1.Severity of SARS-CoV-2 infection in children with kidney disease undergoing immunosuppressive therapy

Objective:To investigate the impact of immunosuppressive therapy on the severity of SARS-CoV-2 infection and cytokine levels in pediatric patients with kidney diseases.Methods:A retrospective analysis was conducted on the clinical data of 40 hospitalized pediatric patients who were diagnosed with SARS-CoV-2 infection at the 900th Hospital of PLA Joint Logistic Support Force from December 2022 to February 2023. Based on their immunosuppressive status prior to SARS-CoV-2 infection, these patients were categorized into immunosuppressive group and non-immunosuppressive group. Independent sample t-tests, Mann-Whitney U tests, and χ2 test were employed to compare the clinical baseline characteristics and laboratory data, the severity of SARS-CoV-2 infection, and the levels of cytokines between the 2 groups. Results:Among the 40 patients, 11 were in the immunosuppressive group (aged 13 (8, 14) years, 9 males and 2 females) and 29 in the non-immunosuppressive group (aged 2 (1, 4) years, 15 males and 14 females). In the immunosuppressive group, 2 were asymptomatic cases, 8 were mild cases, and 1 was moderate case, and there was no severe or critical cases. In the non-immunosuppressive group, 8 were mild cases, 5 were moderate, 15 were severe cases, 1 was critical case, and no asymptomatic cases. The underlying diseases in the immunosuppressive group included nephrotic syndrome (6 cases), IgA vasculitis nephritis (2 cases), lupus nephritis (1 case), post-renal transplantation (1 case), and renal failure (1 case), with a mean total immunosuppression score (TIS) of (3.6±1.4) points. In the non-immunosuppressive group, 2 patients had a history of epilepsy, and the remaining 27 cases had no underlying conditions, all with TIS scores of 0. Compared to the children in the non-immunosuppressive group, those in the immunosuppressive group were more likely to exhibit asymptomatic or mild infection, with lower risks of severe disease, cytokine storm, fever, and cough, but a higher risk of fatigue ( OR=1.22, 2.66, 0.48, 0.12, 0.12, 0.13, 1.22; 95% CI 0.93-1.62, 0.99-7.15, 0.33-0.70, 0.03-0.57, 0.03-0.57, 0.03-0.65, 0.93-1.62; all P<0.05). The levels of cytokine IL-6, interferon-α and interferon-γ in the immunosuppressive group were all lower than those in the non-immunosuppressive group ( Z=2.23, 2.51, 2.92, respectively; all P<0.05). Conclusion:Pediatric patients with kidney diseases receiving appropriate immunosuppressive therapy may mitigate the severity of SARS-CoV-2 infection by suppressing the expression of cytokines.

3.A redox-responsive self-assembling COA-4-arm PEG prodrug nanosystem for dual drug delivery suppresses cancer metastasis and drug resistance by downregulating hsp90 expression.

Metastasis and resistance are main causes to affect the outcome of the current anticancer therapies. Heat shock protein 90 (Hsp90) as an ATP-dependent molecular chaperone takes important role in the tumor metastasis and resistance. Targeting Hsp90 and downregulating its expression show promising in inhibiting tumor metastasis and resistance. In this study, a redox-responsive dual-drug nanocarrier was constructed for the effective delivery of a commonly used chemotherapeutic drug PTX, and a COA-modified 4-arm PEG polymer (4PSC) was synthesized. COA, an active component in oleanolic acid that exerts strong antitumor activity by downregulating Hsp90 expression, was used as a structural and functional element to endow 4PSC with redox responsiveness and Hsp90 inhibitory activity. Our results showed that 4PSC/PTX nanomicelles efficiently delivered PTX and COA to tumor locations without inducing systemic toxicity. By blocking the Hsp90 signaling pathway, 4PSC significantly enhanced the antitumor effect of PTX, inhibiting tumor proliferation and invasiveness as well as chemotherapy-induced resistance in vitro. Remarkable results were further confirmed in vivo with two preclinical tumor models. These findings demonstrate that the COA-modified 4PSC drug delivery nanosystem provides a potential platform for enhancing the efficacy of chemotherapies.
4.Case Report of Esophageal Rupture, Empyema, and Aortic Dissection Potentially Caused by Severe Vomiting

Both empyema and type A aortic dissection are life-threatening conditions. The combination, though rare, can cause serious complications and death. There have been rare cases of empyema caused by a co-infection with a Gram-positive bacterium, Granulicatella adiacens, which is difficult to identify, and a Gram-negative bacterium, Escherichia coli. In this case, a 50-year-old man was referred to the emergency department due to sudden chest pain and a suspected diagnosis of type A aortic dissection. The patient was subsequently found to have co-morbid empyema and a fissure in the esophagus when the cause of the disease was investigated. Considering the possibility that the patient’s infection was of digestive origin, a combination of Gram-positive and Gram-negative antibiotics was given immediately. Etiological examination later confirmed the presence of Granulicatella adiacens and Escherichia coli in chest drainage fluid, and the patient was successfully treated with antimicrobial therapy and conservative treatment.

