Objective To explore the impact of number of positive regional lymph nodes (nPRLN) in N1 stage on the prognosis of non-small cell lung cancer (NSCLC) patients. Methods Patients with TxN1M0 stage NSCLC who underwent lobectomy and mediastinal lymph node dissection from 2010 to 2015 were screened from SEER database (17 Regs, 2022nov sub). The optimal cutoff value of nPRLN was determined using X-tile software, and patients were divided into 2 groups according to the cutoff value: a nPRLN≤optimal cutoff group and a nPRLN>optimal cutoff group. The influence of confounding factors was minimized by propensity score matching (PSM) at a ratio of 1 : 1. Kaplan-Meier curves and Cox proportional hazards models were used to evaluate overall survival (OS) and lung cancer-specific survival (LCSS) of patients. Results A total of 1316 patients with TxN1M0 stage NSCLC were included, including 662 males and 654 females, with a median age of 67 (60, 73) years. The optimal cutoff value of nPRLN was 3, with 1165 patients in the nPRLN≤3 group and 151 patients in the nPRLN>3 group. After PSM, there were 138 patients in each group. Regardless of before or after PSM, OS and LCSS of patients in the nPRLN≤3 group were superior to those in the nPRLN>3 group (P<0.001). N1 stage nPRLN>3 was an independent prognostic risk factor for OS [HR=1.52, 95%CI (1.22, 1.89), P<0.001] and LCSS [HR=1.72, 95%CI (1.36, 2.18), P<0.001]. Conclusion N1 stage nPRLN>3 is an independent prognostic risk factor for NSCLC patients in TxN1M0 stage, which may provide new evidence for future revision of TNM staging N1 stage subclassification.
3.Predictive and prognostic role of serum CA19-9 in metachronous peritoneal metastasis following surgery for stage Ⅱ-Ⅲ colorectal cancer

[摘 要] 目的:评估术前血清糖类抗原19-9(CA19-9)水平在Ⅱ~Ⅲ期结直肠癌术后发生异时性腹膜转移(mPM)中的预测和预后作用。方法: 回顾性分析2019年1月至2024年1月于山东第一医科大学第一附属医院肿瘤中心收治的Ⅱ~Ⅲ期结直肠癌患者临床病理信息,包括术前CA19-9和癌胚抗原(CEA)水平。采用ROC曲线及曲线下面积(AUC)评估其诊断效能。采用单因素与多因素Logistic回归及Cox比例风险回归模型分别识别风险因素与预后因素。通过倾向性评分匹配,比较CA19-9升高组和正常组无病生存期(DFS)及无腹膜转移生存期(PMFS)差异,使用Log-rank检验进行生存分析。结果: 本研究共纳入286例患者,术前CA19-9升高与黏液腺癌组织类型(P = 0.005)、T4分期(P < 0.001)、淋巴结转移(P = 0.02)、术前CEA升高(P = 0.024)、神经侵犯(P = 0.02)、低分化程度(P = 0.045)显著相关。升高的CA19-9水平对mPM的预测准确性优于CEA(AUC:0.73,95% CI:0.61~0.85 vs AUC:0.64,95% CI:0.55~0.73)。多因素Logistic回归分析显示,术前CA19-9升高是发生mPM的独立预测因子(OR = 5.59,95% CI:2.00~15.58,P = 0.001)。CA19-9升高患者的中位DFS[54.27个月 vs 未达到(NR),HR = 3.55,95% CI:1.96~6.44,P < 0.001]与PMFS(NR vs NR,HR = 6.93,95% CI:3.25~14.76,P < 0.001)显著缩短。倾向性评分匹配后,DFS(54.27个月 vs NR,HR = 4.26,95% CI:1.40~12.98,P = 0.006)与PMFS(NR vs NR,HR = 4.28,95% CI:1.19~15.34,P = 0.015)均维持显著性差异。多因素Cox回归分析显示,针对PMFS,术前CA19-9升高是患者不良预后的独立因素(HR = 3.70,95% CI:1.55~8.83,P = 0.003)。结论: 术前血清CA19-9升高是Ⅱ~Ⅲ期结直肠癌患者术后发生mPM的敏感生物标志物,可提供独立的风险预测和预后分层。
4.Correlation Between Cardiovascular Events and Traditional Chinese Medicine Syndrome in Patients with Rheumatoid Arthritis:A Cross-Sectional Study

