1.Dual-center study of the clinical and microbiological characteristics of invasive Salmonella infection in children in Zhejiang Province

Objective:To study the clinical and microbiological characteristics of invasive Salmonella infection in children. Methods:Clinical data of children with invasive Salmonella infection diagnosed in the Children′s Hospital Zhejiang University School of Medicine and Women and Children′s Hospital of Ningbo University from January 2018 to June 2022 were retrospectively analyzed, and the serotypes and drug resistance of the pathogens were detected. The chi-square test was used to compare the resistance rates of different Salmonella serogroups to antibiotics. Results:A total of 105 invasive Salmonella infection cases were included, with ages of 20.0 (12.3, 43.5) months, and 67 cases(63.8%)were male. Fifty-seven cases occurred in summer (from June to August). Eighty-seven cases were confirmed by blood culture, 10 cases by bone marrow or bone marrow cavity pus culture, six cases by cerebrospinal fluid and other specimen culture, with an additional two cases where Salmonella was cultured from two types of specimens including blood, bone marrow or cerebrospinal fluid. Thirty-six patients had underlying diseases, of which 58.3%(21/36) were leukemia. The resistance rates of the strains to ampicillin, ceftriaxone, ceftazidime, piperacillin-tazobactam, imipenem, and trimethoprim-sulfamethoxazole were 48.5%(51/105), 12.3%(13/105), 9.5%(10/105), 1.9%(2/105), 0(0/105), and 29.5%(31/105), respectively. Ninety-nine strains (94.3%) belonged to invasive non-typhoidal Salmonella (iNTS), four strains were Salmonella typhi, and 13 strains were Salmonella typhimurium. The resistance rate of group D strains to ceftriaxone was lower than that of group B ( χ2=5.38, P=0.020) and group E ( χ2=5.74, P=0.017). The resistance rate of group D strains to trimethoprim-sulfamethoxazole was lower than that of group C ( χ2=5.67, P=0.017), group B ( χ2=13.02, P<0.001) and group E ( χ2=9.00, P=0.003), and the differences were all statistically significant. The symptoms of fever and diarrhea accounted for 94.3%(99/105) and 51.4%(54/105), respectively. The most common diagnosis was septicemia, accounting for 88.6%(93/105), pyogenic osteomyelitis with pyogenic arthritis accounted for 10.5%(11/105), and four patients (3.8%) developed purulent meningitis. All 105 patients received antibiotic treatment, 101 patients (96.2%) were cured or improved, and four cases (3.8%) died. Conclusions:iNTS infection is more common than typhoid fever in children. Different serotypes of strains have different resistance to antimicrobial drugs. Invasive strains such as bloodstream are more sensitive to the third-generation cephalosporin. The third-generation cephalosporin is still the first choice for the treatment of iNTS diseases.

3.Clinical efficacy of neoadjuvant chemotherapy combined with radical surgery for elderly patients with locally advanced gastric cancer

Objective:To evaluate the safety and feasibility of neoadjuvant chemotherapy (NACT) combined with radical surgery for elderly patients with locally advanced gastric cancer (LAGC).Methods:One hundred and fourty eight patients with LAGC after NACT and gastrectomy between 2012 and 2020 were retrospectively reviewed. They were divided into two groups: (1) <65 years old (111 cases) and (2) ≥65 years old (37 cases) and their clinicopathological and prognostic data were compared.Results:There was no significant difference between the two groups in the incidence of hematological complications such as anemia ( χ2=0.235, P=0.628), leukopenia ( χ2=0.613, P=0.434), neutropenia ( χ2=0.011, P=0.918) and thrombocytopenia ( χ2=0.253, P=0.615) and non-hematological complications such as nausea ( χ2=0.092, P=0.762), vomiting ( χ2=0.166, P=0.683), diarrhea ( χ2=0.015, P=0.902) and mucositis ( χ2=0.199, P=0.766) due to NACT. There were no statistical differences between the older patients and the younger in operation duration ( t=0.270, P=0.604), intraoperative bleeding ( t=1.140, P=0.250) and R 0 resection rate ( χ2=0.105, P=0.750). The incidence of postoperative complications was 25.2% and 37.8% in the younger patients and the olders ( χ2=2.172, P=0.141). Pleural effusion ( χ2=7.007, P=0.008) and pulmonary infection ( χ2=10.204, P=0.001) was significantly higher in the older patients than in the youngers. The 3-year progression-free survival rate ( t=0.494, P=0.482) and 3-year overall survival rate ( t=0.013, P=0.908) were comparable between the two groups. Conclusions:NACT combined with radical surgery is safe and effective in elderly patients with LAGC, except for higher perioperative pulmonary-related complications.
4.Investigation and analysis of reproductive health status of female street cleaners in a district

