1.Cross-sectional Analysis of ASMI and Muscle Strength in Older Adults: Based on the 2024 National Routine Physical Fitness Surveillance Database

To explore the dose-response correlation between appendicular skeletal muscle index (ASMI) and muscle strength in older adults. This study included adults aged 60-79 years from the 2024 National Routine Physical Fitness Surveillance database. ASMI was used to assess muscle mass, while handgrip strength and 30-second chair stand test (30-s CST) repetitions were employed to evaluate muscle strength. Linear regression analyses, adjusted for age and body mass index (BMI), were performed to examine the associations between ASMI and handgrip strength/30-s CST. Additionally, restricted cubic spline (RCS) models were applied for dose-response analysis. A total of 68 038 older adults (56.3% female, mean age 68.0±5.4 years) were included. Males exhibited significantly higher handgrip strength(33.6± 7.8 kg s Muscle mass and strength are significantly positively correlated in older adults. Personalized muscle health interventions should be considered to reduce the rate of decline in muscle strength, when ASMI falls below 13.55 kg/m2 in men or 11.25 kg/m2 in women.
2.False-positive HIV-1 nucleic acid testing results in patients with severe thalassemia after receiving cell and gene therapy

A 11-year old female patient with severe thalassemia, receipt a lentivirus-based cell and gene therapy (CGT) therapy in Shenzhen Children′s Hosptial on July 27th, 2021. At the two follow-up visits after discharge, patient were continuously tested positive for HIV screening through HIV Ag/Ab Combo assay (chemiluminescence Immunoassay), and the viral load results of HIV-1 nucleic acid testing (NAT) were both>5 000 copies/ml. The patient can be diagnosed with HIV infection according to the National Guideline for Detection of HIV/AIDS(2020 Revised Edition). The thorough investigation findings and supplementary experiment results indicated that the false-positive HIV-1 NAT results was caused by cross-reactivity between the target sites detected by conventional HIV-1 NAT reagents and the lentiviral vectors fragments integrated into the genome of patient′s hematopoietic stem/progenitor cells. In conclusion, it is important for laboratories to select appropriate HIV-1 NAT testing platforms which won′t cause cross-reactivity for the testing of samples from patients who have been treated with HIV-derived vectors. It is also recommended to design and develop NAT testing platforms with multiple target regions labeled by different fluorescents for HIV NAT supplementation experiment to reduce the risk of false-positive diagnoses of HIV infection.
3.Psychological well-being and its relationship with exercise among Chinese residents aged 20~79 years

Objective:To explore the mental health level of Chinese adults aged 20~79 years and its association with exercise based on the surveillance data.Methods:A cross-sectional study was conducted in a total of 161 901 samples from the Fifth National Physical Fitness Surveillance, including 121 928 adults aged 20-59 years and 39 973 elderly aged 60-79 years. The mental health status was assessed by a comprehensive questionnaire, and the participation in frequency and duration of exercise were investigated. Chi-square test and Logistic regression were applied to test the relationship between exercise and mental health using SPSS 26.0 software.Results:The rate of depressive symptoms among residents aged 20-79 years in China was 16.6%(26 311/158 518). In the adults aged 20-59 years, the rates of depression and anxiety symptoms were 18.0%(21 465/119 409) and 13.8%(16 435/119 444), respectively, and the proportion of those who often or always felt stressed was 15.3%(18 373/120 444). In the elderly aged 60-79 years, the rate of depression symptoms was 12.4%(4 846/39 109), the proportion of those with loneliness scale scores higher than 25 was 32.8%(12 650/38 525), and 4.0%(1 587/39 395) elderly often or always felt stressed. Compared with the non-exercisers, the risk of depression symptoms in the exercisers was reduced by 20% ( β=-0.22, OR=0.80, 95% CI: 0.78-0.82). With increased frequency and intensity of physical exercise, the mental health condition improved significantly. For those who exercised at least 3 days a week and had moderate intensity or above, the detection rate of depression symptoms decreased by 35% ( β=-0.39, OR=0.65, 95% CI: 0.63-0.67).The negative associations between exercise and depression were found in different ages, genders and resident subgroups. The risk of anxiety was 6% lower in the adults who participated in physical activity ( β=-0.06, OR=0.94, 95% CI: 0.91-0.97) and 15% lower in those who exercised over 3 days per week at moderate-to-vigorous intensity ( β=-0.16, OR=0.85, 95% CI: 0.81-0.89). No association between exercise and risk of stress was observed ( P>0.05), but the positive effect was also found as exercise frequency and intensity increased. Older adults with exercise had a reduced risk of high loneliness scores by 50% ( β=-0.70, OR=0.50, 95% CI: 0.47-0.52) and a 35% lower detection of stress risk ( β=-0.43, OR=0.65, 95% CI: 0.59-0.72). Conclusion:Exercise was significantly and meaningfully associated with self-reported mental health.Exercise can actively and effectively prevent and improve mental health.
4.The value of transcranial Doppler sonography and digital subtraction angiography in evaluating collateral circulation in ischemic stroke and its correlation with prognosis