ObjectiveTo explore the correlation between the occurrence of cardiovascular events in rheumatoid arthritis(RA) and traditional Chinese medicine(TCM) syndrome. MethodsThe cross-sectional study selected 6713 RA patients from 122 centres nationwide, in which general information such as name, gender, age, height, body weight, and course of disease were collected by completing a questionnaire; patients were classified into eight types of syndrome according to the information of their four examinations,i.e. wind-dampness obstruction syndrome, cold-dampness obstruction syndrome, dampness-heat obstruction syndrome, phlegm-stasis obstruction syndrome, stasis-blood obstructing collateral syndrome, qi-blood deficiency syndrome, liver-kidney insufficiency syndrome, and qi-yin deficiency syndrome. According to the occurrence of cardiovascular events, they were divided into the occurrence group and the non-occurrence group, and the condition assessment data and laboratory examination indexes were recorded. The test of difference between groups was used to analyse the possible risk factors for the occurrence of RA cardiovascular events, and binary logistic regression was used to analyse the correlation between TCM syndromes and RA cardiovascular events. ResultsA total of 6713 RA patients were included, including 256 cases in occurrence group and 6457 in non-occurrence group. There was no statistically significant difference between groups in terms of height, gender, insomnia, appetite, white blood cell(WBC), hemoglobin(HGB), platelets(PLT), rheumatoid factor(RF), anti-cyclic peptide containing citrulline(CCP), alanine aminotransferase(ALT), aspartate aminotransferase(AST), γ-glutamyl transpeptidase(GGT), urea creatinine(CREA), and glucose(GLU)(P>0.05). The TCM syndromes between groups showed significant statistic differences(P<0.05). Patients in occurrence group had longer disease duration, heavier body weight, and older age; more severe conditions such as disease activity(DAS-28), number of painful joints(TJC), number of swollen joints(SJC), health questionnaire scores(HAQ), visual analog scores(VAS), restlessness, and fatigue; higher blood sedimentation rate(ESR), low-density lipoprotein(LDL-C), triglyceride(TG), total cholesterol(TC), D-Dimer, and lower high-density lipoprotein(HDL-C)(P<0.05). The distribution of syndrome types showed that dampness-heat obstruction syndrome accounted for the largest proportion of patients in both groups and was higher in RA cardiovascular events. Logistic regression analysis showed that the occurrence of RA cardiovascular events was strongly associated with dampness-heat obstruction syndrome[OR=5.937, 95%CI (4.434, 7.949), P<0.001]. ConclusionThe occurrence of RA cardiovascular events were associated with TCM syndromes, and the probability of cardiovascular events in the RA patients with dampness-heat obstruction syndrome was 5.937 times higher than patients with other TCM syndromes.
5.Characterization of the shared microbial profile between infected extraction socket and maxillary sinus in patients with odontogenic maxillary sinusitis

