1.Double spontaneous coronary artery dissection in a 54-year-old male patient at a tertiary hospital: A case report.

BACKGROUND
This case report describes a rare presentation of double-vessel Spontaneous Coronary Artery Dissection (SCAD) in a 54-year-old male. One dissection occurred in a coronary artery with atherosclerotic plaque deposition, while the second vessel exhibited dissection without evidence of plaque deposition, trauma or iatrogenic manipulation such as percutaneous coronary intervention. The patient, a known hypertensive and dyslipidemic, had a prior diagnosis of heart failure with reduced ejection fraction (HFrEF). He presented with progressively worsening shortness of breath and orthopnea (two-pillow orthopnea for three years. An initial coronary angiogram was recommended due to a high suspicion of underlying coronary artery disease (CAD).Subsequent diagnostic workup revealed multisegmental wall motion abnormalities on 2D echocardiography with Doppler imaging. Laboratory tests, including electrolytes, were unremarkable. A follow-up coronary angiogram revealed mild to moderate CAD in the left anterior descending (LAD) artery and spontaneous coronary artery dissection involving both the LAD and the right coronary artery (RCA). The patient was managed conservatively with aspirin and discharged on a regimen of sacubitril/valsartan, ivabradine, trimetazidine, and rosuvastatin. As typically seen in SCAD cases, conservative medical management was preferred over invasive procedures such as PCI or coronary artery bypass grafting (CABG), which are reserved for select clinical situations.

Suicide left ventricle describes the development of dynamic intraventricular gradients after aortic valve replacement due to acute hemodynamic changes that happen after relieving the obstruction leading to hemodynamic collapse. This is a rare complication in transcatheter aortic valve replacement and is underreported in surgical aortic valve replacement (SAVR). We present the case of a male patient who presented with suicide left ventricle after SAVR.

4.A rare case of acute perimyocarditis with associated acalculous cholecystitis in a 28-year-old Female: A case report.

BACKGROUND
Perimyocarditis due to inflammation of the pericardium and myocardium results in myocellular damage. Myocarditis, or myocardial inflammation, occurs after cardiac injury. Gallbladder edema, in the absence of cholecystitis, may occur in numerous conditions including cardiac inflammation.
CASE PRESENTATIONA 28-year-old previously healthy female presented with chest pain, orthopnea, exertional dyspnea and a history of fever. She also reported intermittent right upper quadrant pain. Physical exam revealed a pericardial friction rub. Electrocardiogram (ECG) showed sinus rhythm with nonspecific ST-T changes; troponin I was elevated. Echocardiography demonstrated segmental wall motion abnormalities, pericardial thickening and preserved systolic function. Initially managed as acute coronary syndrome, she was later diagnosed with perimyocarditis. On the second hospital day, she developed recurrence of right upper quadrant abdominal pain. Abdominal ultrasound revealed gallbladder edema with pericholecystic fluid, but no stones. Liver enzymes were elevated. Acalculous cholecystitis was considered and cholecystostomy offered instead due to aspirin therapy. However, repeat imaging showed resolution of cardiac and gallbladder findings, and surgery was deferred. Cardiac MRI postdischarge was unremarkable.
CONCLUSIONPerimyocarditis may present with gallbladder edema mimicking acalculous cholecystitis, potentially leading to unnecessary surgical intervention. This case emphasizes the importance of considering cardiac etiologies in atypical abdominal presentations.

5.A Rare Case of Pituitary Apoplexy Associated with Middle Cerebral Artery Infarct: A Correlation or Coincidence?

Introduction:
Pituitary apoplexy is a rare, life-threatening condition
resulting from hemorrhage or infarction of the pituitary
gland, most commonly in patients with pre-existing
pituitary tumors. It typically presents with a sudden
headache, visual disturbance, ophthalmoplegia, and altered
mental status. An uncommon but serious complication
is ischemic stroke in the middle cerebral artery (MCA) territory, particularly in the absence of direct internal
carotid artery (ICA) compression.
Case:
We report a case of a 36-year-old male who presented
with headache, visual impairment, and fever. Initial
computed tomography (CT) brain imaging demonstrated
a heterogeneous sellar lesion with peripheral calcification
measuring 2.4 × 3.2 × 2.7 cm, suggestive of a pituitary mass
with possible apoplexy, without evidence of acute cerebral
infarction. The patient subsequently developed dysarthria,
hemianopia, and reduced consciousness, prompting repeat
neuroimaging. Follow-up CT revealed a large hypodense
area in the right fronto-parieto-temporal region consistent
with ischemic infarction. Magnetic resonance imaging
confirmed an acute infarct in the right MCA territory without
hemorrhagic transformation. A sellar-suprasellar mass
measuring 2.3 × 2.8 × 3.8 cm was identified, consistent with
a pituitary macroadenoma with intratumoral hemorrhage
compressing the optic chiasm, but without direct right ICA
compression. Time-of-flight mineralocorticoid receptor
antagonists demonstrated attenuated flow in the right
ICA (C2–C7), suggesting intracranial ICA thrombosis and
reduced perfusion in the right MCA and its branches.
Laboratory evaluation revealed hyperthyroidism and
hypocortisolism, with no evidence of coagulopathy. The
patient was treated with corticosteroid replacement and
carbimazole and referred for neurosurgical management.
Conclusion
This case highlights a rare association between pituitary
apoplexy and MCA stroke, possibly mediated by
vasospasm, inflammation, or hypercoagulability. More
research is needed to understand this connection and
improve treatment strategies.

6.Finite Element Simulation Analysis of a Nickel-Titanium Alloy Patent Foramen Ovale Occluder.