Objective:To explore the value of transcranial Doppler sonography (TCD) and digital subtraction angiography (DSA) in evaluating collateral circulation in ischemic stroke (IS) and its correlation with prognosis.Methods:The 350 IS patients admitted to Shaoxing People′s Hospital from January 2017 to December 2020 were selected as the research objects. According to the results of DSA, they were divided into collateral circulation group and non-collateral circulation group. According to the prognosis, they were divided into good prognosis group and poor prognosis group. The consistency between the evaluation results of collateral circulation on IS of TCD and DSA was analyzed, and its correlation with prognosis was analyzed.Results:The DSA results showed that among 350 patients, 118 had no collateral circulation and 232 had collateral circulation, including 130 cases of grade 1 collateral circulation and 102 cases of grade 2 collateral circulation. TCD results showed that there were 117 cases without collateral circulation and 233 cases with collateral circulation, including 131 cases with grade 1 collateral circulation and 102 cases with grade 2 collateral circulation. The consistency test showed that the evaluation results of TCD on collateral circulation was highly consistent with the gold standard DSA ( Kappa>0.9, P<0.05). Repeated measures analysis of variance showed that time-point effects, inter-group effects, time-point and inter-group interaction effects can significantly affect the changes in modified Rankin scale(mRS) scores ( P<0.05); and the mRS scores in the collateral circulation group were less than those in the non-collateral circulation group at admission the hospital, discharge of the hospital and 3 months after discharge the hospital: (1.89 ± 0.82) scores vs. (2.98 ± 0.98) scores, (1.13 ± 0.53) scores vs. (2.45 ± 0.80) scores, (0.50 ± 0.45) scores vs. (1.86 ± 0.80) scores, there were statistical differences ( P<0.05). The collateral circulation rate in the good prognosis group was higher than that in the poor prognosis group: 78.95%(150/190) vs. 47.50%(76/160), there was statistical difference ( P<0.05). Conclusions:The evaluation value of TCD for IS collateral circulation is high, and collateral circulation is closely related to prognosis.
5.Distribution characteristics of antibiotic resistance genes in the domestic water of residents in Haimen, Jiangsu Province

ObjectiveAntibiotic resistance genes (ARGs) have received wide attention all over the world. The purpose of this study was to explore the bacterial community structure, the types and levels of antibiotic resistance genes in a water body in east China, and to compare and analyze the characteristics of microbial species distribution and antibiotic resistance gene distribution in various water environments. MethodsA total of 10 households in Haimen City, Jiangsu Province were selected and their surrounding water environment samples were collected. 21 water samples including river water (4), Mingou water (9) and well water (8) were collected for metagenomics sequencing, assembled with MetaWRAP, annotated with CARD database, and analyzed with R software. ResultsIn various water bodies, the dominant bacteria phyla was Proteobacteria, the dominant bacteria genera were Deuterostomia, Pseudomonas, Flavobacteriales and Streptomycetaceae. The ARGs annotated were mainly composed of quinolones, aminoglycosides, macrolides and beta-lactams antibiotic resistance genes. The top four relative abundance of resistance genes were macB, RanA, evgS and TxR, The average absolute abundance and expression of resistance genes in well water and Mingou water were higher than those in river water. ConclusionMultiple ARGs are detected to varying degrees in well water, river water, and Mingou water bodies, and the expression of resistance genes in well water and Mingou water bodies is higher than that in river water bodies, possibly due to human production and living activities.
6.Spatial Distribution of Biomechanical Characteristics for Trabecular Bone in the Femoral Head with Osteonecrosis

