To thoroughly implement the strategic deployment outlined in the Opinions of the Central Committee of the Communist Party of China and the State Council on Promoting the Inheritance and Innovative Development of Traditional Chinese Medicine regarding research on dominant diseases of traditional Chinese medicine and to uphold the development philosophy of equal emphasis on traditional Chinese medicine and western medicine,the China Association of Chinese Medicine has fully played a leading academic role by systematically organizing and conducting a series of academic youth salons on clinical dominant diseases of traditional Chinese medicine. On September 13,2024,the 36th Youth Salon on Clinical Dominant Diseases was successfully held in Nanjing,focusing on the advantages of traditional Chinese medicine and the integrative traditional Chinese medicine and western medicine in the diagnosis and treatment of erectile dysfunction (ED). The conference brought together leading experts from traditional Chinese medicine,western medicine,and interdisciplinary fields,facilitating in-depth multidisciplinary discussions that led to key consensus on optimizing traditional Chinese medicine treatment protocols for ED,researching and developing new drugs of traditional Chinese medicine,and advancing interdisciplinary development in traditional Chinese medicine. This salon systematically sorted out the clinical strengths and distinctive features of traditional Chinese medicine in the diagnosis and treatment of ED. Based on current research foundations and clinical needs,it identified key directions for future scientific layout and scientific research tackling: (1) Standardization of syndrome differentiation system of traditional Chinese medicine for ED. (2) Optimization and standardization of intervention methods of integrated traditional Chinese medicine and western medicine. (3) High-quality clinical research guided by evidence-based medicine. (4) In-depth analysis of the pharmacological mechanisms of traditional Chinese medicine in the treatment of ED. (5) Clinical translation and application promotion of new drugs of traditional Chinese medicine. (6) Interdisciplinary integration and innovation in traditional Chinese medicine. For each research direction,key focus areas,expected objectives,and clinical value were further refined,along with the establishment of a scientifically sound priority funding level evaluation system. Therefore,building on the series of salons on the ED-focused dominant diseases of traditional Chinese medicine,this paper provides standardized guidance for clinical practice of traditional Chinese medicine in ED management,effectively contributing to the high-quality development of traditional Chinese medicine. It serves as a valuable reference for national scientific and technological strategic layout, research and development decision-making in new drugs of traditional Chinese medicine,research topic planning,and clinical guideline formulation.
2.Role of Innate Trained Immunity in Diseases

The innate immune system can be boosted in response to subsequent triggers by pre-exposure to microbes or microbial products, known as “trained immunity”. Compared to classical immune memory, innate trained immunity has several different features. Firstly, the molecules involved in trained immunity differ from those involved in classical immune memory. Innate trained immunity mainly involves innate immune cells (e.g., myeloid immune cells, natural killer cells, innate lymphoid cells) and their effector molecules (e.g., pattern recognition receptor (PRR), various cytokines), as well as some kinds of non-immune cells (e.g., microglial cells). Secondly, the increased responsiveness to secondary stimuli during innate trained immunity is not specific to a particular pathogen, but influences epigenetic reprogramming in the cell through signaling pathways, leading to the sustained changes in genes transcriptional process, which ultimately affects cellular physiology without permanent genetic changes (e.g., mutations or recombination). Finally, innate trained immunity relies on an altered functional state of innate immune cells that could persist for weeks to months after initial stimulus removal. An appropriate inducer could induce trained immunity in innate lymphocytes, such as exogenous stimulants (including vaccines) and endogenous stimulants, which was firstly discovered in bone marrow derived immune cells. However, mature bone marrow derived immune cells are short-lived cells, that may not be able to transmit memory phenotypes to their offspring and provide long-term protection. Therefore, trained immunity is more likely to be relied on long-lived cells, such as epithelial stem cells, mesenchymal stromal cells and non-immune cells such as fibroblasts. Epigenetic reprogramming is one of the key molecular mechanisms that induces trained immunity, including DNA modifications, non-coding RNAs, histone modifications and chromatin remodeling. In addition to epigenetic reprogramming, different cellular metabolic pathways are involved in the regulation of innate trained immunity, including aerobic glycolysis, glutamine catabolism, cholesterol metabolism and fatty acid synthesis, through a series of intracellular cascade responses triggered by the recognition of PRR specific ligands. In the view of evolutionary, trained immunity is beneficial in enhancing protection against secondary infections with an induction in the evolutionary protective process against infections. Therefore, innate trained immunity plays an important role in therapy against diseases such as tumors and infections, which has signature therapeutic effects in these diseases. In organ transplantation, trained immunity has been associated with acute rejection, which prolongs the survival of allografts. However, trained immunity is not always protective but pathological in some cases, and dysregulated trained immunity contributes to the development of inflammatory and autoimmune diseases. Trained immunity provides a novel form of immune memory, but when inappropriately activated, may lead to an attack on tissues, causing autoinflammation. In autoimmune diseases such as rheumatoid arthritis and atherosclerosis, trained immunity may lead to enhance inflammation and tissue lesion in diseased regions. In Alzheimer’s disease and Parkinson’s disease, trained immunity may lead to over-activation of microglial cells, triggering neuroinflammation even nerve injury. This paper summarizes the basis and mechanisms of innate trained immunity, including the different cell types involved, the impacts on diseases and the effects as a therapeutic strategy to provide novel ideas for different diseases.
3.Protective Effects of Yao Medicine Shuilian Fengkui Formula on Sequelae of Pelvic Inflammatory Disease in Rats Through Inhibiting TLR4/Myd88/NF-κB Signaling Pathway

