1.Application of Classic Prescription Xiao Chaihutang in Treatise on Febrile Diseases and Miscellaneous Diseases in Gynecology: A Review

Xiao Chaihutang,an ancient classic prescription,originated from the Treatise on Febrile Diseases and Miscellaneous Diseases (hereinafter referred to as Treatise on Febrile Diseases) written by ZHANG Zhongjing in the Eastern Han Dynasty. It is a representative prescription for reconciling Shaoyang. It can strengthening the body resistance to eliminate pathogenic factors,clear away the evil heat,and harmonize the stomach Qi and can be used to treat various diseases caused by insufficient vital energy,lingering evil energy,dysregulation of ascending and descending,disharmony between the exterior and interior,and dysfunction of the liver,gallbladder,and stomach,such as liver-gallbladder damp heat syndrome,disharmony between the exterior and interior (intermittent chills and fever,malaria),abnormal ascending and descending (vomiting,thirst,irritability,constipation, and depression),and disharmony between Yin and Yang. The core point of the pathogenesis is that the lack of positive energy and the invasion of evil energy,with an unfavorable Shaoyang cardinal system,which is consistent with the pathophysiological characteristics of women. In gynecology,on the one hand,women are prone to Qi and blood deficiency,which affects the body's ability to fight off evil influences. On the other hand,women's liver function is the most important,and they are prone to the pathological manifestations of catharsis dysfunction,Qi stagnation,and loss of function. Therefore,Xiao Chaihutang is used in the treatment of various gynecological diseases such as menstruation,leukorrhea,childbirth,and lactation. The key to using ancient formulas lies in flexible application. As long as the additions and subtractions are appropriate,different diseases can be treated in the same way. If the clinical syndrome can be adjusted according to the changes,the ratio of monarch drug and ministerial drug group,the nature of cold and warm,and the ability to tonify and diarrhea,it can be widely used in the treatment of a variety of gynecological diseases based on the reconciliation of the pathogenesis of Shaoyang,the essence of the same treatment of different diseases,and flexible tailoring. This article aims to study the application of the classic prescription Xiao Chaihutang from the Treatise on Febrile Diseases in gynecology,summarize its characteristics of pathological mechanism integration,and help peers in the treatment of gynecological diseases.
2.Construction of a predictive model for lateral lymph node metastasis in patients with rectal cancer below peritoneal reflection

Objective To explore the lateral lymph node metastasis in patients with rectal cancer below peritoneal reflection and construct predictive models.Methods A total of 102 patients with rectal cancer be-low peritoneal reflection admitted in the hospital from January 2020 to April 2023 were selected as the re-search objects.According to the existence of lateral lymph node metastasis(LLNM),the patients were divided into metastatic group(n=31)and non-metastatic group(n=71).The general clinical data,tumor pathologi-cal features,laboratory indexes[carcinoembryonic antigen(CEA).carbohydrate antigen 199(CA199),neutro-phil/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR),transforming growth factor(TGF)-α and TGF-β1]between the two groups were compared.The risk factors of LLNM in patients with rectal cancer be-low peritoneal reflection were screened by Spearman correlation analysis and Logistic regression analysis,and the predictive model was constructed and the efficacy of the model was evaluated.Results The mean maxi-mum tumor diameter,the short diameter of lateral lymph nodes,the proportion of poorly differentiated pa-tients and the proportion of patients with T3-T4 stage in the metastasis group were significantly higher than those in the non-metastasis group(P<0.05).The average levels of TGF-α,TGF-β1 and NLR in the metasta-sis group were significantly higher than those in the non-metastasis group(P<0.05).Spearman correlation a-nalysis,univariate and multivariate Logistic regression analysis showed that poor differentiation,T3-T4 stage,maximum tumor diameter,longer short diameter of lateral lymph nodes,higher levels of TGF-α,TGF-β1 and NLR were independent risk factors for LLNM in patients with rectal cancer below peritoneal reflec-tion.Receiver operating characteristic(ROC)curve analysis showed that the area under the curve(AUC)of the predictive model based on the above risk factors was 0.915(95%CI:0.847-0.984),which had high pre-dictive performance.Conclusion Patients with LLNM and rectal cancer below peritoneal reflection often have longer tumor diameter and lateral lymph node short diameter,and higher levels of TGF-α,TGF-β1 and NLR.Lateral lymph node dissection should be actively carried out for patients with above characteristics,and timely monitoring should be carried out to see if there is still lymph node metastasis after surgery,so as to provide certain clinical basis for improving the judgment of LLNM in rectal cancer below peritoneal reflection.
3.Improvement on Quality Standard of Yuanhu Zhitong Oral Liquid