Objective:To explore the reproductive health status of the female street cleaners in Chaoyang district of Beijing and its influencing factors.Methods:In July 2018, a total of 647 questionnaires were sent out to female road cleaning and sanitation workers in the whole district, 613 of which were valid, with an effective rate of 94.7%. Frequency or percentage (%) is used for statistical description of counting data, and mean standard deviation is used for measurement data. The influencing factors were analyzed by 2 test.Results:The average age of 613 female road sweepers in Chaoyang District of Beijing was 42.01 (SD=6.69) years old, including 535 married female workers (87.28%) , 356 middle school and below educated (58.08%) , 292 non Beijing registered (47.63%) , accounting for (/613) , (/613) female workers working hours >8 hours per day 110 (17.94%) , weekly rest <2 days 341 (55.63%) . 144 (23.49%) women workers did not have regular gynecological examination, 119 (19.41%) had reproductive tract infections, 177 (28.87%) had abnormal menstruation in recent 6 months. Drinking, sexual behavior outside marriage, night shift and job satisfaction were all the influencing factors ( P<0.05) . The increasing age, working years, drinking, household registration in other places and the decreasing satisfaction of women workers' rights and interests protection were all the influencing factors ( P<0.05) . Conclusion:The reproductive health status of female road sweepers in Chaoyang District of Beijing is not optimistic.

8.Expressions and significance of enhancer of zeste homology 2 and signal transducer and activator of transcription 3 in gastric cancer tissues

Objective To detect the expressions of enhancer of zeste homology 2 (EZH2) and signal transducer and activator of transcription 3 (STAT3) in gastric cancer tissues,and analyze the relationship between their expressions and clinicopathological factors and prognosis of patients.Methods The clinical data of 67 patients with gastric cancer who were admitted to the People's Hospital of Peking University from May 1999 to April 2000 were retrospectively analyzed.Gastric cancer tissues and adjacent normal tissues were harvested and fixed in 4% formaldehyde,and then to make the tissue chips.The protein expressions of EZH2 and STAT3 were detected by immunohistochemical staining,and the relationship between the expressions of EZH2 and STAT3 and the prognosis of patients was analyzed.The correlation between the protein expressions of EZH2 and STAT3 in the gastric and colorectal tissues and the clinicopathological parameters was analyzed using the paired chi-square test.The survival curve was drawn by Kaplan-Meier method,and the survival rate was analyzed by using the Logrank test.The relationship between the clinicopathological factors and the prognosis of patients was analyzed using the COX proportional hazard model.Results The positive protein expression rates of EZH2 and STAT3 in the gastric cancer tissues were 73.1% (49/67) and 67.2% (45/67),which were significantly higher than 32.8% (22/67) and 37.3% (25/67) in the adjacent normal tissues (x2=21.839,11.964,P <0.05).The protein expression of STAT3 was correlated with the age,TNM staging,lymph node metastatic status (x2=5.475,9.998,5.475,P < 0.05).The protein expression of EZH2 was correlated with TNM staging and lymph node metastatic status (x2=11.573,5.902,P < 0.05).The co-expression rate of EZH2 and STAT3 proteins was 73.1% (49/67),EZH2 and STAT3 had the common expression trend (r =0.397,P < 0.05).The co-expression rate of EZH2 and STAT3 was increased as the increase of TNM stages,and the co-expression of EZH2 and STAT3 in the gastric cancer tissue was correlated with the TNM stages (x2 =6.997,P < 0.05).The 5-year survival rate of patients with low protein expression of EZH2 was significantly higher than those with high protein expression of EZH2 (x2=7.386,P < 0.05).The 5-year survival rate of patients with low proteins expression of STAT3 was significantly higher than those with high proteins expression of STAT3 (x2=12.253,P < 0.05).The 5-year survival rate of patients with low co-expression of EZH2 and STAT3 protein was significantly higher than those with high co-expression of EZH2 and STAT3 proteins (x2 =8.765,P < 0.05).The results of univariate analysis showed that age,TNM staging,EZH2 and STAT3 expression,lymph node metastasis and distal metastasis were the factors influencing the survival of patients with gastric cancer (RR =2.136,3.452,3.179,8.341,11.773,6.873,P <0.05).The results of multivariate analysis showed that TNM staging and STAT3 expression were independent factors influencing the prognosis of patients with gastric cancer (RR =2.453,7.535,P < 0.05).Conclusions There is significant correlation between EZH2 and STAT3 protein expressions in gastric cancer tissues,and co-expression of EZH2 and STAT3 is associated with the TNM staging and the prognosis of the patients with gastric cancer.
9.A study of protein expression of MAGE-A3, MAGE-A4 and MAGE-A10 genes in colorectal carcinoma and its clinical significance