Objective To investigate the protective effects and mechanism of Shuilian Fengkui Formula(SFF)of Yao medicine on sequelae pelvic inflammatory disease(SPID)in rats.Methods The SPID rat model was established by mechanical injury coupled with bacterial infection,and then the rats were randomly divided into:the SPID group,the positive drug(Fukeqianjin tablets)group,SFF groups at low,medium and high doses,and the sham-operation group,with 8 rats in each group;SPID group,SFF groups,TLR4 agonist CRX-527(CRX-527)group,SFF plus CRX-527 group,with 8 rats in each group.Corresponding drug intervention was given for 21 consecutive days.The indexes of uterus,spleen,thymus and other organs were calculated.The histopathological changes in rat uterus were observed by HE staining.The levels of interleukin-1β(IL-1β),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),immunoglobulin G(IgG),immunoglobulin M(IgM)in serum were detected by ELISA;The protein expression levels of Toll receptor 4(TLR4),myeloid differentiation factor(MyD88),nuclear factor kappa B p65(NF-κB p65),and phosphorylated nuclear factor kappa B p65(p-NF-κB p65)in rat uterine tissues were detected by Western blot.Results Compared with the sham operation group,the uterine index of SPID group significantly increased(P<0.01),the spleen index and thymus index significantly decreased(P<0.01),the pathological damage of uterine tissue was serious,the contents of IL-1β,IL-6,TNF-α in serum significantly increased(P<0.01),the contents of IgG and IgM significantly decreased(P<0.01),the TLR4,MyD88 protein and the ratio of p-NF-κB p65/NF-κB p65 protein in uterine tissue significantly increased(P<0.01).Compared with the SPID group,the uterine index of the positive drug group and the SFF groups significantly decreased(P<0.05),the spleen index and thymus index significantly increased(P<0.05),the pathological damage of uterine tissue significantly improved,the contents of IL-1β,IL-6 and TNF-α in serum were significantly lowered(P<0.05),the contents of IgG and IgM significantly increased(P<0.05),and the TLR4,MyD88 protein and the ratio of p-NF-κB p65/NF-κB p65 protein in uterine tissue significantly downregulated(P<0.01).However,CRX-527 significantly reversed the effects of SFF on SPID in rats.Conclusion SFF could improve SPID in rats by impeding the inflammatory response,and its mechanism might be attributed to inhibition of TLR4/MyD88/NF-κB signaling pathway.
4.Effect of Huishen Suxing Decoction on Intestinal Mucosal Barrier in Acute-on-Chronic Liver Failure Rats by Regulating HMGB1/TLR4 Signaling Pathway

