1.Oral health-related quality of life status and risk factors in patients with mental disorders.

OBJECTIVES:
This study aims to explore the current status and risk factors of oral health-related quality of life OHRQoL in patients with mental disorders and provide evidence for effective intervention measures.
METHODS:
A total of 397 patients diagnosed with mental illness were selected by convenience sampling, and investigation was carried out using general data questionnaire, health literacy in dentistry-14 (HeLD-14), oral health impact profile-14 (OHIP-14), and oral health status checklist.
RESULTS:
The total score of OHIP-14 in patients with mental disorders was 8(2, 14). The score of HeLD-14 was negatively correlated with the score of OHIP-14 (r=-0.142, P<0.01). The results of multiple linear regression showed that six variables including annual family income, schizophrenia, sweets, frequency of visits to the dentist, dental caries, and missing teeth affected OHRQoL of patients with mental disorders (P<0.05).
CONCLUSIONS
The poor OHRQoL of psychiatric patients is associated with many factors. Medical personnel should pay attention to their oral health problems and develop targeted oral care programs throughout the course of disease to improve oral health and related quality of life of patients.

2.Diagnostic value of multi-slice spiral CT combined with vimentin in dedifferentiated liposarcoma

Objective To explore the diagnostic value of multi-slice spiral CT combined with vimentin in dedifferentiated liposarcoma(DDL).Methods A total of 88 patients with suspected DDL admitted to No.904 Hospital of Joint Logistics Support Force of PLA from June 2017 to June 2022 were enrolled.Multi-slice spiral CT examination and vimentin detection were performed in all the patients.Pathological results were taken as the golden standard of diagnosis.The consistency of multi-slice spiral CT examination and pathological results was investigated.The expression of vimentin was compared between DDL patients and non-DDL patients.The diagnostic value of multi-slice spiral CT combined with vimentin in dedifferentiated liposarcoma was analyzed.Results Pathological diagnosis showed that 64 patients were diagnosed as DDL and 24 patients were not DDL.The results of multi-slice spiral CT showed that 63 patients were diagnosed as DDL.CT results of 59 patients with DDL and 20 patients with non-DDL were consistent with pathological diagnosis.The multi-slice spiral CT examination and pathological results were generally consistent(Kappa=0.746,P<0.01).The expression of vimentin was positive in 59 DDL patients and negative in 5 DDL patients.The expression of vimentin was positive in 6 non-DDL patients and negative in 18 non-DDL patients.Kappa consistency test showed that and the 2 diagnostic methods were generally consistent(Kappa=0.681,P<0.01).Sixty-three patients were diagnosed as DDL by multi-slice spiral CT combined with vimentin detection.The combined detection of 61 patients with DDL and 22 patients with non-DDL were consistent with pathological diagnosis.Kappa consistency test showed that,the 2 diagnostic methods were well consistent(Kappa=0.866,P<0.01).The sensitivities of multi-slice spiral CT,vimentin detection and their combination were 92.18% ,92.18% and 95.31% ,respectively;the specificities were 83.34% ,75.00% and 91.67% ,respectively;the Youden indexes were 0.755,0.671 and 0.870,respectively.The combined use of multi-slice spiral CT and vimentin detection had higher diagnostic value for DDL.Conclusion The result of multi-slice spiral CT combined with vimentin detection is consistent with pathological diagnosis and this method can be widely used in clinic.
3.Protective effect of berberine against ionizing radiation injury in rats and its mechanism of action

Objective To investigate the protective effect of berberine (BBR) against ionizing radiation injury in rats and its mechanism of action. Methods Sprague-Dawley rats were divided into seven groups: normal control group, 1-Gy radiation group, 1-Gy radiation plus low-dose BBR (50 mg/kg) group, 1-Gy radiation plus high-dose BBR (150 mg/kg) group, 3-Gy radiation group, 3-Gy radiation plus low-dose BBR (50 mg/kg) group, and 3-Gy radiation plus high-dose BBR (150 mg/kg) group. All the groups except the normal control group were exposed to external irradiation with a medical electron linear accelerator, followed by BBR administration by gavage for consecutive ten days. The serum levels of superoxide dismutase (SOD), reduced glutathione (GSH), and malondialdehyde (MDA) were measured by using the micromethod. The pathological changes of the bone marrow and small intestine were observed with HE staining. Results Compared with the normal control group, the radiation groups showed significantly increased MDA levels (P < 0.05), significantly decreased SOD and GSH levels (P < 0.05), and more severe pathological damage of the bone marrow and small intestine. Compared with the radiation groups, the BBR groups showed significantly decreased MDA levels (P < 0.05), significantly increased SOD and GSH levels (P < 0.05), and reduced pathological damage to the bone marrow and small intestine, which were more marked in the high-dose BBR group. Conclusion BBR has a certain protective effect against radiation injury in rats, which may be through increasing the activity of antioxidant substances, enhancing free radical clearance, and thereby alleviating free radicals-caused oxidative damage.
4.Study on the sensitivity of a volumetric modulated arc therapy plan verification equipment on multi-leaf collimator opening and closing errors and its gamma pass rate limit.

