1.Analyzing the heart-oriented view of the treatment of mental illness in Synopsis of Golden Chamber based on the theory of five-spirit-viscera

Mental state is an important part of the normal life activities of the human body, and it is also the most external expression and the most easily obtained information of the physical condition. The normal activities of the mind depend on the normal operation of the viscera, qi, and blood, and are a unified whole that prospers together and suffers together. The theory of the five-spirit-viscera in the Yellow Emperor’s Inner Classic revealed that the normal mental activities of the human body were dominated by the five internal organs, that is, the five internal organs were the body and the five spirits were the function. And it highlighted the viewpoint that the five internal organs store the spirits and are actually one. The heart governs the spirit and belongs to the four internal organs. On this basis, Synopsis of Golden Chamber used the internal organs to diagnose and treat mental diseases, integrating the theory of the five spirits into it, forming a unique method of diagnosis and treatment with the heart as the leading factor and regulating the qi and blood of the four internal organs. It identified the pathogenesis of diseases such as pathogenic crying, lily disease, and hysteria from five levels: heart deficiency and weak qi, heart-lung disharmony, heart-liver disharmony, the heart of the loss of the spleen nourishment, and disharmony between heart and kidney. The treatment was mainly to replenish the deficiency of the viscera and eliminate the pathogens, reflecting the characteristics of regulating the mind and calming the four internal organs. This unique view on diagnosis and treatment has profoundly influenced the diagnosis and treatment theories of mental illnesses by later doctors, and is of great significance to the current clinical treatment of such illnesses.
2.Study on residents’willingness to pay for community pharmacies’medication guidance services and its influencing factors

OBJECTIVE To explore the willingness to pay (WTP) of Urumqi residents for community pharmacies’ medication guidance services and its influencing factors, so as to provide data support for the optimization of community pharmacy services and the establishment of a fee structure for medication guidance services. METHODS A stratified quota sampling method was employed to select 14 communities in Urumqi City. From April to June 2025, a combined offline and online questionnaire survey was conducted among adult residents in these communities. The contingent valuation method was used to construct three hypothetical scenarios (namely, basic, enhanced and extended services) of medication counselling in community pharmacies to assess residents’ WTP for these services. Binary Logistic regression was employed to analyze the influencing factors of WTP. RESULTS A total of 576 valid questionnaires were obtained. Under the scenarios of basic, enhanced and extended services, 38.54%, 49.65% and 67.19% of the respondents expressed WTP for the services, respectively. Occupational type, type of basic medical insurance, annual income, perception of pharmacists’ profession, and acceptance level of the service were identified as major influencing factors for WTP (P<0.05). CONCLUSIONS The willingness of residents in Urumqi to pay for medication counseling services provided by pharmacists in community pharmacies significantly increases with the enrichment of service content. It is recommended to incorporate basic medication counselling services provided by pharmacists in community pharmacies into medical insurance payment, while value-added services should be partially or fully self-paid by residents. Additionally, efforts should be made to strengthen the promotion of the professional and service value of licensed pharmacists, so as to facilitate the high-quality development of pharmaceutical care.
3.Study on the effectiveness and safety of a novel intravascular shock wave balloon for pre-treatment of severe coronary artery calcification lesions

Objective To evaluate the efficacy and safety of a novel intravascular lithotripsy(IVL)balloon—Vesscrack shockwave balloon—for vascular preparation before stent implantation in patients with severe coronary artery calcification(CAC).Methods This was a prospective,single-arm,multicenter study conducted in China from June 2022 to October 2022.Patients with severe CAC were treated with the Vesscrack shockwave balloon for lesion preparation,followed by drug-eluting stent(DES)implantation.Of these,33 patients underwent optical coherence tomography(OCT).The primary endpoint was procedural success,defined as successful stent implantation with residual stenosis≤30%and the absence of in-hospital major adverse events,including cardiac death,target vessel-related myocardial infarction,or target lesion revascularization.Results A total of 170 patients[mean age:(65.9±7.9)years,116 males]were enrolled.After treatment with IVL and DES,the minimum lumen diameter increased significantly compared to baseline[(2.34±0.40)mm vs.(0.95±0.33)mm,P<0.001],the degree of stenosis was significantly reduced[(13.24±6.60)%vs.(65.18±10.59)%,P<0.001].Procedural success was achieved in 100%of cases,and device success was 98.8%.The 30-day patient-related cardiovascular clinical composite endpoint(POCE)rate was 0.0,with no target lesion failure,no confirmed or potential thrombotic events were observed.The shockwave energy generator demonstrated excellent stability and ease of use.Among the 33 patients assessed with OCT,after IVL intervention,the maximum calcified area of the lumen[(3.51±1.51)mm2 vs.(2.85±1.80)mm2,P<0.001],and the minimum lumen area within the target lesion[(3.08±1.04)mm2 vs.(2.02±0.75)mm2,P<0.001],and after DES intervention,the luminal area of the largest calcified site[(6.59±1.64)mm2 vs.(2.85±1.80)mm2,P<0.001]and the minimum luminal area within the target lesion[(6.19±1.45)mm2 vs.(2.02±0.75)mm2,P<0.001]were significantly increased,and the differences were statistically significant.Conclusions The Vesscrack shockwave balloon is effective and safe for vascular preparation in patients with severe CAC prior to stent implantation.It achieves significant calcified plaque modification,high procedural success rates,and minimal complications.
4.Interventional blockade of the internal mammary artery for the treatment of coronary artery disease:anatomy,clinical studies,and future perspectives

