1.Analyses of community chronic disease management efficiency in Shanghai: a case study of hypertension and diabetes

ObjectiveTo analyze the output and efficiency of chronic disease management services for hypertension and diabetes in community health service institutions in Shanghai, to evaluate the allocation and efficiency of chronic disease management resources, and to provide references for improving the efficiency of chronic disease management in Shanghai’s communities. MethodsA total of 73 community health institutions from 15 districts of Shanghai were selected. Based on the chronic disease management resource data of community health institutions in 2019, 2021, and 2023, descriptive methods and the BCC model in Data Envelopment Analysis (DEA) were used to analyze the resource allocation, service output, and the efficiency of chronic disease management services in community health institutions in Shanghai. ResultsFrom 2019 to 2023, the indicators of resource allocation and service output of community health service institutions in Shanghai all showed upward trends. In terms of service efficiency, the average number of chronic disease patients managed by each health technician decreased from 237 to 231, and the number of patients served per 1 000 working hours decreased from 54 to 44. DEA efficiency analyses showed that the average overall efficiency increased from 0.867 in 2019 to 0.886 in 2021 and then decreased to 0.811 in 2023. The average technical efficiency decreased from 0.901 to 0.847, which was the main reason for the decline in overall efficiency. The average scale efficiency showed an initial increase followed by a decrease, rising from 0.961 in 2019 to 0.977 in 2021 and dropping back to 0.955 in 2023. The number of strong effective districts (overall efficiency=1.000) decreased from 6 in 2019 to 3 in 2023, and the efficiency differences between districts expanded. The range of overall efficiency widened from 0.622‒1.000 to 0.513‒1.000, indicating a decline in overall efficiency and an increase in inter-district heterogeneity. ConclusionFrom 2019 to 2023, the total volume of resource allocation and service output for chronic disease management in community health service institutions in Shanghai increased continuously. However, both the service output efficiency and the overall resource allocation efficiency declined, driven primarily by a decrease in technical efficiency, and accompanied by widening inter‑district disparities.
2.The value of Th17/Treg imbalance in the evaluation of intravenous immunoglobulin resistance in children with Kawasaki disease and Kobayashi score ≤4

Objective:To investigate the value of T helper 17 cells(Th17)/regulatory T cells(Treg)imbalance in the evaluation of intravenous immunoglobulin(IVIG)resistance in children with Kawasaki disease and Kobayashi score≤4.Methods:A total of 78 children with Kawasaki disease and Kobayashi score ≤ 4 admitted to Hunan Children's Hospital from January 2020 to December 2023 were prospectively selected as the study subjects,all of whom received IVIG treatment.In the acute phase,the proportion of Th17 cells and Treg cells was detected.Children were divided into IVIG sensitive group and IVIG resistance group based on their responsiveness to IVIG treatment.Baseline data of children with different IVIG treatment responsiveness,acute Th17 cell inflammatory factors [interleukin(IL)-17,IL-21,tumor necrosis factor-α(TNF-α)],Treg cell inflammatory factors [IL-10,IL-35,transforming growth factor-β(TGF-β)] levels,and Th17/Treg values were compared.The correlation between Th17/Treg values and IVIG resistance in children with Kawasaki disease was analyzed using a restricted cubic spline model(RCS).According to the threshold of correlation between Th17/Treg values obtained from RCS analysis and drug resistance in children,Th17/Treg was grouped,with a focus on analyzing the predictive value and clinical benefits of Th17/Treg values for IVIG resistance in children with Kawasaki disease.Results:Among the 78 children with Kawasaki disease,16 were resistant to IVIG treatment,accounting for 20.51%.The levels of C-reactive protein(CRP),IL-17,and Th17/Treg in the acute phase of children in the IVIG resistance group were higher than those in the IVIG sensitive group,while the levels of IL-10 were lower than those in the IVIG sensitive group( P<0.05).RCS analysis showed that there was a non-linear dose-response relationship between IVIG resistance and acute Th17/Treg values in children with Kawasaki disease( P<0.05).When the acute Th17/Treg value was greater than 1.05,the risk of IVIG resistance in children with Kawasaki disease increased with the increase in indicator levels.The levels of CRP and IL-17 in the acute phase of children with Th17/Treg>1.05 were higher than those in the Th17/Treg < 1.05 group,while IL-10 levels were lower than those in the Th17/Treg<1.05 group.The proportion of children resistant to IVIG treatment was higher than that in the Th17/Treg<1.05 group( P<0.05).Multivariate Logistic regression analysis showed that CRP,IL-17,IL-10,and Th17/Treg were the influencing factors of IVIG resistance in children with Kawasaki disease( P<0.05).It was found through a nomogram that the C-index of the acute phase Th17/Treg values and their secretion of inflammatory factors in children with Kawasaki disease and Kobayashi score ≤ 4,as well as other major indicators,predicted the risk of IVIG resistance.The C-index was 0.975(95% CI 0.944-1.000),indicating good discrimination.When drawing the decision curve,it was found that compared to using each indicator separately,the Th17/Treg value and its secreted inflammatory factors in the acute phase assisted other major indicators in drawing the decision curve with a higher net benefit rate,with a maximum net benefit rate of 0.205. Conclusion:IVIG resistance in children with Kawasaki disease and Kobayashi score≤4 is related to Th17/Treg imbalance.When the Th17/Treg value in the acute phase of the disease is greater than 1.05,the risk of IVIG resistance is higher.The inflammatory factors IL-17 and IL-10 secreted by the two can assist other known indicators related to IVIG resistance in Kawasaki disease patients,improving the accuracy of predicting resistance risk.
3.Clinical Observation on the Improvement of Postoperative Delirium in Elderly Patients with Hip Fractures by Adding Modified XinjiaHuanglong Decoction Combined with Ear Point Application Pressure

