Osteoporosis is a prevalent skeletal condition characterized by reduced bone mass and strength, leading to increased fragility. Buqi-Tongluo (BQTL) decoction, a traditional Chinese medicine (TCM) prescription, has yet to be fully evaluated for its potential in treating bone diseases such as osteoporosis. To investigate the mechanism by which BQTL decoction inhibits osteoclast differentiation in vitro and validate these findings through in vivo experiments. We employed MTS assays to assess the potential proliferative or toxic effects of BQTL on bone marrow macrophages (BMMs) at various concentrations. TRAcP experiments were conducted to examine BQTL's impact on osteoclast differentiation. RT-PCR and Western blot analyses were utilized to evaluate the relative expression levels of osteoclast-specific genes and proteins under BQTL stimulation. Finally, in vivo experiments were performed using an osteoporosis model to further validate the in vitro findings. This study revealed that BQTL suppressed receptor activator of NF-κB ligand (RANKL)-induced osteoclastogenesis and osteoclast resorption activity in vitro in a dose-dependent manner without observable cytotoxicity. The inhibitory effects of BQTL on osteoclast formation and function were attributed to the downregulation of NFATc1 and c-fos activity, primarily through attenuation of the MAPK, NF-κB, and Calcineurin signaling pathways. BQTL's inhibitory capacity was further examined in vivo using an ovariectomized (OVX) rat model, demonstrating a strong protective effect against bone loss. BQTL may serve as an effective therapeutic TCM for the treatment of postmenopausal osteoporosis and the alleviation of bone loss induced by estrogen deficiency and related conditions.

2.Research progress on the application of nanozyme materials in the treatment of orthopaedic diseases

Nanomaterials can be used in drug delivery systems to enhance drug targeting and efficacy, and reduce adverse reactions. At the same time, they can also be used in tissue engineering and regenerative medicine to promote bone tissue repair and regeneration. Nanozymes are special nanomaterials with the catalytic activity of biological enzymes, which can mediate efficient biochemical reactions and provide a new strategy for the diagnosis and treatment of orthopaedic diseases. In the treatment of tendon-related diseases, the enzymatic nanohybrid encapsulated by extracellular vesicles can effectively mimic catalase to remove reactive oxygen species, continuously release zinc ions, and induce immune regulation through extracellular vesicles. It can significantly promote functional recovery and matrix reconstruction, restore tendon morphology, and inhibit scar formation and adhesion around the tendon. In the treatment of bone and joint diseases, photothermal nanozymes with bionic characteristics can generate thermal energy under near-infrared radiation, enhance joint lubrication performance, reduce cartilage wear in early osteoarthritis, effectively remove reactive oxygen species and reactive nitrogen species, increase the production of hyaluronic acid inside and outside the cells, and help to restore the lubrication and function of articular cartilage. Hollow Prussian blue nanoenzyme prepared by template-free hydrothermal synthesis can inhibit osteoclast formation and bone resorption, inhibit intracellular reactive oxygen species production and mitogen-activated protein kinase and nuclear factor kappa-B signaling pathways, thereby improving osteoporosis. In the treatment of spinal diseases, Prussian blue nanozymes can not only remove excessive reactive oxygen species, maintain the normal Redox level of nucleus pulposus cells, but also escape lysosomal phagocytosis, achieve more effective mitochondrial targeting, and effectively alleviate intervertebral disc degeneration.
3.Effect of mild anemia on prognosis of proliferative diabetic retinopathy after pars plana vitrectomy