ObjectiveTo improve the quality standard of Yuanhu Zhitong oral liquid in order to strengthen the quality control of this oral liquid. MethodThin layer chromatography(TLC) was used for the qualitative identification of Corydalis Rhizoma and Angelicae Dahuricae Radix in Yuanhu Zhitong oral liquid by taking tetrahydropalmatine, corydaline reference substances and Corydalis Rhizoma reference medicinal materials as reference, and cyclohexane-trichloromethane-methanol(5∶3∶0.5) as developing solvent, Corydalis Rhizoma was identified using GF254 glass thin layer plate under ultraviolet light(365 nm). And taking petroleum ether(60-90 ℃) -ether-formic acid(10∶10∶1) as developing solvent, Angelicae Dahuricae Radix was identified using a silica gel G TLC plate under ultraviolet light(305 nm). High performance liquid chromatography(HPLC) was performed on a Waters XSelect HSS T3 column(4.6 mm×250 mm, 5 μm) with acetonitrile(A)-0.1% glacial acetic acid solution(adjusted pH to 6.1 by triethylamine)(B) as the mobile phase for gradient elution(0-10 min, 20%-30%A; 10-25 min, 30%-40%A; 25-40 min, 40%-50%A; 40-60 min, 50%-60%A), the detection wavelength was set at 280 nm, then the fingerprint of Yuanhu Zhitong oral liquid was established, and the contents of tetrahydropalmatine and corydaline were determined. ResultIn the thin layer chromatograms, the corresponding spots of Yuanhu Zhitong oral liquid, the reference substances and reference medicinal materials were clear, with good separation and strong specificity. A total of 12 common peaks were identified in 10 batches of Yuanhu Zhitong oral liquid samples, and the peaks of berberine hydrochloride, dehydrocorydaline, glaucine, tetrahydropalmatine and corydaline. The similarities between the 10 batches of samples and the control fingerprint were all >0.90. The results of determination showed that the concentrations of corydaline and tetrahydropalmatine had good linearity with paek area in the range of 0.038 6-0.193 0, 0.034 0-0.170 0 g·L-1, respectively. The methodological investigation was qualified, and the contents of corydaline and tetrahydropalmatine in 10 batches of Yuanhu Zhitong oral liquid samples were 0.077 5-0.142 9、0.126 1-0.178 2 g·L-1, respectively. ConclusionThe established TLC, fingerprint and determination are simple, specific and reproducible, which can be used to improve the quality control standard of Yuanhu Zhitong oral liquid.
4.Assessment of different bone marrow infiltration patterns with MR whole-body diffusion weighted imaging in multiple myeloma

Objective To evaluate the apparent diffusion coefficient(ADC)values of different bone marrow infiltration patterns in multiple myeloma(MM)patients with MR whole-body diffusion weighted imaging(WB-DWI)and to determine the ADC thresholds for different bone marrow infiltration patterns.Methods Nineteen MM patients diagnosed for the first time were selected.The lesions types of each site(cervical spine,ribs,sternum,humerus,scapula,sacral spine,ilium,femur,thoracic spine,and lumbar spine)after the WB-DWI images were visually evaluated,which were divided into focal group(including focal lesion in combined focal and diffuse infiltration)[region of interest(ROI)=141],pure diffuse infiltration group(ROI=150),diffuse lesion in combined focal and diffuse infiltration group(ROI=127),"salt-and-pepper"group(ROI=54),and normal appearance group(ROI=68).ADC values were measured and compared between each group and the receiver operating characteristic(ROC)curve was drawn to distinguish different patterns of bone marrow infiltration.Results There was no statistically significant difference in ADC values between the diffuse lesion in combined focal and diffuse infiltration group and the"salt-and-pepper"group(P>0.99),and there was statistically significant difference in ADC values between the other groups(P<0.05).The ROC curve showed that the area under the curve(AUC)for identifying focal group and the"salt-and-pepper"group was 0.889[95%confidence interval(CI)0.844-0.934],the AUC for identifying pure diffuse infiltration group and the normal appearance group was 0.968(95%CI 0.949-0.987).ADC values were able to accurately and visually differentiate between the different patterns of bone marrow infiltration.Conclusion The ADC values can be used as a quantitative tool to objectively distinguish different bone marrow infiltration patterns in MM patients.
5.Advances in prostate cancer biomarkers.