Objective To investigate the protective effect of Huishen Suxing decoction on intestinal mucosal barrier in rats with acute-on-chronic liver failure(ACLF)and elucidate its possible mechanism.Methods The rats were randomly divided into control group(Con group),model group,Bifico group(positive control),and HSD-L,M and H groups(low,medium and high dose of Huishen Suxing decoction).The ACLF model was established by CCl4 combined with D-galactosamine(D-Gal N).Intragastric administration of CCl4 was started on the 6th week of intraperitoneal injection,once daily for 6 weeks,and continued until the acute challenge with D-Gal N injection.The liver function of rats was detected by kits.HE staining was used to observe the pathological changes of liver tissue and ileum tissue.The levels of endotoxin(ET),diamine oxidase(DAO),IL-1β,IL-6,TNF-α in serum and secretory immunoglobulin A(sIgA)in ileum were detected by ELISA.Intestinal mucosal permeability was detected by FITC-Dextran tracer.Selective medium was used to detect the amount of intestinal flora.Western blotting was used to detect the protein expression levels of HMGB1,TLR4,NF-κB p65,p-NF-κB p65,Occludin and ZO-1 in ileum tissue of each group.Results Compared with the Con group,the liver tissue and ileum tissue of the Model group showed obvious pathological damage.The levels of ALT,AST,TBIL,ET,DAO,IL-1β,IL-6 and TNF-α in serum significantly increased(P<0.01).The content of sIgA in ileum tissue significantly decreased(P<0.01).The permeability of intestinal mucosa and the number of Enterobacter and Enterococcus in the intestine significantly increased(P<0.01).The amount of Bifidobacterium and Lactobacillus significantly decreased(P<0.01).The expressions of HMGB1,TLR4 protein and p-NF-κB p65/NF-κB p65 protein ratio in ileum tissue significantly increased(P<0.01),and the expressions of Occludin and ZO-1 protein significantly decreased(P<0.01).Compared with the model group,the Bifico group and HSD-L,M,H groups had significantly alleviated liver and ileum injury.The levels of ALT,AST,TBIL,ET,DAO,IL-1β,IL-6 and TNF-α in serum significantly decreased(P<0.01).The permeability of intestinal mucosa and the amount of Enterobacteriaceae and Enterococcus in the intestine significantly decreased(P<0.01),while the amount of Bifidobacterium and Lactobacillus significantly increased(P<0.01).The expressions of HMGB1,TLR4 protein and p-NF-κB p65/NF-κB p65 protein ratio in ileum tissue significantly decreased(P<0.01),and the expressions of Occludin and ZO-1 protein significantly increased(P<0.01).Conclusion Huishen Suxing decoction can reduce inflammatory response,regulate the structure of intestinal flora,improve the intestinal mucosal barrier,and thus alleviate liver injury in ACLF rats.The mechanism may be associated to the inhibition of HMGB1/TLR4/NF-κB signaling pathway activation.
5.Preliminary application of augmented reality(AR)system in cerebrovascular disease surgery

Objective To assess the clinical feasibility of a self-developed augmented reality(AR)system in surgeries for cerebral arteriovenous malformation(AVM),dural arteriovenous fistula(DAVF),moyamoya,and internal carotid artery stenosis.This system integrates preoperative vascular imaging(CTA,MRA,DSA)with intraoperative real-time visualization through high-precision patient-image registration and virtual-real integration technology.Methods A retrospective analysis was conducted on 6 patients(1 cerebral AVM,1 DAVF,3 moyamoya and 1 internal carotid artery stenosis)collected between March 2023 and April 2024.AR with three-dimensional reconstruction was used for preoperative precise localization and intraoperative navigation guidance.Clinical feasibility was evaluated and analyzed using an intraoperative self-controlled method.Results All 6 patients with diverse etiologies successfully underwent preoperative precise localization and intraoperative navigation guidance under AR three-dimensional reconstruction modeling.This technology achieved visualization of intracranial arteriovenous structures and precise lesion locations,providing surgeons with a visual reference for accurate planning of the surgical approach and operative field.Conclusion The application of AR with three-dimensional reconstruction is safe and feasible in neurosurgical procedures for cerebrovascular diseases.It demonstrates satisfactory effectiveness in preoperative localization and intraoperative navigation guidance.
6.Application of esophageal-tubular gastric asymmetric anastomosis in esophageal and esophagogastric junction cancer

