1.Endovascular Treatment for Vertebral Artery Occlusion with Anterior Spinal Artery Involvement: Contrasting Outcomes in Two Cases

Vertebral artery (VA) occlusion is often managed conservatively, but anterior spinal artery (ASA) involvement may cause rapid neurological deterioration. We report two contrasting cases of VA occlusion with ASA compromise. One patient achieved complete recovery after timely endovascular treatment (EVT) restoring ASA flow, whereas the other developed severe bilateral medial medullary infarctions following unsuccessful EVT. These cases highlight the prognostic importance of ASA involvement and suggest that EVT should be considered when ASA perfusion is threatened.
2.Efficacy and Safety of Latanoprostene Bunod 0.024% Ophthalmic Solution in Korean Patients

Purpose:
To evaluate the efficacy and safety of latanoprostene bunod 0.024% (LBN) in Korean patients with glaucoma or ocular hypertension in a real-world clinical setting.
Methods:
This retrospective study included patients who initiated LBN treatment between March 1, 2022, and December 31, 2024. Patients were categorized into monotherapy, switched therapy, and concomitant therapy groups. Changes in intraocular pressure (IOP) were assessed at baseline, month 1, and month 6 according to treatment group and diagnosis. The type and frequency of adverse events were also analyzed.
Results:
Among the 138 patients initially identified, 129 were included in the analysis. The mean IOP for all patients was 17.4 ± 4.9 mmHg at baseline, 16.0 ± 4.6 mmHg at month 1, and 14.6 ± 3.7 mmHg at month 6. The mean IOP reduction was -1.4 ± 2.8 mmHg at month 1 (p < 0.001) and -2.2 ± 3.3 mmHg at month 6 (p < 0.001). Subgroup analyses by treatment type (monotherapy, switched, and concomitant) and diagnosis revealed significant IOP reductions at both time points across all groups. Adverse events occurred in 22 patients (17.1%), with eye pain being the most frequent. No serious adverse events were observed, including systemic side effects, severe visual impairment, or significant ocular complications.
Conclusions
LBN significantly reduced IOP in Korean patients with glaucoma and ocular hypertension, irrespective of prior IOP-lowering medication use and was well tolerated with a favorable safety profile. These findings support the use of LBN as an effective treatment option for Korean patients with glaucoma or ocular hypertension in diverse clinical settings.
4.Biportal Endoscopic Spine Surgery for Epidural Metastatic Tumors: A Surgical Technical Note With a Case Series

Endoscopic spine surgery has rapidly evolved as a minimally invasive technique for treating various spinal pathologies. However, its use in removing epidural metastatic tumors remains insufficiently explored. This video article presents 2 cases utilizing unilateral biportal endoscopic spine surgery for resection of epidural metastatic tumors causing spinal cord compression. The first case involved a 63-year-old woman with metastatic non-small cell lung cancer at T4–5, and the second case an 86-year-old man with prostate cancer metastases at T6–9. Both patients presented with motor weakness (American Spinal Injury Association [ASIA] grade C) and potential spinal instability (SINS [Spinal Instability Neoplastic Scale] score 7). The surgical techniques emphasized precise identification and dissection of the tumor–dura interface to minimize dural injury and bleeding—an essential consideration when managing vascular lesions, particularly under antiplatelet therapy. Both cases achieved complete tumor resection with minimal blood loss (60–90 mL) and operative times of 71 and 109 minutes, respectively. Postoperatively, both patients improved to ASIA grade D and began early radiotherapy, underscoring the advantages of this minimally invasive approach in enabling prompt adjuvant treatment. Endoscopic epidural tumor removal represents a safe and less invasive alternative to open surgery for selected patients, though further long-term evaluation is warranted.
5.Biportal Endoscopic Foramen Magnum Decompression in an Arnold-Chiari Malformation: A Technical Note With a Case Report

Chiari malformation, when accompanied by progressive neurological symptoms or syringomyelia, often necessitates surgical decompression. Although endoscopic spinal surgery continues to advance, its application in foramen magnum decompression remains limited. This video article demonstrates the biportal endoscopic foramen magnum decompression technique for Chiari type I malformation. We present the case of a 19-year-old female patient with progressive headache, motor weakness, and radiological evidence of 7-mm tonsillar descent with C2–7 syringomyelia. She successfully underwent biportal endoscopic foramen magnum decompression with C1 laminectomy. The procedure employed a triportal approach with safe docking on the C2 spinous process, allowing a minimally invasive C1 laminectomy and foramen magnum decompression extended to the suboccipital area. Postoperatively, computed tomography confirmed adequate decompression, and magnetic resonance imaging revealed expansion of the posterior fossa with resolution of tonsillar herniation. The patient experienced no complications or symptom recurrence at the 3-month follow-up. Despite a minor intraoperative–postoperative measurement discrepancy, which highlights the anatomical considerations required to achieve sufficient decompression, this video supports the feasibility of biportal endoscopic surgery. This approach may provide comparable clinical outcomes to conventional open surgery while offering minimally invasive advantages, though long-term follow-up remains essential.
6.The Functional Study of Prostaglandin E2 via EP Receptor on Pacemaker Activity in Interstitial Cells of Cajal from Murine Colon