To investigate the γ pass rate limit of plan verification equipment for volumetric modulated arc therapy (VMAT) plan verification and its sensitivity on the opening and closing errors of multi-leaf collimator (MLC), 50 cases of nasopharyngeal carcinoma VMAT plan with clockwise and counterclockwise full arcs were randomly selected. Eight kinds of MLC opening and closing errors were introduced in 10 cases of them, and 80 plans with errors were generated. Firstly, the plan verification was conducted in the form of field-by-field measurement and true composite measurement. The γ analysis with the criteria of 3% dose difference, distance to agreement of 2 mm, 10% dose threshold, and absolute dose global normalized conditions were performed for these fields. Then gradient analysis was used to investigate the sensitivity of field-by-field measurement and true composite measurement on MLC opening and closing errors, and the receiver operating characteristic curve (ROC) was used to investigate the optimal threshold of γ pass rate for identifying errors. Tolerance limits and action limits for γ pass rates were calculated using statistical process control (SPC) method for another 40 cases. The error identification ability using the tolerance limit calculated by SPC method and the universal tolerance limit (95%) were compared with using the optimal threshold of ROC. The results show that for the true composite measurement, the clockwise arc and the counterclockwise arc, the descent gradients of the γ passing rate with per millimeter MLC opening error are 10.61%, 7.62% and 6.66%, respectively, and the descent gradients with per millimeter MLC closing error are 9.75%, 7.36% and 6.37%, respectively. The optimal thresholds obtained by the ROC method are 99.35%, 97.95% and 98.25%, respectively, and the tolerance limits obtained by the SPC method are 98.98%, 97.74% and 98.62%, respectively. The tolerance limit calculated by SPC method is close to the optimal threshold of ROC, both of which could identify all errors of ±2 mm, while the universal tolerance limit can only partially identify them, indicating that the universal tolerance limit is not sensitive on some large errors. Therefore, considering the factors such as ease of use and accuracy, it is suggested to use the true composite measurement in clinical practice, and to formulate tolerance limits and action limits suitable for the actual process of the institution based on the SPC method. In conclusion, it is expected that the results of this study can provide some references for institutions to optimize the radiotherapy plan verification process, set appropriate pass rate limit, and promote the standardization of plan verification.

5.Emergency medical disposal for a case with the wound suspected of radioactive waste water contamination

A patient with scalp laceration suspected of radioactive waste water contamination had the wound ruled out of radioactive contamination, psychological fear eliminated, and well-healed wound, through rapid emergency medical response and scientific and effective disposal. The treatment process and psychological intervention for batch wounded after a nuclear accident are still applicable to patients with a small amount of sudden radioactive contamination. This article summarizes the relevant disposal process for reference.

Effective methods for visualizing neurovascular morphology are essential for understanding the normal spinal cord and the morphological alterations associated with diseases. However, ideal techniques for simultaneously imaging neurovascular structure in a broad region of a specimen are still lacking. In this study, we combined Golgi staining with angiography and synchrotron radiation micro-computed tomography (SRμCT) to visualize the 3D neurovascular network in the mouse spinal cord. Using our method, the 3D neurons, nerve fibers, and vasculature in a broad region could be visualized in the same image at cellular resolution without destructive sectioning. Besides, we found that the 3D morphology of neurons, nerve fiber tracts, and vasculature visualized by SRμCT were highly consistent with that visualized using the histological method. Moreover, the 3D neurovascular structure could be quantitatively evaluated by the combined methodology. The method shown here will be useful in fundamental neuroscience studies.