Objective:To evaluate the anti-reflux effect of digestive tract reconstruction using esophageal-tubular gastric asymmetric anastomosis after radical resection of esophageal and esophagogastric junction cancer.Methods:The main steps were as follows:(1)oblique incision of the lower esophagus;(2)curved incision of the tubular anterior gastric wall;(3)the lower end of the esophagus was anastomosed to the tubular gastric incision with a 90-degree torsion; (4)The anterior wall of the anastomosis was reinforced with a transverse-inverted suture,the posterior wall with a folded suture,and the corners of the gastric stump were buried with sutures.The anastomosis operation time,postoperative complications and postoperative hospital stay were recorded;the reconstructed structure and anti-reflux effect of the anastomosis were observed by digestive tract radiography,gastroscopy and follow-up investigation.Results:The Department of Gastrointestinal and Thoracic Surgery of Huaian First People's Hospital, affiliated to Nanjing Medical University, treated 5 patients of esophagogastric junction cancer and 20 esophageal cancer cases between August 2022 and November 2024, including 19 men and 6 women, with a mean age of (66.7±7.4) years. The mean anastomosis time was (35.4±5.9) minutes, the intraoperative blood loss was (117.6±33.4) ml and the mean postoperative hospital stay was(16.6±5.2) days, with no complications such as anastomotic leakage and bleeding. Postoperative digestive tract radiography (Trendelenburg position)showed that all the patients had no contrast reflux,gastroscopy showed no signs of reflux esophagitis and bile reflux gastritis, the anastomosis showed an inverted whiskers valve-like structure. The median follow-up time was (16.8±6.3) months, and all patients had no reflux symptoms such as acid reflux and belching,and no acid suppressive medication was needed.Conclusion:The esophageal-tubular gastric asymmetric anastomosis is a safe and effective antireflux reconstruction technique.
7.Establishment and application of a method for detecting Toxoplasma gondii based on recombinant polymerase amplification technology

To establish a method for detecting Toxoplasma gondii based on recombinant polymerase amplification(RPA)technology and apply it to clinical sample validation of pet cats.Using the 529 repeat sequence of the Toxoplasma gondii gene as the target gene sequence,primers and probes were designed,and the Rep-529 recombinant plasmid was constructed as the standard.A fluorescent RPA reaction system was established.Dilute the plasmid standard 10 times to different concentrations as the detection template for sensitivity testing;Specific testing was conducted using genomic DNA from several parasitic spe-cies,including Toxoplasma gondii,Cryptosporidium,Neosporidium,Trichinella spiralis,Giardia flagellata,Babesia bo-vis and Theileria annulata as templates;Simultaneously,fluorescence RPA and RT-PCR were used to detect 52 positive and 40 negative cats clinical samples,and the coincidence rate of the detection results of the two methods were compared and ana-lyzed.The RPA reaction system was successfully established using PTRep recombinant plasmid as the standard,ToxD-F/ToxD-R as the primer,and RepD-P as the fluorescent probe.The reaction temperature was constant at 39 ℃,the reaction time was 30 minutes,and the detection sensitivity was 1 copy/μL.There is no significant cross reaction with parasites such as Cryptosporidium,Neosporidium,Trichinella spiralis,Giardia,Babesia bovis and Theileria annulata,and the specificity is good.A total of 92 clinical fecal samples from cats were tested,and the positive coincidence rate of fluorescence RPA detection method was higher than that of conventional RT-PCR method(98.08%vs.82.69%),and the difference of the positive rate was not statistically significant(X2=1.392,P>0.05).The fluorescence RPA detection method for Toxoplasma gondii suc-cessfully established in this study has the characteristics of being fast,sensitive,specific,accurate,and reliable.It can be used as a rapid clinical detection kit for Toxoplasma gondii in cats and other animals,providing new technical support for the subsequent epidemiological monitoring and precise clinical diagnosis of toxoplasmosis in cats,other animals,and humans in the future.
8.Analysis of the proactive coping status and influencing factors of schizophrenia patients with auditory hallucinations