The interstitial cells of Cajal (ICC) generate spontaneous pacemaker activities responsible for producing slow waves in the gut smooth muscles. Prostaglandin E2 (PGE2 ) is one of the most important prostanoids in the gut, playing a crucial role in multiple physiological and pathological processes. Because information regarding the effects of PGE2 on colonic ICC is limited, we investigated the effects of PGE2 and its underlying signaling pathways in murine colonic ICC. Whole-cell patch-clamp recordings and reverse transcription polymerase chain reaction (RT-PCR) analyses were performed to evaluate the effects of PGE2 on mouse colonic ICC. PGE2 had a dual effect on pacemaker potential in the current-clamp mode. RT-PCR data suggested the expression of EP3 and EP4 receptors in colonic ICC. Low PGE2 concentrations induced membrane depolarization and generation of pacemaker activities. And this effect is mimicked by sulprostone (an EP3 agonist). The low PGE2 concentration-induced effect was inhibited by anoctamin-1 (ANO1) or a T-type Ca2+ channel blocker. Conversely, high PGE2 concentrations induced membrane hyperpolarization and blocked pacemaker potential generation. However, this inhibitory effect was blocked by an ATP-sensitive potassium (K ATP) channel blocker but not by other K+ channel blockers. Low concentrations of PGE2 activated EP3 receptors, leading to excitation of pacemaker activity through regulation of ANO1 and T-type Ca2+ channels in colonic ICC. In contrast, higher concentrations of PGE2 activated EP4 receptors and opened ATP-sensitive K+ channels, resulting in inhibition of pacemaker activity in colonic ICC.
7.Guideline for the Management of Obesity in Adult Patients With Obstructive Sleep Apnea

Objectives:
. Obstructive sleep apnea (OSA) is a common sleep-related breathing disorder that is strongly influenced by obesity, the most important modifiable risk factor. Effective weight management is therefore essential for optimal OSA care. This guideline provides updated, evidence-based recommendations for the evaluation and management of obesity in adults with OSA, incorporating recent advances in lifestyle, pharmacologic, and surgical interventions.
Methods:
. The Korean Society of Sleep and Breathing convened a multidisciplinary panel. A systematic literature search was performed across major databases through 2025. Evidence was appraised using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach, and recommendations were formulated by consensus using the Evidence-to-Decision framework.
Results:
. Eight statements were developed. Key recommendations include recognizing obesity as a major risk factor; routinely assessing overweight and obesity; and implementing structured weight reduction through diet, exercise, and behavioral therapy. Glucagon-like peptide-1 receptor agonists are recommended when lifestyle interventions are insufficient, and bariatric/metabolic surgery is advised for severe obesity unresponsive to nonsurgical treatment. Weight loss improves the apnea-hypopnea index, symptoms, and cardiometabolic markers and may reduce positive airway pressure needs. Continued follow-up is recommended even after clinical improvement.
Conclusion
. Weight management is a core component of OSA treatment. This guideline offers practical, evidence-based strategies to support clinicians in delivering effective, obesity-focused care for adults with OSA.

10.Cervical Spinal Melanocytoma: A Case Report and Literature Review

Spinal melanocytoma (SMC) is a rare, slow-growing tumor arising from melanocytes in the spinal cord. We report a patient with a cervical intra- and extradural spinal tumor causing progressive weakness and numbness. On magnetic resonance imaging (MRI), the lesion showed intense homogeneous enhancement, similar to that seen in common neurogenic spinal tumors. After complete resection, pathological examination confirmed melanocytoma. A review of previously reported cases identified 26 reports of this tumor in the cervical spine, most of which were treated with complete surgical resection. Gross total resection is the preferred treatment, although radiation therapy may be considered when residual tumor remains. We report a 25-year-old male patient who presented with progressive weakness and numbness in both the upper and lower extremities for 3 months. MRI showed homogeneous enhancement. The mass compressed the spinal cord at C6–7 and extended through the neural foramen. Based on the MRI findings, spinal schwannoma was suspected preoperatively. Surgical resection was performed with laminectomy, durotomy, and right facetectomy. A dark-colored mass with well-demarcated margins was exposed and removed. Postoperative MRI confirmed complete removal of the mass. The patient recovered well, and his preoperative myelopathic symptoms gradually improved. SMC is a rare benign tumor that may be mistaken for schwannoma. The treatment of choice is gross total resection.
