Objective To analyze the clinical characteristics of recipients with interstitial lung disease (ILD) who underwent second lung transplantation and summarize the diagnostic and therapeutic experience. Methods A retrospective analysis was conducted on the clinical data of 14 patients who underwent first and second lung transplants for ILD at the First Affiliated Hospital of Guangzhou Medical University from January 2015 to December 2024. The preoperative conditions, intraoperative events, postoperative treatments and prognoses of the first and second lung transplantation were compared, and the postoperative survival of ILD patients after the second lung transplantation was analyzed. Results Among the 14 recipients of the second lung transplant, 13 underwent the procedure due to chronic lung allograft dysfunction, and 1 due to airway complications. The median interval time from the first to the second lung transplant was 32 (2, 80) months. Before the second transplantation, 2 recipients required endotracheal intubation and mechanical ventilation, and 2 required endotracheal intubation, mechanical ventilation, and extracorporeal membrane oxygenation (ECMO) support. The surgical time for the second lung transplantation was longer than that for the first, with increased intraoperative red blood cell and plasma transfusion volumes, the proportion of ECMO support during the second lung transplantation was higher than that during the first (all P<0.05). However, the cold ischemia time for one-sided lung transplant completion in the first lung transplant was similar to that in the second lung transplantation (P>0.05). The median follow-up time after the second lung transplantation was 32 (1, 63) months. The 1-month, 6-month and 1-year survival rates after the second lung transplantation were 79%, 57% and 50%, respectively, with causes of death being infection, multiple organ failure and gastrointestinal bleeding. Conclusions For ILD patients undergoing second lung transplantation after the first lung transplantation, the second lung transplantation is more challenging, with longer surgical time and higher intraoperative blood loss. It requires higher surgical skills and perioperative management. Non-emergency second transplantation may still achieve good results.

Objective:To investigate the influences and mechanism of extracellular vesicles from dermal papilla cells (DPC-EVs) of mice on human hypertrophic scar fibroblasts (HSFs).Methods:The study was an experimental research. The primary dermal papilla cells (DPCs) of whiskers were extracted from 10 6-week-old male C57BL/6J mice and identified successfully. The DPC-EVs were extracted from the 3 rd to 5 th passage DPCs by ultracentrifugation, and the morphology was observed through transmission electron microscope and the particle diameter was detected by nanoparticle tracking analyzer ( n=3) at 24 h after culture. The 3 rd passage of HSFs were divided into DPC-EV group and phosphate buffer solution (PBS) group, which were cultured with DPC-EVs and PBS, respectively. The cell scratch test was performed and cell migration rate at 24 h after scratching was calculated ( n=5). The cell proliferation levels at 0 (after 12 h of starvation treatment and before adding DPC-EVs or PBS), 24, 48, 72, and 96 h after culture were detected by using cell counting kit 8 ( n=4). The protein expressions of α-smooth muscle actin (α-SMA) and collagen typeⅠ (ColⅠ) in cells at 24 h after culture were detected by immunofluorescence method and Western blotting, and the protein expression of Krüppel-like factor 4 (KLF4) in cells at 24 h after culture was detected by Western blotting. After the 3 rd passage of HSFs were cultured with DPC-EVs for 24 h, the cells were divided into blank control group, KLF4 knockdown group, and KLF4 overexpression group according to the random number table. The cells in blank control group were only routinely cultured for 48 h. The cells in KLF4 knockdown group and KLF4 overexpression group were incubated with KLF4 knockdown virus for 24 h, then the cells in KLF4 knockdown group were routinely cultured for 24 h while the cells in KLF4 overexpression group were incubated with KLF4 overexpression virus for 24 h. The protein expressions of KLF4, α-SMA, and ColⅠ in cells were detected by Western blotting at 48 h after culture. Results:At 24 h after culture, the extracted DPC-EVs showed vesicular structure with an average particle diameter of 108.8 nm. At 24 h after scratching, the migration rate of HSFs in PBS group was (54±10)%, which was significantly higher than (29±8)% in DPC-EV group ( t=4.37, P<0.05). At 48, 72, and 96 h after culture, the proliferation levels of HSFs in DPC-EV group were significantly lower than those in PBS group (with t values of 4.06, 5.76, and 6.41, respectively, P<0.05). At 24 h after culture, the protein expressions of α-SMA and ColⅠ of HSFs in DPC-EV group were significantly lower than those in PBS group, while the protein expression of KLF4 was significantly higher than that in PBS group. At 48 h after culture, compared with those in blank control group, the protein expression of KLF4 of HSFs in KLF4 knockdown group was down-regulated, while the protein expressions of α-SMA and ColⅠ were both up-regulated; compared with those in KLF4 knockdown group, the protein expression of KLF4 of HSFs in KLF4 overexpression group was up-regulated, while the protein expressions of ColⅠ and α-SMA were down-regulated. Conclusions:The DPC-EVs of mice can inhibit the proliferation and migration of human HSFs and significantly inhibit the expressions of fibrosis markers α-SMA and ColⅠ in human HSFs by activating KLF4.

