cruited and randomly allocated into a control group(receiving conventional treatment)and a study group(receiving Angong Niuhuang pill in combination with conventional treatment)using the random number table method,with 65 cases in each group.Post-treatment assessments included the National Institutes of Health Stroke Scale(NI-HSS)score,Glasgow Coma Scale(GCS)score,modi-fied Rankin scale(mRS)score,as well as tissue lev-els of AMP-activated protein kinase(AMPK),peroxi-some proliferator-activated receptor gamma coacti-vator-1α(PGC1α),and nuclear factor erythroid 2-related factor 2(NRF2).Additionally,serum levels of superoxide dismutase(SOD),malondialdehyde(MDA),reactive oxygen species(ROS),interleukin-10(IL-10),tumor necrosis factor-α(TNF-α),inter-leukin-6(IL-6),glutathione peroxidase 4(GPX4),ac-yl-coenzyme A synthetase long-chain family mem-ber 4(ACSL4),iron ions,tumor volume,brain ede-ma volume,and the incidence of adverse reactions were evaluated.RESULTS:Each group comprised 65 cases.Post-treatment,the NIHSS score(9.97±1.22)and mRS score(2.29±0.33)in the Research Group were significantly lower than those in the Control Group(11.17±1.52 and 2.64±0.45,respec-tively),with p-values less than 0.05.Additionally,the GCS score for the Research Group(14.57±1.19)was notably higher compared to the control group(13.18±1.05)(P<0.05).After treatment,the expres-sions of AMPK[(3.34±0.81)ng/mL],PGC1α[(2.30±0.67)ng/mL]and NRF2[(3.72±0.85)ng/mL]in the tissues of the group were higher than those of the control group[(2.63±0.65)ng/mL,(1.83±0.70)ng/mL,(2.91±0.96)ng/mL](P<0.05).After treatment,the levels of serum SOD[(98.76±6.67)U/mL]and IL-10[(40.37±5.61)pg/mL]in the research group were higher than those in the Control group group[(89.65±6.89)U/mL,(35.69±4.65)pg/mL](P<0.05);the serum levels of MDA[(3.36±0.62)nmol/mL],ROS[(126.35±23.74)U/mL],TNF-α[(17.22±2.07)pg/mL]and IL-6[(33.37±3.76)pg/mL]in the group were lower than those in the control group group[(3.76±0.83)nmol/mL,(159.85±26.67)nmol/mL,(19.15±2.34)nmol/mL,(41.26±4.91)nmol/mL](P<0.05).After treatment,the serum GPX4 level in the Research Group[(9.87±1.25)ng/mL]was higher than that in the control group[(8.16±1.12)ng/mL](P<0.05).The serum levels of ACSL4[(8.74±1.45)ng/mL]and iron ion[(27.44±3.35)μmol/L]in the study group were lower than those in the control group(10.12±2.11)ng/mL,(30.46±3.17)μmol/L](P<0.05).After treatment,the hematoma volume[(6.46±1.13)mL]and brain edema volume[(11.47±1.76)mL]in the research group were bet-ter than those in the Control group[(8.71±1.02)mL,(3.41±2.04)mL](P<0.05).CONCLUSION:The combination of Angong Niuhuang pill with conven-tional treatment demonstrates significant efficacy in improving neurological function,promoting brain tissue repair,and alleviating oxidative stress,thereby exhibiting high application value in clinical practice.
2.Clinical efficacy of Tirellizumab combined with lenvatinib in the treat-ment of advanced liver cancer

AIM:To investigate the efficacy and safety of checkpoint inhibitor lenvatinib combined with Tirelizumab in the treatment of advanced he-patocellular carcinoma(HCC).METHODS:A retro-spective analysis was performed on 52 patients with advanced HCC treated with Renvatinib,tirelli-zumab or their combination from January 2021 to December 2022 in Zhongda Hospital Affiliated to Southeast University,with disease progression,death and intolerance as endpoints.Efficacy was as-sessed according to RECIST 1.1 criteria and follow-up was conducted up to June 2023.The main end-points of the study were objective response rate(ORR),disease control rate(DCR),progression-free survival,PFS)and security.RESULTS:The combina-tion therapy significantly improved ORR and mPFS in patients compared with tirelizumab or lenvatinib monotherapy.Compared with single tirellizumab or lenvastinib,there was no significant difference in the adverse reactions associated with the combina-tion treatment,suggesting that the combination of the two was generally well tolerated and the side effects are controllable.CONCLUSION:Lonvastinib combined with tirelizumab is more effective in the treatment of HCC,can significantly prolong PFS,and is generally well tolerated,which may be a po-tential treatment for advanced HCC.

5.Microneedles as transdermal drug delivery system for enhancing skin disease treatment.