Objective:
To explore whether infected granulation tissue in tooth extraction sockets and maxillary sinus pus share a common microbial profile at the subspecies-strain level in patients with odontogenic maxillary sinusitis (OMS), providing evidence for infection origin tracing and precise antimicrobial therapy in OMS.
Methods:
This study was reviewed and approved by the institutional ethics committee. Nine consecutive OMS patients who underwent synchronous endoscopic sinus surgery and tooth extraction from October 2020 to August 2022 were prospectively enrolled. Under general anesthesia, paired specimens were collected from infected extraction-socket granulation tissue and maxillary sinus pus. Bacterial DNA was extracted, and the full-length 16S rRNA gene was sequenced on the Illumina MiSeq platform. Amplicon sequence variants (ASVs) were generated using the DADA2 algorithm and taxonomically annotated to the subspecies level against the Human Oral Microbiome Database. The detection rate of shared ASVs between the two sites and their relative abundance in sinus pus were compared. Functional profiles were predicted using Phylogenetic Investigation of Communities by Reconstruction of Unobserved States 2 (PICRUSt2).
Results:
Shared ASVs were identified in seven of the nine patients. Fusobacterium, Parvimonas, Porphyromonas, and Prevotella were the most prevalent genera. Porphyromonas gingivalis and Fusobacterium nucleatum were co-detected in multiple patients, with relative abundances exceeding 5% in sinus pus of several cases. Identical ASVs of F. nucleatum or Porphyromonas spp. were detected in six patients; the ASVs corresponding to F. nucleatum subsp. nucleatum and Porphyromonas endodontalis were significantly more abundant in sinus pus than in extraction-socket granulation tissue. PICRUSt2 functional profiling revealed that the proportion of socket-derived microbes in sinus pus was strongly correlated with 10 pathways, including ferroptosis, adipocytokine signaling, and apoptosis, et al. Except for biotin metabolism, the remaining pathways showed weak correlation with the proportion of extraction socket-derived ASVs in the extraction-socket granulation tissue and maxillary sinus pus. Removing F. nucleatum ASVs markedly attenuated these associations
Conclusion
At the subspecies-strain level, this study confirmed the presence of a shared microbial profile between infected extraction-socket granulation tissue and maxillary sinus pus in patients with odontogenic maxillary sinusitis. The co-detected subspecies-strains with high relative abundance in maxillary sinus pus included Fusobacterium nucleatum subsp. nucleatum and Porphyromonas endodontalis, thus providing strain-level microbiological evidence for infection source tracing in OMS.
6.Development and validation a predictive model for distinguishing malignant pleural effusion

Objective To development and validate a predictive model for distinguishing between malignant pleural effusion(MPE)and benign pleural effusion(BPE).Methods A total of 428 patients diagnosed with pleural effusion(PE)and hospitalized at the First Hospital of Huai'an Affiliated to Nanjing Medical University from July 2020 to May 2022 were selected.The patients were divided into BPE group(211 cases)and MPE group(217 cases)according to diagnostic criteria.The basic information and clinical data of these patients were collected.Boruta method was used for univariate screening,followed by multivariate Logistic regression to construct a basic nomogram model.Bootstrap method was used for internal validation to evaluate the performance of the nomogram,including dis-crimination,accuracy,and clinical applicability.Results The model included 8 key variables:dyspnea,chest pain,general symp-toms,X-ray/CT with malignant tumor features,serum carcinoembryonic antigen,serum neuron-specific enolase,pleural lactate dehy-drogenase,and pleural carcinoembryonic antigen.Internal validation showed that the area under the receiver operating characteristic curve(AUCROC)of the model was 0.933(95%confidence interval:0.912-0.954),with good accuracy(P>0.05).Decision curve a-nalysis(DCA)indicated that this predictive model for predicting MPE risk had a significant net benefit when the probability threshold exceeded 1%.Conclusion The constructed prediction model could effectively distinguish between MPE and BPE.

To analyze the standard establishment approach and compilation rationale for metallic pharmaceutical packaging standard development in the 2025 edition of the Pharmacopeia of the People's Republic of China.This article systematically explained the background and process of establishing the guiding principles for metallic materials and containers used in pharmaceutical packaging in the Chinese Pharmacopoeia through basic information,relevant domestic and international standards,the establishment of key quality attributes of metallic pharmaceutical packaging materials,and the construction of metallic pharmaceutical packaging material standards.The newly established guidelines,the Pharmacopeia of the People's Republic of China 9625,prioritized product critical quality attributes(CQAs)and real-world applicability.This dual emphasis on rigidity and adaptability enhances drug safety,meets the regulatory requirements,and promotes the globalization and scientific advancement of China's pharmaceutical packaging industry.

