1.Evaluation value of urinary 8-oxo-7, 8-dihydroguanosine in the short-term prognosis of sepsis in frail elderly patients

Objective:To investigate the significance of urinary 8-oxo-7, 8-dihydroguanosine(8-oxoGuo)in assessing the short-term prognosis of sepsis in frail elderly patients.Methods:We conducted a cross-sectional study involving 62 frail elderly patients diagnosed with sepsis who were admitted to the Emergency Intensive Care Unit(EICU)at Beijing Hospital between March 2021 and March 2022.Based on their 28-day prognosis, the patients were categorized into two groups: those who died and those who survived.Upon admission, we collected urine samples and clinical data from both groups.We employed isotope dilution high-performance liquid chromatography-mass spectrometry to measure the levels of the RNA oxidation marker 8-oxoGuo in the urine.Results:A total of 62 frail elderly patients[aged(85.1±6.3)years]diagnosed with sepsis were included in the study, comprising 36 patients in the 28-day mortality group and 26 patients in the survival group.Univariate analysis revealed that the survival group had significantly lower body temperature, blood calcitonin(PCT)levels, sequential organ failure assessment(SOFA)scores, and urinary 8-oxoGuo levels compared to the mortality group.Additionally, the survival group exhibited a higher mean arterial pressure(MAP)than the mortality group, with all differences reaching statistical significance(all P<0.05).Spearman correlation analysis indicated that urinary 8-oxoGuo levels were positively correlated with both PCT and SOFA scores in frail elderly sepsis patients( r=0.426, 0.768, both P<0.05).Furthermore, logistic regression analysis identified urinary 8-oxoGuo and SOFA as independent risk factors for 28-day mortality in this population( OR=1.936, 1.427; P=0.006, 0.002).The area under the receiver operating characteristic curve(AUC)for urinary 8-oxoGuo and SOFA in predicting the 28-day prognosis of frail elderly sepsis patients was 0.761 and 0.741, respectively, both demonstrating statistical significance(both P<0.001). Conclusions:Our findings suggest that urinary 8-oxoGuo possesses strong predictive value for the short-term prognosis of sepsis in this vulnerable population.
2.Optimization of Extraction Process of Shanzhiside Methyl Ester from Mussaenda pubescens by Response Surface Methodology and its Antioxidant Activity

Objective To optimized the ultrasonic-assisted extraction process of methyl geniposide from Mussaenda pu-bescens and investigated its antioxidant activity in vitro.Methods The yield of shanzhiside methyl ester from Mussaenda pu-bescens was used as an indicator.The optimal extraction process of shanzhiside methyl ester from Mussaenda pubescens w as ex-plored using single factor experiments and response surface methodology.The factors included methanol concentration,extrac-tion time,extraction temperature,and liquid-solid ratio.The in vitro antioxidant activity of Shanzhiside methyl ester from Mus-saenda pubescens was evaluated by the scavenging rate of DPPH free radical,hydroxyl radical,and superoxide anion free radi-cal.Results The optimal process for extracting glycosides from Mussaenda pubescens using ultrasound assisted method is meth-anol with a mass concentration of 55%,53℃extraction temperature,32∶1 liquid to material ratio,and extraction time of 98 mi-nutes.The yield of methyl ester of shanzhiside was(9.516±1.66)mL/g,and the yield of mountain hawthorn glycoside by re-gression model was 9.532 mg/g,with a relative error of less than 0.2%.After 5.5 hours of antioxidant testing reaction,4 μmol/L of geniposide methyl ester showed significant effects on DPPH·and the clearance rates of ABTS·were 95.18%and 98.75%,respectively,and the total antioxidant capacity of FRAP was 1.054 mmol/L Vc equivalent.Conclusion Response sur-face methodology can optimize the extraction process of shanzhiside methyl ester from Mussaenda pubescens,and the obtained Shanzhiside methyl ester exhibits excellent antioxidant activity in vitro.

6.Level of inflammatory factors and correlation with prognosis in chronic cervicitis patients with HPV infection

Objective To explore the level of inflammatory factors and their correlation with prognosis in chronic cervicitis patients with human papillomavirus(HPV)infection.Methods Totally 100 patients from January 2022 to December 2023 were selected as the observation group.Among them,67 patients were 100 patients with low-risk HPV and 33 patients with high-risk HPV.At the same time,100 female personnel with physical examination in our hospital were selected as the control group.All of the patients were treated routinely.The C-reactive protein(CRP),interleukin(IL)-6,and serum amyloid A(SAA)levels were measured,and the patients were followed up for 6 months after treatment to observe the prognosis.Results Compared with the control group,The expression of CRP,IL-6 and SAA level increased in the observation group(P<0.05),Compared with the low-risk HPV group,The expression of CRP,IL-6 and SAA level increased in high-risk HPV group(P<0.05).Compared with patients with a good prognosis,Higher expression of CRP,IL-6 and SAA in patients with poor prognosis(P<0.05).The positive correlation between CRP,IL-6,SAA and low-risk HPV and high-risk HPV,Statistical difference(P<0.05).Negative correlation between CRP,IL-6,SAA and good prognosis,All showed a positive correlation between CRP,IL-6,and SAA and poor prognosis,There were statistical differences(P<0.05).Receiver operating characteristic(ROC)curve analysis showed that the diagnostic value of CRP,IL-6 and SAA(P<0.05).Conclusion Patients with chronic cervicitis have increased CRP,IL-6 and SAA in HPV infection,and the abnormal increased or decreased expression level is the risk predictor of chronic cervicitis with HPV infection,which may be closely related to the diagnosis of the disease,pathological characteristics,disease development and prognosis.

Objective: To elucidate the anti-inflammatory mechanism of Reduning Injection (RDN) by analyzing the potential biomarkers and metabolic pathways of the carrageenan-induced inflammatory model from the overall metabolic level. Methods: Rat inflammatory model was established by carrageenan. UPLC-Q-TOF/MS was used to detect and analyze changes of endogenous metabolites in the serum and urine of carrageenan-induced inflammatory rats. Combined with multivariate analysis and databases analysis, inflammatory-related potential biomarkers were screened and identified to analyze possible metabolic pathways. The reliability and biological significance of these biomarkers was verified by metabolic network analysis and correlation analysis with pharmacodynamic indicators. Results: A total of 16 potential biomarkers were screened and identified by multivariate analysis and metabolite databases, among which 13 species could be adjusted by RDN. The metabolism pathway analysis revealed that histidine metabolism, sphingolipid metabolism, and tyrosine metabolism were greatly disturbed. Their biomarkers involved urocanic acid, sphingosine, and norepinephrine, all of which showed a callback trend after RDN treatment.