Microneedles (MNs) serve as a revolutionary paradigm in transdermal drug delivery, heralding a viable resolution to the formidable barriers presented by the cutaneous interface. This review examines MNs as an advanced approach to enhancing dermatological pathology management. It explores the complex dermis structure and highlights the limitations of traditional transdermal methods, emphasizing MNs' advantage in bypassing the stratum corneum to deliver drugs directly to the subdermal matrix. The discourse outlines the diverse typologies of MNs, including solid, coated, hollow, hydrogel, and dissolvable versions. Each type is characterized by its unique applications and benefits. The treatise details the deployment of MNs in the alleviation of cutaneous cancers, the administration of inflammatory dermatoses such as psoriasis and atopic dermatitis, and their utility in wound management. Additionally, the paper contemplates the prospects of MNs within the realm of aesthetic dermatology and the burgeoning market traction of cosmetic MN formulations. The review summarizes the scientific and commercial challenges to the clinical adoption of MN therapeutics, including dosage calibration, pharmacodynamics, biocompatibility, patient compliance, sterilization, mass production, and regulatory oversight. It emphasizes the need for ongoing research, innovation, and regulatory harmonization to overcome these obstacles and fully realize MNs' potential in treating skin diseases and improving patient welfare.
6.Clinical efficacy and learning curve of robot-assisted thymectomy via subxiphoid approach

Objective To explore the clinical efficacy and learning curve of robot-assisted thymectomy via subxiphoid approach. Methods The clinical data of patients with robot-assisted thymectomy surgery via subxiphoid approach performed by the same surgical team in the Department of Thoracic Surgery of Shanghai Pulmonary Hospital from February 2021 to August 2022 were retrospectively analyzed. The cumulative sum (CUSUM) analysis and best fit curve were used to analyze the learning curve of this surgery. The general information and perioperative indicators of patients at different learning stages were compared to explore the impact of different learning stages on clinical efficacy of patients. Results A total of 67 patients were enrolled, including 31 males and 36 females, aged 57.10 (54.60, 59.60) years. The operation time was 117.00 (87.00, 150.00) min. The best fitting equation of CUSUM learning curve was y=0.021 2x3–3.192 5x2 +120.17x–84.444 (x was the number of surgical cases), which had a high R2 value of 0.977 8, and the fitting curve reached the top at the 25th case. Based on this, the learning curve was divided into a learning period and a proficiency period. The operation time and intraoperative blood loss in the proficiency stage were significantly shorter or less than those in the learning stage (P<0.001), and there was no statistical difference in thoracic drainage time and volume between the two stages (P>0.05). Conclusion The learning process of robot-assisted thymectomy via subxiphoid approach is safe, and this technique can be skillfully mastered after 25 cases.
7.Lycorine alleviates oxaliplatin-induced chemotherapy induced pain in mice via reducing COX-2 /NLRP3 inflammatory signaling

Objective:
To explore the effect and mechanism of lycorine on oxaliplatin ( OXA) induced chemotherapy pain in mice.
Methods :
40 mice were randomly divided into 4 groups,10 mice per group,which were respectively divided into control group,model group,administration group,and inhibitor group.A mouse model of chemotherapy induced pain was established by intraperitoneal injection of OXA for 5 consecutive days.Intrathecal administration of lycorine was performed.Behavioral changes and expression levels of inflammatory related proteins were detected .
Results :
Compared with control group,model group mice exhibited the increased number of spontaneous flinches,decreased mechanical nociceptive threshold,decreased movement distance and latency,and up-regulated expression levels of interleukin-1 β (IL-1 β) ,astrocytic marker glial fibrillary acidic protein ( GFAP) ,cyclooxygenase-2( COX-2) ,NOD-like receptor protein 3 ( NLRP3 ) ,cysteinyl aspartate and specific proteinase 1 ( Caspase- 1) .Compared with model group,lycorine administration reduced the number of spontaneous flinches,increased mechanical nociceptive threshold ,enhanced the movement distance and latency ,bound and reduced COX-2 expression,down-regulated the expression levels of IL-1 β , GFAP ,NLRP3 and Caspase-1.
Conclusion
Lycorine reduces COX-2 expression,inhibits NLRP3 inflammasome activation,suppresses spinal inflammation,consequently alleviates pain behaviors and improved motor ability of mice.
8.Comparison of different obesity indices in predicting risk of hypertension among adults in Gansu Province

【Objective】 To compare the ability of body mass index (BMI), body fat percentage (BFP), waist circumference (WC), waist-to-height ratio (WHtR), visceral fat index (VFI) and the combinations of two kinds of obesity indices to predict the risk of hypertension. 【Methods】 Data collected in the baseline survey of “Gansu Province’s Urban and Rural Natural Population Cohort Establishment and Tumor Follow-up Study” were analyzed. Area under the curve (AUC) of ROC curve with covariates was used to analyze and compare the effects of individual obesity evaluation index and the combination of two kinds of obesity indices in predicting the risk of hypertension. 【Results】 Analyses of data of 20,079 adults showed that the AUC of BMI, WC, WHtR, BFP and VFI was 0.636, 0.604, 0.615, 0.614 and 0.619, respectively. AUC of the combination of BMI and WC (0.643) was higher than that of BMI (0.636); however, the change rate of AUC was only 1.09%. AUC of the combinations of WC, WHtR and VFI, the three central obesity evaluation indices, and BFP, a general obesity evaluation index, were lower than that of BMI. The optimal cutoff value for BMI was 24.2 kg/m2. 【Conclusion】 The effect of BMI in predicting the risk of hypertension is better than that of BFP, WC, WHtR and VFI. The effects of the combinations of the two kinds of obesity evaluation indices are not better than that of BMI. To prevent and control hypertension, adults should keep their BMI under overweight.
9.Research progress of the orexinergic system in migraine attack generation

Migraine is a complex disorder of brain function. The hypothalamus has been identified to play a crucial role in attack generation and secretes various neuropeptides. The orexinergic system plays a role in energy metabolism, arousal, sleep, stress and pain modulation. These disorders are closely related to clinical symptoms of migraine. Therefore, the study of the mechanism of hypothalamic orexinergic system in migraine may provide a new perspective for treatment. This article discusses the relationship between hypothalamic orexin system and migraine premonitory symptoms, and briefly summarizes the possible role and mechanism of hypothalamic orexin system in migraine.
