1.Effects and mechanisms of glycocholic acid on the radiosensitivity of mice with lung adenocarcinoma transplantation tumors

[摘 要] 目的:探究甘氨胆酸(GCA)对肺腺癌A549细胞移植瘤小鼠放射治疗敏感性的影响及其机制。方法:建立A549人肺腺癌细胞裸鼠移植瘤模型,随机分为移植瘤对照组(对照组)、GCA组、放疗组(RT组)和GCA + 放疗组(GCA + RT组)。RT组和GCA + RT组接受单次10 Gy照射,GCA组及GCA + RT组连续7 d每日灌胃GCA 280 mg/kg。间隔2 d测量1次移植瘤体积,末次给药后处死小鼠并取移植瘤组织,检测移植瘤组织中超氧化物歧化酶(SOD)与谷胱甘肽过氧化物酶(GSH-Px)活性,qPCR法和WB法分别检测放疗关键基因(MCM6、ITGA6、CASP3等)mRNA和蛋白表达水平,H-E染色观察移植瘤组织的形态变化。通过GEO(GSE276500、GSE294906、GSE218171)及TCGA数据库数据验证放疗关键基因。结果:GCA单用对瘤体生长有一定抑制作用,但联合放疗的GCA + RT组相比单纯放疗组表现出放疗抵抗的效应(P < 0.05)。GCA处理显著提高移植瘤组织SOD活性(P < 0.01)、降低GSH-Px活性(P < 0.01),提示GCA可改变移植瘤抗氧化酶平衡,减弱放疗诱导的氧化应激。GCA干预上调移植瘤组织中MCM6与ITGA6 mRNA表达、下调CASP3 mRNA表达(均P < 0.05)。GCA + RT组移植瘤组织中的MCM6蛋白表达显著高于对照组(P < 0.05)。H-E染色显示,GCA组部分瘤组织坏死,而GCA + RT组坏死组织面积较RT组有所缩小。GEO和TCGA数据库验证支持MCM6、ITGA6高表达与放疗抵抗和预后不良相关。结论:GCA通过增强SOD活性、降低GSH-Px活性并上调ITGA6、MCM6的表达改变氧化应激与关键信号网络,从而削弱A549移植瘤对放疗的敏感性。

To thoroughly implement the strategic deployment outlined in the Opinions of the Central Committee of the Communist Party of China and the State Council on Promoting the Inheritance and Innovative Development of Traditional Chinese Medicine regarding research on dominant diseases of traditional Chinese medicine and to uphold the development philosophy of equal emphasis on traditional Chinese medicine and western medicine,the China Association of Chinese Medicine has fully played a leading academic role by systematically organizing and conducting a series of academic youth salons on clinical dominant diseases of traditional Chinese medicine. On September 13,2024,the 36th Youth Salon on Clinical Dominant Diseases was successfully held in Nanjing,focusing on the advantages of traditional Chinese medicine and the integrative traditional Chinese medicine and western medicine in the diagnosis and treatment of erectile dysfunction (ED). The conference brought together leading experts from traditional Chinese medicine,western medicine,and interdisciplinary fields,facilitating in-depth multidisciplinary discussions that led to key consensus on optimizing traditional Chinese medicine treatment protocols for ED,researching and developing new drugs of traditional Chinese medicine,and advancing interdisciplinary development in traditional Chinese medicine. This salon systematically sorted out the clinical strengths and distinctive features of traditional Chinese medicine in the diagnosis and treatment of ED. Based on current research foundations and clinical needs,it identified key directions for future scientific layout and scientific research tackling: (1) Standardization of syndrome differentiation system of traditional Chinese medicine for ED. (2) Optimization and standardization of intervention methods of integrated traditional Chinese medicine and western medicine. (3) High-quality clinical research guided by evidence-based medicine. (4) In-depth analysis of the pharmacological mechanisms of traditional Chinese medicine in the treatment of ED. (5) Clinical translation and application promotion of new drugs of traditional Chinese medicine. (6) Interdisciplinary integration and innovation in traditional Chinese medicine. For each research direction,key focus areas,expected objectives,and clinical value were further refined,along with the establishment of a scientifically sound priority funding level evaluation system. Therefore,building on the series of salons on the ED-focused dominant diseases of traditional Chinese medicine,this paper provides standardized guidance for clinical practice of traditional Chinese medicine in ED management,effectively contributing to the high-quality development of traditional Chinese medicine. It serves as a valuable reference for national scientific and technological strategic layout, research and development decision-making in new drugs of traditional Chinese medicine,research topic planning,and clinical guideline formulation.
3.Determination of benzothiazoles and benzotriazoles in drinking water using automated solid-phase extraction combined with liquid chromatography-tandem mass spectrometry