9.Key mediating factors of ABO glycosyltransferases affecting human diseases: ABH blood group antigens and von Willebrand factor

Epidemiology research has found that ABO blood group and the gene coding ABO glycosyltransferases are associated with many human diseases. The activity of ABO glycosyltransferases varies with different blood types, mediating different glycosylation modifications. The variation in glycosylation level might be the risk factor of specific disease. Based on the literature retrieval and analysis, glycosylation levels regulated by ABO glycosyltransferases mainly affect the ABH blood group antigens and von Willebrand factor (vWF). By modulating key glycosylation components, ABO glycosyltransferases partly determine the activity or expression levels of the ABH antigens and vWF, thereby affecting the development and progression of diseases. Exploring the pathogenic mechanisms of ABO glycosyltransferases can improve the understanding of the molecular pathology of related diseases and provide reference for clinical research and application.
10.Effect of side-to-end anastomosis on postoperative bowel function in rectal cancer surgery: a prospective single-center randomized controlled trial

Objective:To compare bowel function 12 months after surgery between side-to-end anastomosis (SEA) and end-to-end anastomosis (EEA) groups of patients who had undergone rectal cancer resection.Methods:This single-center, prospective, open-label, phase III randomized controlled trial was approved by the Ethics Committee of Peking University People's Hospital (2018PHB040-01) and registered at ClinicalTrials. org (NCT03669237). Inclusion criteria were as follows: (1) histologically confirmed rectal adenocarcinoma; (2) tumor located 0 to 12 cm from the anal verge; (3) age≥18 years; and (4) planned R0 resection with primary reconstruction. Exclusion criteria included: (1) emergency surgery; (2) cognitive impairment; (3) non-primary anastomosis; (4) history of left-sided colonic or anorectal surgery; and (5) preexisting chronic defecation dysfunction. Eligible rectal cancer patients scheduled for elective sphincter-preserving surgery at Peking University People's Hospital were prospectively enrolled between October 2018 and March 2021 and randomly assigned to either the EEA group or the SEA group via computer-generated numbers prior to entering the operating room. All patients underwent standard radical tumor resection. Bowel function was evaluated by the low anterior resection syndrome (LARS) questionnaire. It consists of five single-choice questions and yields a total score ranging from 0 to 42. Defecation function is categorized into three levels: no LARS (0-20 points), minor LARS (21-29 points), and major LARS (30-42 points). The primary endpoint was the LARS score 12 months after surgery. Secondary endpoints included LARS scores from 1 to 11 months and during long-term follow-up(>12 months). The final follow-up was completed in July 2022. All randomized patients were included in the intention-to-treat set (ITTS). The full analysis set (FAS) was defined as ITTS patients with valid outcome data. All primary statistical analyses were performed in the FAS, and results were further compared in the per-protocol set (PPS) based on the actual treatment received.Results:A total of 323 patients underwent eligibility assessment, of whom 71 did not meet the inclusion criteria and 52 declined to participate. Ultimately, 200 patients were randomized. Median age was 64 years and 85 were women. The SEA and EEA groups comprised 102 and 98 patients, respectively. A total of 181 patients (90.5%) were included in the FAS, and 170 (85.0%) were included in the PPS. Among these, the 12-month LARS score was evaluated in 178 patients (98.3%) in the FAS and in 167 (98.2%) in the PPS. Median LARS score at 1–12 months were significantly lower in the SEA group in both the FAS dataset [12 months:8 (interquartile range [IQR], 0–22) vs. 14 (IQR, 8–29); Z=2.687, P=0.007] and the PPS dataset [12 months: 8 (IQR, 0–22) vs. 14 (IQR, 6–29); Z=2.543, P=0.011]. During long-term follow-up, the median LARS score was also significantly lower in the SEA group in the FAS dataset [2 (IQR, 0–4) vs. 11 (IQR, 2–23); Z=2.968, P=0.003] and the PPS dataset [2 (IQR, 0–14) vs. 11 (2, 27); Z=2.687, P=0.007]. Conclusion:Compared with the EEA group, bowel function was superior in the SEA group 1 year after surgery and during long-term follow-up.
