The original meaning of pathogenesis refers to the cardinal mechanism of the occurrence, development and change of disease, which has three characteristics: cardinal, dominant and directional. The original pathogenesis of primary osteoporosis includes aging and renal failure, which was determined through ancient Chinese medicine books and modern researches. The basic contradiction and evolutionary law of diseases from the level of original pathogenesis were identified, that contribute to the prevention and treatment of primary osteoporosis with Traditional Chinese Medicine.

Objective:To explore the preventive effects of hyperbaric oxygen combined with upper limb functional exercise on venous thrombosis in patients with peripherally inserted central catheter (PICC).Methods:A total of 144 patients with PICC admitted to the First Department of General Surgery of Hengshui People’s Hospital from December 2019 to June 2020 were retrospectively selected as the research subjects and divided into control group ( n=70) and observation group ( n=74) according to treatment methods. The control group was given upper limb functional exercise on the basis of conventional treatment, while the observation group was given hyperbaric oxygen treatment on the basis of the treatment of the control group. The incidences of venous thrombosis after treatment were compared between the two groups. The changes of coagulation and fibrinolytic functions and of hemorheology indicators of patients were detected before and after treatment. The predictive value of each indicator for the occurrence of venous thrombosis was evaluated by receiver operating characteristic (ROC) curve. Results:Compared with those before treatment, the prothrombin time (PT), the activated partial thromboplastin time (APTT), the thrombin time (TT), and the levels of tissue plasminogen activator (t-PA) in the two groups after treatment were significantly prolonged or increased, and the levels of fibrinogen (FIB), D-dimer, fibrinogen degradation product (FDP), and plasminogen activator inhibitor (PAI) in the two groups after treatment were significantly decreased ( P<0.05); compared with those in the control group after treatment, PT, APTT, TT, and the level of t-PA in the observation group after treatment were significantly prolonged or increased, and the levels of FIB, D-dimer, FDP, and PAI in the observation group after treatment were significantly lower; and all the differences were statistically significant ( P<0.05). Compared with those before treatment, the values of peak systolic velocity (PSV) and resistance index (RI) in the two groups after treatment were significantly lower, and the values of mean flow velocity (Vm) were significantly higher; compared with those in the control group after treatment, the values of PSV and RI in the observation group after treatment were significantly lower, and the value of Vm was significantly higher; and all the differences were statistically significant ( P<0.05). ROC curve analysis showed that the areas of FIB, D-dimer, FDP, and t-PA under the ROC curve predicting the occurrence of venous thrombosis were 0.823, 0.794, 0.684, and 0.787, respectively; the sensitivities were 75.0%, 66.7%, 61.2%, and 64.4%, respectively; the specificities were 81.5%, 83.3%, 74.6%, and 78.3%, respectively; and the accuracies were 74.3%, 72.6%, 68.4%, and 70.2%, respectively. The accuracy of FIB for the diagnosis of venous thrombosis was better than those of D-dimer, FDP, and t-PA. Conclusion:Hyperbaric oxygen combined with upper limb functional exercise can improve the coagulation function, fibrinolysis, and hemorheology of patients. Clinically, it can help predict the occurrence of venous thrombosis in upper limb by detecting the levels of FIB, D-dimer, FDP, and t-PA.

Clinically, it can help predict the occurrence of venous thrombosis in upper limb by detecting the levels of FIB, D-dimer, FDP, and t-PA.

Three hundred and nine patients with thyroid nodules detected by physical examination in Harrison International Peace Hospital from October 2013 to October 2017 were divided into intervention group (155 cases) and control group (154 cases). Patients in intervention group received oral levothyroxine sodium 25 g / d for 12 months and those in control group had no treatment, patients were followed up every 3 months to 12 months. After treatment, the maximum diameter and thyroid nodule volume of the intervention group were (31.87±3.84) mm and (17.32±0.94) cm3, which were significantly smaller than those of the control group [(34.01±3.72) mm and (24.25±1.21)cm3, P<0.05]. TSH in intervention group was lower than that in control group [(2.24±0.41) vs. (2.52±0.58) mIU/L, P<0.05] and free T4 (FT4) was higher than that in control group [(25.64 ± 3.85) vs. (16.39 ± 3.28) pmol/ L, P<0.05]. TC, TG and LDL?C in intervention group, were lower than those in the control group .The HDL?C level in intervention group was higher than that in control group (all P<0.05). After treatment, there were no malignant changes in the intervention group, while the malignant change rate in control group was 2.6％ (4/154). It is suggested that levothyroxine treatment can reduce TSH level in patients with benign thyroid nodules, inhibit the growth of thyroid nodules.

Objective To explore clinical curative effects of the moist burn cream combined with ozone in treatment of fat liquefaction of abdominal incision after appendectomy.Methods 112 patients who suffered with fat liquefaction of abdominal incision after appendectomy in hospital from January 2012 to January 2015 were chosen.According to the random number table method, they were randomly divided into the observation group and control group, 56 cases in each group.Before treatment, two groups of patients were liquefied suture removal and thoroughly removed subcutaneous tissue necrosis.Then, the control group accepted the pure moist burn cream treatment, while the observation group were given the moist burn cream combined with ozone treatment.The growth condition of granulation tissue after treatment, frequency of dressing change, wound cicatrisation, and length of hospital stay were compared between two groups, and the condition of wound healing was evaluated.Results The grade A healing rate of the observation group was 89.28%(50/56), while that of control group was 71.43%(40/56)(χ2 =5.65,P<0.01).The time of the granulation tissue beginning to grow and the time when the granulation tissue flushed leather face of observation group were significantly shorter than those of control group ( P<0.05 ) .In addition, times of changing a medical prescription of observation group was less than that of control group (P<0.05), and the incision healing time, length of stay in hospital were also shorter than those of control group (P<0.05).Conclusion It has better clinical curative effect to apply the moist burn cream combined with ozone in the treatment of fat liquefaction of incision after appendectomy.Moreover, it has some advantages, such as the quick growth of granulation, the well-healed wound, and the short hospitalization time.Therefore, it is worthy of clinical popularization and application.