Objective:To analyze the clinical effect of XinjiaHuanglong Decoction add and subtract combined with auricular point sticking on delirium after hip fracture in elderly patients.Methods:Select 80 elderly patients with postoperative delirium after hip fracture admitted to our hospital from January 2022 to June 2024,and divide them into a matched group and an observation group,with 40 cases in each group.The matched group was treated with olanzapine,and the observation group was treated with XinjiaHuanglong Decoction add and subtract combined with auricular point sticking.Inflammatory response indexes(IL-6,TNF-α,hs-CRP)and stress response indexes(dopamine(DA),5-hydroxytryptamine(5-HT)were detected before and Post-treatment in both groups.Compare the Delirium Rating Scale 98(DRS-R-98)scores,Mini Mental State Examination(MMSE)scores,and duration of delirium between two groups before and Post-treatment,comprehensively evaluate the efficacy,and record the occurrence of adverse reactions.Results:Post-treatment,the DRS-R-98 score in the observation group was lower than that in the matched group,and the MMSE score was higher than that in the matched group(P<0.05);The duration of delirium in the observation group was shorter than that in the matched group,and the overall effective rate was higher than that in the matched group(P<0.05).The levels of inflammatory response indicators in the observation group were lower than those in the matched group Post-treatment(P<0.05);The level of stress response indicators in the observation group was lower than that in the matched group Post-treatment(P<0.05);There was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:XinjiaHuanglong Decoction add and subtract combined with auricular point sticking has a certain effect on improving delirium after hip fracture in the elderly,and can shorten the duration of delirium,which may be related to reducing inflammation and stress response,and it is safe and worthy of clinical application.

5.Investigation of the Mechanism of Atractylodes Ⅰ Inhibiting Colorectal Cancer via the AKT/S6K1 Signaling Pathway