In the mid-20th century,internal mammary artery(IMA)ligation attracted attention for improving myocardial ischemia survival and relieving angina symptoms in animals,but was eliminated after sham-operation trials confirmed that its efficacy stemmed from a placebo effect.Recent advances in interventional techniques have led to the revival of distal IMA blockade,which has been shown to improve myocardial perfusion by modulating collateral circulation.Randomized controlled trials further showed that angina relief was significantly better in the blockade group than in the sham-operated group,especially in right coronary artery(RCA)and left anterior descending(LAD)lesions,but not in the ileocecal branch due to the lack of IMA collateral connections.Anatomy suggests that the internal mammary artery forms a natural collateral pathway to the RCA/LAD via the pericardial phrenic artery,which is the anatomic basis for its efficacy.Current technical limitations include incomplete occlusion,device-related complications,and insufficient evidence of long-term prognosis.Future studies are needed to optimize the design of occlusion devices,explore combined therapeutic strategies,and conduct controlled studies with conventional revascularization techniques.In this review,we systematically review the 100-year evolution of IMA occlusion,the paradigm shift from surgical ligation to precision intervention,analyze its anatomical and hemodynamic mechanisms,integrate evidence-based evidence and propose screening criteria for indications,and provide a new pathway for individualized treatment of high-risk patients who cannot tolerate coronary artery bypass grafting/percutaneous coronary intervention.
5.Effects of robust optimization parameters on radiation dose in proton radiotherapy for localized prostate cancer

The effects of different robust optimization parameters on the doses to organs-at-risk(OAR)and the clinical target volume(CTV)in proton therapy plans for localized prostate cancer are explored for identifying the optimal robust optimization parameters.A retrospective analysis is conducted on 10 cases in which proton plans with a total dose of 76 Gy delivered in 38 fractions are designed.In robust optimization,uncertainties of 3.5%in range and setup errors of 3,5 and 7 mm are considered.After being grouped by setup errors,3 groups of plans are obtained.The effects of setup errors on the doses to CTV and OAR are analyzed,and the robustness of the CTV dose is assessed,including the worst-case values of dosimetric parameters and the passing rates under different scenarios.The results show that as the setup error increased,the doses to OAR tended to rise.Compared with the 3 mm plan group,the 5 mm and 7 mm plan groups experience increases of 1.99%and 5.15%in rectal V70,3.71%and 10.01%in rectal V45,0.93%and 2.55%in bladder V70,and 1.71%and 5.27%in bladder V45,respectively;similar patterns are observed for the doses to sigmoid colon and bulbous urethra,and the differences are statistically significant(P<0.05).In robustness analysis,the CTV D99 in the 5 mm and 7 mm plan groups increases by 0.68 Gy and 0.95 Gy as compared with the 3 mm plan group,with passing rates improving by 7.2%and 9.6%,respectively(passing criterion:D95 receives at least 100%of the prescribed dose),with significant differences(P<0.05).Considering both OAR dose and CTV robustness,the setup error of 5 mm is found to be a reasonable choice for robust optimization in proton therapy plans for localized prostate cancer,as it can effectively balance the enhancement of CTV dose robustness with the control of dose escalation to OAR.
6.Dynamic changes and influencing factors of Mycobacterium tuberculosis infection among senior high school students