Objective To investigate the proactive coping of schizophrenia patients with auditory hallucinations and to analyze its influencing factors.Methods A total of 240 schizophrenia patients with auditory hallucinations hospitalized in Shandong Mental Health Center from February to October 2023 were selected as subjects.General information questionnaire,simplified coping style questionnaire,perceived social support scale,insight and treatment attitude questionnaire were used to conduct the survey.Results A total of 236 valid questionnaires were collected,and effective questionnaire response rate was 98.33%.The proactive coping score of schizophrenia patients with auditory hallucinations was(18.55±7.74)points,which was at a moderate level.The results of multiple linear regression analysis showed that education level,perceived of social support,and self-awareness were the influencing factors of proactive coping to schizophrenia patients with auditory hallucinations(P<0.05).Conclusion The proactive coping of schizophrenia patients with auditory hallucinations needs to be further improved,and nursing staff can focus on patients with low education level,low ability to perceive social support and poor self-awareness,and formulate targeted interventions to improve their proactive coping to auditory hallucinations.
9.Construction of drug utilization evaluation criteria for Dezocine injection based on evidence-based methodology and Delphi method

OBJECTIVE To construct the drug utilization evaluation criteria for Dezocine injection,so as to provide reference for the rational drug use in medical institutions.METHODS On the basis of evidence methodology,relevant guidelines/expert consensus,systematic reviews/meta-analysis were consulted;the evaluation criteria framework for Dezocine injection was established after screening evidence.Delphi method was employed,whereby 28 clinicians and clinical pharmacists from secondary and above-level medical institutions across eight provinces,including Tianjin,Beijing and Shandong,were selected to participate in two rounds of questionnaire surveys.The final indicators were determined based on the experts'enthusiasm coefficient,authority coefficient,and degree of coordination.RESULTS The effective recovery rate of questionnaire was 100%in the first round and 92.86%in the second round;expert authority coefficient was 0.82 in the first round and 0.81 in the second round;the coordination degree of experts in the first round was 0.29,and in the second round was 0.31(P<0.001).Drug utilization evaluation standard system for Dezocine injection was formed finally,including three dimensions of medication indications,medication process and medication results,with a total of 11 first-level indicators(such as indications,usage and dosage)and 33 second-level indicators(such as labor analgesia and the management of severe pain following major or moderate surgeries combined with other analgesic drugs).The average importance scores for each indicator ranged from 4.08 to 5.00 points,with an overall average score of 4.61 points and coefficient of variation ranging from 0 to 0.19.CONCLUSIONS The drug utilization evaluation criteria for Dezocine injection established based on evidence-based methodology and Delphi method is authoritative and scientific,which provides a reference for subsequent evaluation of the rationality of clinical medication.
10.Efficacy of 450 nm blue laser vaporization resection of the prostate in the treatment of 132 patients with benign prostatic hyperplasia

Objective To explore the clinical efficacy,safety and changes in sexual function of 450 nm blue laser vaporization resection of the prostate in the treatment of benign prostatic hyperplasia(BPH).Methods The clinical data of 132 BPH patients treated in Yingsheng Branch of Taian Central Hospital during Jan.and Oct.2023 were retrospectively analyzed.The perioperative related indicators,incidence of complications were recorded.The international prostate symptoms(IPSS)score,maximum urine flow rate(Qmax),post voiding residual(PVR),quality of life score(QoL),prostate-specific antigen(PSA)level,the Chinese index of premature ejaculation score(CIPE)and international erectile function index-5(IIEF-5)score before operation,and 1,3,6,and 12 months after operation were analyze.Results The operation time was(18.37±5.38)min,intraoperative blood loss(6.73±2.57)mL,postoperative hospital stay(4.31±1.54)d,catheter indwelling time(2.45±1.27)d,and bladder irrigation time(19.61±2.45)h.There were significant differences in Qmax,PVR,PSA level,QoL and IPSS before and after operation(P<0.05).There was no statistically significant difference in IIEF-5 and CIPE before operation and 3,6 and 12 months after operation(P>0.05).The retrograde ejaculation rate remained 6.25%3,6 and 12 months after operation.Conclusion The 450 nm blue laser vaporization resection of the prostate can relieve lower urinary tract symptoms,preserve the sexual function,and improve patients'quality of life.