Prostate cancer is one of the most common malignant tumors in men and posing a serious threat to men's health. Detection methods such as prostate-specific antigen (PSA), prostate biopsy, and magnetic resonance imaging are widely used for prostate cancer screening, but they have low specificity, high cost, and significant risks. Therefore, there is an urgent need to develop highly specific, low-cost, easily obtained, stable, and reliable biomarkers, and use them as the basis to establish non-invasive screening and diagnostic methods for prostate cancer. This paper reviewed the recent advances in the use of prostate cancer biomarkers and combined detection methods for prostate cancer diagnosis and prognosis assessment and provides an in-depth analysis and comparison of different biomarkers and combined detection methods, as well as points out the directions and challenges for future research. The paper emphasizes the importance of developing efficient, cost-effective and easy-to-implement biomarkers to increase the early diagnosis rate of prostate cancer, improve patient prognosis, and reduce the waste of healthcare resources. This paper provides an important theoretical basis and technical guidance for early diagnosis, precise treatment and prognostic evaluation of prostate cancer, and has important reference value for promoting clinical research and practice of prostate cancer.

6.Research progress on the effects of micronutrients on immune function, respiratory infection, and SARS-Cov-2 infection

Micronutrients are important factors affecting the immune system. Micronutrients contribute to maintaining the normal functions of immune system through maintaining the integrity of skin and mucosa, regulating the activity of cytokines, and mediating the proliferation and differentiation of T cells and B cells. Human immune system changes at different stages of growth and development, and micronutrient deficiency can impair the immune system and increase the susceptibility to respiratory infections. Therefore, maintaining the optimal nutritional status is beneficial for the immune system to prevent respiratory infections. Severe acute respiratory syndrome coronavirus 2 (SARS-Cov-2) infection is one of the respiratory infectious diseases that is of most concern recently. Micronutrient supplementation has been shown to have positive effects on the prevention of, symptom relief of and recovery from SARS-Cov-2 infection. Further studies are needed to systematically evaluate the effects of micronutrients on immunity and respiratory infections, and to identify the appropriate timing and dosage.
7.Biochemical characteristics of type 2 diabetes mellitus and its microvascular complications in animal models

Objective By summarizing experimental data of animal models of type 2 diabetes mellitus(T2DM)and its microvascular complications in rats and mice as animal models,the biochemical indexes were analyzed in order to provide theoretical reference for future experiments.Methods The main topics of"type 2 diabetes""diabetic nephropathy""diabetic foot"and"diabetic retinopathy"were searched on China National Knowledge Infrastructure and PubMed databases to screen out the journal literatures related to T2DM and microvascular complications in clinical and animal experiments published from October 31,2019 to October 31,2022.The subjects,disease types,detection indicators and other contents recorded in the literature were sorted out and summarized,and a database was established for statistical analysis.Results A total of 510 literatures were selected,among which fasting blood glucose was the most frequently used biochemical index in T2DM experiment,urine protein level was the most frequently used biochemical index in diabetic nephropathy experiment,and serum inflammatory factors were the most frequently used biochemical index in diabetic foot and diabetic retinopathy experiment.Conclusions By analyzing the application frequency of biochemical indicators in T2DM and its microvascular complications,this study can provide an important reference for clinical diagnosis and treatment of the disease and the development of animal experiments.
8.Effects of low-frequency rTMS combined with paroxetine on clinical symptoms and social abilities in patients with panic disorder