ObjectiveTo investigate the effect and mechanism of Yiqi Wenyang Huwei decoction (YWHD) on airway inflammation in bronchial asthma (BA) rats based on transforming growth factor-β1 (TGF-β1)/SMAD family member 3 (Smad3) signaling pathway. MethodSixty male SD rats were randomly divided into a normal group, a model group, a dexamethasone (DEX) group, and low-, medium-, and high-dose YWHD groups, with 10 rats in each group. The BA model was induced by intraperitoneal injection of 1 mL of ovalbumin (OVA)-aluminum hydroxide suspension for sensitization, followed by nebulization with 2% OVA. One hour before daily OVA nebulization, the control group was treated with saline, the DEX group with DEX solution at 0.2 g·L-1, and the low-, medium-, and high-dose YWHD groups with YWHD at 1, 2, 4 g·mL-1, respectively. General conditions and lung function were observed. Bronchoalveolar lavage fluid (BALF) and serum were collected to count inflammatory cells in BALF and measure immunoglobulin E (IgE) levels in serum and inflammatory cytokines in BALF using enzyme-linked immunosorbent assay (ELISA). Pathological changes in lung tissues, collagen deposition, and airway mucus secretion were observed by hematoxylin-eosin (HE), Masson, and periodic acid-Schiff (PAS) staining. TGF-β1/Smad3-related mRNA and protein levels in lung tissues were determined by Real-time fluorescent quantitative polymerase chain translation (Real-time PCR) and Western blot analysis. ResultCompared with the normal group, the model group showed increased total airway resistance (RL) and decreased dynamic compliance (Cdyn) (P<0.05, P<0.01), elevated serum IgE levels, increased inflammatory cell counts, and higher inflammatory cytokine levels in BALF (P<0.01). Additionally, there was significant inflammatory cell infiltration, collagen deposition, and mucus secretion in lung tissues. The levels of TGF-β1, α-smooth muscle actin (α-SMA), and Smad3 phosphorylation in lung tissues were significantly increased (P<0.01). Compared with the model group, the DEX group and high-dose YWHD group exhibited significantly reduced RL (P<0.01), improved lung dynamic compliance (P<0.05), and lower serum IgE levels, inflammatory cell counts, and inflammatory cytokine levels in BALF (P<0.05). Moreover, these treatments alleviated pathological damage in lung tissues and reduced the levels of TGF-β1, α-SMA, and Smad3 phosphorylation (P<0.01). ConclusionYWHD reduces airway inflammation, improves pathological damage, and mitigates airway remodeling in bronchial asthma rats, possibly by downregulating TGF-β1, α-SMA protein levels, and Smad3 phosphorylation.

BACKGROUND:Osteonecrosis of the femoral head is one of the refractory diseases in orthopedic diseases. The natural collapse course of osteonecrosis of the femoral head under different stages and types affects the progression and prognosis of the disease. OBJECTIVE:To explore the progression of natural collapse within 5 years in patients under the different classifications of China-Japan Friendship Hospital(CJFH)with stage Ⅱ osteonecrosis of the International Association for Research Circulation Osseous(ARCO),and to analyze the collapse rate and collapse risk of the femoral head under the different classifications of CJFH. METHODS:A retrospective study was performed to select patients diagnosed with ARCO Ⅱ stage osteonecrosis of the femoral head without collapse in the Honghui Hospital Affiliated to Xi'an Jiaotong University from October 2016 to October 2017.According to whether it collapsed, the number of hips was divided into the collapse group(n=82)and the non-collapsed group(n=70).The collapse risk of patients with osteonecrosis of the femoral head under different CJFH classifications,as well as the collapse time, number of collapses, and collapse rate within 5 years were counted, and then the Kaplan-Meier survival curve of the femoral head under different classification of CJFH was plotted. RESULTS AND CONCLUSION:(1)A total of 97 patients with 152 hips were enrolled, and 82 hips collapsed during the follow-up period, with a total collapse rate of 53.9%,of which the collapse rates of M type,C type,L1 type,L2 type, and L3 type were 0.0%,36.7%,51.4%,72.2%,and 77.8%,respectively,and the comparison between the groups was statistically significant(P<0.05).(2)In terms of collapse risk, the collapse risk of L1 type was 1.704 times that of C-type(P>0.05),while the collapse risks of L2 type and L3 type were 3.866 times and 6.423 times that of C type(P<0.05),respectively.(3)In terms of the Kaplan-Meier survival curve, the median survival time of the femoral head of ARCO Ⅱ stage patients was 3 years, with a 95%confidence interval of 2.885-3.471 years, and the survival rates of the femoral head at the first, third and fifth years were 65.1%(99/152),50.7%(77/152),and 46.1%(70/152),respectively.(4)These findings conclude that different CJFH classifications affect the collapse rate of ARCO Ⅱ stage osteonecrosis of the femoral head patients, among which L3 type patients have the highest collapse rate, followed by L2 type and L1 type patients;C type patients have a lower collapse rate, and M type patients do not collapse, which indicates that the preservation of the lateral column of the femoral head is of great significance for the natural collapse course of osteonecrosis of the femoral head.