Background Benzothiazoles and benzotriazoles are ubiquitously detected in drinking water, posing potential health risks. Developing a reliable and sensitive analytical method is critical for assessing their exposure levels and associated risks. Objective To establish an automated solid-phase extraction (SPE) coupled with liquid chromatography-tandem mass spectrometry method for the simultaneous determination of benzothiazole, 2-aminobenzothiazole, benzotriazole, and 5-chlorobenzotriazole in drinking water. Methods Key parameters were systematically optimized. Three mobile phase systems were compared to evaluate their effects on chromatographic peak shape and separation; three SPE cartridges were tested for extraction efficiency; the influences of water pH on extraction recoveries and matrix effects were investigated; the contributions of pretreatment steps to benzothiazole blank were analyzed, and control measures were established; nitrogen evaporation temperatures were tested to ensure analyte stability. The optimized parameters were used to develop the method, validate its performance, and analyze drinking water samples. Results Drinking water samples were first adjusted to pH 6.0 with formic acid, and then extracted using HLB cartridges. After elution with 6 mL methanol, the eluate was concentrated under nitrogen evaporation at 45 °C, followed by separation on a C18 column using a gradient elution of acetonitrile-0.1% formic acid solution, and tandem mass spectrometry detection. Avoiding nitrogen blow-drying during SPE and eliminating the use of polypropylene materials during nitrogen evaporation can effectively minimize benzothiazole contamination. Good calibration linearity was obtained for the target analytes in the concentration range of 5.0-250 μg·L−1, with a correlation coefficient of r > 0.995. The method detection limits were in the range of 1.0-5.0 ng·L−1. The recoveries of the target analytes in pure water and tap water were 80.2%-119.5% and 72.2%-115.6%, respectively, with relative standard deviations of were 3.2%-12.8% and 2.3%-11.6%, respectively. When applying this method to actual water samples, benzotriazole was detected in 100% of treated and tap water samples, with median concentrations of 79.4 ng·L−1 and 114 ng·L−1, respectively. Benzothiazole was detected in 83.3% of treated water samples and 100% of tap water samples, with median concentrations of 48.3 ng·L−1 and 65.4 ng·L−1, respectively. In addition, 5-chloro-benzotriazole exhibited low detection rates and concentrations, while 2-amino-benzothiazole was undetected. Conclusion The developed method demonstrates high accuracy, reliability, and sensitivity, making it suitable for the analysis of trace levels of benzothiazoles and benzotriazoles in drinking water.
4.Survival impact of radiotherapy for patients with de novo metastatic rectal cancer

