1.Theoretical Basis and Application Practice of Chiqilin Powder in the Treatment of Atherosclerosis

Atherosclerosis(AS)is the important pathological foundation of cardiovascular and cerebrovascular diseases.There is a high degree of consistency between AS and yin-nature carbuncles in terms of etiology,disease-location,severity of illness,pathogenesis,therapies and progression of disease.Based on the theory of yin-nature carbuncles and after the analysis of the characteristics of zang-fu organs mainly involved in AS and the intricate pathogenesis of AS,this paper put forward the use of warming-eliminating,warming-expulsing and warming-tonifying therapies together for treating AS,and established Chiqilin Powder,the experienced prescription including triplet medicinals,i.e.,Notoginseng Radix et Rhizoma,Draconis Sanguis,and Fermentum Rubrum.In the prescription,Notoginseng Radix et Rhizoma mainly has the warming-tonifying and warming-expulsing actions,and also exerts the warming-eliminating actions;Draconis Sanguis exerts eliminating,expulsing and tonifying actions simultaneously;Fermentum Rubrum mainly has the warming-tonifying and warming-eliminating actions,and also exerts the warming-expulsing actions.In clinical practice,the application of Chiqilin Powder in the treatment of AS patients with yang deficiency and phlegm blended with blood stasis syndrome has achieved remarkable therapeutic effects,which will provide reference for the clinical use of Chinese herbal medicine in the treatment of AS.
2.Stage-Based Intervention in Atherosclerosis Using the "Attacking,Supplementing,Dispersing,Dissipating" Method Based on the Accumulation Syndrome Theory

Atherosclerosis is a complex pathological condition resulting from lipid deposition, chronic inflammatory responses, and fibrosis, with a prolonged disease course and multifactorial etiology. Based on the traditional Chinese medicine (TCM) theory of accumulation syndrome, atherosclerosis can be classified under this category, with its pathogenesis involving phlegm, blood stasis, deficiency, and accumulation. This paper proposed a stage-based intervention strategy using the four therapeutic principles of "attacking, supplementing, dispersing, dissipating", and divided into six stages based on the pathological progression, including the stage of accumulation before formation, the stage of accumulation already formed, the stage of nucleus accumulation, the stage of nucleus accumulation decay, the stage of nucleus accumulation consolidation, and the stage of severe stenosis of nucleus. At different stages, the intervention focuses on reinforcing healthy qi and consolidating the root, tonifying the kidneys and spleen, dispersing and removing turbidity, removing phlegm stagnation, promoting qi circulation, dispersing accumulations and removing stasis, attacking accumulation and expelling stasis, directing the turbid downward and dispersing accumulation, and treatment would be adjusted based on specific symptoms, which provides a theoretical framework for the prevention and treatment of atherosclerosis with TCM.
3.The Role of Physical and Mental Exercise in the Association Between General Anesthesia and Mild Cognitive Impairment

[Objective]To explore the correlation between general anesthesia and mild cognitive impairment in older adults so as to provide new ideas for early prevention and timely intervention of mild cognitive impairment(MCI).[Methods]Based on the baseline survey of the Hubei memory and aging cohort study(2018-2023),the participants completed a thorough neuropsychological assessment and physical examination,and self-reported a history of general anesthesia and surgery.The association of general anesthesia and MCI in the elderly was analyzed using the logistic regression model.In addition,the stratification and interaction analysis of anesthesia history,anesthesia number and physical intellectual exercise were conducted separately.[Results]A total of 5 069 older adults aged 65 and above were included in this study,including 3 692 city dwellers and 1 377 rural people,among whom were 2 584 women(51%).Out of the 1 472 participants with history of general anesthesia,249 people(17.4%)had MCI.After controlling for confounding factors,there was a 39.6%increased risk of MCI in older adults who underwent general anesthesia[OR=1.396,95%CI(1.169,1.668),P<0.001],suggesting that general anesthesia may be an independent influence on MCI.For the older adults who had one general anesthesia[OR=1.235,95%CI(1.001,1.523),P=0.049],two general anesthesia[OR=1.779,95%CI(1.292,2.450),P<0.001],and three OR more general anesthesia[OR=2.395,95%CI(1.589,3.610),P<0.001],their risks of MCI were increased by 23.5%,77.9%,and 139.5%,respectively.Compared with the older adults without a history of general anesthesia who did not exercise,the risk of developing MCI was significantly negatively correlated with the exercise group,cognitive exercise group,and combined exercise and cognitive exercise groups(all P<0.001).The risk of developing MCI in the exercise group was 60.2%of that in the no exercise group[OR=0.602,95%CI(0.456,0.795)],the risk in the cognitive exercise group was 42.4%of that in the no exercise group[OR=0.424,95%CI(0.294,0.613)],and the risk in the combined exercise and cognitive exercise group was 27.0%of that in the no exercise group[OR=0.270,95%CI(0.208,0.353)].In the older adults with a history of general anesthesia,compared with the no exercise group,the risk of developing MCI was significantly negatively correlated with the cognitive exercise group and the combined exercise and cognitive exercise group(all P<0.05).The risk of developing MCI in the cognitive exercise group was 47.7%of that in the no exercise group[OR=0.477,95%CI(0.256,0.892)],the risk in the combined exercise and cognitive exercise group was 34.5%of that in the no exercise group[OR=0.345,95%CI(0.220,0.540)],while the risk in the exercise-only group did not show a significant difference.[Conclusion]The risk of MCI increased significantly in older adults with a history of general anesthesia,and this risk increased with the times of anesthesia.Physical and mental exercise reduces the risk of MCI.it is recommended that older adults with a history of anesthesia incorporate physical and mental exercise into their daily lives to prevent mild cognitive impairment.
4.Summary of the best evidence for blood glucose monitoring and management in patients with diabetes of the exocrine pancreas

