Objective:To investigate the clinical efficacy of a self-designed assistant device for distal ulnar osteotomy and reduction in the surgical treatment of ulnar impaction syndrome. Methods:A retrospective analysis was performed to study the clinical data from the 27 patients with ulnar impaction syndrome who had been treated by distal ulnar shortening and fixation with 2 screws between January 2022 and August 2024 at Department of Hand and Foot Surgery,Affiliated Hospital of Jining Medical University. The cohort included 6 males and 21 females, with 13 left and 14 right sides affected and a mean age of (40.3±10.8) years (range: from 17 to 59 years). Based on their different assistant methods in osteotomy, the patients were divided into group A (15 cases) subjected to conventional freehand osteotomy and group B (12 cases) subjected to distal ulnar osteotomy and reduction assisted by our self-designed assistant device. Comparisons were made between the 2 groups regarding operative time, bone healing time, Mayo wrist function score at postoperative 16 weeks, number of the patients returning to their original occupations and complications. Results:There were no significant dif- ferences in the baseline characteristics between the 2 groups ( P>0.05). All patients were followed up postoperatively for (27.1±11.1) weeks (range: from 16 to 50 weeks). In group B, the operative time [50.0 (50.0, 62.5) min] and bone healing time [6.5 (6.0, 7.0) weeks] were significantly shorter than those in group A [80.0 (67.5, 92.5) min and 7.5 (6.8, 9.0) weeks] ( P<0.05). At postoperative 16 weeks, the Mayo wrist function score was 90.0 (85.0, 96.8) points for group A and 92.5 (85.0, 98.8) points for group B, showing no significant difference ( P>0.05). Five patients in group A and 4 ones in group B returned to their original work status, showing no significant difference either ( P>0.05). One case of non-union occurred in group A while no complication occurred in group B, demonstrating no significant difference either ( P>0.05). Conclusion:In the surgical treatment of ulnar impaction syndrome, compared with conventional freehand osteotomy, application of our self-designed assistant device for distal ulnar osteotomy and reduction is simple, less invasive, and comparable in functional recovery of the wrist, but superior in operative time and bone healing time.

Chronic visceral pain is a persistent and debilitating condition arising from dysfunction or sensitization of the visceral organs and their associated nervous pathways. Increasing evidence suggests that imbalances in central nervous system function play an essential role in the progression of visceral pain, but the exact mechanisms underlying the neural circuitry and molecular targets remain largely unexplored. In the present study, the ventral tegmental area (VTA) was shown to mediate visceral pain in mice. Visceral pain stimulation increased c-Fos expression and Ca2+ activity of glutamatergic VTA neurons, and optogenetic modulation of glutamatergic VTA neurons altered visceral pain. In particular, the upregulation of NMDA receptor 2A (NR2A) subunits within the VTA resulted in visceral pain in mice. Administration of a selective NR2A inhibitor decreased the number of visceral pain-induced c-Fos positive neurons and attenuated visceral pain. Pharmacology combined with chemogenetics further demonstrated that glutamatergic VTA neurons regulated visceral pain behaviors based on NR2A. In summary, our findings demonstrated that the upregulation of NR2A in glutamatergic VTA neurons plays a critical role in visceral pain. These insights provide a foundation for further comprehension of the neural circuits and molecular targets involved in chronic visceral pain and may pave the way for targeted therapies in chronic visceral pain.

Glaucoma is a group of optic nerve disorders characterized by progressive optic nerve atrophy and visual field defects, which can lead to irreversible blindness. Early diagnosis of glaucoma is essential for preventing visual loss. However, due to the absence of obvious early symptoms, the diagnosis of glaucoma remains challenging. Visual electrophysiological examinations, an objective approach for evaluating visual function, have the potential to be used in the early diagnosis of glaucoma. This review integrates the latest publications to introduce visual electrophysiological examination techniques, including electroretinography(ERG)and visual evoked potential(VEP). It also explores the mechanisms underlying these techniques and their application value in the early diagnosis of glaucoma. In addition, this review summarizes the advantages, limitations, and applicable scenarios of different visual electrophysiological techniques. Finally, the review provides an outlook on the development prospects of visual electrophysiological techniques in the early diagnosis of glaucoma. The findings of this review can assist clinicians in selecting appropriate diagnostic methods, promote the innovation and development of early visual electrophysiological diagnostic techniques for glaucoma, and contribute to reducing the risk of blindness caused by glaucoma.

As the first defense of respiratory system, airway epi-thelial cells(AECs)play an important role in separating the re-spiratory internal and external environment. They are essential for the natural immune function.Small airway lesions are an im-portant early pathology of chronic obstructive pulmonary disease(COPD),when AECs are exposed to harmful particles or gases for a long time, the epithelial barrier is damaged, and the signa-ling pathways which involved in differentiation, repair, and in-flammatory are disordered, resulting in epithelial cell cycle stag-nation and accelerated aging.A number of studies have sugges-ted that AECs of COPD patients express high levels of aging markers, suggesting that senescence of AECs is closely related to COPD.This review discusses the potential mechanisms of AECs senescence in COPD,the impact of AECs senescence on the de-velopment and severity of the disease, and highlights potential targets for modulating cellular senescence in airway epithelium as a therapeutic approach in COPD.
