1.Meta-analysis of the diagnostic value of 18F-FDG PET/CT in postoperative recurrence or metastasis of ovarian cancer patients with elevated CA125 levels

Objective:To systematically review the diagnostic value of 18F-fluorodeoxyglucose positron emission tomography/computed tomography(18F-FDG PET/CT)on recurrence or metastasis of patients with ovarian cancer whose postoperative serum carbohydrate antigen 125(CA125)occurred abnormal elevation.Methods:A systematic search was conducted in foreign databases,including PubMed,Embase,Cochrane Library(Central)and Web of Science(SCI-Expanded),as well as Chinese databases,such as China National Knowledge Infrastructure(CNKI),VIP Network,Wanfang Database,and China Biology Medicine(CBM),for the studies that were published between January 2010 and January 2023 on the diagnostic value of 18F-FDG PET/CT on recurrence or metastasis of patients with ovarian cancer whose postoperative serum CA125 levels occurred abnormal elevation.The quality of the included studies was assessed by using the QUADAS-2 quality evaluation tool in Review Manager 5.4 software.The effective data were extracted,and effect quantities were merged,and heterogeneity test was conducted through Meta-Disc 1.4 software.Publication bias of the included studies was tested by using Stata 14.0 software.Results:A total of seven literatures were involved in this research,and 290 patients with ovarian cancer were included to conduct heterogeneity test,which revealed I2 values all were greater than 50%.Therefore,the a fixed-effect model was used to merge effect quantity.The results demonstrated that the diagnostic accuracy was higher,and the sensitivity,specificity,positive likelihood ratio,negative likelihood ratio,and diagnostic odds ratio(DOR)of the mergement were respectively 0.95(95%CI:0.91-0.98),0.83(95%CI:0.70-0.92),4.84(95%CI:2.82-8.31),0.07(95%CI:0.04-0.13)and 73.53(95%CI:28.00-187.90).The area under curve(AUC)value of receiver operating characteristic curve(AUC)was 0.9527,and the diagnostic accuracy of 18F-FDG PET/CT for metastasis or recurrence in patients with ovarian cancer,whose postoperative serum CA125 levels occurred elevation,reached to 95.27%.Publication bias testing showed there was no evidence of publication bias.Conclusion:18F-FDG PET/CT exhibits higher accuracy in diagnosing recurrence or metastasis in patients with ovarian cancer whose postoperative serum CA125 levels occurred elevation,which can sensitively and accurately confirmed postoperative recurrence or metastasis of patients with ovarian cancer,and early find the lesions of metastasis or recurrence of ovarian cancer.

3.A case of chorea-acanthocytosis with psychiatric and behavioral symptoms as the primary clinical manifestation

Chorea-acanthocytosis (ChAc) is a rare, severe, and progressive hereditary neurodegenerative disease with a poor prognosis. The patient in this case was a 29-year-old female who presented with gradually worsening behavioral abnormalities and slurred speech as the main symptoms. Gene sequencing revealed a mutation in the VPS13A gene, leading to a diagnosis of"Chorea-acanthocytosis". This case report aims to improve the awareness among psychiatrists in distinguishing movement disorders and behavioral disorders. Combined with a literature review on the clinical manifestations, differential diagnosis, and genetic testing, this paper may help psychiatrists in early identification and accurate diagnosis of ChAc.
4.Lamotrigine augmentation in the treatment of schizophrenia with body dysmorphic symptoms: a case report

Body dysmorphic symptom is a symptom of obsessive-compulsive-related disorder and can be one of the symptoms of schizophrenia, causing severe distress and impairment of social functioning. Selective serotonin reuptake inhibitors (SSRIs) are the first line of therapeutic agents for body dysmorphic disorder (BDD). The combination of SSRIs and adjuvant drug therapy is a commonly used strategy for patients when their treatment is ineffective. This article reported a case of a young female patient with schizophrenia accompanied by body dysmorphic symptoms who was effectively treated with lamotrigine augmentation therapy, and discussed the clinical features of BDD, pharmacotherapy, potential pharmacological effects and adverse effects of lamotrigine based on the literature, in order to provide new perspectives on the treatment of body dysmorphic symptoms for clinical staff.
5.Improvement of tardive syndrome in three cases treated with modified electroconvulsive therapy

Tardive syndrome (TS) is a movement disorder caused by long-term treatment with dopamine-blocking agents, posing a significant clinical challenge for psychiatrists. Lowering the antipsychotic dosage, switching antipsychotics, and combining a vesicular monoamine transporter-2 inhibitor often fail to adequately control both psychiatric and motor symptoms. Additionally, TS can manifest in different subtypes, such as tardive dystonia and tardive hyperkinesia, each requiring distinct treatment approaches. This article reports on the improvement observed in three cases of different TS subtypes treated with modified electroconvulsive therapy, aiming to provide new clinical insights for treating TS.

Although social cognitive impairment is commonly observed in patients with schizophrenia, how to effectively assess it and the clinical value of such evaluations warrant further discussion. In 2022, the release of the "European Psychiatric Association guidance on assessment of cognitive impairment in schizophrenia" provided guidance for research and clinical practices in this field. This article aims to analyze the necessity, effectiveness, and feasibility of conducting assessments in clinical settings based on the recommended approaches outlined in the guidance, considering the current clinical landscape in China.

Although cognitive impairment is considered a core symptom of schizophrenia and significantly impacts functional outcomes, it is often overlooked in clinical practice. The European Psychiatric Association released “Guideline on Assessment of Cognitive Impairment in Schizophrenia” in 2022, providing recommendations for research and clinical practice. This paper interprets the neurocognitive aspects of schizophrenia covered in the guidelines, introduces neurocognitive assessment tools used in China and their application settings, examines the relationship between subjective and objective assessments of cognitive impairment, and briefly discusses the connections between neurocognition and positive and negative symptoms. The paper also considers the future development of neurocognitive assessment tools in China.

Cognitive impairment in schizophrenia emerges early and persists, is a core symptom that severely affects social functioning. In 2022, the European Psychiatric Association published the " European Psychiatric Association guidance on assessment of cognitive impairment in schizophrenia", presenting the latest evidence and systematic review results. The guidance recommends comprehensive and detailed cognitive assessments for all individuals with schizophrenia, emphasizing the phase of early intervention. In conjunction with the recommendations of the guidance and the current state of research both domestically and internationally, this article provides an overview of cognitive impairment in schizophrenia, including definition, impact, selection of assessment tools, and limitations of cognitive assessments. It also connects with clinical practice in China, outlining the development and shortcomings of cognitive assessments for schizophrenia. It is suggested that a clinically simple questionnaire suitable for the Chinese population is needed to help physicians quickly identify cognitive impairment.

Cognitive impairment is one of the core features of schizophrenia and severely affects the prognosis of the disorder. Improving cognitive impairment is of great significance for alleviating symptoms, preventing relapses, enhancing the quality of life, and reducing the social burden in patients with schizophrenia. Based on the "Guidelines for the Assessment of Cognitive Impairment in Schizophrenia" published by the European Psychiatric Association in 2022, the article focuses on the intervention and management of cognitive impairment in schizophrenia patients following a systematic assessment, while also considering the specific context of practice in China. This paper reviews recent progress in cognitive impairment interventions for schizophrenia, providing clinical guidance across three key aspects: pharmacotherapy, physical therapy, and psychotherapy. Other interventions are also discussed. Additionally, it addresses the existing challenges and unresolved issues, aiming to provide a basis for decision-making in the current treatment of cognitive impairment in schizophrenia.