Objective To explore protein expression and significance of MAGE genes in colorectal carcinoma(CRC)tissues.Methods The expression of MAGE genes were studied by using tissue chip and immunochemistry methods in primary CRC tissue and paired adjacent tissue samples in 97 cases.Data were analyzed with x2-test by SPSS 16.0 software.Results The protein expression of MAGE-A3,MAGE-A4 and MAGE-A10 genes were 57%(56/97),63%(61/97)and 28%(27/97)respectively in 97 cases of primary adenocarcinoma.The protein expression frequency of MAGE-A3 in poor colorectal adenocarcinomas was significantly higher than in well-and moderately disfferentiated adenocarcinomas(x2 =9.133,P =0.010).MAGE-A10 in poor colorectal primary adenocarcinomas was significantly higher than in well and moderately adenocarcinomas(x2 =15.280,P =0.000); MAGE-A10 protein expression was significantly higher in stage TNM Ⅲ + Ⅳ than in stage TNM Ⅰ + Ⅱ(x2 =4.227,P=0.040); MAGE-A10 gene expression was higher in metastasis lymphoid node than in no metastasis lymphoid node(x2 =5.557,P =0.018),and the expression level was higher in primary lesion with the increasing of the numbers of lymphoid node metastasis(x2 =7.296,P =0.026).Conclusions The protein expression of MAGE genes is associated with the tumor differentiation,TNM stage and lymphoid node metastasis.MAGE-A3 and MAGE-A10 genes are the possible prognosis marker and potential target of immunotherapy of CRC.
10.Clinical study of laparoscopic cholecystectomy combined with endoscopic retrograde cholangiopancreatography/endoscopic Oddi sphincterotomy on the treatment of cholecystolithiasis complicated by choledocholithiasis

Objective To discuss the clinical value of laparoscopic cholecystectomy (LC) combined with endoscopic retrograde cholangiopancreatography (ERCP)/endoscopic Oddi sphincterotomy (EST) on treating choledocholithiasis with cholecystolithiasis. Method The clinical data of 48 patients with choledocholithiasis complicated by cholecystolithiasis treated with LC combined with ERCP/EST from January 2005 to August 2010 was collected and analyzed retrospectively. Results Forty-six patients achieyed success by ERCP,and 45 patients finished LC,all patients underwent LC combined with ERCP/EST were recovered more rapidly,with shorter hospital stays. No severe complications or residual stone and refluent cholangitis in the follow-up of 3-12 months of 46 patients. Conclusions Combining the advantages of LC with ERCP/EST treating patients with choledocholithiasis complicated by cholecystolithiasis,according to the theoretics of minimally invasive surgery, with less invasive and the advantages of shorter hospital stays and rapid recovery. It is the comparatively ideal choice for the treatment of choledocholithiasis complicated by cholecystolithiasis at present.