Objective:To observe the effect of mild anemia on prognosis of hyperplastic diabetic retinopathy (PDR) treated by pars plana vitrectomy (PPV).Methods:A retrospective case-control study. From January 2021 to December 2022, 147 PDR patients with 147 eyes who received PPV treatment at Shenyang He Eye Hospital were included in the study. There were 147 eyes in 147 cases, 82 eyes in 82 males and 65 eyes in 65 females. Age was 25-79 (53.54±12.74) years old. A hemoglobin (Hb) of 90 to 120 g/L in men and 90 to 110 g/L in women was defined as mild anemia. According to Hb results, the patients were divided into mild anemia group and non-anemia group, with 39 cases (39 eyes) and 108 cases (108 eyes), respectively. The best corrected visual acuity (BCVA) test was performed using the international standard logarithmic visual acuity scale before and after the surgery, which was statistically converted to minimum resolved logarithmic (logMAR) visual acuity. The logMAR BCVA of the affected eye was 0.2-2.5 (1.60±0.62) before surgery. According to whether the BCVA improved by more than 3 lines 6 months after surgery, the affected eyes were divided into BCVA improved by more than 3 lines and BCVA improved by less than 3 lines, 117 eyes and 30 eyes, respectively. The patient's body mass index (BMI), BCVA at 6 months after surgery, and the occurrence of neovascular glaucoma (NVG) and recurrence of vitreous hemorrhage (RVH) were recorded. Independent sample t test was used for comparison between groups. Multivariate logistic regression analysis was performed to analyze the influencing factors of increased BCVA ≥3 rows after surgery. Results:Six months after surgery, the logMAR BCVA of patients in mild anemia group and non-anemia group were 0.97±0.87 and 0.68±0.63, respectively. BCVA increased ≥3 in 26 (66.7%, 26/39) and 91 (84.3%, 91/108) eyes, respectively. logMAR BCVA ( t=2.234) and BCVA increased by ≥3 rows of eyes ( χ2=5.460) between the two groups, the differences were statistically significant ( P=0.027, 0.019). Logistic regression analysis showed that baseline logMAR BCVA [odds ratio ( OR) = 4.291, 95% confidence interval ( CI) 1.918-9.600, P<0.001)], BMI ( OR=1.248, 95% CI 1.057-1.472, P=0.009), the number of retinal laser photocoagulation during surgery ( OR=1.001, 95% CI 1.000-1.002, P=0.038), mild anemia ( OR=0.360, 95 % CI 0.131-0.993, P=0.048), postoperative NVG ( OR=0.156, 95% CI 0.033-0.726, P=0.018) and RVH ( OR=0.264, 95% CI 0.086-0.808, P=0.020) were independent factors for BCVA improvement ≥3 lines after PPV. Conclusions:Mild anemia has a certain effect on BCVA improvement after PPV of PDR. Mild anemia, post-operative NVG and RVH were independent risk factors for increasing BCVA ≥3 lines after PPV in PDR patients.

6.Effects of Electroacupuncture on Skeletal Muscle Mitochondrial Dynamics in SAMP8 Mice

Objective To observe the effects of electroacupuncture on the motor function and mitochondrial dynamics of skeletal muscle of SAMP8 mice;To explore the mechanism of electroacupuncture in improving the motor dysfunction of Alzheimer disease(AD)from the perspective of mitochondrial dynamics.Methods Totally 18 SAMP8 mice were divided into model group and electroacupuncture group,with 9 mice in each group,and the SAMR1 mice with the same age were set as control group."Baihui","Dazhui"and"Shenshu"were selected in the electroacupuncture group,and electroacupuncture was performed daily for 20 min,8 d as a course of treatment.Each course of treatment was separated by 2 d,for a total of 3 courses of treatment.The model group and the control group were not intervened.The motor function of mice was tested by grip strength test,suspension test,hind limb extension test and Morris water maze experiment.The morphology and structure of gastrocnemius were observed by HE staining,ATP content in gastrocnemius was determined by colorimetry,the mRNA expression of optic atrophy 1(OPA1),mitofusin 2(MFN2)and dynamin-related protein 1(DRP1)in gastrocnemius were detected by real-time quantitative PCR,the expressions of OPA1,MFN2 and DRP1 in gastrocnemius were detected by Western blot.Results Compared with the control group,the grip strength,the score in suspension test,and the average speed and maximum speed of Morris water maze experiment of mice in model group significantly decreased(P<0.01);the arrangement of fibers in the gastrocnemius muscle tissue was disordered,the gaps become wider,and the distribution of nuclei was uneven;the ATP content in the gastrocnemius muscle tissue was significantly decreased(P<0.01),the mRNA and protein expressions of OPA1 and MFN2 were significantly decreased(P<0.01),and the expression of DRP1 mRNA and protein significantly increased(P<0.01).Compared with the model group,the grip strength,the score in suspension test,and the average speed and maximum speed of Morris water maze experiment in electroacupuncture group significantly increased(P<0.01);the arrangement of gastrocnemius muscle tissue was relatively neat,the gaps become narrower,and the distribution of nuclei was more uniform;the ATP content in gastrocnemius muscle tissue significantly increased(P<0.01),while the mRNA and protein expressions of OPA1 and MFN2 significantly increased(P<0.05,P<0.01),the expression of DRP1 mRNA and protein significantly decreased(P<0.01).Conclusion Electroacupuncture can improve the skeletal muscle morphological structure and motor dysfunction of SAMP8 mice,and the mechanism may be related to the correction of skeletal muscle mitochondrial dynamic imbalance and the increase of skeletal muscle ATP content.
7.Effects of Electroacupuncture on Motor Function and the Expressions of Irisin,Decorin and Myostatin in SAMP8 Mice