6.Effect of amsulpride on the improvement of cognitive function in adolescent patients with first-episode schizophrenia

Objective To investigate the efficacy of sulforubicin combined with olanzapine in the treatment of adolescents with schizophrenia and to observe its effect on cognitive function.MethodsFrom August 2013 to August 2016, 124 adolescents diagnosed with schizophrenia were randomly divided into control group and observation group,each of 62 cases.The control group was given olanzapine treatment, the observation group was given olanzapine combined with amisulpride treatment.Positive and negative symptom scale (PANSS), Wisconsin Card Sorting Test (WCST) and Wechsler Memory Scale (WMS) were used to evaluate the patient and clinical symptoms and cognitive function after 8 weeks of treatment The clinical effect and adverse changes of two groups were compared.ResultsThe PANSS score, WCST score and WMS score of the two groups were not statistically significant.After 8 weeks of treatment, the PANSS score, WCST score and WMS score of the two groups were significantly higher than those before treatment (P<0.05), and the improvement of the above scores in observation group was better than that of the control group (P<0.05).The total effective rate was 93.55% in the observation group, which was significantly higher than that in the control group (67.74%, χ2=14.49, P<0.05).There was no serious adverse reaction between the two groups.ConclusionThe combination of sulforubicin and olanzapine for the first episode of schizophrenia is very effective in improving the clinical symptoms and cognitive function of patients, and has better safety.
7.Effects of military functional food NB-5 on psychological stress-induced oxidative stress

Objective To explore the protective effects of a new military functional food NB-5 on psychological stress-induced oxidative stress .Methods Rat whiskers were completely removed to induce the oxidative stress , and the concen-trations of MDA and protein carbonyl in various organs were detected to study the damage to membrane lipid and protein . Rats were fed with NB-5 for 4 weeks, and the oxidative stress was induced by whisker cutting .Biochemical marks men-tioned above were detected to explore the protective effects of NB-5.Results and Conclusion Lipid and protein peroxida-tion occurred in the brain , heart, liver, spleen and kidney after whisker removal due to emotional stress , while the catalase ( CAT) activity decreased significantly in these organs except the spleen .In this experiment model , NB-5 showed a good free radical scavenging activity to reduce the lipid and protein peroxidation among whisker -cutting rats fed with NB-5 in ad-vance.So NB-5 can serve as a good food for soldiers in case of emergency incidents .
8.Sequential treatment of neonatal hypoxic ischemic encephalopathy with a modified oxygen mask in an air-pressurized chamber

Objective To observe the efficacy, benefits and shortcomings of pressurized air therapy for neo- natal hypoxic ischemic encephalopathy (HIE). Methods One hundred and nine neonates with HIE were treated with hyperbaric oxygen (HBO) with a continuing oxygen supplement from an improved oxygen mask plus a 2.5-1itre breathing sacculus proprius in a large air-pressurized oxygen chamber. Among them there were 70 cases treated with 3-6 courses of HBO + drug therapy, 39 cases treated with a single session of HBO + drug therapy, and 32 treated with drug therapy alone. Motor development was assessed using the Chinese infantile intelligence development test scale at the ages of 3, 6 and 12 months. Results The sequential HBO + drug group achieved significantly better average motor development than the single session group or the drug only group. There was mo significant difference between the single treatment group and the drug only group. The proportion of abnormal CT results 12 months after treatment was significantly higher in the drug only group than in the sequential HBO + drug group. Conclusion Sequential HBO + drugs therapy with the improved oxygen mask is preferable to a single session of HBO + drug treatment or drug therapy alone.
9.Clinical and pathological analysis collision carcinoma: report of 7 patients.

OBJECTIVETo study the epidemiological, pathological, clinical features and therapy of collision carcinoma.
METHODSSeven patients were studied retrospectively against the background of the literature from 1980 to 2001, including the incidence, age of onset, sex ratio, predilection sites, clinical signs, pathological features and therapies.
RESULTSCollision carcinoma is rare, but more common in males than in females. The peak onset age is more than 65 years. It is found frequently in the stomach, liver, uterine, esophagogastric junction, and lung. Without special clinical features, it is difficult to diagnose. Pathological findings are the only way to make a correct diagnosis before operation. Operation is the major therapy, assisted by chemotherapy or/and radiotherapy when necessary. Collision carcinoma represents a coexistence of two adjacent but histologically different malignant neoplasmas without histological admixture in an organ.
CONCLUSIONSCollision carcinoma is rare but composed of mosaics of two malignant tumours. The clinical features of collision carcinoma need further investigation.

10.Surgical treatment of solid pseudopapillary tumors of the pancreas

Objective To study the characteristics of the solid pseudopapillary tumors of the pancreas (SPT) ,and to improve the diagosis and management of SPT. Methods Fourteen cases of solid pseudopapillary tumors of the pancreas admitted from Jan 1999 to Jul 2005 were analyzed retrospectively. Results There were 1 male and 13 females. Tumors were all single and mainly located in the head of the pancreas (9/14, 64. 3% ) , tumors in the other 5 cases were in the body and tail of the pancreas. The average diameter was 7.2 cm, with no specific symptom. On CT scan tumors were of low density, well-circumscribed with calcification, solid tumors were enhanced slightly on contrast medium, but cystic tumors were not enhanced . Simple resection was performed in 8 cases. Partial resection of the body of the pancreas and pancreaticojejunostomy was performed in 2 cases. One patient underwent distal pancreatectomy and splenectomy. Pancreaticoduodenectomy was performed in 3 cases. All the cases were followed up for an average of 20. 1 months with no recurrences. Conclusions Solid-pseudopapillary tumor of the pancreas is of low-grade malignancy. Surgical resection is effective. Surgical procedures adopted depend on the location and size of the tumor. Complete resection results in good prognosis.
