1.Application research of analytic hierarchy process-based fuzzy comprehensive evaluation model for quality assessment of hemodialysis machines

Objective To construct a fuzzy comprehensive evaluation model based on the analytic hierarchy process(AHP)to evaluate the quality of hemodialysis machines.Methods Firstly,the influencing factors for the quality of hemodialysis machines were determined with considerations on the requirements for medical technology assessment of medical devices and the hospital's many years of experience in use,maintenance and management of hemodialysis machines,and the indicators of the evaluation model were set with the general attributes of the product quality as the criterion level and the influencing factors as the sub-criterion level.Secondly,the AHP was used to qualitatively and quantitatively analyze relevant factors to construct a hierarchical judgment matrix,and the quality evaluation indicator weights for hemodylysis machines were determined with the consistency test.Finally,the operational principles of fuzzy mathematics were applied to establishing a fuzzy judgment matrix,and the evaluation results were calculated using Type Ⅳ fuzzy comprehensive evaluation model.Results The constructed fuzzy comprehensive evaluation model included one first-level indicator,seven second-level indicators and 21 third-level indicators.All the first-and second-level indicators passed the consistency tests.The evaluation results obtained with this model were practical and consistent with the hospital's long-term record data on hemodialysis machines.Conclusion The constructed fuzzy comprehensive evaluation model demonstrates a certain degree of scientific rigor for quality assessment of hemodialysis machines and can provide decision-making support for their procurement justification.[Chinese Medical Equipment Journal,2025,46(9):81-87]
2.To explore the safety and efficacy of applying Bivalirudin during perioperative period of percutaneous coronary intervention in complex high-risk and indicated patients with extracorporeal membrane oxygenation

Objective To explore the safety and efficacy of applying Bivalirudin in complex high-risk and indicated patients(CHIP)undergoing percutaneous coronary intervention(PCI)while on extracorporeal membrane oxygenation(ECMO).This study will provide additional evidence for making informed decisions regarding anticoagulation therapy during ECMO.Methods A total of 83 CHIP who underwent VA-ECMO assisted PCI in the Heart Center of the First Hospital of Lanzhou University from March 2019 to December 2022 were retrospectively enrolled as the research objects.According to the use of anticoagulant drugs during ECMO support and PCI,they were divided into Bivalirudin group(40 cases)and heparin group(43 cases).The cost of anticoagulants,coefficient of variation of activated partial thromboplastin time(APTT),APTT compliance rate,bleeding events,thrombotic events,blood product transfusion,and net adverse clinical events(NACE)at 30 days after PCI were compared between the two groups to determine the safety and efficacy of Bivalirudin in CHIP undergoing ECMO assisted PCI.Results The proportion of previous PCI(25.00%vs.4.65%),fibrinogen level[(3.52±1.14)g/L vs.(2.92±1.28)g/L],CRUSADE score[(32.00±12.69)scores vs.(26.14±10.60)scores],anticoagulation cost[1 400.00(700.00,2 100.00)RMB vs.107.69(58.74,205.59)RMB]and compliance rate of APTT[(57.76±33.11)%vs.(31.44±27.63)%]in the Bivalirudin group were higher,while the coefficient of variation for APTT[10.64(7.72,21.11)vs.19.47(10.48,31.28)]was lower than those in the heparin group.The total bleeding events(25.00%vs.46.51%),Bleeding Academic Research Consortium(BARC)1-2 bleeding events(12.50%vs.32.56%),and total thrombotic events(12.50%vs.32.56%)of the Bivalirudin group was significantly lower compared with the heparin group.Kaplan-Meier curves showed that cumulative incidence of 30-days NACE was significantly lower in the Bivalirudin group than in the heparin group.The differences between the above two groups were statistically significant(Log-rank P<0.001).Conclusions Compared with heparin,Bivalirudin has better safety and efficacy in CHIP undergoing PCI assisted by VA-ECMO.

6.Proteomics reveals biomarkers for sepsis-associated acute kidney injury: a prospective multicenter cohort study.