Purpose:
Metastatic rectal cancer (mRC) is a highly lethal and complex disease that demands a multidisciplinary treatment approach. However, the clinical effectiveness of radiotherapy (RT) for de novo mRC remains controversial and uncertain.
Methods:
This retrospective cohort study examined medical records from Kaohsiung Chang Gung Memorial Hospital for patients with histologically confirmed de novo mRC diagnosed between January 2015 and December 2020. All patients received standard systemic therapy and radical surgery when feasible. The primary outcome, overall survival (OS), was assessed using the Kaplan-Meier method. Multivariable analysis was performed using a Cox regression model.
Results:
Among 271 patients included in the analysis, 117 received RT and 154 did not. The median OS was significantly longer in the RT group compared with the non-RT group (27.8 months vs. 21.9 months; P=0.046). Multivariate analysis identified several independent predictors of OS: age ≥65 years (hazard ratio [HR], 1.69; 95% confidence interval [CI], 1.26–2.27; P=0.001), primary tumor resection (HR, 2.62; 95% CI, 1.90–3.61; P<0.001), M1b or M1c disease (HR, 1.97; 95% CI, 1.44–2.69; P<0.001), and receipt of RT (HR, 1.41; 95% CI, 1.02–1.94; P=0.036).
Conclusion
RT significantly improves OS in patients with mRC, underscoring its role in treatment strategies. These findings support its inclusion in therapeutic protocols and highlight the need for larger, multicenter trials to confirm and extend these results.
5.Treatment of 149 cases of large-volume benign prostatic hyperplasia with blue laser vaporization

Objective To explore the efficacy and safety of blue laser vaporization in the treatment of large-volume (>80 mL) benign prostatic hyperplasia (BPH). Methods A retrospective analysis was conducted on the clinical data of 149 BPH patients with prostate volume>80 mL who underwent blue laser vaporization at our hospital during Aug. 2023 and Aug. 2025. The 1-month postoperative maximum urinary flow rate (Qmax) and international prostate symptom score (IPSS), operation time, postoperative hemoglobin drop, bladder irrigation volume, catheter indwelling time, and incidence of postoperative complications were recorded and analyzed. Results All operations were successfully completed, without conversion to other approaches. The operation time was (41.47±11.05) minutes, postoperative hemoglobin drop (2.78±1.19) g/L, bladder irrigation volume (9.97±1.49) bags, and catheter indwelling time (3.30±1.85) days. One month after surgery, the Qmax was significantly higher than the preoperative value [(20.19±3.39) mL/s vs. (2.39±2.17) mL/s], while the IPSS was significantly lower [(4.46±0.87) vs. (27.69±2.06)], with statistically significant differences (P<0.001). Postoperative complications included 1 case of incontinence (0.67%), 2 cases of urethral stricture/bladder neck contracture (1.34%), and 1 case of secondary bleeding (0.67%), all of which improved with symptomatic treatment. Conclusion Blue laser vaporization is effective for treating large-volume BPH, with advantages such as minimal bleeding risk, short operation time, fast recovery and safety.

Methods:
Patients with hepatic steatosis were enrolled in this prospective study and underwent UGAP measurements. MRI-PDFF values of ≥5%, ≥15%, and ≥25% were used as references for the diagnosis of steatosis grades ≥S1, ≥S2, and S3, respectively. Spearman correlation coefficients and area under the receiver operating characteristic curves (AUCs) were calculated.
Results:
Between July 2023 and June 2024, the study included 88 patients (median age, 40 years; interquartile range [IQR], 36 to 46 years), of whom 54.5% (48/88) were men and 45.5% (40/88) were women. Steatosis grades exhibited the following distribution: 22.7% (20/88) had S0, 50.0% (44/88) had S1, 21.6% (19/88) had S2, and 5.7% (5/88) had S3. The success rate for UGAP measurements was 100%. The median UGAP value was 0.74 dB/cm/MHz (IQR, 0.65 to 0.82 dB/ cm/MHz), and UGAP values were positively correlated with MRI-PDFF (r=0.77, P<0.001). The AUCs of UGAP for the diagnoses of ≥S1, ≥S2, and S3 steatosis were 0.91, 0.90, and 0.88, respectively. In the subgroup analysis, 98.4% (60/61) of patients had valid controlled attenuation parameter (CAP) values. UGAP measurements were positively correlated with CAP values (r=0.65, P<0.001).
Conclusion
Using MRI-PDFF as the reference standard, UGAP demonstrates good diagnostic performance in the detection and grading of hepatic steatosis in patients with MASLD.