OBJECTIVE To investigate the pharmacological efficacy and mechanism of action of Atractylenolide Ⅰ(Atr-Ⅰ)in inhibiting colorectal cancer.METHODS Among three active compounds of Atractylodes macrocephala,Atr-Ⅰ exhibited the highest anti-tumor potency by MTT assay.The optimal concentration of Atr-Ⅰ was determined.The effect of Atr-Ⅰ on LoVo cell prolifera-tion was assessed via a clonogenic assay,while its impact on apoptosis and cell cycle progression was evaluated using flow cytometry.The influence of Atr-Ⅰ on the migration and invasion of LoVo cell line was examined through wound healing and Transwell migration assays.Western blot analysis was performed to explore the effects and mechanisms of Atr-Ⅰ on proteins associated with mi-gration,proliferation,and epithelial-mesenchymal transition(EMT)in LoVo cells.The CT26 mouse subcutaneous tumor model was established,and histopathological analysis was conducted using hematoxylin-eosin(HE)staining.Western blot was also used to assess the effects of Atr-Ⅰ on EMT-related proteins in mouse tissues to elucidate underlying mechanisms.RESULTS Atr-Ⅰ significantly reduced colorectal cancer cell viability,with statistically significant differences between treatment and control groups(P<0.05,P<0.01).Atr-Ⅰ induced apoptosis in LoVo cells,with the treatment group showing significant differences compared to the control(P<0.05,P<0.01).Cell cycle analysis revealed that Atr-Ⅰ exerted anti-tumor effects by inducing G2/M phase arrest,with increased G2 phase cell numbers in the LoVo treatment group compared to the control(P<0.05).Wound healing and Transwell migration assays confirmed that Atr-Ⅰ significantly inhibited tumor cell migration and invasion(P<0.05,P<0.01).Western blot analysis demonstra-ted that Atr-Ⅰ specifically suppressed the expression of c-Myc and Bcl-2(P<0.05),as well as cell cycle-related proteins CDK1,Cyclin B1,and Cyclin D1(P<0.05),and angiogenesis-related proteins VEGF and MMP9(P<0.05).Additionally,Atr-Ⅰ down-regulated EMT-related protein N-cadherin and upregulated E-cadherin expression(P<0.05).It also reduced the expression of p-AKT and p-S6K1(P<0.05).CONCLUSION Atr-Ⅰ exhibits potent anti-tumor effects against colorectal cancer,potentially through modulation of the AKT/S6K1 signaling pathway.

7.Comparative effectiveness of small optical zone orthokeratology lenses and repeated low-level red-light therapy for controlling myopia in adolescents

Objective To compare the effect of small optical zone orthokeratology lenses(OK lenses)and repeated low-level red-light(RLRL)therapy in controlling myopia progression for adolescents,and the therapeutic effectiveness of RLRL is evaluated.Methods A retrospective analysis was conducted on the clinical data of 80 adolescent myopic patients in the First Affiliated Hospital of Army Medical University.The patients were divided into the RLRL group and the OK lenses group according to different intervention methods,with 40 cases in each group.Patients in the RLRL group received RLRL therapy combined with single-vision spectacles,and patients in the OK lenses group were treated with OK lenses.The changes of spherical equivalent(SE),axial length and subfoveal choroidal thickness(SFCT)1,3,6,and 12 months after treatment compared with the baseline,and color vision of patients were assessed.Based on the mean baseline axial length of the RLRL group,the patients in this group were subdivided into the short axial group and the long axial group,and the changes of the axial length and SFCT were further analyzed.Results The diopter 1,3,6,and 12 months after treatment in the RLRL group were not significantly different from the baseline(P>0.05).Axial lengths of patients in the RLRL group progressively shortened after treatment and returned close to the baseline 12 months after treatment.In contrast,axial lengths of patients in the OK lens continued to grow within 12 months after treatment;the axial lengths at each time point after treatment of patients in the two groups were significantly different from the baseline(P<0.05).The changes of axial length of patients in the RLRL group at each time point after treatment were significantly smaller than those in the OK lenses group(P<0.05).The SFCT changes of patients in the RLRL group at each time point after treatment were all greater than those in the OK lenses group(P<0.05).The SFCT at each time point after treatment in the RLRL group were significantly different from the baseline(P<0.05),whereas the SFCT at each time point after treatment in the OK lenses group were not significantly different from the baseline(P>0.05).The changes of axial length at each time point after treatment of patients in the long axial group were all greater than those in the short axial group(P<0.05);the SFCT changes 6 months after treatment of patients in the long axial group was greater than that in the short axial group(P<0.05);the SFCT at each time point after treatment of patients in the two groups were signifi-cantly different from the baseline(P<0.05).Color vision tests revealed no abnormities after treatment in the RLRL group and the OK lenses group.Conclusion RLRL therapy is effective in controlling myopia progression and demonstrates superior axial length control compared to orthokeratology lenses.