8.Risk factors for mortality in pediatric acute respiratory distress syndrome requiring extracorporeal membrane oxygenation support

Objective:To explore the risk factors for mortality in pediatric acute respiratory distress syndrome (PARDS) requiring extracorporeal membrane oxygenation (ECMO) support.Methods:Clinical data of 109 patients with severe PARDS supported by ECMO, who were hospitalized in 6 ECMO centers in China from September 2012 to February 2020, were retrospectively analyzed. They were divided into survival group and death group according to the prognosis. Chi-square test and rank sum test were used to compare the variables between the two groups, including the demographic data, laboratory examination results, clinical data before and after ECMO, and other supportive treatment. Univariate and multivariate Logistic regression models were used to analyze the prognostic risk factors.Results:In these 109 cases, 54 died and 55 survived. Compared with the survival group, the death group had higher incidences of acute kidney injury (AKI) (48.1% (26/54) vs. 21.8% (12/55) , χ2=8.318, P=0.004) and coagulation dysfunction (22.2% (12/54) vs. 7.3% (4/55) , χ2=4.862, P=0.027), and higher rate of renal replacement therapy (48.1% (26/54) vs. 21.8% (12/55) , χ2=9.694, P=0.008) during ECMO support. Logistic regression analysis showed that continuous renal replacement therapy (CRRT) and AKI were independent risk factors for death in patients with severe PARDS requiring ECMO support ( HR=3.88,95% CI 1.04-14.52, HR=4.84,95% CI 1.21-19.46, both P<0.05). Conclusion:AKI and CRRT are independent risk factors for predicting mortality in patients with severe PARDS requiring ECMO support.
9.A phase Ⅱ prospective study on raltitrexed combined with concurrent radiotherapy for elderly esophageal carcinoma

Objective To evaluate the clinical effects and adverse reaction of raltitrexed combined with radiation for esophageal carcinoma in elderly patients.Methods Sixty patients were randomly divided into two groups by the envelope method, 30 patients in experimental group received raltitrexed combined with radiotherapy and 30 patients in control group received radiotherapy only.Patients in both groups received conventional radiotherapy with a total dose of 56-60 Gy/28-30 F.In experimental group, raltitrexed 2.6 mg/m2 was administered concurrently with the radiotherapy on d1 and d22.Two cycles of concurrent chemotherapy were administered during radiotherapy.The short-term effects, survival times and adverse reactions of the two groups were compared.Results The total effective rates of experimental group and control group were 93.3% and 73.3%, respectively, and there was statistically significant difference between the two groups (χ2=4.320, P=0.038).The median survival times of experimental group and control group was 24.0 months and 12.0 months, respectively, and there was statistically significant difference by Log-rank test (χ2=6.048, P=0.014).The major adverse reactions of grade 3-4 in experimental group and control group were radiation-induced esophagitis (10.0% vs.3.3%;χ2=0.268, P=0.605), leukopenia (13.3% vs.10.0%;χ2=0.000, P=1.000), thrombocytopenia (3.3% vs.0;P=1.000), nausea and vomiting (6.7% vs.0;χ2=0.517, P=0.472), and the differences were not statistically significant.Conclusion Raltitrexed combined with radiotherapy can enhance the short-term effect and prolong the survival time for the elderly esophageal carcinoma patients, and the adverse reactions are mild.It is worthy of further clinical study.
10.Optimized design of casualty triage flow in urgent medical support of nuclear accidents

Objective To explore routes for the optimization of casualty triage flow in urgent medical support of nuclear acci -dents.Methods The present working procedures were reorganized and redesigned in accordance with the feature and pattern of the medi -cal mission , as well as the practical organization of medical support teams in case of nuclear accidents .Results Through optimized proce-dures, the casualties were integrated from multiple types to simpler types:i.e.the A1 casualty type that would go through 3 steps and A2 would go through 4 steps;B1 casualty type would go through 3 steps and B2 casualty type would go through only 2 steps.The triage work flow was optimized to 2 steps, and treatment work flow was also optimized and clarified .Conclusion Casualty triage was permeated in the whole process of medical rescue mission , for this reason, the responsibilities and divisions of work for various teams should be elaborated and clarified on the basis of optimized work procedures , and they should be demonstrated and further rectified in practical work .