Objective To analyze the dynamic changes and influencing factors of Mycobacterium tuberculosis(TB)infection in grade-1 students in senior high school at the time of enrollment and one year after enrollment.Methods In 2022,TB screening was carried out among grade-2 students in 4 senior high schools in Binhai and Xiqing Districts of Tianjin.Basic information of students was collected,and TB infection detection results of grade-1 students in senior high school at the time of enrollment were collected retrospectively.Positive rate,strong positive rate and positive conversion rate from tuberculin skin test(TST)results of students were compared.Univariate and multiva-riate logistic regression models were used to analyze the relevant influencing factors for positive conversion of TST results.Results The overall positive rate of 1 839 students one year after enrollment was higher than that at the enrollment(46.82%vs 33.12%),while strong positive rate was lower(12.51%vs 13.00%),both with statisti-cally significant differences(both P<0.001).One year after enrollment,64 out of 1 230 students who were origi-nally negative for TST in their first year of senior high school turned positive,with a positive conversion rate of 5.20%(95%CI:3.93%-6.48%).One year after enrollment,TST results showed that the average diameter of induration was 5(2,8)mm,diameter was 2(0,5)mm at the enrollment of the first year of senior high school,but the difference was not statistically significant(P=0.478).The difference range of average diameter of induration was±21 mm,39.70%of students being within±2 mm.Students with increased and decreased average diameter of induration accounted for 65.09%and 28.49%,respectively.Multivariate logistic regression analysis result showed that students in boarding school had a higher risk of positive conversion compared with non-boarding school students(OR=4.842,95%CI:2.794-8.392).Conclusion The screening of TB infection among grade-2 students in senior high school should be strengthened,with a focus on boarding school students.Early detection of tuberculosis patients and newly infected individuals can provide reference for implementing precise prevention and control.
7.Progress in the application of non-intubated video-assisted thoracoscopic surgery for non-small cell lung cancer

With advances in minimally invasive techniques,non-intubated video-assisted thoracoscopic surgery(NIVATS)is increasingly applied in the treatment of early-stage non-small cell lung cancer(NSCLC).NIVATS preserves the patient's spontaneous breathing,avoiding laryngeal injury,airway edema,and pulmonary complications associated with mechanical ventilation and double-lumen endotracheal intubation,demonstrating significant advantages in postoperative recovery.Studies indicate that NIVATS is particularly suitable for NSCLC patients with good lung function and smaller tumors.However,the application of NIVATS in complex cases remains limited,such as in cases requiring extensive lymph node dissection,tumors close to major blood vessels,and severe pleural adhesions,where patients may need to switch to intubated anesthesia mid-procedure.Additionally,in obese patients or those with severe cardiopulmonary disease,the safety and efficacy of NIVATS are reduced due to inadequate respiratory function.
8.Analysis on regional disparities and dynamic evolution of primary healthcare resource allocation in China under high-quality development:Based on Dagum Gini Coefficient Decomposition and Kernel Density Estimation

Objective:To explore the regional equity status and dynamic evolution patterns of primary healthcare resource allocation in China from 2009 to 2023.Methods:Based on statistical data of China's primary healthcare resources,the Dagum Gini coefficient decomposition method was used to analyze regional equity differences in primary healthcare resource allocation in China from 2009 to 2023,and Kernel density estimation was employed to investigate the spatial distribution and dynamic evolution process of resource allocation.Results:From 2009 to 2023,the total volume of primary healthcare resources in China continued to grow,with institutions,beds,and personnel increasing by 15.20%,65.53%,and 101.83%respectively,but significant regional disparities persist.Different types of resource allocation showed divergent equity trends:the Gini coefficient for personnel allocation decreased from 0.14 to 0.07,while the Gini coefficient for bed allocation increased from 0.15 to 0.22,and institutional allocation remained relatively stable.Kernel density estimation revealed that personnel allocation transformed from a bimodal distribution to a concentrated unimodal distribution,while bed allocation became more dispersed.Conclusion:China's primary healthcare resource allocation exhibits a divergent phenomenon with concurrent improvement in personnel allocation equity and deterioration in bed allocation equity.It is necessary to establish a differentiated allocation system with targeted policies,implement regional coordinated development strategies,improve monitoring and evaluation mechanisms,and strengthen technological support.
9.Correlation between the expression level of miRNA-4319 and miRNA-4262 in early gastric carcinoma tissue and recurrence after endoscopic submucosal dissection

