1.Correlation analysis of cervical anteversion and curvature changes with sagittal parameters in patients with cervical spondylosis

Objectives:To apply the anterior tilt slope(ATS)for the first time to assess the degree of cervical anteversion,and to explore and analyze the correlations between the cervical anteversion and cervical curvature changes and sagittal spinal alignment parameters in patients with cervical spondylosis.Methods:A total of 201 patients with cervical spondylosis who were treated in the Department of Orthopedics(Cervical Spine)outpatient clinic from May 2020 to July 2023 were included in the study,consisting of 79(39%)males and 122(61%)females,aged 25.6±6.5(18-40)years.Imaging parameters were measured on the total spinal lateral X-ray films,including the cervical parameters,such as cervical lordosis(CL),upper cervical Cobb an-gle,C1-7 Cobb angle,and T1 slope(T1S),cervical sagittal vertical axis(cSVA),Borden value of cervical cur-vature,anterior tilt slope(ATS);thoracolumbar parameters,such as thoracic kyphosis(TK)and lumbar lordosis(LL);as well as pelvic parameters,such as sacral slope(SS),pelvic tilt(PT),pelvic incidence(PI),sagittal vertical axis(SVA).The curvature changes and cervical anteversion were staged according to the Borden value and ATS in the cervical spine parameters,and their relationships with each spinal sagittal parameter in different classification were compared.Results:According to ATS,the cercical anteversion of patients was classified into type Ⅰ of 67 cases,type Ⅱ of 70 cases,type Ⅲ of 40 cases,and type Ⅳ of 24 cases.According to the Borden value,the cervical curvature was classified into type kyphotic of 65 cases,type straight of 92 cases,type lordotic of 29 cases,and type sigmoid of 15 cases.There were significant positive correlations between Borden values and CL,C1-7 Cobb angle,and T1S(P<0.001).ATS had significant positive correlations with T1S and cSVA(P<0.001),and positive correlations with CL,C1-7 Cobb angle,and Borden value(P<0.05).SVA was significantly positively correlated with C1-7 Cobb angle,T1S,cSVA,Borden value,ATS,and SS(P<0.001),whereas there was no significant correlation between each of the cervical spine parameters and pelvic parameters.In different cervical anteversion classifications,significant differences were observed between cSVA,T1S,TK,and SVA(P<0.001),and severe degree of cervical anteversion further aggravated the sagittal imbalance.In different types of cervical curvature,there were significant differences between CL,C1-7 Cobb angle,T1S,Borden value,TK,SS,SVA,and ATS(P<0.05),and when the curvature straightened or became kyphotic,it would cause compensatory reduction of the cervicothoracic junction.Conclusions:In patients with cervical spondylosis,cervical anteversion and curvature changes exhibit significant correlations with thoracic parameters among the sagittal parameters.When cervical anteversion is aggravated and changes in cervical curvature mainly cause compensatory changes in the T1S and TK angles,the SVA is significantly larger in those with significant cervical anteversion.

3.Construction and validation of a simple model for predicting the risk of prenatal depression

BackgroundMental illness during pregnancy has become a major public health problem in China over the recent years, and depression is the most common psychological symptom during pregnancy. Current research efforts are directed towards the therapy on prenatal depression, whereas the construction of prediction model for prenatal depression risk has been little studied. ObjectiveTo construct a simple model for predicting the risk of prenatal depression, thus providing a valuable reference for the prevention of maternal depression during pregnancy. MethodsA total of 803 pregnant women attending three hospitals in Nanchong city were consecutively recruited from May 2021 to February 2022. A self-administered questionnaire was developed for the assessment of social demographic variables, obstetrical and general medical indexes and psychological status of all participants, and Self-rating Depression Scale (SDS) was utilized to screen for the presence of maternal depression. Subjects were randomly assigned into modelling group (n=635) and validation group (n=168) at the ratio of 8∶2 under simple random sampling with replacement. The candidate risk factors of maternal depression during pregnancy were screened using binary Logistic regression analysis, and the predictive model was constructed. Then the performance of the predictive model was validated using receiver operating characteristics (ROC) curve. Results① Lack of companionship (β=-0.692, OR=0.501, 95% CI: 0.289~0.868), low mood during the last menstrual period (β=-1.510, OR=0.221, 95% CI: 0.074~0.656), emotional stress during the last menstrual period (β=-1.082, OR=0.339, 95% CI: 0.135~0.853), unsatisfactory relationship between mother-in-law and daughter-in-law (β=-1.228, OR=0.293, 95% CI: 0.141~0.609), and indifferent generally relationship between mother-in-law and daughter-in-law (β=-0.831, OR=0.436, 95% CI: 0.260~0.730) were risk factors for prenatal depression in pregnant women (P<0.05 or 0.01). ② Model for predicting the prenatal depression risk yielded an area under curve (AUC) of 0.698 (95% CI: 0.646~0.749), the maximum Youden index was 0.357 in modelling group with the sensitivity and specificity was 0.606 and 0.751, and an AUC of 0.672 (95% CI: 0.576~0.767) and maximum Youden index of 0.263 in validation group with the sensitivity and specificity of 0.556 and 0.707. ConclusionThe simple model constructed in this study has good discriminant validity in predicting of the risk of prenatal depression. 4.Resisin stimulates the expression of CCL3 and CCL4 in chondrocytes

BACKGROUND:Previous studies have indicated that resistin stimulates a large set of chemokines in chondrocytes that are known to be important in inflammatory joint lesions.
OBJECTIVE:To further investigate the mechanism of co-regulation roles of transcription and post-transcription in the up-regulation of two chemokine genes CCL3 and CCL4 in chondrocytes in response to resistin.
METHODS:Human chondrocytes, T/C-28a2 and ATDC5 cels were cultured. The function of resistin on the chemokine genes, and the expression of C/EBPβ, nuclear factor-κB isoforms and chondrogenic specific miRNAs were tested by qPCR. The co-regulation of C/EBPβ and nuclear factor-κB was investigated by nuclear factor-κB inhibitor (IKK-NBD) and C/EBPβ inhibitor (SB303580) treatments, and subcelular localization was detected with or without resistin stimulation.
RESULTS AND CONCLUSION:Resistin could increase the expression of chemokine genes independently. Chondrocytes reacted in a non-restrictedly cel-specific manner to resistin; C/EBPβ inhibitor, nuclear factor-κB and some chondrogenic specific miRNAs in a combinatorial manner regulated chemokine gene expression. The activity of C/EBPβ was augmented by a transient increase in activity of nuclear factor-κB, and both transcription factors acted independently on the chemokine genes, CCL3 and CCL4.