Objective To retrieve,evaluate and summarize the best evidence for blood glucose monitoring and management in patients with diabetes of the exocrine pancreas(DEP),and to provide evidence-based basis for clinical practice.Methods According to the"6S"evidence model,relevant evidence on blood glucose management in patients with DEP was searched from computer decision support systems,guideline networks,professional association websites and databases from top to bottom,covering clinical decision-making,clinical practice guidelines,evidence summaries,systematic reviews,expert consensuses,and randomized controlled trials.The search scope was from the establishment of the databases to June 30,2024.Totally 2 researchers independently evaluated the quality of the included literature,and extracted data and summarized evidence that met the criteria.Results A total of 10 articles were included,including 3 guidelines,1 expert consensus,2 systematic reviews,l clinical decision,1 RCT,and 2 cohort studies.Finally,26 pieces of best evidence were formed from 5 aspects,including blood glucose management team construction and goals,blood glucose monitoring methods,glucose control medication management,lifestyle health education and treatment of hypoglycemia.Conclusion This study summarizes the best evidence for blood glucose monitoring and management in DEP patients,which can provide resource preparation for clinical translation and a basis for medical staff to carry out blood glucose management in DEP patients.
5.Perioperative care of a patient undergoing total pancreatectomy with autogenous islet cell autotransplan-tation for pancreatic cancer

To summarize the nursing experience of a patient with pancreatic cancer who underwent total pancreatectomy combined with islet cell transplantation.Nursing points:goal-oriented management of endocrine function,being alert to the occurrence of"brittle diabetes";the dose of pancreatic enzymes was dynamically adjusted by doctors and nurses to improve exocrine insufficiency;the whole-process nutritional risk assessment and individualized nutritional support strategies were implemented.Early identification and standardized management of gastroparesis syndrome can improve gastric motility of patients.After careful treatment and nursing care,the patient's islet function was stable,and the endocrine and exocrine functions of the pancreas recovered well.The patient was discharged 15 days after the operation.During the follow-up of 2 months,the patient's condition was stable,and no serious blood glucose disorder and other serious complications occurred.

8.Nursing care of a patient with bilothorax after autologous liver transplantation

To summarize the nursing experience of a patient with bilothorax after autolo-gous liver transplantation.The key points of nursing include:early identification of reexpan-sion pulmonary edema,the start of the hospital rapid response team to give efficient rescue;early identification of bilothorax,and maintenance of effective drainage combined with accurate anti-infection treatment;individual and stage lung rehabilitation training to promote patients lung rehabilitation;step anticoagulant therapy is safely adminis-tered under dynamic assessment of bleeding risk.Through careful treatment and nursing,the patient was discharged 63 days after surgery.
9.Path analysis on influencing factors of health information-seeking behavior in stroke patients

Objective:To explore the influencing factors of health information-seeking behavior in stroke patients and analyze its influencing path.Methods:A total of 280 stroke patients in Xuzhou Municipal Hospital Affiliated to Xuzhou Medical University from February to July 2022 were selected as research objects by convenience sampling method. General Data Questionnaire, Stroke Self-Efficacy Scale, Medical Social Support Scale, Electronic Health Literacy Scale and Health Information-Seeking Behavior Questionnaire were used to investigate patients. Multiple linear regression analysis was used to investigate the influencing factors of health information seeking behavior in stroke patients. Amos 26.0 was used to establish the structural equation model and analyze the path. A total of 280 questionnaires were distributed in this study, and 268 were effectively collected, with an effective recovery rate of 95.71% (268/280) .Results:The score of health information-seeking behavior in stroke patients was (108.53±28.34). Multiple linear regression analysis showed that education, residence, number of hospitalizations due to stroke, self-efficacy and social support were the influencing factors of health information searching behavior of stroke patients ( P<0.05), which could explain 88.3% of the total variation. The structural equation model results showed that self-efficacy of stroke patients had a complete mediating effect between social support and health information searching behavior, and the indirect effect accounted for 80.29% of the total effect. Conclusions:The health information-seeking behavior of stroke patients is at a moderate level. Medical staff should formulate targeted intervention measures according to its influencing factors, and give full play to the mediating role of self-efficacy between social support and patients' health information-seeking behavior.