OBJECTIVE:
To identify and validate novel biomarkers for the early diagnosis of sepsis-associated acute kidney injury (SA-AKI) and precise continuous renal replacement therapy (CRRT) using proteomics.
METHODS:
A prospective multicenter cohort study was conducted. Patients with sepsis admitted to five hospitals in Taizhou City of Zhejiang Province from April 2019 to December 2021 were continuously enrolled, based on the occurrence of acute kidney injury (AKI). Sepsis patients were divided into SA-AKI group and non-SA-AKI group, and healthy individuals who underwent physical examinations during the same period were used as control (NC group). Peripheral blood samples from participants were collected for protein mass spectrometry analysis. Differentially expressed proteins were identified, and functional enrichment analysis was conducted on these proteins. The levels of target proteins were detected by enzyme linked immunosorbent assay (ELISA), and the predictive value of target protein for SA-AKI were evaluated by receiver operator characteristic curve (ROC curve). Additionally, sepsis patients and healthy individuals were selected from one hospital to externally verify the expression level of the target protein and its predictive value for SA-AKI, as well as the accuracy of CRRT treatment.
RESULTS:
A total of 37 patients with sepsis (including 19 with AKI and 18 without AKI) and 31 healthy individuals were enrolled for proteomic analysis. Seven proteins were identified with significantly differential expression between the SA-AKI group and non-SA-AKI group: namely cystatin C (CST3), β 2-microglobulin (β 2M), insulin-like growth factor-binding protein 4 (IGFBP4), complement factor I (CFI), complement factor D (CFD), CD59, and glycoprotein prostaglandin D2 synthase (PTGDS). Functional enrichment analysis revealed that these proteins were involved in immune response, complement activation, coagulation cascade, and neutrophil degranulation. ELISA results demonstrated specific expression of each target protein in the SA-AKI group. Additionally, 65 patients with sepsis (38 with AKI and 27 without AKI) and 20 healthy individuals were selected for external validation of the 7 target proteins. ELISA results showed that there were statistically significant differences in the expression levels of CST3, β 2M, IGFBP4, CFD, and CD59 between the SA-AKI group and non-SA-AKI group. ROC curve analysis indicated that the area under the curve (AUC) values of CST3, β 2M, IGFBP4, CFD, and CD59 for predicting SA-AKI were 0.788, 0.723, 0.723, 0.795, and 0.836, respectively, all exceeding 0.7. Further analysis of patients who underwent CRRT or not revealed that IGFBP4 had a good predictive value, with an AUC of 0.84.
CONCLUSIONS
Based on proteomic analysis, CST3, β 2M, IGFBP4, CFD, and CD59 may serve as potential biomarkers for the diagnosis of SA-AKI, among which IGFBP4 might be a potential biomarker for predicting the need for CRRT in SA-AKI patients. However, further clinical validation is required.

7.Research progress in effect of traditional Chinese medicine on aerobic glycolysis in colorectal cancer.

Colorectal cancer(CRC) is a common malignant tumor worldwide. Due to the treatment intolerance and side effects, CRC rank the top among various cancers regarding the incidence and mortality rates. Therefore, exploring new therapies is of great significance for the treatment of CRC. Aerobic glycolysis(AEG) plays an important role in the microenvironment formation, proliferation, metastasis, and recurrence of CRC and other tumor cells. It has been confirmed that intervening in the AEG pathway can effectively curb CRC. The active ingredients and compound prescriptions of traditional Chinese medicine(TCM) can effectively inhibit the proliferation, metastasis, and drug resistance and regulate the apoptosis of tumor cells by modulating AEG-associated transport proteins [eg, glucose transporters(GLUT)], key enzymes [hexokinase(HK) and phosphofructokinase(PFK)], key genes [hypoxia-inducible factor 1(HIF-1) and oncogene(c-Myc)], and signaling pathways(MET/PI3K/Akt/mTOR). Accordingly, they can treat CRC, reduce the recurrence, and improve the prognosis of CRC. Although AEG plays a key role in the development and progression of CRC, the specific mechanisms are not yet fully understood. Therefore, this article delves into the intrinsic connection of the targets and mechanisms of the AEG pathway with CRC from the perspective of tumor cell glycolysis and explores how active ingredients(oxymatrine, kaempferol, and dioscin) and compound prescriptions(Quxie Capsules, Jiedu Sangen Decoction, and Xianlian Jiedu Prescription) of TCM treat CRC by intervening in the AEG pathway. Additionally, this article explores the shortcomings in the current research, aiming to provide reliable targets and a theoretical basis for treating CRC with TCM.