In this paper, a preliminary stress/strain analysis of the design structure of a nickel-titanium alloy patent foramen ovale occluder is conducted with the finite element simulation analysis method. In the analysis, solid structure modeling is carried out on three different specifications of domestic patent foramen ovale occluders. Referring to the test method of fatigue performance in inspection standard YY/T 1553-2017, an initial installation deformation is applied to the model, and then the fatigue displacement of 2 mm is applied to the sample to make the model fatigue deformation. The fatigue safety factors of each type of occluder are obtained by strain simulation analysis. The results indicate that the minimum fatigue safety factors of the three specifications of domestic patent foramen ovale occluders are 2.09, 2.35 and 2.06 respectively, which all meet the design of fatigue safety factor greater than 1. Among them, 1818 and 3030 specifications of patent foramen ovale occluders have close values in minimum fatigue safety factors, and both are lower than that of 1825 model. Therefore, it is recommended to carry out physical fatigue tests on both 1818 and 3030 specifications to further verify the fatigue performance of the products.

7.Current Situation and Development Trends Analysis of Innovative Medical Devices for Heart Valve Intervention.

In recent years, the field of heart valve interventional treatment has developed rapidly, and various new technologies have been continuously applied. The study provides a comprehensive review and analysis of the research progress and challenges related to interventional treatment products for the aortic valve, pulmonary valve, mitral valve, and tricuspid valve both domestically and internationally. The study also summarizes the status of heart valve interventional devices entering Special Review of Innovative Medical Device of National Medical Products Administration (NMPA). The results show that the field of valve interventional treatment is developing rapidly, but there is still room for improvement in terms of valve durability, operational suitability, and complication reduction. Lastly, the study looks forward to the future development trend of new technologies for heart valve interventional therapy, and puts forward suggestions for the promotion and application of new technologies of valve interventional therapy from the perspective of supervision.

8.A Novel Coronary Knobby Scoring Balloon and Biomechanical Study in Intravascular Dilation.

This study investigated a novel coronary knobby scoring balloon through finite element analysis (FEA) and in vitro anti-slippage testing, evaluating its dilation process under various vascular conditions and comparing it with other balloons. The FEA results indicated that in the cases of healthy artery and diseased artery with different stenosis rates, the stress on the vessels caused by the knobby scoring balloon was significantly smaller than that of the scoring balloon, and was close to that of the plain balloon. In vitro anti-slippage testing showed that the slippage distance of a plain balloon was 0.11±0.06 mm, and there was no slippage for knobby scoring balloon under nominal pressure. Knobby scoring balloon can effectively expand calcified lesion while providing anti-slippage function, and has a lower risk of vascular injury.

9.Optimization of flow rate and orientation of outflow graft at implantation for patients with left ventricular assist device.

OBJECTIVES:
A ventricular assist device (VAD) is an electromechanical device used to assist cardiac blood circulation, which can be employed for the treatment of end-stage heart failure and is most commonly placed in the left ventricle. Despite enhancing perfusion performance, the implantation of left ventricular assist device (LVAD) transforms the local intraventricular flow and thus may increase the risk of thrombogenesis. This study aims to investigate fluid-particle interactions and thromboembolic risk under different LVAD configurations using three-dimensional (3D) reconstruction models, focusing on the effects of outflow tract orientation and blood flow rates.
METHODS:
A patient-specific end-diastolic 3D reconstruction model was initially constructed in stereo lithography (STL) format using Mimics software based on CT images. Transient numerical simulations were performed to analyze fluid-particle interactions and thromboembolic risks for LVAD with varying outflow tract orientations under 2 flow rates (4 L/min and 5 L/min), using particles of uniform size (2 mm), and a blood flow rate optimization protocol was implemented for this patient.
RESULTS:
When the LVAD flow rate was 5 L/min, helicity and flow stagnation of the blood flow increased the particle residence time (RT) and the risk of thrombogenesis of the aortic root. The percentage of particles traveling toward the brachiocephalic trunk was up to 20.33%. When the LVAD flow rate was 4 L/min, blood turbulence in the aorta was reduced, the RT of blood particles was shortened, and then the percentage of particles traveling toward the brachiocephalic trunk decreased to 10.54%. When the LVAD blood flow rate was 5 L/min and the direction of the outflow pipe was optimal, the RT of blood particles was shortened, and then the percentage of particles traveling toward the brachiocephalic trunk decreased to 11.22%. A 18-month follow-up observation of the patient revealed that the LVAD was in good working order and the patient had no complications related to the implantation of LVAD.
CONCLUSIONS
Implantation of LVAD results in a higher risk of cerebral infarction; When implanting LVAD with the same outflow tract direction, optimizing flow velocity and outflow tract can reduce the risk of cerebral infarction occurrence.

10.Protective effects of exerkine on cardiovascular system.

Cardiovascular disease (CVD) poses a serious threat to human health. Exercise plays an important role in both the prevention and treatment of CVD and is one of the key non-pharmacological interventions. Exercise can regulate the level of exerkine secreted by different tissue cells, directly affect the cardiovascular system or play a role in cardiovascular protection by improving cardiovascular risk factors. Exerkine such as meteorin-like protein (Metrnl), brain-derived neurotrophic factor (BDNF), fibroblast growth factor 21 (FGF21), and exosomal microRNA (miRNA) play an important role in regulating vascular and cardiac diseases such as atherosclerosis, heart failure, cardiac ischemia-reperfusion and myocardial infarction, as well as their risk factors. Exploring the signaling pathways and mechanisms by which Metrnl, BDNF, FGF21, and exosomal miRNAs exert cardiovascular protective effects can provide novel insights into exercise-based strategies for preventing and treating cardiovascular diseases.
