1.Application value of two-phase arterial spin labeling combined with territorial arterial spin labeling in the evaluation of cerebral perfusion after moyamoya disease revascularization

Objective To evaluate the changes in cerebral perfusion after moyamoya disease revascularization using two-phase arterial spin labeling(ASL)combined with territorial arterial spin labeling(t-ASL).Methods A retrospective selection was conducted on 34 moy-amoya disease patients with 37 cerebral hemispheres who underwent revascularization surgery.ASL[post labeling delay(PLD)of 1.5 s and 2.5 s],t-ASL,and digital subtraction angiography(DSA)were performed before and after surgery.The cerebral blood flow(CBF)and relative cerebral blood flow(rCBF)values in the middle cerebral artery(MCA)supply area from ASL were com-pared before and after surgery.t-ASL and DSA were used to score the area of revascularization on a 3-point scale,and weighted Kappa was used to compare the consistency of the revascularization area assessments.A DSA score greater than 1 point was classified as good revascularization.The diagnostic efficacy of ASL(PLD 1.5 s,2.5 s)and t-ASL for revascularization were compared.Results In ASL PLD 1.5 s,the preoperative and postoperative CBF values were(32.65±12.71)mL·100 g-1·min-1 and(43.09±18.05)mL·100 g-1·min-1,respectively,and the rCBF values were 0.76±0.23 and 1.02±0.31;In ASL PLD 2.5 s,the preoperative and postoperative CBF val-ues were(47.07±13.28)mL·100 g-1·min-1and(52.85±14.98)mL·100 g-1·min-1,respectively,and the rCBF values were 1.21±0.31 and 1.33±0.29.There were statistically significant differences between preoperative values and postoperative values above(P<0.05).The revascularization area score showed strong consistency(weighted Kappa coefficient=0.729).Receiver operating charac-teristic(ROC)curve analysis revealed that t-ASL reflected the revascularization area with area under the curve(AUC)of 0.982,while ASL PLD 1.5 s and 2.5 s CBF reflected the revascularization area with AUC of 0.813 and 0.794,espectively.Conclusion The com-bination of two-phase ASL and t-ASL more effectively evaluates cerebral blood after moyamoya disease revascularization.Furthermore,t-ASL demonstrates higher efficacy in evaluating postoperative revascularization.

3.Effect of MSC-exo,a New Cell Delivery Tool,on Gene Delivery and Proliferation of Pancreatic Cancer

Objective To observe the effect of a new cell delivery tool(MSC exo)on the proliferation of pancreatic cancer by transferring targeted genes.Methods Transmission Electron Microscope(TEM)and Nanoparticle Tracking Analysis(NTA)were used to identify human mesenchymal stem cell exosomes(MSC-exo)and transport miR-450a-5p into CFPAC-1,to explore the effect of miR-450a-5p targeting BZW2 on inhibiting the proliferation of pancreatic cancer cells.Results The expression of miR-450a-5p was low in pancreatic cancer tissue(P<0.05),and the expression of CD63 and TSG101 of MSC-exo-miR-450a-5p in CFPAC-1 cells was higher than that of MSC-exo by Western blot(P<0.05).CCK-8 and EdU results showed that MSC-exo-miR-450a-5p significantly inhibited the proliferation of CFPAC-1 cells(P<0.05).Cell scratch and Transwell experiments showed that MSC-exo-miR-450a-5p can inhibit the migration and invasion of CFPAC-1 cells(P<0.05).Through dual luciferase assay,it was confirmed that miR-450a-5p targets BZW2,and RT-qPCR and Western blotting showed a negative correlation(P<0.05)between miR-450a-5p and BZW2 expression.Overexpression of BZW2,CCK-8,EdU,cell scratch,and Transwell experiments confirmed that pc-BZW2 reversed the anti-cancer function of MSC-exo-miR-450a-5p on CFPAC-1.Western blot detected PCNA,Ki-67,MMP2,MMP9,and the results were consistent with the above experiments(P<0.05).Conclusion hMSC exo is a new delivery system,targeting BZW2 to transport miR-450a-5p to inhibit the biological malignancy of pancreatic cancer cells,which provides an important clue for the research of targeted treatment of pancreatic cancer.
4.Application of Sihuang Powder application on Yongquan Point in patients undergoing hip and knee replacement

Objective:To explore the application effect of Sihuang Powder application on Yongquan Point in patients undergoing hip and knee replacement.Methods:The convenient sampling method was used to select 172 patients after hip and knee replacement in Yantai Mountain Hospital from October 2021 to August 2022 as the research objects. They were divided into the control group ( n=85) and the experimental group ( n=87) by the random number table method. Patients in the control group were treated with routine nursing intervention, while patients in the experimental group were treated with Sihuang Powder application on Yongquan Point on the basis of control group. The fibrinogen content and D-dimer before and after intervention, the incidence of deep vein thrombosis (DVT) in lower limbs during hospitalization and the length of postoperative hospitalization were compared between the two groups. Results:On the thrid day after operation, fibrinogen content and D-dimer level of the experimental group were lower than those of control group, and the differences were statistically significant ( P< 0.05). The incidence of DVT during hospitalization in the experimental group was 5.75% (5/87), lower than 15.29% (13/85) in the control group, and the difference was statistically significant (P<0.05). The postoperative hospitalization days in the experimental group was shorter than that in the control group, and the difference was statistically significant ( P<0.05) . Conclusions:Applying Sihuang Powder to Yongquan Acupoint can effectively improve blood hypercoagulability, reduce the incidence of lower limb DVT in patients after hip and knee replacement surgery, reduce postoperative complications and effectively shorten the length of hospital stay.
5.Application of green channel nursing model based on information intelligent platform in emergency trauma patients

