1.Therapeutic Effects of Intravitreal Methotrexate Injection for Intraocular Lymphoma Diagnosed Using Immunocytochemical Staining

Purpose:
This study evaluated the therapeutic efficacy of intravitreal methotrexate (MTX) injections in patients diagnosed with intraocular lymphoma via vitrectomy and immunocytochemical staining.
Methods:
In a retrospective analysis of medical records, we reviewed data from four patients (six eyes) diagnosed with intraocular lymphoma cytologically after undergoing vitrectomy at our hospital between December 2021 and December 2023. Each case was followed for a minimum of 6 months after treatment, with comparisons made between pre- and post-treatment observations
Results:
The mean age of the patients was 63.5 ± 9.8 years, with an average interval of 29.3 ± 32.0 months from initial symptom onset to intraocular lymphoma diagnosis. Diagnosis was confirmed through cytological and immunocytochemical analysis of vitreous specimens, identifying diffuse large B-cell lymphoma in four eyes and atypical lymphoid cells in two eyes. On average, 14.0 ± 1.7 intravitreal MTX injections were administered per eye. The mean best-corrected visual acuity improved from 1.18 ± 0.90 pre-treatment to 0.37 ± 0.70 post-treatment. Ophthalmic complications included toxic keratopathy in three eyes and retinal hemorrhage in one eye. Additionally, nasal cavity lymphoma was diagnosed in two patients.
Conclusions
Diagnostic vitrectomy combined with cytology and immunocytochemical staining is essential for the early diagnosis of intraocular lymphoma and differentiation from inflammatory diseases, such as uveitis. Intravitreal MTX injections can induce clinical remission in intraocular lymphoma cases.
3.Morphologic Analysis of Fracture Patterns in the Anterior Region of Femoral Intertrochanteric Fractures Using Computed Tomography

Purpose:
Intertrochanteric fractures often involve comminuted fractures, particularly around the intertrochanteric ridge, including the greater and lesser trochanters. In cases with comminuted fractures in the posterior region, maintaining a stable reduction of the anteriormedial region of the fracture surface is essential. Therefore, this study examined the morphologic features of the anterior femur for stable reduction by analyzing the anterior region of intertrochanteric fractures using three-dimensional computed tomography.
Materials and Methods:
This retrospective study analyzed the imaging data of 51 patients aged 60 years and older who underwent a surgical treatment for intertrochanteric fractures at a single institution from April 2013 to March 2023. The sample contained 15 male patients with an average age of 81.9 years and 36 female patients with an average age of 83.7 years. Forty patients (78.4%) had two or more comorbidities. The bone mineral density was assessed in all cases except for five (9.8%).
Results:
In 49 cases (96.1%), the anterior fracture line followed the intertrochanteric line and passed through two bony prominences.In two cases (3.9%), the fracture line ran below the intertrochanteric line and involved a large fragment of the lesser trochanter. As the fracture line passed through the inferior medial bony prominence, it exhibited an S-shaped pattern rather than a straight line. The inferior medial bony prominence consisted of cortical bone, with an average thickness of 6.4±1.4 mm, which was significantly thicker than the surrounding average cortical bone thickness (1.5±0.6 mm) but thinner than the cortical bone thickness of the femoral shaft (7.3±1.3 mm).The fracture line in the superior lateral region of the intertrochanteric line included cancellous bone with a cortical bone thickness of 1.1±0.6 mm, which was thin.
Conclusion
The anterior fracture line followed the femoral intertrochanteric line and curves as it passed through the inferior medial bony prominence. The inferior medial bony prominence, consisting of cortical bone, was significantly thicker than the surrounding average cortical bone.

Background:
s/Aims: Hepatocellular carcinoma (HCC) is the sixth most common cancer and second leading cause of cancer-related deaths in South Korea. This study evaluated the characteristics of Korean patients newly diagnosed with HCC in 2016-2018.
Methods:
Data from the Korean Primary Liver Cancer Registry (KPLCR), a representative database of patients newly diagnosed with HCC in South Korea, were analyzed. This study investigated 4,462 patients with HCC registered in the KPLCR in 2016-2018.
Results:
The median patient age was 63 years (interquartile range, 55-72). 79.7% of patients were male. Hepatitis B infection was the most common underlying liver disease (54.5%). The Barcelona Clinic Liver Cancer (BCLC) staging system classified patients as follows: stage 0 (14.9%), A (28.8%), B (7.5%), C (39.0%), and D (9.8%). The median overall survival was 3.72 years (95% confidence interval, 3.47-4.14), with 1-, 3-, and 5-year overall survival rates of 71.3%, 54.1%, and 44.3%, respectively. In 2016-2018, there was a significant shift toward BCLC stage 0-A and Child-Turcotte-Pugh liver function class A (P<0.05), although survival rates did not differ by diagnosis year. In the treatment group (n=4,389), the most common initial treatments were transarterial therapy (31.7%), surgical resection (24.9%), best supportive care (18.9%), and local ablation therapy (10.5%).
Conclusions
Between 2016 and 2018, HCC tended to be diagnosed at earlier stages, with better liver function in later years. However, since approximately half of the patients remained diagnosed at an advanced stage, more rigorous and optimized HCC screening strategies should be implemented.
9.Incidence and Clinical Course of Post-infectious Irritable Bowel Syndrome in Patients Admitted to University Hospitals: 1-year Prospective Follow-up Study

Background/Aims:
Post-infectious irritable bowel syndrome (PI-IBS) is characterized by chronic gastrointestinal symptoms that arise following an episode of infectious enteritis. The incidence rates vary, ranging from 5% to 32% and the risk factors are not well known. We aim to investigate the incidence and risk factors of PI-IBS in enteritis patients admitted to university hospitals in Korea.
Methods:
This multi-center prospective study was conducted in patients hospitalized for infectious enteritis. Each patient underwent 1 outpatient visit and 3 telephone surveys during the first year after discharge to determine if PI-IBS occurred within the follow-up period.
Results:
In the 3-month survey, 7 out of 354 patients (2%) were diagnosed with PI-IBS, and after 1 year, only 1 patient met the criteria for IBS.No statistically significant difference was found between the PI-IBS group and the non-PI-IBS group in terms of age, sex, underlying diseases, medication history, gastrointestinal symptoms, enteritis location, causative strain, hospitalization and treatment periods, and laboratory findings. Female sex (P = 0.003), enteropathogenic Escherichia coli (EPEC) infection (P = 0.044), and a longer total treatment period (P = 0.018) were independent risk factors for diarrhea lasting ≥ 3 months after enteritis.
Conclusions
The incidence of PI-IBS in Korea was relatively low, and most cases improved over time. No risk factors associated with the development of PI-IBS were found. However, persistent diarrhea after enteritis was associated with female sex, EPEC infection, and severe or long-lasting enteritis. IBS symptoms may persist after severe enteritis but usually improve with time.