Objective To observe the effects of electroacupuncture on the motor function and expressions of Irisin,Decorin and Myostatin in skeletal muscle of SAMP8 mice;To explore the mechanism of electroacupuncture in the treatment of the motor dysfunction of Alzheimer disease(AD).Methods Totally 247-month-old male SAMP8 mice were randomly divided into model group and electroacupuncture group,with 12 mice in each group,and the 12 male SAMR1 mice with the same age were set as the control group."Baihui","Dazhui"and"Shenshu"were selected in the electroacupuncture group,once a day,8 days as one course of treatment,with an interval of 2 days,for a total of 3 courses.The control group and the model group were not intervened.The motor function of mice was tested by grip strength test,pole climbing test and open field test,the mRNA expressions of Irisin,Decorin and Myostatin in quadriceps muscle were detected by RT-qPCR,and the protein expressions of Irisin,Decorin and Myostatin in quadriceps muscle were detected by immunohistochemistry and Western blot.Results Compared with the control group,the grip peak and duration of the mice in the model group decreased,head turning time and pole climbing time were prolonged(P<0.01),the mRNA and protein expressions of Irisin and Decorin decreased(P<0.05,P<0.01),and the expressions of Myostatin mRNA and protein increased(P<0.05).Compared with the model group,the grip peak and duration of the mice in the electroacupuncture group increased,head turning time and pole climbing time were decreased(P<0.05),the mRNA and protein expressions of Irisin and Decorin increased(P<0.05),and the expressions of Myostatin mRNA and protein decreased(P<0.05).Conclusion Electroacupuncture can improve the motor dysfunction of SAMP8 mice,and its mechanism may be related to regulating the expressions of Irisin,Decorin and Myostatin in skeletal muscle.
8.Material Basis and Molecular Mechanism of Linggui Qihua Prescription Against Myocardial Fibrosis in Heart Failure with Preserved Ejection Fraction

ObjectiveTo explore the material basis and molecular mechanism of Linggui Qihua prescription (LGQH) against myocardial fibrosis in heart failure with preserved ejection fraction (HFpEF). MethodLiquid chromatography-mass spectrometry (LC-MS) was used to qualitatively analyze the active components of LGQH. AutoDock software was employed for molecular docking between the active components of LGQH and target proteins including α-smooth muscle actin (α-SMA), type Ⅰ collagen (ColⅠ), type Ⅲ collagen (ColⅢ), matrix metalloproteinase-9 (MMP-9), and tissue inhibitor of metalloproteinase-1 (TIMP-1). In vivo experiments were conducted on 40 spontaneously hypertensive rats (SHRs) aged 4 weeks, which were divided into an HFpEF group, an Entresto group (0.018 g·kg-1), and low- and high-dose LGQH groups (3.87, 7.74 g·kg-1). A high-fat, high-salt, and high-sugar diet was administered for 16 weeks along with intraperitoneal injection of streptozotocin solution for 8 weeks to establish an HFpEF model in rats. The blank group consisted of 10 Wistar Kyoto (WKY) rats and 10 SHRs. After successful modeling, the WKY, SHR, and HFpEF groups were given equal volumes of normal saline, while the other three groups received predetermined interventions. Daily oral gavage was performed for 6 weeks. After intervention, echocardiography was conducted to measure left ventricular (LV) anterior wall thickness (LVAWd), LV posterior wall thickness (LVPWd), LV internal diameter at end-diastole (LVIDd), LV ejection fraction (LVEF), isovolumic relaxation time (IVRT), early diastolic peak velocity of mitral valve inflow (E), and early diastolic mitral annular velocity (e'). The E/e' ratio was calculated. Enzyme-linked immunosorbent assay (ELISA) was used to detect serum atrial natriuretic peptide (ANP), B-type natriuretic peptide (BNP), and galectin-3 (Gal-3). Myocardial fibrosis was observed through Masson staining of pathological sections, and collagen volume fraction (CVF) and perivascular fibrosis ratio (PFR) were calculated. Real-time polymerase chain reaction (PCR) and Western blot were employed to detect LV myocardial mRNA and protein expression of α-SMA, ColⅠ, ColⅢ, MMP-9, and TIMP-1. ResultLC-MS identified 13 active components in LGQH. Molecular docking indicated stable binding of the 13 compounds with five target proteins. In vivo experiments showed that compared with the blank group, the HFpEF group had significantly increased LVAWd, LVPWd, LVIDd, IVRT, E/e', ANP, BNP, Gal-3, CVF, and PFR. LV myocardial α-SMA, ColⅠ, and ColⅢ mRNA and protein expression was significantly upregulated, while MMP-9/TIMP-1 mRNA and protein ratios were significantly downregulated (P<0.05, P<0.01). Compared with the HFpEF group, LGQH might dose-dependently reduce LVAWd, LVPWd, LVIDd, IVRT, E/e', ANP, BNP, Gal-3, CVF, and PFR, downregulated myocardial α-SMA, ColⅠ, ColⅢ mRNA expression, α-SMA, and ColⅠ protein expression, and upregulated MMP-9/TIMP-1 mRNA and protein expression (P<0.05, P<0.01). ConclusionLGQH contains multiple active components and may inhibit myocardial fibrosis in HFpEF rats. It may further alleviate LV hypertrophy, dilation, and diastolic dysfunction, making it an effective Chinese medicinal prescription for treating HFpEF.
9.Screwdriver aspiration during implant surgery: case report and literature review