Objective:To analyze the effect of green channel nursing model based on the information intelligent platform on the rescue time and quality of emergency trauma patients.Methods:Using the convenient sampling method, a total of 156 emergency trauma patients admitted to Yantai Mountain Hospital in Yantai City from April to October 2023 were selected as the research objects. The 78 patients admitted from April to July 2023 were set as the control group and they were given the conventional nursing model, while 78 patients admitted from August to October 2023 were set as the observation group and they were given green channel nursing mode based on the information intelligent platform on the basis of the control group. The rescue time and quality of patients were compared between two groups.Results:The establishment time of venous channel, medication application time, completion time of CT examination and stay time in emergency room in the observation group were shorter than those in the control group, and the scores of all dimensions of nursing quality were all higher than those in control group, and the differences were statistically significant ( P<0.01) . Conclusions:The green channel nursing model based on the information intelligent platform can improve the rescue time and nursing quality of emergency trauma patients, which has certain clinical significance.
6.Study on the mechanism of visual dysfunction and pathological changes in mice with acute hepatic encephalopathy

Objective:To study the changes in visual function and its mechanism in mice with acute hepatic enceph-alopathy(AHE)model.Method:Twelve male mice were divided into experimental and control groups,with six mice in each.A mouse model of AHE was created using TAA,and serum ALT,AST,and ammonia levels were measured using ELISA and colorimetric assays.Behavioral tests,including open-field and elevated cross maze experiments,were conducted,and neurological function scores were evaluated.HE staining was used to evaluate visual function scores in mice.Behavioural tests were conducted to assess neurological function scores.HE staining was used for pathological examination of the liver and retina.Visual electrophysiology was used to test visual function in both eyes.Retrograde tracer adeno-associated virus was used to label the ventrolateral thalamic nucleus(VL)of thalamus in c-Fos-CreERT2 mice.Results:Compared to the control group,the AHE mice had abnormal liver function and neurological changes.The HE results showed liver tissue damage.The retinal tissues were not significantly different from the control group.The visual electrophysiological results showed changes in visual function in both eyes of the AHE mice.The retrograde tracer virus injections revealed no significant difference in VL activation between the AHE model and the control group.In the AHE model,VL nucleus accumbens neurons primarily received input from ipsilateral visual cortical neurons.Conclusion:The visual deficits in AHE mice caused by TAA were primarily functional rather than organic changes,and were associated with the activation of the visual cortex-to-VL pathway.

9.CT manifestations of primitive neuroectodermal tumor of adrenal gland

Objective To explore the CT manifestations of primitive neuroectodermal tumor (PNET) of adrenal gland,aiming to increase the cognition on this disease.Methods The CT manifestations of 7 patients who were pathologically diagnosed with adrenal gland PNET were analyzed retrospectively.Results The age of patients was 24-63 years old,with average age of (47.6±13.7) years,including 6 females and 1 male.Location of PNET:4 in left adrenal gland and 3 in right adrenal gland.Clinical manifestations:3 cases had upper abdominal distension,2 cases suffered from pain of waist and back,1 case was discovered in physical examination and another 1 had fever.No abnormality was found in laboratory and endocrinological examinations.After operation,4 cases had recurrence and 2 had distant metastasis.During follow-up,6 patients died and only 1 still alive.CT showed that the tumor,whose maximum diameter was 10.1-19.4 cm,was elliptic or lobulated in shape.The tumor was clear in boundary in 2 patients,but was unclear in boundary and involved adjacent organs in 5 patients.The plain scanning showed equidensity or slightly low density,complicated with necrosis and cystic lesions without calcification.The enhanced scanning suggested progressive uneven enhancement,with multiple interatrial septum-like changes and latticed enhancement inside.Conclusion CT manifestations of adrenal gland PNET can prompt the diagnosis,pathological examination is still required for confirmation.CT can accurately describe the internal structures,adjacent relationship and the metastasis of tumors.
10.Giant cell glioblastoma imaging findings and pathologic correlation

Objective To investigate the imaging findings of giant cell glioblastoma(GCG).Methods Imaging findings of 8 cases with pathology-confirmed GCG were retrospectively reviewed and correlated with pathologic findings.Results All of the 8 cases were located in the cerebral hemisphere.On CT images, 1 lesion case was shown appeared as mixed density,4 lesions were shown as cystic and solid mass.Mural nodule were slightly hyperdense.On MRI images, all of 4 cases lesions were hypointensity on T1WI, mixed signal intensity or hyperintensity on T2WI and FLAIR, low signal intensity on DWI.The lesion showed nonuniform enhancement,with enhanced mural nodules and cyst walls,and mildmoderate or no peritumoral edema.Microscopically, the cells were arranged in perivascular pseudorosettes to form palisades.Cellular pleomorphism was a typical feature, including monstrous and multinucleated giant cells.Mitotic figures were usually numerous.Reticulin was abundant in the stroma.Immunohistochemistry showed staining for S-100, Vim, p53, GFAP.The proliferation rate determined by Ki-67 immunohistochemistry was 30%.Conclusion GCG has certain imaging characteristics,CT and MRI are effective in detecting the disease.