9.Application of Zeolitic Imidazolate Framework-8 in Bone Tissue Regeneration

How to effectively achieve regenerative repair of large bone defects remains a significant chal-lenge in current clinical treatment.Traditional bone regeneration materials have yet to yield ideal results,while the emergence of nanomaterials offers new options for bone regeneration.In recent years,metal-or-ganic frameworks(MOFs)have been widely applied in fields such as biomedicine,among which zeolitic imidazolate framework-8 nanoparticles(ZIF-8 NPs)have been extensively studied.ZIF-8 NPs are easy to synthesize,with controllable particle sizes,large specific surface areas,high thermal stability,and pH responsiveness.The nanoscale size of ZIF-8 NPs can be utilized to adjust the mechanical properties of biomaterials,further aligning with the elastic modulus of bone tissue.Additionally,ZIF-8 NPs facilitate osteogenic differentiation,vascular regeneration,and exhibit antibacterial properties,while also serving as carriers for controlled release of drugs or biomacromolecules.This article summarizes the characteris-tics and biocompatibility of ZIF-8 NPs and reviews their application progress in the field of bone tissue re-generation,including the enhancement of mechanical properties,cell adhesion,osteoinductive activity,angiogenic ability,anti-inflammatory ability and antibacterial ability of bone regeneration materials.This review aims to provide references for advancing the preparation and application of ZIF-8-based bioactive materials.

Objective:To investigate the value of T helper 17 cells(Th17)/regulatory T cells(Treg)imbalance in the evaluation of intravenous immunoglobulin(IVIG)resistance in children with Kawasaki disease and Kobayashi score≤4.Methods:A total of 78 children with Kawasaki disease and Kobayashi score ≤ 4 admitted to Hunan Children's Hospital from January 2020 to December 2023 were prospectively selected as the study subjects,all of whom received IVIG treatment.In the acute phase,the proportion of Th17 cells and Treg cells was detected.Children were divided into IVIG sensitive group and IVIG resistance group based on their responsiveness to IVIG treatment.Baseline data of children with different IVIG treatment responsiveness,acute Th17 cell inflammatory factors [interleukin(IL)-17,IL-21,tumor necrosis factor-α(TNF-α)],Treg cell inflammatory factors [IL-10,IL-35,transforming growth factor-β(TGF-β)] levels,and Th17/Treg values were compared.The correlation between Th17/Treg values and IVIG resistance in children with Kawasaki disease was analyzed using a restricted cubic spline model(RCS).According to the threshold of correlation between Th17/Treg values obtained from RCS analysis and drug resistance in children,Th17/Treg was grouped,with a focus on analyzing the predictive value and clinical benefits of Th17/Treg values for IVIG resistance in children with Kawasaki disease.Results:Among the 78 children with Kawasaki disease,16 were resistant to IVIG treatment,accounting for 20.51%.The levels of C-reactive protein(CRP),IL-17,and Th17/Treg in the acute phase of children in the IVIG resistance group were higher than those in the IVIG sensitive group,while the levels of IL-10 were lower than those in the IVIG sensitive group( P<0.05).RCS analysis showed that there was a non-linear dose-response relationship between IVIG resistance and acute Th17/Treg values in children with Kawasaki disease( P<0.05).When the acute Th17/Treg value was greater than 1.05,the risk of IVIG resistance in children with Kawasaki disease increased with the increase in indicator levels.The levels of CRP and IL-17 in the acute phase of children with Th17/Treg>1.05 were higher than those in the Th17/Treg < 1.05 group,while IL-10 levels were lower than those in the Th17/Treg<1.05 group.The proportion of children resistant to IVIG treatment was higher than that in the Th17/Treg<1.05 group( P<0.05).Multivariate Logistic regression analysis showed that CRP,IL-17,IL-10,and Th17/Treg were the influencing factors of IVIG resistance in children with Kawasaki disease( P<0.05).It was found through a nomogram that the C-index of the acute phase Th17/Treg values and their secretion of inflammatory factors in children with Kawasaki disease and Kobayashi score ≤ 4,as well as other major indicators,predicted the risk of IVIG resistance.The C-index was 0.975(95% CI 0.944-1.000),indicating good discrimination.When drawing the decision curve,it was found that compared to using each indicator separately,the Th17/Treg value and its secreted inflammatory factors in the acute phase assisted other major indicators in drawing the decision curve with a higher net benefit rate,with a maximum net benefit rate of 0.205. Conclusion:IVIG resistance in children with Kawasaki disease and Kobayashi score≤4 is related to Th17/Treg imbalance.When the Th17/Treg value in the acute phase of the disease is greater than 1.05,the risk of IVIG resistance is higher.The inflammatory factors IL-17 and IL-10 secreted by the two can assist other known indicators related to IVIG resistance in Kawasaki disease patients,improving the accuracy of predicting resistance risk.