The traditional Chinese medicine(TCM) industry is a crucial part of China's pharmaceutical sector and plays a strategic role in ensuring public health and promoting economic and social development. In response to the practical demand for high-quality development of the TCM industry, this paper focused on the bottlenecks encountered during the digital and intelligent transformation of TCM production systems. Specifically, it explored technical strategies and methodologies for constructing the best TCM production mode. An innovative artificial intelligence(AI)-centered technical architecture for TCM production was proposed, focusing on key aspects of production management including process modeling, state evaluation, and decision optimization. Furthermore, a series of critical technologies were developed to realize the best TCM production mode. Finally, a novel AI-driven TCM production mode characterized by a closed-loop system of "measurement-modeling-decision-execution" was presented through engineering case studies. This study is expected to provide a technological pathway for developing new quality productive forces within the TCM industry.

9.Domestication progress of endangered Chinese medicinal material Fritillariae Cirrhosae Bulbus.

Fritillariae Cirrhosae Bulbus is the dried bulb of perennial herbaceous plants in the Fritillaria genus(Liliaceae family) and is a representative traditional Chinese medicinal material with distinctive regional characteristics. Clinically, it is widely used in the treatment of dry cough, bronchial asthma, and other respiratory diseases, possessing significant medicinal and economic value and being highly esteemed in TCM. Currently, Fritillariae Cirrhosae Bulbus primarily relies on wild harvesting. However, due to excessive collection, its wild resources have drastically declined, and all source species have been classified as category Ⅱ in the List of National Key Protected Wild Plants, exacerbating the supply-demand imbalance in the market. To mitigate this issue, large-scale cultivation through the domestication of wild Fritillariae Cirrhosae Bulbus has become an inevitable trend. However, its strict environmental requirements, low propagation efficiency, high seedling mortality, and immature cultivation techniques have severely hindered industrialization. This study investigates the domestication process of Fritillariae Cirrhosae Bulbus, focusing on seed propagation, seedling cultivation, and medicinal material production. It also reviews the species and distribution of wild resources, their endangered status, market supply-demand dynamics, and the historical and current development of domestication. The findings indicate that enhancing propagation efficiency, optimizing cultivation models, and distinguishing between seed propagation and medicinal material production are key measures to accelerate the industrialization of domesticated Fritillariae Cirrhosae Bulbus. This research aims to promote the industrialization of Fritillariae Cirrhosae Bulbus domestication and provide a reference model for the conservation and sustainable utilization of rare and endangered medicinal plant resources.

10.Multi-Phase Contrast-Enhanced CT Clinical-Radiomics Model for Predicting Prognosis of Extrahepatic Cholangiocarcinoma After Surgery: A Single-Center Retrospective Study.

OBJECTIVES:
To develop and validate a preoperative clinical-radiomics model for predicting overall survival (OS) and disease-free survival (DFS) in patients with extrahepatic cholangiocarcinoma (eCCA) undergoing radical resection.
METHODS:
In this retrospective study, consecutive patients with pathologically-confirmed eCCA who underwent radical resection at our institution from 2015 to 2022 were included. The patients were divided into a training cohort and a validation cohort according to the chronological order of their CT examinations. Least absolute shrinkage and selection operator (LASSO)-Cox regression was employed to select predictive radiomic features and clinical variables. The selected features and variables were incorporated into a Cox regression model. Model performance for 1-year OS and DFS prediction was assessed using calibration curves, area under receiver operating characteristic curve (AUC), and concordance index (C-index).
RESULTS:
This study included 123 patients (mean age 64.0 ± 8.4 years, 85 males/38 females), with 86 in the training cohort and 37 in the validation cohort. The OS-predicting model included four clinical variables and four radiomic features. It achieved a training cohort AUC of 0.858 (C-index = 0.800) and a validation cohort AUC of 0.649 (C-index = 0.605). The DFS-predicting model included four clinical variables and four other radiomic features. It achieved a training cohort AUC of 0.830 (C-index = 0.760) and a validation cohort AUC of 0.717 (C-index = 0.616).
CONCLUSIONS
The preoperative clinical-radiomics models show promise as a tool for predicting 1-year OS and DFS in eCCA patients after radical surgery.