Objective To explore the spatial distribution of microstructural parameters and mechanical properties for trabecular bone in the femoral head with osteonecrosis. Methods Microstructural parameters and mechanical properties of trabecular bone in different regions were analyzed by combined use of imaging measurements and numerical simulation method, and the spatial distribution of biomechanical properties for trabecular bone along coronal, sagittal and vertical directions was investigated. Results Microstructural characteristics and mechanical properties of trabecular bone were Y-shaped distributed along coronal and sagittal directions, and mechanical properties of trabecular bone in Y-shaped region were higher than those in the other regions. Such distribution characteristics was consistent with the location of principle compressive group in the femoral head. Conclusions Necrotic lesions in Y-shaped region had a greater influence on stress distribution of the femoral head and might cause the deterioration of osteonecrosis. The spacial correlation between necrotic lesions and Y-shaped region should be fully considered during clinical diagnosis.
7.Mechanical Properties of Calcification from Human Aortic Valve

Objective To measure mechanical properties of calcified tissues from human aortic valve by nanoindentation test. Methods Leaflets from 5 patients with aortic stenosis were collected. Elastic modulus and hardness of the calcified leaflets were obtained by nanoindentationtest. Results The elastic modulus and hardness of the calcified leaflets were (15.69±3.89) GPa and (0.59±0.15) GPa, respectively. Conclusions The elastic modulus and hardness of the calcified valve tissues can be obtained by nanoindentationtest, which provides experimental data for biomechanical research of the valve.

In 2019, the rate of primary tooth caries rate among 1 598 preschool children in Shinan District and Shibei District of Qingdao was 59.4%. The multivariate logistic regression model showed that compared with children ≤ 2 years old with history of iron deficiency anemia (IDA), suffering IDA, breastfeeding to 2 years old and no tooth melanin, children ≤ 2 years old without history of IDA, not suffering IDA, breastfeeding to less than 2 years old and heavy tooth melanin had a lower risk of primary tooth caries, with OR ( 95%CI) values about 0.328 (0.197-0.549), 0.354 (0.208-0.603), 0.636 (0.437-0.926) and 0.301 (0.143-0.635), respectively.

10.A case report of simultaneous multiple osteosarcoma and a review of systematic literature

Synchronous multifocal osteosarcoma (SMOS) was analyzed for its predisposing age, sex, location, oncology characteristics, and survival time with different treatment. The key words about "multifocal osteosarcoma" had been used to search articles which includ Synchronous multifocal osteosarcoma patients databases from 1949 to 2020. The articles have been filtratedby title, abstract and full text. There were 80 articles used for thisstudy. All the patients were objects of thisstudy. Butthe same patients' data in different articles had not been used repeatedly. The patients' data had been collected as much aspossible, including age, location, treatment, survival timeand so on. All the patients' data had been used forsystematic analysis. All of the 80 articles and 264 patients had been studied. The mean onset age was 16.17 years old and the peak age of onset was 10-20 years old. The gender difference had been uncovered and the sex ratio was 1.76∶1. The incidence site of 188 patients (92.16%) was located in the extremities. Alkaline phosphatase was elevated in 135 patients (95.10%). The pathological type was osteoblastic osteosarcoma in 134 patients (76.14%). There were 3 patients with hypocalcemia and 2 patients with anemia. The mean survival time of 15 patients (15/58) who gave up treatment was 4.51 months. The mean survival time of 23 patients with chemotherapy was 8.97 months. The mean survival time was 16.17 months in 11 patients with preoperative chemotherapy and surgical treatment. Nine patients with neoadjuvant chemotherapy, surgery and postoperative chemotherapy had an average survival time of 23.28 months. Multiple osteosarcoma of the same type was rare, with high degree of malignancy and poor prognosis. The age of high incidence was 10-20 years old. Currently, the most effective treatment was neoadjuvant chemotherapy, surgery and postoperative chemotherapy.
