Objective:To explore the clinical efficacy of the splenic vessel-oriented anatomical plane priority strategy in Da Vinci robotic Kimura distal pancreatectomy.Methods:A retrospective analysis was conducted on 26 patients who underwent robotic-assisted distal pancreatectomy at Zhejiang Provincial People’s Hospital from January 2019 to September 2024. The cohort included 7 male and 19 female patients, aged (49.3±16.7) years. Surgical outcomes, including operative time, intraoperative blood loss, postoperative complications, and hospital stay, were analyzed, and surgical techniques were summarized.Results:All 26 patients successfully completed the surgery. Pathological diagnoses included 5 cases of intraductal papillary mucinous neoplasm, 5 serous cystadenomas, 1 pancreatic neuroendocrine tumor, 6 solid pseudopapillary neoplasms, 4 mucinous cystic neoplasms, and 5 neuroendocrine tumors. The maximum tumor diameter was (2.3±1.1) cm, and the operative time was (183.2±77.4) min. The spleen preservation rate was 100% (26/26). Intraoperative blood loss was 50.0 (17.5, 125) ml, and postoperative hospital stay was (10.1±3.7) d. No Clavien-Dindo grade III or higher complications occurred. The post-operative pancreatic fistula (POPF) rate was 53.8% (14/26), including 38.5% (10/26) biochemical leak and 15.3% (4/26) grade B POPF, with no grade C POPF.Conclusion:The splenic vessel-oriented anatomical plane priority strategy in robotic-assisted spleen-preserving distal pancreatectomy (Kimura technique) is safe and feasible, significantly improving the spleen preservation rate.

Objective:Based on " the surveillance, epidemiology, and end results" (SEER) database, we constructed a nomogram model for predicting cancer-specific survival time (CSST) in patients with T2 stage gallbladder cancer.Methods:Clinical data on 486 patients with T2 stage gallbladder cancer between 2018 and 2020 were retrospectively collected from the SEER database. The cohort comprised 147 male and 339 female patients with the age at diagnosis of (70±13) years. Clinical information including age, gender, tumor size, tumor stage, surgical type, number of lymph node dissection, postoperative treatment, and patients prognosis were extracted from the SEER database. We analyzed the factors influencing CSST in patients with T2 stage gallbladder cancer using Cox risk-proportional regression. The nomogram model was constructed based on independent risk factors obtained from multivariate Cox regression analysis, and the area under curve (AUC) of receiver operating characteristic curves (ROC) were used to evaluate the predictive accuracy of the nomogram model, while calibration plots, decision curve analysis, and clinical impact curves were used to evaluate the model's practicality and effectiveness.Results:The results of multivariate Cox regression analysis showed that patients with tumor size ≥30 mm ( HR=1.775, 95% CI: 1.123-2.806), AJCC stage ⅢB ( HR=6.083, 95% CI: 2.961-12.495), 1-3 lymph node dissection ( HR=6.139, 95% CI: 2.876-13.106), no postoperative chemotherapy ( HR=1.743, 95% CI: 1.096-2.771) had a higher risk of short CSST (all P<0.05). A nomogram model for predicting CSST was constructed based on the above risk factors, and the AUC of the ROC of which for predicting 1-year and 2-year CSST in patients with T2 stage gallbladder cancer was 0.778 and 0.696, respectively. Calibration plots demonstrated excellent collinearity between predicted and actual probabilities. Decision curve analysis and clinical impact curves confirmed high net benefit and clinical validity of the nomogram model. Conclusions:The tumor size ≥30 mm, AJCC stage ⅢB, 1-3 lymph node dissection and no postoperative chemotherapy are risk factors for short CSST in patients with T2 gallbladder cancer. The nomogram model based on the above risk factors have excellent performance in predicting CSST in patients with T2 stage gallbladder cancer.

Based on the theory of "spleen dispersing essence" in Huangdi's Inner Canon, combined with the discussion of medical experts in previous dynasties and modern researches, this paper argues that "spleen not dispersing essence" is an important pathogenesis of diabetes, which runs through the whole process of diabetes, including the stage of its complications. In the pathological state, the spleen disperses semen, affecting the physiological functions of the viscera, and the occurrence of diabetic retinal, heart, kidney, peripheral nerve and other complications. In clinical practice, the general principle of "promoting the spleen and dispersing essence" is used to provide the idea of TCM syndrome differentiation for the treatment of diabetes and its complications.