Objective :
To discuss and summarize the preventive measures and treatment methods for aspiration/ingestion during dental procedures.
Methods :
One case of aspiration during an implant operation was reported, and the literature on aspiration/ingestion during oral procedures was reviewed.
Results:
An implant screwdriver accidentally slipped into the mouth of the patient during implant surgery. The patient experienced no obvious discomfort except a few coughs. The surgeon and assistant paused the procedure immediately to search for the screwdriver, but it was not found. The patient declared that there was no special abnormality, such as breathing disorder or chest distress, so we considered that the foreign body was ingestion. After the implant surgery was completed, no foreign body was found in the stomach via gastroscopy. Chest X-ray and CT showed a dense metal shadow in the lower lobe of the left lung. Under local anesthesia, bronchoscopy and biopsy forceps were used by respiratory physicians to clip out the foreign body. After removal of the foreign body, the patient had no obvious discomfort but a slight cough. Cephalexin and metronidazole were given for three days to prevent infection. Three days later, the patient had no complaints of respiratory discomfort. After reviewing the literature, we found that the operation should be paused immediately after aspiration/ingestion occurs during dental procedures and that the dental chair should be laid down to prevent the foreign body from descending deeper, which may increase the difficulty of removal and cause gastrointestinal and respiratory tract injury. The position of the foreign body should be determined by imaging examination, and the corresponding means to remove the foreign body should be performed.
Conclusion
Patients may have no obvious symptoms after aspiration/ingestion during dental procedures, and the foreign body can be removed after imaging examination.
10.Clinical efficacy of artificial vascular replacement for external iliac artery dissection during renal transplantation

Objective:To explore the clinical efficacy of treating external iliac artery dissection in renal transplantation by artificial vascular replacement.Methods:Four sudden intraoperative cases of external iliac artery dissection were selected.After removing vascular sutures, intimal arterial peeling blocked external iliac artery( n=3)and transplanted renal artery( n=1). Artificial vascular replacement of external iliac artery was performed using artificial vessels made from puffed polytetrafluoride ethylene(ePTFE). Secondary perfusion was performed in four transplanted kidneys for anastomosing with internal iliac artery. Results:One patient regained normal renal function within 1 week post-operation.Two cases had delayed graft function.Another case had delayed graft function plus acute rejection.After hemodialysis, renal function normalized at 2-3 weeks post-operation.During a follow-up period of(0.5-5.0)years, transplanted kidney function remained stable, blood supply, skin temperature and movement of operated lower extremities normalized.Conclusions:The incidence of vascular dissection of external iliac artery is not high during renal transplantation.However, the disease has a rapid and dangerous progression.The consequences of delayed intervention are quite serious.Treating external iliac artery dissection with renal transplantation may achieve satisfactory clinical outcomes.