Objective To explore the effects of the combination of low-frequency repetitive transcranial magnetic stimulation(rTMS)and paroxetine on clinical symptoms and social abilities in patients with panic disorder.Methods A total of 90 patients with panic disorder were randomly divided into observation group(n=45,low-frequency rTMS combined with paroxetine)and control group(n=45,paroxetine alone).All were continuously treated for 6 weeks.The clinical symptoms were evaluated with panic disorder severity scale(PDSS)and Hamilton anxiety scale(HAMA)for assessing the therapeutic effect;and the social abilities were evaluated with global assessment function(GAF)and perceived social support scale(PSSS).The occurrence of adverse reactions during treatment was recorded.Results After 4 and 6 weeks of treatment,compared with control group,observation group had lower PDSS score and HAMA score(Ftime=566.213,533.124,P<0.05;Fbetween-group=13.211,4.466,P<0.05),and higher GAF score and PSSS score(Ftime=486.312,497.112,P<0.05;Fbetween-group=62.601,123.510,P<0.05).After 4 and 6 weeks of treatment,the response rates in observation group were 80.00%and 91.11%,higher than 60.00%and 71.11%in control group(P<0.05).There was no significant difference in total incidence of adverse reactions between observation group and control group(22.22%vs 17.78%,P>0.05).Conclusion The combination of low-frequency rTMS and paroxetine can effectively relieve anxiety,improve clinical symptoms,therapeutic effect and social abilities in patients with panic disorder,with certain safety and bringing more benefits as compared with paroxetine alone.
9.Application Progress of Alginate Composite Hydrogels in Cancer Therapy

Alginate composite hydrogel is one of the research hotspots of tumor drug delivery system materials at present. Alginate hydrogel has good biocompatibility and regeneration properties. However, the natural alginate saline gel may not achieve the expected results in the body environment due to the slow degradation and instability of the gel. Alginate has found extensive application in cancer therapy through its combination with other materials and the utilization of ionic cross-linking, covalent cross-linking, and free radical polymerization to form hydrogels. Based on the composite system of alginate brine gel, this paper summarizes the structure and properties of alginate hydrogel, with a focus on recent research into alginate composite hydrogel in common cancer treatments application, summarizes the current research focus and discuss the existing problems of alginate composite hydrogel, so as to provide reference for further expanding the research of alginate composite hydrogel in clinical cancer treatment.
10.Reasons, safety and efficacy analysis for conversion of HAART to TAF/FTC/BIC among HIV-infected patients.

BACKGROUND:
This study aimed to determine the reasons for conversion and elucidate the safety and efficacy of transition to tenofovir alafenamide/emtricitabine/bictegravir sodium (TAF/FTC/BIC) in highly active antiretroviral therapy (HAART)-experienced HIV-infected patients in real-world settings.
METHODS:
We conducted a retrospective cohort study. The treatment conversion rationales, safety, and effectiveness in 1684 HIV-infected patients with previous HAART experience who switched to TAF/FTC/BIC were evaluated at Beijing Ditan Hospital from September 2021 to Auguest 2022.
RESULTS:
Regimen simplification (990/1684, 58.79%) was the most common reason for switching, followed by osteoporosis or osteopenia (375/1684, 22.27%), liver dysfunction (231/1684, 13.72%), decline in tenofovir alafenamide/emtricitabine/elvitegravir/cobicistat (TAF/FTC/EVG/c) with food restriction (215/1684, 12.77%), virological failure (116/1684, 6.89%), and renal dysfunction (90/1684, 5.34%). In patients receiving non-nucleotide reverse transcriptase inhibitors (NNRTI)-containing regimens, lipid panel changes 1 year after switching indicated a difference of 3.27 ± 1.10 mmol/L vs . 3.40 ± 1.59 mmol/L in triglyceride ( P = 0.014), 4.82 ± 0.74 mmol/L vs . 4.88 ± 0.72 mmol/L in total cholesterol ( P = 0.038), 3.09 ± 0.70 mmol/L vs . 3.18 ± 0.66 mmol/L in low-density lipoprotein ( P <0.001), and 0.99 ± 0.11 mmol/L vs . 0.95 ± 0.10 mmol/L in high-density lipoprotein ( P <0.001). Conversely, among patients receiving booster-containing regimens, including TAF/FTC/EVG/c and lopinavir/ritonavir (LPV/r), lipid panel changes presented decreased trends. We also observed an improved trend in viral load suppression, and alanine transaminase (ALT), aspartate transaminase (AST), estimated glomerular filtration rate (eGFR), and serum creatinine levels after the transition ( P <0.001).
CONCLUSION
The transition to TAF/FTC/BIC demonstrated good treatment potency. Furthermore, this study elucidates the motivations behind the adoption of TAF/FTC/BIC in real-world scenarios, providing clinical evidence supporting the stable conversion to TAF/FTC/BIC for HAART-experienced patients.