9.Effect of rs28362491 polymorphism in NF-κB1 gene on the efficacy of atorvastatin

Objective To investigate the effect of polymorphism at the-94ins/del ATTG(rs28362491)site in the promoter region of nuclear transcription factor-κB(NF-κB)1 on the lipid-lowering and anti-inflammatory efficacy of atorvastatin in patients with coronary heart disease(CHD).Methods A total of 180 patients with CHD in Jinhua People's Hospital from June 2022 to June 2024 were selected as the research objects.The genotypes of rs28362491 locus of NF-κB1 gene were detected.The levels of blood lipids and inflammatory factors before treatment,1 month and 6 months after treatment were analyzed,and the incidence of adverse reactions during treatment were observed.Results A total of 180 CHD patients underwent genetic testing and were divided into DD genotypes 46 cases,ID genotypes 76 cases,and II genotypes 58 cases.The levels of total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),interleukin(IL)-6,IL-8,and tumor necrosis factor-α(TNF-α)in DD genotypes were higher than those in ID genotypes and Ⅱ genotypes(P<0.05).After 1month and 6 months treatment,the levels of TC,LDL-C,IL-6,IL-8,and TNF-α in three groups of patients decreased significantly compared to before treatment(P<0.05).At the same time point,the change rates of TC,LDL-C,IL-6,IL-8,and TNF-α levels in ID genotypes and Ⅱgenotypes were higher than those in DD genotypes(P<0.05).Conclusion The efficacy of atorvastatin in CHD patients is associated with the rs28362491 polymorphism of the NF-κB1 gene,with II genotypes and ID genotypes showing better efficacy than DD genotypes.
10.Retrospecitve study on clinical treatment of 214 children with high altitude pulmonary edema

Objective:To analyse the clinical characteristics of 214 cases of paediatric high altitude pulmonary edema(HAPE)and the efficacy of dexamethasone in adjunctive therapy.Methods:This retrospective study analyzed 214 pediatric cases of HAPE admitted to the Department of Paediatrics of the General Hospital of Tibetan Military between June 2015 to June 2017 and June 2019 to June 2021.Patients were divided into dexamethasone-treated group and dexamethasone-untreated group.Baseline data,clinical characteristics were collected to evaluate the treatment efficacy and drug side effects.Results:There were 107 children in each of the two groups with a median age of 8(5,11)years. The median age of the dexamethasone-treated group was 9(6,12)years and the mean age of the dexamethasone-untreated group was 7(3,10)years. The proportion of male children was 69.60%(149/214);the onset of illness was mostly concentrated within 72 hours,accounting for 97.20%(208/214)of the cases;83.18%(178/214)of the cases had symptoms of combined upper respiratory tract infection before entering the plateau. The most important clinical symptoms of the children were cough(86.92%,186/214),cyanosis(70.09%,150/214),and shortness of breath(66.36%,142/214). The proportion of auscultatory rhonchi was 83.18%(178/214),and all cases showed positive findings on chest radiography. After the dexamethasone regimen,the overall cure rate of the children was 94.39%,the average disappearance time of the symptoms and signs was(40.52±7.85)h,and the average hospital stay was(3.60±1.90)d. After treatment with the dexamethasone-free regimen,the overall cure rate was 92.52%,the mean time to disappearance of symptoms and signs was(42.10±7.62)h,and the mean length of stay in the hospital was(3.84±2.08)d. There was no significant difference in the cure rate,the disappearance time of symptoms and signs,and the average hospitalisation days between the two groups( P>0.05),but a total of 11 children in the dexamethasone-treated group experienced adverse drug reactions,and no children in the dexamethasone-untreated group experienced adverse drug reactions. Conclusion:Han Chinese male children,particularly those with upper respiratory infections,should be closely monitored for HAPE risk within three days of ascending to high altitudes. This study does not recommend the use of dexamethasone for pediatric HAPE due to the lack of therapeutic benefits and potential adverse effects.