The iatrogenic splenic injuries occurred in 49 patients who underwent abdominal surgery from January 2005 to January 2020; including grade I injury in 40 cases and grade Ⅱ injury in 9 patients. The normal saline-soaked gauze was placed on the wound surface of the injured spleen, then the high-frequency electric coagulator was employed for hemostasis. The bleeding was successfully stopped and the spleens were preserved without postoperative complications in all cases. The results show that moist electrothermal coagulation can be effectively and safely applied in treatment of grade I and grade Ⅱ injury iatrogenic splenic injuries, and no special equipment required.

A total of 521 patients underwent laparoscopic radical gastrectomy from January 2013 to January 2020 in the First People′s Hospital of Wenling. In 242 cases the splenomental fold was severed before dissecting the left half of omentum or spleen (pretreatment group), and in 279 cases the splenomental fold was not severed priorly (routine group). For pretreatment group the introoperative splenic injury occurred in 4 cases (1.65%), including 3 cases (1.24%) with class Ⅰ injury and 1 case (0.41%) with class Ⅱ injury; while for routine group splenic injury occurred in 24 cases (8.60%), including 22 cases (7.89%) with class Ⅰ injury and 2 cases (0.72%) with class Ⅱ injury, and the rupture of splenic capsule caused by tracting splenomental fold occurred in 19 cases(6.81%). There were significant differences in total number of splenic injuries, splenic injuries with class Ⅰ and rupture of splenic capsule caused by tracting splenomental fold between two groups ( P<0.05). The operation time of 28 cases with splenic injuries was (185±89) min, which was longer than that in 493 cases without splenic injuries [(172±95) min, P<0.05]. The results show that rupture of splenic capsule by tracting splenomental fold is main cause of splenic injury in laparoscopic radical gastrectomy and most of them are class Ⅰ injuries. To sever the splenomental fold priorly can reduce the incidence of iatrogenic splenic injury.

Clinical data of 67 patients with recurrent inguinal hernia and chronic pain after anterior repair of transversalis fascia who underwent Kugel procedure through the primary incision in Wenling First People′s Hospital between June 2015 and January 2019 were retrospectively reviewed. During the operation the surrounding scar tissue and previous mesh or plug were removed, the transversalis fascia was separated and the patch was placed to overlay myopectineal orifice. The mean operative time was 57 min (51-85 min).The peritoneum rupture occurred during the operation in 6 cases (8.9%), and seroma was developed in 5 patients(7.5%). After a mean 18 months of follow-up, no recurrence occurred in all patients. Three months after operation, the mild, moderate or severe pain in all 67 patients were all improved remarkably with a total pain relief rate of 92.5%. The results show that Kugel procedure through the primary incision is safe and effective in treatment of recurrent inguinal hernia with chronic pain after anterior repair of transversalis fascia.

Objective To study the operation method of modified Lichtenstein herniorrhaphy in patients with femoral hernia.Methods The clinical data of 37 femoral hernia patients having underwent the modified Lichtenstein herniorrhaphy from January 2010 to January 2017 were retrospectively analyzed. Results The average operation time was 50 min, and average hospitalization day was 5.6 d. During the follow-up period,the postoperative pain and foreign body sensation were mild,no recurrence of femoral hernia was found,and no complications such as postoperative infection,testicular atrophy and painful ejaculation occurred. Conclusions Modified Lichtenstein herniorrhaphy for femoral hernia is effective.

Objective To observe the effect of Chuanxiongqin injection combined with nimodipine on primary open angle glaucoma.Methods 112 patients with primary open angle glaucoma in ophthalmology department were grouped two groups,each with 56 cases.Two groups were treated with nimodipine and observation group was treated another with Chuanxiongqin injection to observe effect.Results After treatment,retinal light sensitivity,visual field defect,blood flow,blood flow velocity,erythrocyte migration rate,PSV,EDV and RI in central retinal artery of observation group were better than control group(P<0.05).There was no significant difference in adverse reactions between groups.Conclusion Chuanxiongqin injection can improve blood flow status of central retinal artery and optic papillary laminar, retinal light sensitivity and visual field defect for patients with primary open angle glaucoma.