Objective To investigate the correlation between the expression of microRNA-4319(miRNA-4319)and microRNA-4262(miRNA-4262)in early gastric carcinoma tissue and postoperative recurrence after endoscopic submucosal dissection(ESD).Methods From May 2018 to March 2020,396 patients with early gastric carcinoma who underwent ESD were regarded as the disease group,meantime,98 patients with normal gastric mucosa gastritis confirmed by pathological examination were as the control group.Quantitative reverse transcription polymerase chain reaction(qRT-PCR)method was applied to detect the relative expression levels of miRNA-4319 and miRNA-4262 in tissues;follow up on postoperative recurrence of early gastric carcinoma patients,and the expression levels and clinical pathological characteristics of miRNA-4319 and miRNA-4262 were compared between recurrent and non recurrent patients.Multivariate Logistic regression analysis was applied to analyze the influencing factors of recurrence in early gastric carcinoma patients after ESD;receiver operating characteristic curve(ROC curve)was applied to analyze the predictive value of miRNA-4319 and miRNA-4262 in early gastric carcinoma tissue for recurrence after ESD.Results The expression levels of miRNA-4319 and miRNA-4262 in the disease group were lower than those in the control group,the differences were statistically significant(P<0.05);The expression levels of miRNA-4319 and miRNA-4262 in the recurrence group were decreased,and the proportion of submucosal invasion was increased,the differences were statistically significant(P<0.05).The depth of invasion to the submucosal level(OR=3.158,95%CI:1.395~7.151)was a risk factor for recurrence after ESD in patients with early gastric carcinoma(P<0.05),and miRNA-4262 ≥ 0.76(OR=0.561,95%CI:0.370~0.852)and miRNA-4319 ≥ 0.69(OR=0.482,95%CI:0.255~0.911)were protective factors for recurrence after ESD in patients with early gastric carcinoma(P<0.05);The area under the curve(AUC)of miRNA-4319,miRNA-4262 and their combination for predicting recurrence after ESD in patients with early gastric carcinoma was 0.889(95%CI:0.854~0.918),0.914(95%CI:0.882~0.940)and 0.964(95%CI:0.940~0.980),respectively,which was superior to the individual predictions of miRNA-4319 and miRNA-4262 respectively(P<0.05).Conclusion MiRNA-4319 and miRNA-4262 are low expressed in tissues of early gastric carcinoma patients,they are closely related to postoperative recurrence after ESD.
10.Full genome analysis of G4P23porcine rotavirus and its pathogenicity in suckling mice and piglets

To perform the phylogenetic characterization of an isolated porcine rotavirus(PoRV)and investigate its pathogenicity in suckling mice and piglets.A G4P[23]genotype PoRV strain JSJR2023 was successfully isolated from the diarrheic piglet feces through propagation in MA104 cells.The viral proliferation kinetics were analyzed using TCID50 assays,followed by complete genome sequencing through Sanger sequencing platforms.Comprehensive genotyping and phylogenetic reconstruction were conducted using MEGA7.0 with maximum likelihood algorithms.Pathogenicity was assessed in the following animal models:5-day-old C57BL/6 mice and 3-day-old piglets.Multidimensional evaluation included clinical monitoring(diarrhea scoring,growth parameters),virological detection,and histopathological analysis of intestinal tissues.The virus strain JSJR2023 could replicate efficiently in MA104 cells,achieving peak titers of 107.5 TCID50/mL.Whole genome genotype analysis showed that the strain belonged to G4-P[23]-I5-R1-C1-M1-A8-N1-T1-E1-H1.Phylogenetic analysis indicated that the VP3 and NSP4 genes of JSJR2023 strain were most closedrelated to human species rotaviruses,suggesting genetic reassortment between human and porcine RV strains.The animal experiments in suckling mice showed that the JSJR2023 strain infection caused diarrhea symptoms,intestinal edema and congestion,and shedding of intestinal villus epithelial cells.The pathogenicity experiments in piglets showed that compared with the control group,the challenged group of pig-lets had severe diarrhea symptoms,accompanied by reduced appetite and listlessness.Post-mortem examination revealed that the intes-tines were significantly thinner,congested,and filled with yellow watery contents.The challenged piglets showed typical pathological changes such as thinning of the intestinal wall and shortening and shedding of intestinal villi.In conclusion,this study successfully iso-lated a human-porcine recombinant G4P[23]PoRV strain and established the infection models in suckling mice and piglets,providing important tools for investigating the pathogenic mechanism of PoRV,evaluating vaccines and developing antiviral drug.
